  • This paper presents a new approach to diagnose and detect faults in oil-filled power transformers based on various dissolved gas analyses. A theoretic fuzzy information model is introduced, An inference scheme which yields the 'most' consistent conclusion proposed. A framework is established that allows various dissolved gas analyses to be combined in a systematic way such as the Dempster-Shafer rule. Good diagnosis accuracy is obtained with the proposed approach.

Two Cases of Gastric Volvulus in Neonates

  • Gastric volvulus in neonates is an extremely uncommon disorder, which is challenging to diagnose because of its non-specific clinical manifestations. Early diagnosis of gastric volvulus is important to avoid life-threatening complications, such as gastric ischemia, necrosis, and perforation. A definitive diagnosis could be made with radiological upper gastrointestinal series. In this report, we present two cases of neonate gastric volvulus, which were confirmed by radiological upper gastrointestinal series, and the patients underwent surgical treatment.

  • Transcranial magnetic stimulation (TMS) is a safe and noninvasive tool for investigating the cortical excitability of the human brain and the neurophysiological functions of GABAergic, glutamatergic, and cholinergic neural circuits. Neurophysiological biomarkers based on TMS parameters can provide information on the pathophysiology of dementia, and be used to diagnose Alzheimer's disease and differentiate different types of dementia. This review introduces the basic principles of TMS, TMS devices and stimulating paradigms, several neurophysiological measurements, and the clinical implications of TMS for Alzheimer's disease.

  • Carotid duplex ultrasound is commonly used to diagnose various carotid artery diseases due to it being noninvasive and easy to perform. Carotid atherosclerosis is a major indicator for the need to perform carotid duplex ultrasound, which can determine the degree of stenosis, plaque characteristics, and intima-media thickness. It can also be used to screen and follow-up after carotid revascularization. Here we discuss the standard techniques, interpretations, and clinical indicators for carotid duplex ultrasounds.

  • Rotator cuff disease is a common cause of shoulder pain for which partial-thickness rotator cuff tears occupy a significant proportion. Such tears are often difficult to diagnose and manage in the general clinic setting. A review of the available literature from well-known databases was performed in this study to provide a concise overview of partial-thickness rotator cuff tears to aid physicians in their understanding and management.

  • Computer-assisted polyp characterization (computer-aided diagnosis, CADx) facilitates optical diagnosis during colonoscopy. Several studies have demonstrated high sensitivity and specificity of CADx tools in identifying neoplastic changes in colorectal polyps. To implement CADx tools in colonoscopy, there is a need to confirm whether these tools satisfy the threshold levels that are required to introduce optical diagnosis strategies such as "diagnose-and-leave," "resect-and-discard" or "DISCARD-lite." In this article, we review the available data from prospective trials regarding the effect of multiple CADx tools and discuss whether they meet these thresholds.

  • Esophagogastroduodenoscopy (EGD) has been used to diagnose a wide variety of upper gastrointestinal diseases. In particular, EGD is used to screen high-risk subjects of gastric cancer. Quality control of EGD is important because the diagnostic rate is examiner-dependent. However, there is still no representative quality indicator that can be uniformly applied in EGD. There has been growing awareness of the importance of quality control in improving EGD performance. Therefore, we aimed to review the available and emerging quality indicators for diagnostic EGD.

Clinical Study on the Sasang Constitutional Pulse Using Array Piezoresistive Sensor (어레이 압저항 센서를 활용한 체질맥 임상연구)

  • 1. Objective Pulse diagnosis is generally applied to Traditional Oriental Medicine but not to Sasang Constitution diagnosis. Recently new pulse analyzer using array piezoresistive sensor and multi-channel robot arm developed. It reflects Oriental Medical Doctors' diagnostic processes, and its reproducibility test was done at Korea Institute of Oriental Medicine. We performed this study to set parameters diagnosing Sasang Constitution. 2. Methods One hundred thirty three subjects participated in this study. They are healty and approved this study. Before being tested with pulse analyzer, they had interview with Sasang Constitution Specialist to diagnose their Sasang Constitution. We established some useful parameters from parameters of pulse analyzer according to the Original Texts of Oriental Medicine and clinical experiences to analyze with clinical data of this study. 3. Results (I) There is a significant difference in pre-dicrotic notch time among all parameters of pulse analyzer in Sasang Constitution groups(P=0.047). (2) There is a significant difference in maximum pulse pressure in 33 to 48 year Sasang Constitution groups(P=0.010). (3) There is a significant difference in frequency width in 17 to 32 year Sasang Constitution groups(P=0.002). (4) There is a significant difference in CFS value in groups which OMD diagnoses; Floating & Sinking pulse(P=0.020). (5) There is a significant difference in pulse rate in groups which OMD diagnoses; Rapid & Slow pulse(P=0.000). (6) There is a significant difference in maximum pulse pressure in groups which OMD diagnoses; Deficient & Solid pulse(P=0.000). 4. Conclusions Analyzing parameters in each Sasang Constitution group, we found it shows significant difference in maximum pulse pressure and corresponding tendency in coefficient of floating & sinking pulse with theories of Sasang Consti-tutional Medicine. As we accumulate more clinical data, we will establish algorithm to diagnose Sasang Constitution using a pulse analyzer.

DIAGNOSTIC RELIABILITY OF THE DYNAMIC MRI FOR THE INTERNAL DERANGEMENT OF TEMPORO-MANDIBULAR JOINTS (악관절내장증의 진단에 있어 Dynamic MRI의 효용)

  • The Magnetic Resonance Imaging has been used widely to evaluate the disk position without any interruption of the TMJ structures, and the Dynamic MRI presenting computed serial imaging or the video-recorded simulation images is thought to be very effective to evaluate the disk position under function. This is to study the correlation between the clinical diagnosis and the findings of Dynamic MRI for the diagnosis of internal derangement of the temporomandibular joints. 30 joints(15 patients) were examined clinically, and the movement of TMJ meniscus was reviewed in the dynamic MRI. The comparative results are as follows : 1. All internal derangements of TMJ disk displacement without reduction were consistent with MRI findings. 2. 5 joints (50%) of disk displacements with reduction could not be confirmed by MRI findings. 3. The disk displacements in MRI were found in 55% of painful joints, 50% of clicking joints, and 70% of the joints with restricted movement. 4. The reliability of MRI for the diagnosis of TMD was evaluated as 77% ; 24 of 30 joints who presented with clinical diagnosis of TMD. 5. MRI is very reliable to diagnose the disk displacement without reduction, but it is rather not so effective to diagnose the early derangement or muscle disorders.

Development and Validation of a Situational Judgment Test for a Competencey of Public Ethics: A Case of an University in South Korea (대학생 상황판단형 공적윤리 역량 진단도구 개발과 타당화: S대학교를 중심으로)

  • This paper aims to develop and validate a situational judgement test to diagnose the Competencey of public ethics for undergraduates. A criterion group composing of high performers was formed. Critical incidents and responses were developed from interviews with members of the criterion group. Also an expert committee was composed of six university members who have professional knowledge and experience on Competencey based selection or education. This committee reviewed all the items developed and selected some of them for the final test. Item validity and reliability were examined through the final test that 519 college students took. The result of the confirmatory as well as exploratory factor analysis showed that the Competencey of public ethics consisted of 2 sub-elements which were work ethics and democratic citizenship. The correlation between results of the test and the HEXACO model was statistically significant. Overall, the findings of the study highlight that a situational judgement test can be developed and utilized to diagnose Competencey levels of public ethics for undergraduates.
