Medical treatment for animals is very difficult as the opinions of pets' masters take priority over treatment regardless of the seriousness of pets' disease or the needs of medical treatment. In case that a pet has heart disease, especially, it is difficult to get the direct answer from the pet's master on the decision or confirmation of treatment. For those reasons, it is almost impossible to predict and treat the pet before an emergency like the heart failure or an unexpected death happens. Using stethoscope can be the first diagnosis method to check the heart or any kinds of disease inside the body. High-tech equipments like CT, X-ray or Ultrasound can be used, but they can only be used as a second choice of diagnosis method since it requires professional skills and its high price. That's why stethoscope is still the best diagnostic tool when one makes the first diagnosis. In this study, we give a detailed account of digital diagnosis system in which veterinarians can analyze the sound from stethoscope without bringing it to their ears and make a diagnosis wherever they are. And we suggest a new concept of diagnosis system surrounding, which shows the relativeness of disease through Level Crossing Rate(LCR) and energy level from the stethoscope sound made in this system.

Purpose: The United States of America have pre-migration screening program are mandated through legislation. The one of purpose of this program is to prevent the importation of certain communicable diseases. Examinations of migration applicants are performed by panel physicians who are licensed physicians in the countries of origin that have agreements with the government departments of the country of destination to undertake this activity. In recent decades, the number of US migrants has increased, however COVID-19 impacted this trend of international migrants. The Aim of this study is to examine trends of US applicants and outcomes of tuberculosis and sexual transmitted infections (STIs) screening in South Korean applicants for US visa. Methods: A total of 4,442 applicants participated in US visa health check-up in 2016 to 2020. Results: The numbers of applicant for US visa in males and females was 1,814 and 2,628. The positive results in tuberculosis and STIs screening was noted with 0.66% in nontuberculous mycobacteria (NTM), 0.03% in tuberculosis, 0.03% in gonorrhea, 0.08% in syphilis, and 0.74% in chlamydia. The age groups of 2 to 14 years showed higher positive rate (13.89%) in tuberculin skin test (TST) tests compared to the rate of interferon gamma release assay (IGRA) tests (0.44%, P<0.001). The positive ratio of NTM and abnormal chest X-ray (CXR) which required sputum tests among the applicants over 15 years old has trends to increase with age. The age groups of over 70 years showed high positive rate in NTM and abnormal CXR (4.10%, 20.51%). However, ages 15 to 49 years groups showed 0.00% to 0.30% in NTM and 1.08% to 3.91% in abnormal CXR. In the study population, the positive rates of STIs were 0.03%, 0.08%, and 0.74% in gonorrhea, syphilis, and chlamydia, respectively. Among the participants who underwent sputum smears and cultures were just 1 case of active tuberculosis (0.03%) and 25 cases of NTM infection (0.66%). Conclusion: This study found that rate of positivity in tuberculosis and STIs screening among South Korean applicants for US visa in 2016 to 2020. Regarding tuberculosis, the positivity of CXR among the aged 15 to 59 years applicants was lower than that among over 60 years old. Among 2 to 14 years, positivity of IGRA was very low. And IGRA test can reduce effort of further evaluation compared to TST test in South Korea, so IGRA test among young adults can be useful. High positive rates in STI among young adult could result in high active sexuality, especially chlamydia infection is most common. Therefore Chlamydia polymerase chain reaction should be considered formal protocol among the Korean applicant for US visa.

Digital substraction technique and computer-assisted densitometirc analysis detect minor change in bone density and thus increase the diagnostic accuracy. This advantage as well as high sensitivity and objectivity which precludes human bias have drawn interest in radiologic research area. The objectives of this study are to verify if Radiographic density can be recognized in linear pattern when density profile of standard periapical radiograph with the aluminium stepwedge as the reference, was investigated under varies circumstances which can be encountered in clinical situations, and in addition to that to obtain mutual relationship between the existing standard radiographic system, and future digital image systems, by confirming the corelationship between the standard radiograph and Digora system which is a digital image system currently being used. In order to make quantitative analysis of the bone tissue, digital image system which uses high resolution automatic slide scanner as an input device, and Digora system were compared and analyzed using multifunctional program, Brain3dsp. The following conclusions were obtained. 1. Under common clinical situation that is 70kVp, 0.2 sec., and focal distance 10cm, Al-Equivalent image equation was found to be Y=11.21X+46.62 $r^2=0.9898$ in standard radiographic system, and Y=12.68X+74.59, $r^2=0.9528$ in Digora system, and linear relation was confirmed in both the systems. 2. In standard radiographic system, when all conditions were maintained the same except for the condition of developing solution, Al-Equivalent image equation was Y=10.07X+41.64, $r^2=0.9861$ which shows high corelationship. 3. When all conditions were maintained the same except for the Kilovoltage peak, linear relationship was still maintained under 60kVp, and Al-Equivalent image equation was Y=14.60X+68.86, $r^2=0.9886$ in the standard radiograhic system, and Y=13.90X+80.68, $r^2=0.9238$ in Digora system. 4. When all conditions were maintained the same except for the exposure time which was varied from 0.01 sec. to 0.8 sec., Al-Equivalent image equation was found to be linear in both the standard radiographic system and Digora system. The R-square was distributed from 0.9188 to 0.9900, and in general, standard radiographic system showed higher R-square than Digora system. 5. When all conditions were maintained the same except for the focal distance which was varied from 5cm to 30cm, Al-Equivalent image equation was found to be linear in both the standard radiographic system and Digora system. The R-square was distributed from 0.9463 to 0.9925, and the standard radiographic system had the tendency to show higher R-square in shorter focal distances.
Objective : The aim of this study was to compare the knee joint displaying between the anteroposterior weight bearing(AP-WB) View and the metatarsophalangeal(MTP) view for assessing joint space narrowing(JSN) and osteophytes in osteoarthritis patients. Subjects and Materials : Two hundreds of twenty patients(38 men) who came rheumatoid caused by knee pain, had both AP-WB and MTP views taken on a day. Radiographs were evaluated independently by 13 experienced observers(3 orthopedics surgeon, 2 rheumatogist, 3 radiologist, 5 radiological technologist) They assessed JSN and osteophytes using by PACS monitor JSN was scored by the optic evaluation to the nearest at the narrowest point in medial compartments of the tibiofemoral joint in both knees. Osteophytes were graded 0 to 3(bad 0, not bad 1, good 2 and very good 3) according to a standard atlas. All exam was using by Philips(Buckey Diagnostic-TH) X-ray material. Exposure condition was 60 kv, 8 mAs and 100 cm focus to film distance. Results : JSN was scored $1.32{\pm}0.050$ in AP-WB view, $2.51{\pm}0.046$ in MTP view. MTP view of JSN score is higher to AP-WB view significantly(p<0.05). Osteophytes scored $2.14{\pm}0.054$ in AP-WB view, $2.10{\pm}0.054$ in MTP view. There was no difference(p<0.05) between MTP view and AP-WB view in osteophytes. But MTP view was more reproducible than AP-WB view Conclusions : Joint space narrowing is most important factor to diagnosis with knee joint Osteoarthritis patients. This study was summarized as follows; In comparision of JSN, MTP view was more widely displayed than AP-WB view. In comparision of Osteophytes, there was no difference between MTP view and AP-WB view. It was concluded MTP view was more useful method to diagnosis of knee joint Osteoarthritis patients.

The quality assurance (QA) is very important for diagnostic field and radiation therapy field to evaluate the characteristic of devices. The purpose of this study was to compare different NPS methodologies results which are measuring NPS with regard to horizontal and vertical directions by using megavoltage X-ray energies. The NPS evaluation methods were applied to the International Electro-technical Commission standard (IEC 62220-1). The electronic portal imaging devices (EPID) devices such as Siemens BEAMVIEW$^{PLUS}$, Elekta iViewGT and Varian Clinac$^R$ iX aS1000 were used. NPS data were expressed by corresponding each frequency about average of noise value corresponding the each frequency, and NPS were evaluated quantitatively by totaling up the noise values of average frequency which are on horizontal and vertical directions. In NPS results for Elekta iViewGT, NPS of horizontal and vertical by using 4 methods were indicated the difference of 3~5% between horizontal and vertical direction. In the results of Siemens BEAMVIEW$^{PLUS}$ and Varian Clinac$^R$ iX aS1000, the NPS of horizontal and vertical direction were indicated the difference of 15% when averaging the whole values. This study were evaluated the NPS of each devices by totaling up the noise values of average frequency which are on horizontal and vertical directions suggesting the quantitative evaluation method using the data.
Purpose: We designed this retrospective study to establish the incidence of diagnosic delay in children diagnosed with acute appendicitis and to identify associated factors with delayed diagnosis and its impact on the clinical course. Methods: All cases of children under 15 years of age who underwent appendectomy from 1996 to 2001 at Gangneung Asan Hospital were reviewed. We reviewed signs and symptoms, type of health professional first contacted, the advice given by the health professional and a history of appendicitis in first degree relatives. Diagnostic period is the time elapsed between first complaints and definitive diagnosis. Delay was defined as diagnostic period exceeded the 48 hours. Postoperative course and complications were also reviewed. Results: Incidence of diagnostic delay differed by whether diarrhea and fecalith on X-ray were present. Also children whose parents were advised to observe them at home were more likely to have a diagnostic delay. In almost half of the cases in delayed group, initial diagnosis was not acute appendicitis but gastroenteritis. The perforation rate in non-delayed group was 22%, whereas 87% in delayed group. The delayed group showed a higher number of postoperative complication and a longer hospitalization period. Conclusions: Diarrhea with abdominal pain and fever in children should not be dismissed as gastroenteritis, respiratory infections or other common disorders. Our study suggests that physicians have a responsibility to prevent diagnostic delay and resultant perforation of acute appendicitis in children by having a high index of suspicion about acute appendicitis.
With the recent prevalence of helical CT and multi-slice CT, which deliver higher radiation dose than conventional CT due to overbeaming effect in X-ray exposure and interpolation technique in image reconstruction. Although multi-detector and helical CT scanner provide a variety of opportunities for patient dose reduction, the potential risk for high radiation levels in CT examination can't be overemphasized in spite of acquiring more diagnostic information. So much more concerns is necessary about dose characteristics of CT scanner, especially dose efficient design as well as dose modulation software, because dose efficiency built into the scanner's design is probably the most important aspect of successful low dose clinical performance. This study was conducted to evaluate z-axis geometric dose efficiency in single detector CT and each level multi-detector CT, as well as to compare z-axis dose efficiency with change of technical scan parameters such as focal spot size of tube, beam collimation, detector combination, scan mode, pitch size, slice width and interval. The results obtained were as follows ; 1. SDCT was most highest and 4 MDCT was most lowest in z-axis geometric dose efficiency among SDCT, 4, 8, 16, 64 slice MDCT made by GE manufacture. 2. Small focal spot was 0.67-13.62% higher than large focal spot in z-axis geometric dose efficiency at MDCT. 3. Large beam collimation was 3.13-51.52% higher than small beam collimation in z-axis geometric dose efficiency at MDCT. 4. Z-axis geometric dose efficiency was same at 4 slice MDCT in all condition and 8 slice MDCT of large beam collimation with change of detector combination, but was changed irregularly at 8 slice MDCT of small beam collimation and 16 slice MDCT in all condition with change of detector combination. 5. There was no significant difference for z-axis geometric dose efficiency between conventional scan and helical scan, and with change of pitch factor, as well as change of slice width or interval for image reconstruction. As a conclusion, for reduction of patient radiation dose delivered from CT examination we are particularly concerned with dose efficiency of equipment and have to select proper scanning parameters which increase z-axis geometric dose efficiency within the range of preserving optimum clinical information in MDCT examination.
