전산화단층촬영장치에서 환자피폭선량 (Patient Radiation Exposure Dose in Computed Tomography)

  • 전산화단층촬영장치(computed tomography, CT)을 이용한 검사의 경우 방사선량의 분포가 일반 X선 장치와 차이가 있고 검사로 인한 방사선 피폭이 많은 것으로 알려져 있다. 그러나 검사 결과의 정확성에 대한 신뢰도가 높아 그 이용도는 계속적으로 증가하고 있다. 이와 같은 상황을 고려하여 기존에 발표된 자료를 바탕으로 CT장치, CT검사에서 방사선량, 진단참고준위 그리고 검사 시 방사선량 감소 방안에 대해 알아보았다.

외상성 횡경막 손상 -22례 보고- (Traumatic Diaphragmatic Injuries (Report of 22 Cases))

  • 22 Cases of traumatic diaphragmatic injuries treated at the Department of Thoracic and Cardiovascular Surgery in Chon-Buk National University Hospital from Jan. 1979 to Oct. 1988 were reviewed in this study. Of the 22 cases, 18 were male and 3 were female, a ratio of 4.5:1. This ratio revealed high incidence in male patient. The age distribution ranged from 2 to 60 years and mean age was 31 years. The modes of injury were as follows: 11 stab wound, 5 traffic accident, 2 fall down, 2 fighting injury, 1 compression wound by sand bag, and 1 slip down injury. Useful diagnostic tools were chest X-ray with or without radiopaque dye swallowing, which was the most commonly diagnostic, UGI series, and thoracoscope. Operations were performed in 22 cases, and 18 cases were through thoracotomy. The herniated organs through the ruptured diaphragm were stomach, omentum, liver, spleen, colon, and small bowel. There were associated injuries, and the most commonly associated was rib fracture. There was no postoperative death.

  • PET/CT is a new imaging technology that combines high-quality Positron Emission Tomography (PET) and Computed Tomography (CT). This imaging provides simultaneous anatomical and metabolic information. Therefore PET/CT is useful diagnostic modality for early detection of malignant tumor, accurate staging, decision on therapeutic plan, monitoring response to therapy and rapid detection of recurrence. We report oral and maxillofacial cancers diagnosed by using PET/CT and the usefulness of PET/CT in the evaluation of postoperative recurrence.

  • Many system devices for fluoroscopic and general X-ray studies in diagnostic radiographic system have been being changed from analog mode to digital mode. In addition, among diagnostic imaging and radiologic examinations, fluoroscopic studies that requires functional diagnosis is being widely used. The video recording method of fluoroscopic studies has been useful in functional image diagnosis and dynamic image observation, but the utility of its image quality is being reduced because of limitation in setting play segments of the video player, inconvenience of play, difficulties in preserving reproduced images, the change of image quality, etc. In order to complement these shortages, it is necessary to facilitate access to patient diagnosis information such as storing, editing and sharing functional diagnosis images in response to the trend of the digitalization of digital radiographic & fluoroscopic system(DRF). Thus this study designed and implemented a device of storing functional dynamic images real time using a computer rather than existing video recording, aiming at contribution to functional image diagnosis.

기관협착증에서 3차원적 영상 진단의 의의 (The Significance of 3-Dimensional Imaging in Tracheal Stenosis)

  • Three-dimensional reconstruction of computed tomographic image(3D CT) is a well-established imaging modality which has been investigated in various clinical settings. It is commonly performed in case of congenital or developmental abnormalities, and traumatic fracture of skull and face that requires reconstruction of osseous structure. However reporting the 3D CT in laryngeal or tracheal stenosis is rare and its results are obscure. The authors performed 3D CT in six cases of tracheal stenosis and found diagnostic value of 3D CT. A Comparision of diagnostic information obtained from plain X-ray, 2D CT and 3D CT has performed in total six cases of tracheal stenosis. Surgical treatment of the tracheal stenosis was following in these cases : tracheal end to end anastomosis In 1 case, laryngotracheal end to end anastomosis in 2 cases. 3D CT information was compared with operative finding. In two of six cases, satisfactory information was not obtained from 3D CT in evaluating an exact stenosis of trachea. Future, it will be helped in evaluating of tracheal stenosis by 3D CT.

  • 선천성 기관지 폐쇄증은 단순촬영에서 폐 결절로 보여 악성 종양으로 오인될 수 있다. 그러나 흉부단순촬영과 특히 전산화단층촬영에서는 늘어난 기관지 내의 점액고착, 구역 과팽창 등이 특징적으로 보여 더 이상의 침습적 검사 없이 선천성 기관지 폐쇄증으로 진단할 수 있다.

  • A 13-year-old male was diagnosed with a glenoid fracture following direct shoulder trauma, for which surgical treatment was considered. After referral to a center for pediatric orthopedic care, physical examination, contralateral shoulder X-ray, and detailed computed tomography examination ruled out the presence of fracture; these findings were later confirmed by magnetic resonance imaging. Normal ossification patterns in the adolescent shoulder may simulate a fracture in traumatic settings. To accurately diagnose and manage pediatric shoulder pathology, orthopedic surgeons must be aware of the normal anatomy of the growing shoulder, its secondary ossification centers, and growth plates.

수도권지역 일반촬영 장비의 정도관리 분석 (Analyze for the Quality Control of General X-ray Systems in Capital region)

  • 방사선 의료기술의 발달로 의료 방사선 영역에서 환자와 종사자의 피폭선량을 최소로 유지하고 많은 정보가 포함된 영상을 제공하고 있다. 그러나 장치의 성능 저하는 선량의 증가와 화질의 저하로 환자 및 의료진에게 손실을 줄 수도 있다. 따라서 영상의 질을 저하시키는 원인을 분석하고 X선 발생장치의 부적합을 점검하여야 하며, 장비의 고장 가능성을 예견하고 방지하기 위해서는 정기적인 정도관리가 반드시 필요하다. 본 연구는 수도권에 소재한 의원, 교육기관 그리고 종합병원을 대상으로 진단방사선안전관리규정의 일반방사선장비 검사기준을 준용하여 관전압시험, mAs시험, 광조아야 오차시험, 조사선량 재현성시험, 반가층시험, 조사시간 재현성 시험을 시행하였다. 개인 및 교육기관의 경우 일반촬영장비 22대를 대상으로 시험한 결과 관전압시험에서 18.2%, 조사선량 재현성시험에서 13.6%, mAs시험에서 9.1% 그리고 반가층 시험에서 13.6%가 부적합으로 측정되었다. 종합병원의 경우 28대의 장비에서는 조사선량재현성시험에서 7.1%, 광조사야 오차시험에서 7.1%, 조사시간 재현성시험에서 7.1% 부적합이 나타났다. 종합병원의 경우 정도관리의 중요성을 인식하고 장비업체의 예방점검 체결 및 원내에 정도관리 팀을 구성하여 적극적인 관리가 이루어지고 있어 대부분의 시험에서 기준을 충족하였으나 개인의원에서는 상대적으로 관리가 미흡한 실정이다.

진단방사선촬영에서 광자극형광선량계의 에너지의존성에 대한 보정인자 (Correction Factor for the Eenergy Dependence of a Optically Stimulated Luminescent Dosimeter in Diagnostic Radiography)

  • 이 연구의 목적은 진단방사선촬영에서 환자의 피부선량을 측정하는 나노도트선량계의 에너지의존성에 대한 보정인자들을 구하는 것이다. 보정인자들은 랜다우어사에서 제공한 팬텀 정에 관한 X-선에 상대적인 선량계의 에너지반응그래프와 로사도 등이 발표한 IEC의 RQR 표준방사선 품질들에 대한 평균에너지 값들을 사용하여 구하였다. 결과들은 관전압 40-150 kVp에서 1-1.33의 보정인자들을 나타냈다. 얻어진 보정인자들은 각 관전압에서 정확한 피부선량 측정을 위하여 임상에 사용하는데 유용할 것으로 생각된다.

Measurement of Depth Dose Distribution Using Plastic Scintillator

  • We examined a possibility to use inorganic plastic scintillator, which has the effective atomic number close to that of human soft tissue, for the measurement of dose distributions in a shorter time period. The method was to irradiate a block of plastic scintillator as a phantom, and to measure the distribution of the scintillation light by a wave length analyzer through a thread of plastic optical fiber. By irradiating the diagnostic x-ray, we observed the emission spectrum of the scintillation light from the scintillator. It showed a peak at around 420nm with a full width of 140 nm. The emission spectrum was integrated to determine the total number of photons. The dependences of the amount of photons on the irradiated dose were measured. The results of the experiment show that the amount of emission light is in proportional to the irradiated dose. From this fact, we conclude that the present method can be used for the measurement of the depth dose distribution of the diagnostic x-rays.
