Purpose: To evaluate the effectiveness and safety of postoperative radiotherapy for the treatment of keloid scars administered immediately after Cesarean section. Materials and Methods: A total of 26 postpartum patients with confirmed keloids resulting from previous Cesarean sections received either 12 or 15 Gy radiotherapy. The radiotherapy was divided into three 6 MeV electron beam fractions administered during the postpartum period immediately following the final Cesarean section. To evaluate ovarian safety, designated doses of radiation were estimated at the calculated depth of the ovaries using a solid plate phantom and an ionization chamber with the same lead cutout as was used for the treatment of Cesarean section operative scars and a tissue equivalent bolus. Results: In total, the control rate was 77% (20 patients), while six (23%) developed focally elevated keloids (ranging from 0.5 to 2 cm in length) in the middle of the primary abdominal scar. Five patients experienced mild hyperpigmentation. Nonetheless, most patients (96%) were satisfied with the treatment results. The estimated percentage of the applied radiation doses that reached the calculated depth of the ovaries ranged from 0.0033% to 0.0062%. Conclusion: When administered during the immediate postpartum period, postoperative electron beam radiotherapy for repeated Cesarean section scars is generally safe and produces good cosmetic results with minimal toxicity.

A rectangular-shaped PET system with an adjustable gantry (AGPET) has been developed for imaging small animals. The AGPET system employs a new depth of interaction (DOI) method using a depth dependent reflector patterns and a new digital time pickoff method based on the pulse reconstruction method. To evaluate the performance of the AGPET, timing resolution, intrinsic spatial resolution and point source images were acquired. The timing resolution and intrinsic spatial resolution were measured using two detector modules and Na-22 gamma source. The PET images were acquired in two field of view (FOV) sizes, 30 mm and 90 mm, to demonstrate the characteristic of the AGPET. As a result of in the experiment results, the timing resolution was 0.9 ns using the pulse reconstruction method based on the bi-exponential model. The intrinsic spatial resolution was an average of 1.7 mm and the spatial resolution of PET images after DOI correction was 2.08 mm and 2.25 mm at the centers of 30 mm and 90 mm FOV, respectively. The results show that the proposed AGPET system provided higher sensitivity and resolution for small animal imaging.
The purpose of this study was to compare radiation dose among workers in the radiation zone and to compare the doses of students in clinical practice in the same area to provide basic data on optimization of radiation protection. The subjects were 121 radiation related workers, 36 radiation workers, and 121 students who completed 8 weeks of clinical practice from Jan. 2016 to Dec. The depth and surface dose between the radiation related workers and the radiation workers were the highest with $.7440{\pm}1.676mSv$ and $.7753{\pm}1.730mSv$, respectively, and statistically significant (p<.01). Among the three groups, the depth dose was the highest at $.143{\pm}.136mSv$ for clinical practice students and the highest at surface dose of $.1513{\pm}.139mSv$. The lowest in both cases, The mean difference between the two groups was statistically significant (p<.01). In conclusion, it is necessary to manage thoroughly according to the ALARA(As Low As Reasonably Achievable) principle. Especially, it is necessary to systematically manage the dose of radiation for clinical students who are in the blind spot of radiation safety management.
$\underline{Purpose}$: To evaluate the pathological prognostic factors related to local recurrence after radical surgery and adjuvant radiation therapy in advanced rectal cancer. $\underline{Materials\;and\;Methods}$: Fifty-four patients with advanced rectal cancer who were treated with radical surgery followed by adjuvant radiotherapy and chemotherapy between February 1993 and December 2001 were enrolled in this study. Among these patients, 14 patients experienced local recurrence. Tissue specimens of the patients were obtained to determine pathologic parameters such as histological grade, depth of invasion, venous invasion, lymphatic invasion, neural invasion and immunohistopathological analysis for expression of p53, Ki-67, c-erb, ezrin, c-met, phosphorylated S6 kinase, S100A4, and HIF-1 alpha. The correlation of these parameters with the tumor response to radiotherapy was statistically analyzed using the chi-square test, multivariate analysis, and the hierarchical clustering method. $\underline{Results}$: In univariate analysis, the histological tumor grade, venous invasion, invasion depth of the tumor and the over expression of c-met and HIF-1 alpha were accompanied with radioresistance that was found to be statistically significant. In multivariate analysis, venous invasion, invasion depth of tumor and over expression of c-met were also accompanied with radioresistance that was found to be statistically significant. By analysis with hierarchical clustering, the invasion depth of the tumor, and the over expression of c-met and HIF-1 alpha were factors found to be related to local recurrence. Whereas 71.4% of patients with local recurrence had 2 or more these factors, only 27.5% of patients without local recurrence had 2 or more of these factors. $\underline{Conclusion}$: In advanced rectal cancer patients treated by radical surgery and adjuvant chemo-radiation therapy, the poor prognostic factors found to be related to local recurrence were HIF-1 alpha positive, c-met positive, and an invasion depth more than 5.5 mm. A prospective study is necessary to confirm whether these factors would be useful clinical parameters to measure and predict a radio-resistance group of patients.

Beam quality is changed about magnetic field of bending magnet. Evaluation of beam quality using PDD(Percentage Depth Dose) at 10cm depth at recommendation of AAPM(America Academy of Pain Medicine). However this evaluation shows fragmentary element. Therefore this study is applied to three value, 10cm divided by 5cm depth PDD, 20cm divided by 10cm depth PDD, 30cm divided by 20cm depth PDD, at change the magnetic field. PDD is measured at magnetic field changed ${\pm}1%$, ${\pm}2%$ at 6MV(Mega Voltage), 10MV photon. The plan technique is 3 portal plan using Core-Plan at human pelvic phantom. Conventional and presented methods are compared at maximum and minimum dose. The presented method increased discernment of relieve the unequal distribution and energy area than conventional method. Henceforth, application of presented method will be considered. Development of energy measurement method and detector miniaturization will be needed about continuous study.

Purpose : To reduce the radiation dose to the thyroid that is affected to scattered radiation, the shield was used. And we evaluated the shielding effect for the thyroid during whole brain radiation therapy. Materials and Methods : To measure the dose of the thyroid, 300cGy were delivered to the phantom using a linear accelerator(Clinac iX VARIAN, USA.)in the way of the 6MV X-ray in bilateral. To measure the entrance surface dose of the thyroid, five glass dosimeters were placed in the 10th slice's surface of the phantom with a 1.5 cm interval. The average values were calculated by measured values in five times each, using bismuth shield, 0.5 mmPb shield, self-made 1.0 mmPb shield and unshield. In the same location, to measure the depth dose of the thyroid, five glass dosimeters were placed in the 10th slice by 2.5 cm depth of the phantom with a 1.5 cm interval. The average values were calculated by measured values in five times each, using bismuth shield, 0.5 mmPb shield, self-made 1.0 mmPb shield and unshield. Results : Entrance surface dose of the thyroid were respectively 44.89 mGy at the unshield, 36.03 mGy at the bismuth shield, 31.03 mGy at the 0.5 mmPb shield and 23.21 mGy at a self-made 1.0 mmPb shield. In addition, the depth dose of the thyroid were respectively 36.10 mGy at the unshield, 34.52 mGy at the bismuth shield, 32.28 mGy at the 0.5 mmPb shield and 25.50 mGy at a self-made 1.0 mmPb shield. Conclusion : The thyroid was affected by the secondary scattering dose and leakage dose outside of the radiation field during whole brain radiation therapy. When using a shield in the thyroid, the depth dose of thyroid showed 11~30% reduction effect and the surface dose of thyroid showed 20~48% reduction effect. Therefore, by using the thyroid shield, it is considered to effectively protect the thyroid and can perform the treatment.

Monte Carlo calculations were performed to demonstrate the dose modulation with dynamic magnetic fields in phantom. The goal of this study is to obtain the uniform dose distributions at a depth region as a target on the central axis of photon beam under moving transverse magnetic field. We have calculated the depth dose curves for two cases of moving magnetic field along a depth line, constant speed and optimal speed. We introduced step-by-step shift and time factor of the position of the electromagnet as an approximations of continuous moving. The optimal time factors as a function of magnetic field position were calculated by least square methods using depth dose data for static magnetic field. We have verified that the flat depth dose is produced by varying the speed of magnetic field as a function of position as a results of Monte Carlo calculations. For 3 T magnetic field, the dose enhancement was 10.1% in comparison to without magnetic field at the center of the target.

Central axis percentage depth-doses, P(%), were measured at the points from the 2.5cm depth of reference point to 20 cm depth with 2.5 cm interval. Distance from the X-ray target to the water phantom($30{\times}30{\times}30cm^3$) surface was 1 m, and at this point three different beam sizes of $5cm{\phi},\;10cm{\phi},\;and\;15cm{\phi}$ were used. While the X-ray tube voltage varied from 150 to 250 kV, the tube current remained constant at 5 mA. Absorbed dose rate in water, $\dot{D}_w$, was determined using the air kerma calibration factor, $N_k$, which was derived from the exposure calibration factor, $N_x$, of the NE 2571 ion chamber. The reference exposure rate, $\dot{X}_c$, was measured using the Exradin A-2 ion chamber calibrated at ETL, Japan. The half value layers of the X-rays determined to meet ETL calibration qualities. The absorbed dose rates determined at the calibration point were compared to the values obtained from Burlin's general cavity theory, and the percentage depth-dose values determined from $N_k$ showed a good agreement with the values of the published depth dose data(BJR Suppl. 17).
A hot water layer (HWL hereinafter) was installed at the depth of 1.2 m from the pool surface to reduce the radiation level at the pool top. After the HWL system was improved by the replacement of the filter with the Ion Exchanger to capture the Na-24, to purify the pool water of HWL and finally to reduce the radiation at the pool top. It was confirmed by the performance test of the pump and the measurement of the pressure difference through the Ion Exchanger and the strainer, that the flow characteristics of HWL system was not adversely affected after the system modification. Also the flow analysis using the pressure loss coefficients of the Ion Exchanger and strainer, calculated by the Darcy formula, could predict the flow variations by pressure changes within $10\%$ error in comparison with the field test results. It was also confirmed that HWL was maintained with the depth of 1.2 m from the pool surface because each electric water heater was electrically and thermodynamically maintained at 30 kW and the temperature of HWL was maintained with $5^{\circ}C$ higher temperature than that of pool water. Finally, it was confirmed that the pool top radiation was saturated and stabilized below 10000 nG/hr within 24 hours as the ion exchanger captured the main nucleus, Na-24 and purified the pool water of HWL.

The radiosurgery treatment is one time, non surgical approach to the treatment of patients with intracranial disorders whose conditions would be difficult or dangerous to treat with conventional sugical procedures. The LINAC based radiosurgery is based on the combination of multiple isocentric arc irradiation with small fields centered in the stereotactic target. The absorption of the beam in a tissue equivalent medium, such as water, as well as the uniformity, or profile, of the beam must be precisely documented. The beam characteristics and dosimetric measurememts of the 6MV X-ray beam from a ML-6M linear accelerator are examined. The percent depth dose (PDD) and beam profile (including flatness, symmetry and penumbra) is calibrated with the radiosurgery cone in water phantom. The cone is made of lead which size is from $10{\times}10mm{\phi}$ to $30{\times}30mm{\phi}$ All of these dosimetric measurements sufficiently characterized the beam to permit safe clinical use.
