Descending Controls: The Self-Regulation of Chronic Pain (하행성 조절계 : 만성 통증에 대한 제어 작용)

  • The descending control system is composed of a group of structures in the midbrain, medulla oblongata and pons that form a network of descending inhibitory projections. In the clinical setting, it has been shown that the application of a electrical counterirritant to these structures and diminishes the pain in patients. Thus, depression and anxiety have been shown to predict the development of chronic neuropathic pain state. These factors could influence pain might also involve descending controls. Interestingly, reduced descending controls are seen in patients with irritable bowel syndrome and theses patients had greater anxiety, depression compared to controls. And, the influence of anxiety on the chronicity of pain and on the descending control pathways should be tested in animal models, using modern techniques. Given this Knowledge, it is no wonder that pain is a highly personal experience that is susceptible to a variety of biologic, pharmacologic, and environmental influences.

Effect of Amygdalin from Armeniacae Semen on Ion Currents Changed by Lipopolysaccharide in Rat Periaqueductal Gray Neurons

  • Background : Amygdalin is abundant in Armeniacae semen, and it is recently reported to treat cancers and relieve pain. But modus operandi of amygdalin at the level of neuron has not been reported, yet. Objective : This study aimed to find out the effect of amygdalin on glycine- and glutamate-induced ion currents in periaqueductal gray (PAG) neurons. And it was investigated that amygdalin participates in the regulation of the descending pain control system in the level of PAG neurons. Method : We investigated that the changes of glycine- and glutamate-induced ion currents in PAG neurons through application of lipopolysaccharides (LPS) and application of amygdalin with LPS by using the nystatin-perforated patch clamp method. Result : Application of LPS on PAG neurons resulted in increased glycine-induced ion current, and in decreased glutamate-induced ion current. In contrast, application of amygdalin with LPS resulted in decreased glycine-induced ion current increased by LPS, and increased glutamate-induced ion current decreased by LPS. Conclusion : Amygdalin from Armeniacae semen controls glycine- and glutamate-induced ion current by LPS in PAG neurons, and it is suggested that amygdalin participates in the regulation of the descending pain control system in the level of PAG neurons.

Design of convection current circulation system in reservoir using CFD simulation (CFD모사를 이용한 저수지 물순환장치 유동 설계)

  • Convection Current Circulation System(CCCS) in stratified reservoir controls development of anaerobic condition and algal bloom during summer. In order to increase the CCCS effectiveness, we analyze diverse design parameters to make optimize the flow pattern in reservoir. In this study, we interpret the internal flow with installation and operation condition of CCCS based on CFD in reservoir. Design variables of CCCS is reservoir depth, stratification strength, distance of between CCCS and so on. Since reservoir depth and stratification strength in variables is depending on natural phenomenon, we evaluated current circulation effect by distance of CCCS and proposed the optimal design condition using CFD simulation. Flow and diffusion changes in water body was assessed by temperature and dye test. Changes in water floor temperature at 40m intervals was slowly descending over 37 hours. Dye diffusion simulation at 60m intervals, the radius of the spread between two devices were overlapped after 12 hours.

In Vitro Susceptibility of Diarrhea-Causing Escherichia coli to 9 Antibacterial Agents in Clinical Use (최근 분리된 장내 병원성 대장균의 항균제 감수성)

  • To determine the prevalence of antibiotic resistance in fecal E. coli and to investigate possible associations between antibiotic resistance and other plasmid-mediated virulence properties, antibiotic disk susceptibility tests for nine antibiotics were done on 141 strains of E. coli isolated from diarrheal children and well controls. Eighty two percent of the test strains were resistant to one or more antibiotics. Antibiotics to which the test strains were most resistant in descending order were ampicillin (85%), trimethoprim/sulfamethoxazol (60%), and cephalothin (55%). Seventy nine percent of these resistant strains were resistant to two or more antibiotics. All 141 test strains were sorted into enterotoxigenic E. coli (ETEC), enteropathogenic E. coli (EPEC), enteroadherent E. coli (EAEC) and non-pathogenic E. coli and the percentages of strains resistant to multiple antibiotics were compared. Among ETEC regardless of its source, multiple drug resistance was more frequent in strains producing heatstable enterotoxin (ST) only than in strains producing only heat-labile enterotoxin (LT) or both. In EAEC, multiple resistance was more frequently associated with strains isolated from diarrheal patients than with those from well controls. The major antibiotic resistance patterns possessed by multiple resistant enteropathogenic strains were $SXT^R$ $AM^R$, $CR^R$, and $SXT^R$ $AM^R$ $CR^R$. Of 28 ST- producing $SXT^R$ ETEC, 26(96%) were also resistant to ampicillin and 17 (61%) were resistant to cephalothin. The similar pattern was observed in EAEC and EPEC as well. This study has important implications for the treatment of E. coli diarrhea with antibiotics because it is possible that dissemination of virulence could occur under the force of selective antibiotic pressure. In addition, this study suggests that the in vivo efficacy of SXT in treating diarrheal illness be reevaluated.

Quantitative Analysis of Distribution of the Gastrointestinal Tract Eosinophils in Childhood Functional Abdominal Pain Disorders

  • Background/Aims Although functional abdominal pain disorders (FAPDs) are common in children, the accurate pathogenesis of FAPDs is not known yet. Micro-inflammation, particularly tissue eosinophilia of gastrointestinal (GI) tract, has been suggested as the pathophysiology observed in several GI disorders. We aimed to evaluate eosinophilic infiltration throughout the entire GI tract in children with FAPDs, compared to those with inflammatory bowel diseases (IBD) and to normal reference values. Methods We included 56 children with FAPDs, 52 children with Crohn's disease, and 23 children with ulcerative colitis. All subjects underwent esophagogastroduodenoscopic and colonoscopic examination with biopsies. Tissue eosinophil counts were assessed in 10 regions throughout the GI tract. Results Eosinophil counts of the gastric antrum, duodenum, terminal ileum, cecum, and ascending colon were significantly higher in children with FAPDs compared to normal reference values. Eosinophil counts of the stomach and the entire colon were observed to be significantly higher in children with IBD than in those with FAPDs. Even after selecting macroscopically uninvolved GI segments on endoscopy in children with IBD, eosinophil counts of the gastric body, cecum, descending colon, sigmoid colon, and the rectum were also significantly higher in children with IBD than those with FAPDs. Conclusions Significantly high eosinophil counts of the stomach and colon were observed in the order of IBD, followed by FAPDs, and normal controls, regardless of endoscopically detected macroscopic IBD lesions in children. This suggests some contribution of GI tract eosinophils in the intrinsic pathogenesis of FAPDs in children.

Influence of catecholamines on the free fatty acid metabolism in the tortoise (자라에 있어서 Catecholamine이 유리지산대사(遊離脂酸代謝)에 미치는 영향(影響))

  • 1. In the tortoise, Amyda japonica, a cold-blooded animal readily available in this country, the role of catecholamines in the regulation of free fatty acids(FFA) metabolism was investigated in both in vivo and in vitro studies. 2. Norepinephrine elevated both FFA and glucose levels in plasma. 3. When $50{\mu}g/kg$ of Epinephrine, Norepinephrine and Isopropylarterenol were administered intravenously, the relative effectiveness of mobilizing FFA was in the descending order of potency-Epinephrine, Norepinephrine and Isopropylarterenol. 4. In order to exclude the 'tonic influence of the endogenous catecholamines', reserpine was given to some animals. Two days after the reserpine-treatment, glucose showed a significant increase over the solely vehicle treated controls, FFA but an insignificant one. Excised auricles from those animals showed a diminished response to tyramine. Seven days after the treatment, however, when the depletion of catecholamines from the tissue stores seemed to be complete, judged from the absence of the response of isolated auricles to tyramine, both FFA and glucose levels were definitely lowered. 5. In in vitro experiments Epinephrine enhanced the FFA-release from the adipose tissue. The effect increased proportionately with the concentration until a maximal effect was attained at a concentration of 1x $10^5$ g/ml. 6. The order of potency in releasing FFA from adipose tissue in vitro was the same as in vitro, i.e., Epinephrine, Norepinephrine and Isopropylarterenol, but the differences were much less marked. 7. Ergotamine exerted no lipolytic action, but inhibited the lipolytic effect ef Epinephrine significantly. 8. Nethalide showed a slight lipolytic effect per se but inhibited the Epinephrine-induced lipolysis significantly. 9. Catecholamines play an important role in regulating FFA metabolism in the cold-blooded animal, just as in the warm-blooded animals, and the tortoise, Amyda japonica, may be used in the studies of fat metabolism as well as the rat.

Association of Toll-like receptor 2-positive monocytes with coronary artery lesions and treatment nonresponse in Kawasaki disease

  • Purpose: Activation of Toll-like receptor 2 (TLR2) present on circulating monocytes in patients with Kawasaki disease (KD) can lead to the production of proinflammatory cytokines and interleukin-10 (IL-10). We aimed to determine the association of the frequency of circulating TLR2+/ CD14+ monocytes (FTLR2%) with the outcomes of KD, as well as to compare FTLR2% to the usefulness of sIL-10. Methods: The FTLR2% in patients with KD was measured by flow cytometry. Serum levels of IL-10 (sIL-10) were determined in 31 patients with KD before the initial treatment with intravenous immunoglobulin (IVIG) and in 21 febrile controls by using enzyme-linked immunosorbent assay. Patients were classified as having coronary artery lesions (CALs) based on the maximal internal diameters of the proximal right coronary artery and proximal left anterior descending coronary artery one month after the initial diagnosis. Results: We found that FTLR2% greater than 92.62% predicted CALs with 80% sensitivity and 68.4% specificity, whereas FTLR2% more than 94.61% predicted IVIG resistance with 66.7% sensitivity and 71.4% specificity. Moreover, sIL-10 more than 15.52 pg/mL predicted CALs and IVIG resistance with 40% and 66.7% sensitivity, respectively, and 73.7% and 76.2% specificity, respectively. Conclusion: We showed that measuring FTLR2% before the initial treatment could be useful in predicting CAL development with better sensitivity than sIL-10 and with results comparable to sIL-10 results for the prediction of IVIG resistance in patients with KD. However, further studies are necessary to validate FTLR2% as a marker of prognosis and severity of KD.

Proton Magnetic Resonance Spectroscopic Changes of the Primary Motor Cortex and Supplementary Motor Area in Hemiparetic Patients with Corticospinal Tract Injury due to Deep Intracerebral Hematoma

  • Purpose: To investigate the metabolic changes in the motor and motor association cortices following axonal injury in the internal capsule that was caused by deep intracerebral hematoma. Materials and Methods: Using proton magnetic resonance spectroscopy (1H MRS), the authors studied the primary motor cortices (M-1) and supplementary motor areas (SMA) of 9 hemiparetic patients with documentable hemiparesis of varying severity, and we studied 10 normal volunteers as controls. To measure the M-1 and SMA biochemical changes, 4 separate single volumes of interest (VOIs) were located bilaterally in the affected and unaffected hemisphere (AH and UH). Results: 1H MRS provided a neuronal and axonal viability index by measuring levels of N-acetylaspartate (NAA) and creatine/phosphocreatine (Cr). The M-1/SMA NAA/Cr ratios of the AH and UH in patients, and the AH and normal volunteers were compared. The NAA/Cr ratios of the M-1 and SMA in AH, and the SMA in UH were significantly lower than those of normal volunteers. Conclusion: These 1H MRS findings indicate that axonal injury in the descending motor pathway at the level of internal capsule could induce metabolic changes in the higher centers of the motor pathway.

Coronary Artery Bypass Surgery with Radial Artery -Early Results (요골동맥을 이용한 관상동맥우회술 -조기성적)

  • The radial artery as a graft for myocardial revascularization was introduced by Carpentier in the early 1970s. Mid-term results were unfortunately discoura ing, and the clinical experience with this graft was interrupted. At the end of the 1980s, these authors reproposed the same arterial conduit with more satisfying results, because of improved technique and pharmacological management of the graft. Between October 1994 and July 1995, 36 patients underwent myocardial revascularization with a radial artery graft in Seiong General Hospital. Left internal mammary artery was concomitantly used as a pedicled Vift in 34 patients. Fifteen patients (42%) had a complete arterial waft revascularization. A total of 12) distal anastomoses were performed (average 3.4 per patient), including 36 left internal mammary artery wafts (two sequential in 2 patients), and 23 saphenous vein grafts. The remaining 64 distal anastomoses were perFormed with radial artery grafts (mean 1.8 per patient). The radial arteries were anastomosed to the circumflex (n=38), diagonal (n= 18), right coronary(n=G), and left anterior descending coronary artery(n=2). The percent ge of radial artery graft anastomoses (64) to the total anastomoses(123) was 52%. The radial artery was used as a single graft in 10 patients, as a sequential graft in 25 patients, and two grafts in 1 patient. Twenty patients underwent associated procedures coronary endarterectomy (14), coronary artery patch angioplasty (4), mitral valve repair (1), and repair of ventricular septal rupture (1). One patient died of low cardiac output syndrome and the others had no perioperative myocardial infarction. There are no ischemic and functional complications in the arm or hand aftcr removal of the radial artery. Only 1 patient required reexploration of the am, for the hematoma evacuation, and 2 patients complained transient thumb dysesthesia of the side of the havested arm. This dysesthesia improved within one month. Postoperative angiovaphic controls were obtained in 11 patients(31%) postoperative 79 to 210 days (mean 126 days). The patency rate were as follows : left internal mammary artery (100%), saphcnous vein (100%), and radial artery(95%). We concluded that the radial artery is useful alternative graft, but long term clinical and angiographic studies are required to derterminc whether wider application is warranted.

Effects of Pectinase Treatment on Alcohol Fermentation of Persimmon (Pectinase처리가 감과실 알콜발효에 미치는 영향)

  • This study was conducted to determine whether pectinase treatment would affect the process of alcohol fer-mentation with persimmon. The pectinase did not change pH and total acidity throughout the alcohol fermentation. However, the concentrations of reducing sugar were significantly lowered with the fermentation time, compared with controls. During the alcohol fermertation, the concentration of reducing sugar decreased rapidly up to 60 hours, unchanged from 60 to 72 hours, and then increased thereafter. The total alcohol concentrations of pectinase-treated groups were significantly higher than that of alcohol fermentation containing without pectinase. Among concentration 200 and 500 ppm had the most pronounced increase in the yield (%) of total alcohol (96%, respectively) and then, 300, 400 ppm and control in descending order. The contents of 5 major alcohols (acetaldehyde, methanol, n-propy alcohol, iso-butyl alcohol and iso-amyl alcohol) were measured. Among alcohol constituents, acetaldehyde and methanol were detected to be the lowest at control and methanol the highest at 200 ppm. These observations indicated that pectinase treatment would increase the yield of total alcohol, whereas it also raised methanol production during persimmon alcohol fermentation.
