Effect of hydrogen-rich water on the lactic acid level in metformin-treated diabetic rats under hypoxia

  • The present study aims to investigate the impact of hydrogen-rich water on the lactic acid level in metformin-treated diabetic rats under hypoxia. Thirty Sprague-Dawley rats were randomly divided into five groups, including normal diet group, and diabetes model (DM) group, DM + metformin treatment (DMM) group, DMM + hypoxia treatment (DMMH) group and DMMH + hydrogen-rich water (DMMHR) group. We found that the levels of lactic acid, pyruvate and lactate dehydrogenase were significantly lower in the blood of DMMHR group than DMMH group. Superoxide dismutase and glutathione levels in liver and heart were significantly higher in DMMH group after hydrogen-rich water treatment, while malondialdehyde and oxidized glutathione levels were decreased in DMMHR group when compared with DMMH group, which indicates that hydrogen-rich water could reduce oxidative stress. qPCR analysis demonstrated that that pro-apoptotic genes Bax/Caspase-3 were upregulated in DM group and metformin treatment suppressed their upregulation (DMM group). However, hypoxic condition reversed the effect of metformin on apoptotic gene expression, and hydrogen-rich water showed little effect on these genes under hypoxia. HE staining showed that hydrogen-rich water prevented myocardial fiber damages under hypoxia. In summary, we conclude that hydrogen-rich water could prevent lactate accumulation and reduce oxidant stress in diabetic rat model to prevent hypoxia-induced damages. It could be served as a potential agent for diabetes patients with metformin treatment to prevent lactic acidosis and reduce myocardial damages under hypoxic conditions.

심장외막지방두께와 당뇨와의 연관성 연구 (The Relationship Between Epicardial Adipose Tissue and Diabetes Mellitus)

  • The incidence of cardiovascular disease increases rapidly after 40's. The thickness of the epicardial adipose tissue was measured to analyze the risk factors affecting the thickness change. We present the cut off value for the epicardial adipose tissue thickness for high impacted variables. For the study, 547 patients underwent echocardiography, epicardial adipose tissue thickness, body mass index, abdominal subcutaneous fat thickness and diabetes mellitus were used as analytical variables. As a result, multiple regression analysis of age group showed that diabetes mellitus was highly influential in all age group, so we could predict the cut off value for the epicardial adipose tissue thickness for diabetes mellitus. As a result of ROC curve analysis, cut off value of 7.44 mm was obtained with sensitivity of 79.6% and specificity of 60.6% for diab etes variab le. Logistic regression analysis b ased on this value showed that the risk for diab etes increased 6 times with each increase in the thickness of the epicardial adipose tissue. Among the various obesity indexes, epicardial adipose tissue is used as an important index especially to prevent the risk of cardiovascular disease. Therefore, we suggest that the use of echocardiography as a routine screening method for the prevention of cardiovascular disease and metabolic syndrome, which is rapidly increasing in patients over 40's age, will enable more effective monitoring.   • The purpose of this study was to reseach the effect of Daesihotang and its component groups on diabetes, free radical and antioxidative defense system in alloxan-induced diabetic rats. The experimental group was divided into three groups: Daesihotang (DS), Soyangyak (DS1), Yangmyungyak (DS2). The results were obtained as follows: The level of glucose, triglyceride, total cholesterol, creatinine in serum were considerablely reduced by Daesihotang. And the level of HDL cholesterol, albumin, total protein in serum were increased by Daesihotang significantly. And the level of BUN has some decreased, but with no significancy. In the study of Daesihotang on free radical scavenging effect in vitro, it has shown that Daesihotang and its components group have significant suppressing effect on peroxidation of linoleic acid on concentration. And in other experiments as scavenging effect of DPPH radical, inhibitory effect of superoxide in xanthine-xanthine oxidase system and inhibitory effect on lipid peroxidation reaction by hydroxy radical in $H_2O_2-Fe^{2+}$ system, Daesihatang have some activity, but with no significancy. In the study of Daesihotang on antioxidative defense system in vivo, the activity of GOT and GPT in serum were significantly increased; and the amounts of lipid peroxide in serum was reduced significantly but in liver no significancy. In hepatic catalase activity, Daesihotang has showed significant effect. In the level of hepatic glutathione, Daesihotang increased the amount of glutathione significantly. And in the activity of glutathione-s-transferase, Daesihotang has decreasing effect but has no significancy. These result suggest that Daesihotang has strong effect on diabetes and it is useful to prevent diabetes, but has less effect on peroxidative damage by free radical.

  • The purpose of this study was to reseach the effect of Daehwanggamchoeumja(大黃甘草飮子) and its component groups on diabetes, free radicals, and antioxidants system in Alloxan-induced diabetic rats. The experimental group was divided into three groups: Daehwanggamchoeumja(DG), and its components groups, Gamdutang (Gamcho&Daedu; DG-I) and Daehwanggamchotang(DG-2). The results were obtained as follows: 1. In the study of effect on diabetic metabolic dysfunction(Glucose, Triglyceride, Total Cholesterol, HDL Cholesterol, Total Protein, Albumin, Creatine, BUN), only DG has a significant effect. 2. In the study on free radical scavenging effect in vitro(the suppressing effect on peroxidation of linoleic acid on concentration, the scavenging effect of DPPH radical, inhibitory effect of superoxide in xanthine-xanthine oxidase system, inhibitory effect on lipid peroxidation reaction by hydroxy radical in $H_2O_2Fe^{2-}$system, and the effect on Nitrate reductase activity), DG and DG-2 have more effect than DG-l relatively. 3. In the study on antioxidants system in vivo(The level of serum LPO, The level of hepatic LPO, Catalase, GSH, GST), only DG has a significant effect. These results suggest that Daehwanggamchoeumja(大黃甘草飮子) has an effect on diabetes, peroxidative damage by free radical, so it seems to be useful to prevent and treat diabetes. The mechanisms of these are supposed to be involved in antioxidant and three drugs' cooperative synergy effect.

Transient Neonatal Diabetes Mellitus Managed with Continuous Subcutaneous Insulin Infusion (CSII) and Continuous Glucose Monitoring

  • Neonatal diabetes mellitus can be categorized as transient, permanent, or syndromic, and approximately half of the cases are transient. We present a case involving a term newborn who showed overt progression of transient neonatal diabetes mellitus, with complete remission within 6 months. On the second day of life, the patient presented with tachypnea, hyperglycemia, and decreased serum levels of C-peptide and insulin. Continuous subcutaneous infusion of insulin and continuous glucose monitoring were well tolerated. The patient showed a normal growth pattern, with no hyperglycemic or hypoglycemic episodes at 6 months of age. As it is rare and often asymptomatic, hyperglycemia may be attributed to various factors, including intrauterine environment, perinatal stress, and diverse genetic background. Therefore, consistent blood glucose monitoring and prompt early insulin therapy are crucial for any term newborns with persistent hyperglycemia, to prevent further diabetic complications. Moreover, continuous subcutaneous insulin infusion and the utilization of continuous glucose monitoring devices are the most effective and practical management strategies.

대사증후군 환자 가운데 당뇨환자를 찾기 위한 규칙 도출 (Deriving rules for identifying diabetic among individuals with metabolic syndrome)

  • 본 연구의 목적은 대사증후군이 당뇨병으로 확대되는 것을 방지하는데 이용할 수 있는 구체적인 분류 규칙을 도출하는 것이다. 좀 더 구체적으로 말하면, 대사증후군을 앓고 있는 사람들을 당뇨병이 없는 사람 (class 0)과 당뇨병이 있는 사람(class 1)으로 구별해 내는 분류하는 규칙을 찾는 것이다. 본 연구는 국민건강영양조사 데이터를 수집하여 데이터 전처리 과정들을 거친 후 의사결정나무를 구축하였다. 생성된 의사결정나무로부터 유용한 5개의 분류 규칙을 도출하였는데, 이들의 평균 분류 정확도는 75.8%이었다. 또한, 생성된 의사결정나무로부터 고혈압 여부와 허리둘레가 class 0 그룹과 class 1 그룹으로 분류하는데 있어서 중요한 요인임을 알 수 있었다. 이번 연구 결과는 의사들이 향후 대사증후군 환자가 당뇨환자가 되지 않도록 치료하는데 좋은 지침이 될 것으로 기대된다.

  • Purpose: The purpose of this study was to evaluate the prevalence, awareness, treatment and control of hypertension, diabetes, and dyslipidemia among women with rheumatoid arthritis. Methods: This cross-sectional study of 288 women with rheumatoid arthritis was conducted in a university-affiliated hospital. Data were collected by reviewing the women's medical charts and self-report questionnaires. Results: The prevalence of hypertension, diabetes and dyslipidemia was 27.8%, 5.9% and 48.3%, respectively. Despite the high prevalence of dyslipidemia, the subjects'awareness of it and its treatment rates were the lowest, which resulted in a low control rate. Of the 288 subjects, 158 (55%) had at least one of the three conditions. The prevalence of the combination of cardiovascular risk factors increased with age, and 32.7% of those aged 60 or older had two or three of the cardiovascular risk factors. Combinations of cardiovascular risk factors were significantly related with severe pain and fatigue. Conclusion: Cardiovascular risk factors were highly prevalent among the women with rheumatoid arthritis, but the rates of awareness, treatment, and control were suboptimal. There is a pressing need to facilitate the prevention, early detection and appropriate management of hypertension, diabetes and dyslipidemia among patients with rheumatoid arthritis in order to prevent cardiovascular disease.

당뇨병 노인의 건강행위 실천과 응급실 내원 간의 관련성 (The Association between Personal Health Behaviors and Emergency Room Visit among Elderly Patients with Diabetes)

  • 본 연구는 당뇨병을 진단받은 65세 이상 노인들의 인구학적 및 질병 특성, 건강행위 실천과 질병으로 인한 응급실 내원 간의 관련성에 대해 파악하기 위한 서술적 상관연구이다. 연구대상자는 2014~2017년도의 한국의료패널 연간 데이터에 등록된 대상자 중에서 당뇨병을 진단받은 65세 이상 노인 총 956명을 대상으로 하였다. 자료 분석은 SPSS 26.0 program을 이용하여 기술통계, Mann-Whitney U test, 카이제곱 분석, 일변량 회귀분석으로 분석하였다. 연구결과 당뇨병을 진단받은 노인의 중등도 신체활동과 걷기 실천 일수가 적을수록, 연령이 높을수록, 장애로 인한 활동제한이 있을수록, 우울증이 있을수록, 동반 만성질환 수가 많을수록 응급실 내원의 위험이 증가하는 것으로 나타났다. 따라서 당뇨병을 진단받은 노인들의 응급실 내원을 감소시키기 위해 지속적인 신체활동의 실천을 적극 격려하는 것이 중요하다.

타입 1형 당뇨 환자에서 급작스런 혈당조절에 의해 발생한 신경염 (Treatment-Induced Neuropathy of Diabetes)

  • Treatment-induced neuropathy in diabetes (TIND) is a reversible neuropathy that occurs in patients with diabetes, usually after a fast improvement in glycemic control. TIND is defined as the sudden onset of neuropathic pain or autonomic dysfunction with a large improvement in glycemic control (glycated hemoglobin [HbA1c] level of ≥ 2% over 3 months). We report the first case of a 24-year-old woman with type 1 diabetes mellitus who developed TIND in Korea. Her HbA1c level had decreased from 16.7% to 7.3% within a 3-month period. She developed acute-onset, severe, and continuous burning pain affecting her back and lower extremities. She was administered tapentadol (50 mg), pregabalin (75 mg), and vitamin B with minerals twice daily for neuropathic pain. She complained of orthostatic hypotension; thus, midodrine (2.5 mg) and anti-embolic stockings were prescribed. She almost completely recovered 6 months after the onset. A physician should be aware of TIND and gradually reduce HbA1c levels to prevent the occurrence of TIDN. They must also try to provide relief from severe pain or autonomic dysfunction and emphasize on an almost complete recovery.

프로바이오틱스 섭취를 통한 비만 및 제2형 당뇨의 완화 (Prevention of Obesity and Type 2 Diabetes by Using Probiotics)

  • The beneficial effects of probiotics on human health have been extensively studied. Strains of Lactobacillus and Bifidobacterium have gained considerable attention, as they are major groups of probiotic bacteria that improve gastrointestinal health. Recently, probiotics have been found to have positive effects on obesity and type 2 diabetes mellitus (T2D) that go beyond the improvement of gut health. Probiotics can alter the secretion of substances such as leptin and adiponectin from white adipose tissue via certain mechanisms. Additionally, probiotics can modulate the adaptive immune system against low-grade inflammation caused by obesity. The potential role of probiotics in the treatment of T2D has garnered interest because of increasing evidence of the anti-diabetic effect of probiotics. In this review, we discuss the results of in vivo studies that have examined the use of probiotics to prevent obesity and T2D, while primarily focusing on the studies that explore the cellular and molecular mechanisms underlying the activity of promising probiotic strains. The conclusions of this review could be of help to researchers who are investigating the effects of probiotics on obesity or T2D, and to patients with these diseases.
