Tumor-associated autoantibodies as diagnostic and prognostic biomarkers

  • In the process of tumorigenesis, normal cells are remodeled to cancer cells and protein expression patterns are changed to those of tumor cells. A newly formed tumor microenvironment elicits the immune system and, as a result, a humoral immune response takes place. Although the tumor antigens are undetectable in sera at the early stage of tumorigenesis, the nature of an antibody amplification response to antigens makes tumor-associated autoantibodies as promising early biomarkers in cancer diagnosis. Moreover, the recent development of proteomic techniques that make neo-epitopes of tumor-associated autoantigens discovered concomitantly has opened a new area of 'immuno-proteomics', which presents tumor-associated autoantibody signatures and confers information to redefine the process of tumorigenesis. In this article, the strategies recently used to identify and validate serum autoantibodies are outlined and tumor-associated antigens suggested until now as diagnostic/prognostic biomarkers in various tumor types are reviewed. Also, the meaning of autoantibody signatures and their clinical utility in personalized medicine are discussed.

Application of Vocal Properties and Vocal Independent Features to Classifying Sasang Constitution (음성 특성 및 음성 독립 변수의 사상체질 분류로의 적용 방법)

  • 1. Objectives Vocal characteristics are commonly considered as an important factor in determining the Sasang constitution and the health condition. We have tried to find out the classification procedure to distinguish the constitution objectively and quantitatively by analyzing the characteristics of subject's voice without noise and error. 2. Methods In this study, we extract the vocal features from voice selected with prior information, remove outliers, minimize the correlated features, correct the features with normalization according to gender and age, and make the discriminant functions that are adaptive to gender and age from the features for improving diagnostic accuracy. 3. Results and Conclusions Finally, the discriminant functions produced about 45% accuracy to classify the constitution for every age interval and every gender, and the diagnostic accuracy was meaningful as the result from only the voice.

The Reduction of Question Items in Korea Sasang Constitutional Diagnostic Questionnaire (KS-15) (단축형 사상체질 진단설문지(KS-15)의 문항 축소 연구)

  • Objectives The purpose of this study is to reduce the question items in Korea Sasang Constitutional Diagnostic Questionnaire (KS-15) and to assess the validity. Methods The subjects of this study were 1,553 people who had information on body shape, personality, and symptoms among the data established at the Korean Medicine Data Center. Among them, 155 people were separated into a test group to assess the validity, and the remaining 1,398 were used as training groups for model development. The reduction question items were finally confirmed through discussion by two experts. The model development went through Multinomial Logistic Regression. Results & Conclusions The Percent Correctly Predicted (PCP) in the test group was 60.00% (58.49% in men and 60.78% in women). The PCP of each constitution was 72.6% in Taeeumin, 53.3% in Soeumin, and 50% in Soyangin. The accuracy rate of KS-10 seems to be reasonable considering the ease of use of reduced time required.

IOTA Simple Rules in Differentiating between Benign and Malignant Ovarian Tumors

  • Background: To evaluate the diagnostic performance of IOTA simple rules in differentiating between benign and malignant ovarian tumors. Materials and Methods: A study of diagnostic performance was conducted on women scheduled for elective surgery due to ovarian masses between March 2007 and March 2012. All patients underwent ultrasound examination for IOTA simple rules within 24 hours of surgery. All examinations were performed by the authors, who had no any clinical information of the patients, to differentiate between benign and malignant adnexal masses using IOTA simple rules. Gold standard diagnosis was based on pathological or operative findings. Results: A total of 398 adnexal masses, in 376 women, were available for analysis. Of them, the IOTA simple rules could be applied in 319 (80.1%) including 212 (66.5%) benign tumors and 107 (33.6%) malignant tumors. The simple rules yielded inconclusive results in 79 (19.9%) masses. In the 319 masses for which the IOTA simple rules could be applied, sensitivity was 82.9% and specificity 95.3%. Conclusions: The IOTA simple rules have high diagnostic performance in differentiating between benign and malignant adnexal masses. Nevertheless, inconclusive results are relatively common.

The Technological Concept of Fibroscan and its Clinical Usefulness in the Traditional Korean Medical Field (비침습적 간섬유화 측정기(Fibroscan)의 기술적 개요와 한의임상적 유용성 고찰)

  • Objectives: The liver fibroscan has been developed as a noninvasive and convenient method to assess hepatic fibrosis. This study aimed to review this device in terms of its technological concept and clinical application in traditional Korean medicine (TKM). Methods: The technological background, diagnostic evaluation, and clinical usefulness of fibroscan were reviewed using various literature and clinical studies. One clinical example-a patient with hepatofibrosis who had been treated with TKM-is presented. Results: The liver fibroscan was approved as a medical device for noninvasive measurement of the hepatic fibrosis level by the Korean Food and Drug Administration (KFDA). Numerous clinical studies have confirmed that its sensitivity and specificity allow it to serve as a substitute for liver biopsy, the present gold standard diagnostic method. The accuracy and reproducibility and lack of technical risks are strong points of the fibroscan; however, it has some limitations for application, especially in patients with obesity or severe hepatitis. One clinical example showed the applicability of the liver fibroscan in herbal medicine-based treatments. Conclusions: The requirement for diagnostic medical devices is an important issue in TKM; therefore, this study provides valuable information for practitioners of TKM.

Diagnostic Imaging of Massive Hepatocellular Carcinoma in 5 Dogs (개의 종괴형 간세포암종에 대한 진단 영상 소견에 대한 고찰)

  • Hepatocellular carcinoma (HCC) is a primary hepatic neoplasms in dogs. Three types of HCC such as massive, nodular and diffuse form were reported. Massive HCC is most common and has relatively better prognosis than other forms because this type of HCC can be removed surgically and has low frequency of relapse or metastasis. Diagnostic image can provide useful information for shape and internal structure of the hepatic mass as well as the location and adhesion or invasiveness of the mass to establish surgical plan to remove the mass safely. In this study, we investigated diagnostic features of massive HCC in 5 dogs. Radiography showed soft tissue mass in cranial abdomen in 3 dogs. On ultrasonography, all dogs had a solitary hepatic mass with mixed echo pattern with anechoic cysts, which represented necrosis and hemorrhage. The radiographic and ultrasonographic findings of affected hepatic lobe were compared with the macroscopic findings through laparotomy. Computed tomography was performed to check metastasis and figure out the origin of the mass in two dogs. Ultrasonography is useful and relatively sensitive examination for diagnosis and planning for surgery in canine massive HCC.

  • We discovered the fact that the most important factor for judging adults' internet addiction in the K-scale method which was developed by Korea National Information Society Agency (NIA), has composed of 4 categories including 20 items, is tolerance and preoccupation factor from the experiments by using data mining techniques. In this research, we propose a new internet addiction diagnostic method based on the degree of tolerance considering users' non-duty internet activities. From some questionnaire participants, their feedbacks for the K-scale and the proposed diagnostic method were collected, and then we confirmed that the proposed user-centered diagnostic method is effective to find undiscovered addicts due to individuals's intention in the K-scale.

  • This paper presents an application of power system fault diagnostic algorithm for the PEFP Proton Accelerator Research Center using neural network. Proposed fault diagnostic system is constructed by the radial basis function (RBF) neural network because it has the capabilities of the pattern classification and function approximation of any nonlinear function. Proposed system identifies faulted section in the power system based on information about the operation of protection devices such as relays and circuit breakers. In this paper, parameters of the RBF neural networks are tuned by the GA-TS algorithm, which has the global optimal solution searching capabilities. To show the validity of the proposed method, proposed algorithm has been tested with a practical power system in Proton Accelerator Research Center of PEFP.

Measurement of Apnea Using a Polyvinylidene Fluoride Sensor Inserted in the Pillow (베게에 삽입된 PVDF센서를 이용한 무호흡증 측정)

  • Most sleep apnea patients exhibit severe snoring, and long-lasting sleep apnea may cause insomnia, hypertension, cardiovascular diseases, stroke, and other diseases. Although polysomnography is the typical sleep diagnostic method to accurately diagnose sleep apnea by measuring a variety of bio-signals that occur during sleep, it is inconvenient as the patient has to sleep with attached electrodes at the hospital for the diagnosis. In this study, a diagnostic pillow is designed to measure respiration, heart rate, and snoring during sleep, using only one polyvinylidene fluoride (PVDF) sensor. A PVDF sensor with piezoelectric properties was inserted into a specially made instrument to extract accurate signals regardless of the posture during sleep. Wavelet analysis was used to identify the extractability and frequency domain signals of respiration, heart rate, and snoring from the signals generated by the PVDF sensor. In particular, to separate the respiratory signal in the 0.2~0.5 Hz frequency region, wavelet analysis was performed after removing 1~2 Hz frequency components. In addition, signals for respiration, heart rate, and snoring were separated from the PVDF sensor signal through a Butterworth filter and median filter based on the information obtained from the wavelet analysis. Moreover, the possibility of measuring sleep apnea from these separated signals was confirmed. To verify the usefulness of this study, data obtained during sleeping was used.

Analysis of management status of chestnut cultivation in Chungcheongnam-do

  • In this study, we attempted to estimate the degree of management of chestnut forestry households in Chungcheongnam-do and to provide information for establishing chestnut cultivation-related policies. The chestnut management standard diagnostic table consists of three major categories, namely, management base, management and sales capacity, and production technology levels, along with 19 subcategories. A survey of 309 chestnut forestry households was conducted from 2014 to 2019 in Gongju, Cheongyang, and Buyeo in Chungcheongnam-do. The average score for the 19 subcategories was 65.7 points, indicating that these areas have excellent management conditions. When the total score was higher, the output per hectare and the rate of top-grade products in the total output were also higher, indicating a significant correlation. These findings will be useful for providing consulting services to chestnut growers as they highlight the correlation between the higher scores of the indicators in the chestnut management standard diagnostic table and the management performance of the farmers. We found that the scores of the indicators for management and sale skill, such as management record and analysis, material purchase, and direct transaction with consumers, were relatively lower than those of the indicators for management base and production skill. It is assumed that the chestnut growers aging has led to negligence in recording details on incomes, expenditures, and work and lowered the willingness to make substantial profits. Therefore, it is essential to overcome these problems for profitable chestnut farming.
