  • The aim of FMEA method which is already generalized in the industry engineering is to grasp a possibility that defect forms will appear. And it is a means of prevention step to let person remove obstacles which have an influence on customers beforehand in order. The two parts of FMEA are Design FMEA which we prepare in an early stage and Process FMEA which we prepare in time previous to mass production. This Study apply Process FMEA to execution fields of the construction industry.

  • In the field of safety-critical systems, analyzing the effects of various failure factors (failure modes) is essential through Failure Mode and Effects Analysis (FMEA). However, with the increasing importance of software in systems, applying FMEA analysis to the design phase has become challenging. This paper proposes the use of Automatic FMEA, which can automatically perform FMEA using model-based design techniques, and presents a case study of FMEA for automotive engines. A comparison is made between the model-based Automatic FMEA analysis tool and existing FMEA tools. The study aims to demonstrate the performance of the Automatic FMEA analysis tool and propose future research plans.

  • FMEA (failure mode and effect analysis)is a widely used technique to assess or to improve reliability of product not only at early stage of design and development, but at the process and service phase during the product life cycle. In designing a service system, this study proposes a fuzzy service FMEA with the service blueprints as a tool which describes customer actions, onstage contact employees actions, backstage contact employees actions, support processes, and physical evidences, in order to analyse and inform service delivery system design. We fuzzified only two risk factors, occurrence and severity, to more effectively assess the potential failure modes in service. Proposed fuzzy risk grades are applied to Gaussian membership function, defuzzified into Fuzzy Inference System, and eventually identified the ranks on the potential fail points.

  • An FMEA for the waterjet-lifter of a DSNM is performed to prevent the occurrence of device failure. A waterjet-lifter raises and transports manganese nodules from the deep-sea floor up to a somewhat elevated place, from which a pin-scraper transports the lifted nodules to the inner space of the DSNM. A concept design for a device using the axiomatic design methodology is shown as the mapping between the functional domain and physical domain. The FMEA for a DSNM is introduced briefly and the rating criteria of severity, occurrence, and detection for the DSNM are defined. The FMEA of the functional requirements of a DSNM device is accomplished. Three kinds of failure modes, as well as their effects and causes, are predicted. Current design control methods for detecting potential failures, such as physical or computational experiments, design confirmation, and mathematical calculation, are described and the recommended actions for several significant causes are suggested.

  • FMEA(Failure Mode and Effect Analysis) is a failure analysis method for the system to identify the potential failure modes, and their effects and causes to reduce or mitigate the critical effects of the system. FMEA for railway was introduced with reliability of railway system, and this was used for identifying and analysing the possible hazards qualitatively to meet the requirements in early stage of business. In general, the overall failure data of system could be managed from design stage by FMEA, and also the countermeasures to prevent many predicted failures could be established by identification of failure modes and assement of failure effects by FMEA. Using these advantages of FMEA, the effectiveness of reliability improvement could be expected if FMEA is applied continuously in operation stage. It is essential that railway vehicles are maintained with high level of safety and reliability not to happen any failures in operation. This paper is proposed the proper FMEA for maintenance of railway vehicles compared with existing FMEA.

  • Munitions must be analyzed to identify any risks for quality assurance in development and mass production. Risk identification for parts, compositions, and systems is carried out through failure mode effects analysis (FMEA) as one of the most reliable methods. FMEA is a design tool for the failure mode of risk identification and relies on the RPN (risk priority number). FMEA has disadvantages because its severity, occurrence, and detectability are rated at the same level. Fuzzy FMEA applies fuzzy logic to compensate for the shortcomings of FMEA. The fuzzy logic of Fuzzy FMEA is to express uncertainties about the phenomenon and provides quantitative values. In this paper, Fuzzy FMEA is applied to the failure mode of a rotorcraft landing system. The Fuzzy rule and membership functions were conducted in the Fuzzy model to study the RPN in the failure mode of a landing system. This method was selected to demonstrate crisp values of severity, occurrence, and detectability. In addition, the RPN was obtained. The results of Fuzzy FMEA for the landing system were analyzed for the RPN and ranking by fuzzy logic. Finally, Fuzzy FMEA confirmed that it could use the data in quality assurance activities for rotorcraft.

  • Purpose: This paper proposes a procedure that may infer and identify interaction failures in a module. Methods: In design FMEA, we defined an interaction model between components and proposed a method for selecting a single component by using the standard specification classification table and four methods for choosing the related components. We also introduced the function tree for function and requirement characteristic analysis and proposed utilization of standard stress lists and 1st and 2nd stress analysis tables to determine the effect the stress analysis has on interactions. Finally, the interaction mechanism diagram was proposed and used to infer the failure mechanism. Process FMEA also established procedures in a similar way. Results: We established a procedure for predicting the failure mode due to interaction between components based on Company A's multi-step FMEA procedure. Conclusion: By applying the proposed interaction FMEA procedure to the development model, we were able to confirm the effect of the new derivation on the failure mode of interaction, which was not predicted by the existing FMEA.

  • This study proposes an integrated approach that uses both a fuzzy service FMEA (failure mode and effect analysis) and HOQ (house of quality) matrix algebra in designing and improving a service system. The fuzzy service FMEA methodology applies the customer satisfaction to the fuzzy RPN model. We fuzzify only the service satisfaction that consist in two failure factors, intangible service and tangible service, to more effectively assess the customer satisfactions on service encounters. Proposed fuzzy service satisfactions with triangle membership function are defuzzified by using the Fuzzy Inference System, and these are eventually identified the ranks on the potential fail points. HOQ matrices are constructed from cause-effect relationships. It is possible for these relationship matrix to find a linear approximation solution on the engineering attributes. Thus, in order to demonstrate how the proposed methods work, practical sample of the A/S part in S Electronic Co. provides for the ranking of the engineering attributes which has been successfully implemented.

  • FMEA is a widely used technique to assess or to improve reliability of products at early stage of design and development. In order to implement FMEA effectively, it is important to utilize knowledge or technical know-hows. However, that is very difficult. In fact, if information on the failure modes is enough, it is important to consider a counterplan to eliminate critical failure modes. If engineers do not have such information, however, it is more important to know what failure modes of the component under consideration would undergo. This means that the purposes of performing FMEA varies according to the situation. Therefore, we need different kinds of FMEA charts to meet those purposes. This paper proposes two FMEA charts: one is suitable for detecting potential failure modes and the other is for product improvements.
