Comparative Study of Target Genes and Protocols by Country for Detection of SARS-CoV-2 based on Polymerase Chain Reaction (PCR) (중합효소 연쇄반응 기반의 코로나-19 바이러스 검출법에 대한 국가별 목표 유전자 및 프로토콜 비교 연구)

  • Corona-19, a disease caused by 'Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2)', was declared a global pandemic by the World Health Organization (WHO) in March 2020, and a real-time polymerase chain reaction test is performed as a diagnostic test for screening and confirmation in most countries. However, not only the target genes and protocols differ by countries, but also the procedures for reading the diagnosis results are diverse, so the criteria for confirmed patients differ by country. Therefore, in this review, we discussed the target genes, test techniques, and diagnostic criteria for each country notified by WHO. And the specificity and sensitivity, limits of detection, positive and negative controls, false positive bacteria candidates, and specimens, and the specifics of the control setting were also described. In addition, the characteristics of Korea's test were compared to each country's one. Finally, in order to obtain the same diagnosis result for SARS-CoV-2 in the future, standardized diagnosis methods and result interpretations for Corona-19 diagnosis were proposed.

Therapeutic lumbar facet joint nerve blocks in the treatment of chronic low back pain: cost utility analysis based on a randomized controlled trial

  • Background: Related to escalating health care costs and the questionable effectiveness of multiple interventions including lumbar facet joint interventions, cost effectiveness or cost utility analysis has become the cornerstone of evidence-based medicine influencing coverage decisions. Methods: Cost utility of therapeutic lumbar facet joint nerve blocks in managing chronic low back pain was performed utilizing data from a randomized, double-blind, controlled trial with a 2-year follow-up, with direct payment data from 2016. Based on the data from surgical interventions, utilizing the lowest proportion of direct procedural costs of 60%, total cost utility per quality adjusted life year (QALY) was determined by multiplying the derived direct cost at 1.67. Results: Patients in this trial on average received $5.6{\pm}2.6$ procedures over a period of 2 years, with average relief over a period of 2 years of $82.8{\pm}29.6$ weeks with $19{\pm}18.77$ weeks of improvement per procedure. Procedural cost for one-year improvement in quality of life showed USD $2,654.08. Estimated total costs, including indirect costs and drugs with multiplication of direct costs at 1.67, showed a cost of USD $4,432 per QALY. Conclusions: The analysis of therapeutic lumbar facet joint nerve blocks in the treatment of chronic low back pain shows clinical effectiveness and cost utility at USD $2,654.08 for the direct costs of the procedures, and USD $4,432 for the estimated overall cost per one year of QALY, in chronic persistent low back pain non-responsive to conservative management.

  • When applying web hacking techniques and methods it needs to configure the integrated step-by-step and run an information security. Web hackings rely upon only one way to respond to any security holes that can cause a lot. In this study the diagnostic process of cross-site scripting attacks and web hacking response procedures are designed. Response system is a framework for configuring and running a step-by-step information security. Step response model of the structure of the system design phase, measures, operational step, the steps in the method used. It is designed to secure efficiency of design phase of the system development life cycle, and combines the way in secure coding. In the use user's step, the security implementation tasks to organize the details. The methodology to be applied to the practice field if necessary, a comprehensive approach in the field can be used as a model methodology.

  • Cervical cancer is an issue of foremost importance globally, specifically affecting the developing nations. Significant advances have taken place with regard to diagnosis of cervical cancer, especially with screening. Appropriate screening measures can thus reduce the incidence of cervical cancer. The most desirable screening technique should be less invasive, easy to perform, cost-effective and cover a wide range of diagnostic icons. Manual liquid based cytology (MLBC) can be considered as one of the suitable technique for screening with the above-mentioned benefits. The aim of the current study was to compare two cervical screening techniques on the basis of different morphological parameters and staining parameters by using modified acetic acid Pap staining to see the possibility of reducing time economy involved in conventional Pap staining (CPS). The study was conducted on a total 88 cases and all were analyzed with both MLBC and CPS. Forty eight cases that were regarded as satisfactory on the basis of Bethesda system by both methods were further recruited for investigation. Their morphological parameters and staining quality were compared and scored according to a scoring system defined in the study. Quality indices was calculated for both staining procedures and smear techniques.

Surgical Evaluation of Iatrogenic Hypopharyngo-esophageal Perforation (의인성 하인두-식도천공에 대한 외과적 고찰)

  • Background : Esophageal perforation due to a traumatic endoscopy or intubation is exceedingly rare. If riot noticed immediately or treated promptly, however, the morbidity and mortality is significant. We performed a retrospective review of patients with iatrogenic hypopharyngo-esophageal perforation to assess the outcome of current management techniques. Material and Methods : We retrospectively analyzed all cases iatrogenic hypopharyngo-esophageal perforation diagnosed at our hospital from January, 1999, through April, 2004. The study group consisted of 11 patients (4 men) with a mean age of 47.6 years (range, 21-83 yr). We reviewed the 11 patients with perforated injuries of the hypopharynx or esophagus during the diagnostic or therapeutic procedures. Result: Perforations were due to diagnostic gastroscopy ($54.5\%$, 6/11), esophageal dilation ($27.3\%$, 3/11), endoscopic port insertion ($9.1\%$, l/11), and tracheal intrathoracic ($9.1\%$, 1/11). Seven patients had intrathoracic and 4 had cervical perforations. Treatment included incision and drainage (5), resection and reconstruction (4), drainage only (1), and observation (2). Nonfatal complications included transient pneumonia (1), and wound infection (1). They occurred in advanced mediastinal abscess ]patients. Mortality was $9.1\%$ (1/11) in old patient who managed medically in cervical esophageal perforation. Conclusions : Current mortality rates in iatrogenic esophageal perforation were improved compared to previous published rates of $19\%\;to\;66\%$ for all patients with this condition. We concluded that aggressive and definitive surgery for thoracic esophageal perforations improving the survival rate, whether diagnosed early or late.

  • The recently enacted detailed guidelines on the performance evaluation of track facilities presented the necessary requirements regarding the evaluation procedures and implementation methods of track performance evaluation. However, the grade of rail surface damage is determined by external inspection (visual inspection), and there is no choice but to rely only on qualitative evaluation based on the subjective judgment of the inspector. Therefore, in this study, we attempted to develop a diagnostic application that can diagnose rail internal defects using rail surface damage. In the field investigation, rail surface damage was investigated and patterns were analyzed. Additionally, in the indoor test, SEM testing was used to construct image data of rail internal damage, and crack length, depth, and angle were quantified. In this study, a deep learning model (Fast R-CNN) using image data constructed from field surveys and indoor tests was applied to the application. A rail surface damage diagnosis application (App) using a deep learning model that can be used on smart devices was developed. We developed a smart diagnosis system for rail surface damage that can be used in future track diagnosis and performance evaluation work.

  • Barrett's esophagus (BE) is the precursor to esophageal adenocarcinoma (EAC), and is caused by chronic gastroesophageal reflux. BE can progress over time from metaplasia to dysplasia, and eventually to EAC. EAC is associated with a poor prognosis, often due to advanced disease at the time of diagnosis. However, if BE is diagnosed early, pharmacologic and endoscopic treatments can prevent progression to EAC. The current standard of care for BE surveillance utilizes the Seattle protocol. Unfortunately, a sizable proportion of early EAC and BE-related high-grade dysplasia (HGD) are missed due to poor adherence to the Seattle protocol and sampling errors. New modalities using artificial intelligence (AI) have been proposed to improve the detection of early EAC and BE-related HGD. This review will focus on AI technology and its application to various endoscopic modalities such as high-definition white light endoscopy, narrow-band imaging, and volumetric laser endomicroscopy.

Design and Development of White-box e-Learning Contents for Science-Engineering Majors using Mathematica (이공계 대학생을 위한 Mathematica 기반의 화이트박스 이러닝 콘텐츠 설계 및 개발)

  • This paper deals with how to design and develop white-box based e-learning contents which are equipped with conceptual understanding and step-by-step computational procedures for studying vector calculus for science-engineering majors who might need supplementary mathematics learning. Noting that rewriting rules are often used in school mathematics for students' problem solving, the theoretical aspects of rewriting rules are reviewed for developing supplementary e-learning contents for them. The software design of step-by-step problem solving requires careful arrangement of rewriting rules and pattern matching techniques for white-box procedures using a computer algebra system such as Mathematica. Several modules for step-by-step problem solving as well as producing dynamic display of e-learning contents was coded by Mathematica in order to find the length of a curve in vector calculus after implementing several rules for differentiation and integration. The developed contents are equipped with diagnostic modules and immediate feedback for supplementary learning in terms of a tutorial. At the end, this paper indicates the strengths and features of the developed contents for college students who need to increase math learning capabilities, and suggests future research directions.

Clinical Results of the Limb Salvage Procedure in Bone Tumor (골종양 치료에 있어서 사지 구제술식의 임상적 결과)

  • During the last decade, the use of primary amputation has decreased, and a larger number of patients are being treated with resection and reconstruction with limb preservation. The advantage of chemotherapy, improvements in diagnostic imaging methods and newer techniques for reconstruction have made limb salvage an alternative to amputation for most patients with malignant bone tumor. Clinical results and functional results of the limb salvage operation of bone tumors treated at the Department of Orthopaedic Surgery, Guro Hospital, Korea university was reviewed. Eighteen bone tumors(seven giant cell tumors and eleven malignant bone tumors.) were studied over a period of five years. The limb salvage group included fourteen endoprothesis, two pasteurized autograft, one near total scapulectomy and one rotationplasty. Functional evaluation was performed according to the Enneking's modified system. The results were as followed; 1. At a mean follow-up of thirty two months (15-77 months), fourteen(83%) of the eighteen patients with limb salvage procedures had been continuously disease free. 2. There are no local recurrence but deep infection developed in two patients and three patients with distant metastasis. 3. Thirteen(87%) of the fifteen patients showed above 60% of functional results. 4. Fourteen(93%) of the fifteen patients were satisfied with the limb salvage procedures.

  • Successful treatment of any pathological condition of the shoulder requires an accurate diagnosis. Physical examination represents an integral part of the assessment of shoulder disorders. Pain, loss of strength, decreased range of motion, and instability are the most common symptoms caused by a great variety of different shoulder pathologies. Therefore, a sophisticated clinical approach to the patient with shoulder pathology is essential. A standardized evaluation including a variety of diagnostic tests and clinical assessments improves the diagnostic accuracy by specifically examining one component of the shoulder complex. In most cases a careful physical examination will establish or suggest a diagnosis and special investigative techniques such as imaging procedures can be applied more selectively for confirmation or further evaluation of the pathological entity.
