  • Modern hospitals have been greatly facilitated with information technology (IT) such as hospital information system (HIS). One of the most prominent achievements is medical imaging and image data management so-called Picture Archiving and Communication Systems (PACS). Due to inevitable use of diagnostic images (such as X-ray, CT, MRI), PACS made tremendous impact not only on radiology department but also nearly all clinical departments for exchange and sharing image related clinical information. There is no doubt that better use of PACS leads to highly efficient clinical administration and hospital management. However, due to rapid and widespread acceptance of PACS storage and management of digitized image data in hospital introduces overhead and bottleneck when transferring images among clinical departments within and/or across hospitals. Despite numerous technical difficulties, financing for installing PACS is a major hindrance to overcome. In addition, a mirroring or a clustering backup can be used to maximize security and efficiency, which may not be considered as cost-effective approach because of extra hardware expenses. In this study therefore we have developed a new based on grid of distributed PACS in order to balance between the cost and network performance among multiple hospitals.

  • Thin-film transistor (TFT) arrays for an x-ray detector require quite different design concept from that of the conventional active-matrix liquid crystal devices (AM-LCDs). In this paper anew design of TFT array which uses only SiNx for passivation layer is described to meet the detector performance and the product availability simultaneously. For the purpose of optimizing the design parameters of the TFT array, a Spice simulation was performed. As a result, some parameters, such as the TFT width, the data line capacitance, and the storage capacitance, were able to be fixed. The other parameters were decided within a permissible range of the TFT process especially the photolithography process and the wet etch process. Then we adapted the TFT array which had been produced by the proposed design to our prototype model (FDXD-1417 and evaluated it clinically by comparing with a commercial model (EPEX, Hologic, Beford, USA). The results say that our prototype model is slightly better than EPEX system in chest PA images. So we can prove the technical usefulness and the commercial values of the proposed TFT design.

  • Generally the inverse square law (ISL) is applied by use of intensity scale method, that is distance scale method, when we practice sensitometry of X-ray film. It was reported that the failure of the ISL had occurred in the many experiments. Thus authors obtained some conclusions of the exponent value of distance by use of dosimeter and fluorescence meter in the range of tube voltage 60 - 120 kvp, that is the range of diagnostic energy. Those conclusions were as follows its exponents were produced as 2.1383 - 2.1763 in dosimeter, as 2.1190-2.2152 in fluorescence meter. This revealed that the perfect ISL was failded and that numerical differences were shown in each measurem apparatus. Therefore it is need to study the exponent at each tube voltage for the precise supply of fixed quantity on the characteristic curve's horizontal axis.

  • Developmental pulmonary abnormalities are known as rare condition. diagnosis was made at autopsy in the early cases reported, however, as diagnostic aids such as X-ray, bronchography, bronchoscopy and exploratory thoracotomy have come into use, the condition is being discovered more often recently in living persons, and it appears to occur with sufficient frequency to merit consideration in the differential diagnosis of certain chest conditions. According to Schneider and Boyden there are three main types of this abnormality: [1] Agenesis, in which there is complete absence of one or both lungs; there is no trace of bronchial or vascular supply or of parenchymal tissue. [2] Aplasia, in which there is suppression of all but a rudimentary bronchus which ends in a blind pouch; there are no vessels or parenchyma. [3] Hypoplasia, in which the bronchus is fully formed but is reduced in size and ends in a _ flesh structure which usually lies within the mediastinum. Rudimentary pulmonary parenchyma may be present around the bronchial stump and often is the site of cystic malformation. We experienced one case of hypoplastic lung with cystic malformation which was originated from a small aberrant rudimentary bronchus, and the rudimentary bronchus was branched from the right side of tracheal end. The diagnosis was finally confirmed by the histopathological finding. Now, we report this case with a brief review of literatures.

  • Morgagni's hernia constitutes about 3% of all the congenital diaphragm hernias. It is usually asymptomatic and it is frequently found coincidentally during routine diagnostic testing in adulthood. It is usually diagnosed by simple chest X-ray, but when this condition is without intestinal herniation, then chest CT or other modalities are necessary. Operative repair is desirable when there is the risk of strangulation of the intestine. The trans-thoracic or trans-abdominal approaches are possible to treat this malady. We report here on one case for which we successfully used a laparoscopic approach to treat this problem.

  • Purpose : To evaluate the relative diagnostic accuracy of panoramic radiography and Water's projection in maxillary sinus inflammatory diseases by comparing the radiodensities of the images with those of CT. Materials and Methods : Panoramic radiographs, Waters' projection, and CT images from 55 subjects (11O sinuses) were included in this retrospective study. The radiodensity of each maxillary sinus in panoramic radiography was recorded separately as upper and lower divided horizontally by hard palate. In Waters' projection, the overall sinus radiodensity was recorded. The CT images were considered as gold standard. Results : In panoramic radiography, 83 sinuses had same upper and lower radiodensity and 72 of these were consistent with those of CT, 26 sinuses had different upper and lower radiodensity and 15 of these, upper radiodensity was consistent with CT, the remaining 11, lower radiodensity was consistent with CT. One sinus had upper radiolucency with lower radiopacity and both were consistent with those of CT. Altogether 73 (66.4%) among 110 sinuses in panoramic radiography showed full agreement with CT, 26 (23.6%) showed partial agreement with CT. 9 sinuses had no lower image under the hard palate in panoramic radiography due to the smaller size of sinus. In Waters' projection, the radiodensity of 105 sinuses (95.5%) were consistent with that of CT. Conclusion : The panoramic radiography showed 90.0% of the sinus conditions fully or partially which may appear less accurate than that of Water's view (95.5%) but with more detailed information of the inferior part of sinuses. • A Korean domestic short hair (1-year-old, male) presented with 2 to 3 weeks of seizures, aggressive behavior, vomiting, anorexia, and lethargy. The frequency of seizure had gradually increased from once a week to once every 3 hours. Physical and neurologic examination, diagnostic screening tests, including complete blood count (CBC), serum chemistry, electrolyte, coagulation test, X-ray, ultrasonography, and urinalysis were performed. Feline Leukemia Virus (FeLV), Feline Immunodeficiency Virus (FIV) and Toxoplasma spp. All tested negative, but the Feline Corona Virus (FCoV) kit revealed a positive result. To determine the exact diagnosis, magnetic resonance imaging (MRI) was performed but yielded no specific findings. The patient was then diagnosed with idiopathic epilepsy and treatment of phenobarbital was initiated. A month's treatment with phenobarbital proved ineffective as symptoms worsened. Zonisamide was then selected as an additional anticonvulsant. After adding zonisamide, symptoms improved, and seizures abated for 15 months. This is the first case report in South Korea describing the use of phenobarbital and zonisamide in the treatment of a cat with idiopathic epilepsy.

  • This study is aimed at proposing the national policy for medical device control system in Korea as deeply analysed the present status of the system. On 1979, Korean government had started to inspect 24 kinds of the medical devices such as X-ray diagnostic equipments, medical sterilizer and etc. mostly imported from advanced countries which USA, Germany and Japan for the first time according to the Pharmaceutical Affairs Law. However medical devices were becoming consideration as an important partner of diagnosis, curing and alleviation of diseases by medical doctors and also much important keeping the health at home. Furthermore medical devices industry can be designated as a national growth engine industry. So it is necessary not only to harmonize to international standards but also the harmonization of the system including pre-market application and post-market surveillance, is strongly needed by the Korea Food and Drugs Administration(KFDA).
