Paradoxical Reactions following Intravenous Midazolam in Patients with Severe Anxiety for Dental Treatment -A Case Report- (치과 치료에 대한 심한 공포가 있는 환자들에서 미다졸람 정주 후 나타난 기이반응 -증례보고-)

  • Intravenous midazolam has been frequently used for the relief of anxiety in dental treatment. This is likely the result of the sedative and anterograde amnestic properties of midazolam that are mediated through $\gamma$-Aminobutyric acid agonism. Unfortunately, Paradoxical reactions to midazolam include agitation, talkativeness, confusion, disinhibition, aggression,violent behavior, act of self-injury and need for restraints. These occur in less than 1% of all patients receiving midazolam, may occur at variable times after administration and are difficult to predict and diagnose. Two women with severe anxiety for dental treatment experienced paradoxical reactions associated with the use of intravenous midazolam. We are reviewed the management and prevention of paradoxical reactions and its different etiology.

  • This paper presents the method for analyzing surface temperature of Electric incoming Apparatus. For the experiment, the surface temperature of electric power apparatus was measured and analyzed by using a infrared thermal imaging system. Surface Discharges(SD) have very complex characteristics of discharge patterns, therefore it requires the development of precise analysis methods. recently, studies on infrared thermal imaging system are carried out to analyze temperature distribution of power equipments through condition diagnosis and to diagnose the degradation of power equipments. The changes in suface temperature was measured by using the infrared thermal imaging system under hot line condition. The system was set up based on the diagnostic method of the electric incoming apparatus.

  • Detection methods for atrial tachycardia and fibrillation on the time axis have the advantages of light operational load and are easy to apply to various applications. Despite these advantages, arrhythmia detection algorithm on the time axis cannot stand much noise such as motion artifacts, moreover the peak detection algorithm has high complexity. In this paper, we use a spectrum analysis method for the detection of atrial tachycardia and fibrillation. By applying spectrum analysis and digital filtering on obtained electrogram signals, we can diagnose heart arrhythmia without using peak detection algorithm.

  • A purpose of this paper is to recognize partial discharge pattern for cable insulation. The classification of PD sources was widely studied for two decades. this research sought to use the partial discharge detection method, and to diagnose the interface of cable, which is deemed vulnerable of cable systems. A research abalyzed faults that can occur in the interface of cable joint as well as accident mechanisms, manufactured test 22.9kV CNC cable, invented artificial faults and carried out partial discharge detection experiments. As a result, various PD pattern along defect measured and distinguished.

  • This paper introduces an comprehensive monitoring and management program for implementation of a real-time monitoring system that monitors condition of urban railway AC/DC transformers, disconnecting switches, circuit breakers, regulators, and GIS (Gas Insulated Switchgear). Especially, the system is applied to diagnose the overall condition of urban railway substations by sending acquired data through an OPC server to a database, effectively storing and monitoring conditions simultaneously. The above system is a management based system and is also applicable to small-scale systems.

  • Recently, CV, CNCV, CNCV-W cable are used to transmit and distribute electric power. And a lot of researchers put more effort to realize high performance. The dielectric breakdown strength characteristic is a standard to design insulators. Examination of that is a main factor to determine long term insulation performance, which is used to diagnose Insulation deterioration. In this paper, we prepared XLPE, XLPE/nano-filler, LDPE/nano-filler for comparing each of the dielectric breakdown strength characteristics.

  • In this thesis, the silicone filler with a sample size of 0~75 phr and void size of 2~4.5 mm is prepared in order to diagnose the defect of void which exists in widely used insulation material, silicone rubber. In this silicone rubber sample, electrodes are connected and whilst the voltage changes, applied voltage 7 kV~9 kV is increased constantly over time and discharge quantity, discharge frequency and applied voltage (T-QNV) were measured. The discharge quantity of the applied voltage (VQ) is measured to estimate inception voltage and extinction voltage. In addition, under the condition of maintaining constant applied voltage, discharge quantity and discharge frequency (QN) are measured, and its characteristics are analyzed.

  • In this paper, a method to detect and diagnose faults in Automated Manufacturing Systems(AMS) is proposed. In AMS, it is necessary to monitor the process-status. The detection and diagnosis of faults are often difficult in monitoring level with given passive data. We propose the model-based monitoring system for faults detection and diagnosis using Petri Nets to model AMS efficiently and easily. Simulation results show the validity of proposed method with example of Reverse Mill Process in Automated Mill Lines.

  • It has been proven that the dissolved gas analysis (DGA) is the most effective and convenient method to diagnose the transformers. The DGA is a simple, inexpensive, and non intrusive technique. Among the various diagnosis methods, IEC 60599 has been widely used in transformer in service. But this method cannot offer accurate diagnosis for all the faults. This paper proposes a fault diagnosis method of oil-filled power transformers using IEC code based neuro-fuzzy model. The proposed method proceeds two steps. First, IEC 60599 method is applied to diagnosis. If IEC code can't determine the fault type, neuro-fuzzy model is applied to effectively classify the fault type. To demonstrate the validity of the proposed method, experiment is performed and its results are illustrated.

  • A 74-year-old man suffering from cryptogenic organizing pneumonia (OP) visited our department with arthralgia accompanied with partial swellings of proximal interphalangeal and metacarpophalangeal joints with morning stiffness. A diagnose of rheumatoid arthritis (RA) was made. It was thought that OP was associated with RA. We initiated a treatment with salazosulfapyridine and loxoprofen for RA. Although this treatment was effective, it was discontinued due to the development of drug eruption. As an alternative, the patient was treated with prednisolone (PSL) and clarithromycin (CAM). This treatment demonstrated being effective for OP and RA, to a certain extent; however, the RA activity was not completely suppressed. In order to suppress the RA activity further, tacrolimus (TAC) was successfully added with increasing the dosage of CAM that is assumed to raise blood TAC concentrations. The present case shows that treatment with PSL, CAM and TAC may be effective in some cases of RA.
