Purpose: This study was to examine the effects of foot reflexion massage on sleep disturbance, depression disorder, and the physiological index of the elderly in nursing homes. Method: The research design was a non-equivalent control group pretest-posttest quasi-experimental study. The subjects in this study were 50 elderly people who resided in two different nursing homes in the same region. An experimental group and a control group were organized with 25 subjects respectively, and foot reflexion massage was provided for 12 sessions, 30 minutes per session. The selected dependent variables were sleep disturbance, depression disorder, and physiological indices(blood plasma serotonin, serum cortisol), which were all measured before and after foot reflexion massage was offered. Data analysis included $x^{2}$-test, t-test, paired t-test, and ANCOVA, using the SPSS program package. Result: 1. The experimental group improved sleep quality more than the control group. 2. The experimental group had less depression disorder than the control group. 3. The experimental group had higher serotonin levels than the control group. Conclusion: It's necessary to give foot reflexion massage as a successful nursing intervention to elderly who undergo a change in sleep, and suffer from a depression disorder due to a deterioration in sleep.
Objective : To present effects of depression, optimism and gratitude on suicidal ideation of patients with depressive disorders. Methods : Using analyses of covariance (ANCOVA), scores on the Beck Depression Inventory (BDI), Life Orientation Test-Revised (LOT-R), and Gratitude Questionnaire (GQ-6) were compared between depressive disorder patients with higher and lower levels of suicidal ideations. A linear regression model was fitted to detect independent correlates for suicidal ideations. Results : Significantly greater level of depression and lower level of gratitude were characterized by depressive disorder patients with higher level of suicidal ideations. The fitted regression model presented that depression and gratitude were independent correlates for suicidal ideations in patients with depressive disorders. Conclusion : Our findings suggest that gratitude may be associated with lowering the level of suicidal ideation in patients with depression.
Purpose : Anxiety disorder and depressive disorder are often comorbid with each other, and the comorbidity is associated with poorer psychiatric outcome, resistance to treatment, increased risk for suicide, greater chance for recurrence. We aimed to investigate the comorbidity of anxiety disorder in Korea. Method : Subjects were total of 867 depressed patients recruited CRESCEND-K multicenter trial. We used SCID (Structured Clinical Interview for DSM - IV) to find comorbidity of anxiety disorders in depressed patient. Results : Of 867 patients, total 8.2% had anxiety disorder. Proportion of anxiety disorder Not Otherwise Specified was 3.5%, panic disorder was 1.7%, generalized anxiety disorder was 1.1%, post traumatic stress disorder was 0.9%, obsessive compulsive disorder was 0.6%, social phobia was 0.4%. Conclusion : In this study, anxiety disorder in depression were measured at a low comorbidity rate in compare to previous studies. Selection bias, use of antidepressants at registration, severity of depression symptoms, and point of SICD administration seems to have affected these results. It is probable that comorbidity evaluation would be more precise if shorter, structured interviews such as M. I.N.I.-Plus were used during first clinical interview for depression diagnosis.

본 연구의 목적은 품행장애를 보이는 청소년에서 우울및 불안의 정도를 알아보고 또한 자신의 부모가 보이는 자녀 양육 방식에 대해 그들이 어떠한 지각을 하고 있는가를 살펴봄으로써, 품행장애 환아들에서 흔히 동반되는 우울, 불안 증상과 부모양육방식 사이에는 어떤 상호관련이 있으며 이는 또한 품행장애의 임상경과와 어떤 관련이 있는지를 알아보는데 있다. 이러한 목적하에 저자들은 DSM-Ⅳ의 진단기준에 부합되는 품행장애 입원환아 30명과 정상대조군으로 30명의 중학생을 선정하였으며, 이들에게 소아우울척도(CDI), 소아 상태-특성 불안척도(STAI), 부모-자녀 결합형태 검사(PBI)를 시행한 바 다음과 같은 결과를 얻었다. 1) 품행장애 환아들은 정상대조군에 비해 증상면에서 상태불안(state anxiety)이 높았으나 우울과 특성불안(trait anxiety)은 의미있는 차이를 보여주지 않았다. 2) 품행장애 환아들은 부모의 양육태도에 있어서 아버지와 어머니 모두에 대해 돌봄이 적고 과보호가 많다는 부정적인 인식을 가지고 있는 것으로 나타났다. 3) 어머니의 돌봄은 우울 및 품행문제의 심각도와 부적 상관관계를 보였다. 4) 어머니의 과보호는 특성불안과 정적 상관관계를 보였다. 품행장애 환아에 있어 아버지 양육태도의 특성은 그다지 의미있는 결과를 보여주지 않아 아버지보다는 어머니의 부정적인 양육 방식이 품행장애 소아청소년의 우울, 불안 및 품행문제의 심각도등의 임상경과와 보다 밀접한 관련성을 갖는 것으로 보인다.
Depression and anxiety disorders are common psychiatric illnesses whose etiology remains partially understood. The etiology of depression and anxiety disorders is multi-factorial, and abnormalities in neurotransmitter, neuroendocrine system, and brain activation have been implicated in those conditions. However, the pathophysiology of depression and anxiety disorder is certainly not well understood, and some patients with depression or anxiety disorders do not respond to antidepressant therapy. Recently, immunological factors such as cytokines are known to be closely related to central nervous system as well as depression and anxiety disorders. This review highlights recent progress in understanding the function of cytokines in depression and anxiety disorders.
Purpose: The purpose of this study was to investigate the relationship between eating disorder, self-esteem and depression in college women. Method: Participants were 282 college women. Data were collected between Nov. 1st and 30th, 2003, and provide basic data on eating disorder levels in college women and basic data for health control programs. The instrument was a questionnaire consisting of 8 items on general characteristics and weight control, 24 on eating disorders, 10 on self-esteem and 10 on depression. Analysis of the data was done using numbers, percentages, means and standard deviations, t-test and Pearson correlation coefficients. SPSS WIN 10.0 was used in data analysis. Result: The greatest difference for eating disorders was between the group with normal body weight and the group with low body weight (t=-6.94, p=.000). There was a high positive correlation between Body Mass Index and eating disorder (r=.383, p<.01), between eating disorder and depression (r=.161, p<.01). There was a high negative correlation between eating disorder and self-esteem (r=-.196, p<.01), and between self-esteem and depression (r=-.537, p<.01). Conclusion: These results indicate that college women need more education and counseling on dietary concerns. Also, systematic efforts to establish a more health conscious social standard for beauty should be taken. Further empirical and experimental studies are required to investigate factors influencing attitudes towards eating held by college women and to determine variables which affect various specific dimensions of these attitudes.
Mood disorder is a common psychiatric illness with a high lifetime prevalence in the general population. A serious problem such as suicide is commonly occurring in the patients with depression. Till now, the monoamine hypothesis has been the most popular theory of pathogenesis for depression. However, the more specific pathophysiology of depression and cellular molecular mechanism underlying action of commercial antidepressants have not been clearly defined. Several recent studies demonstrated that glial cells, especially astrocytes, are a promising answer to the pathophysiology of depression. In this article, current understanding of biology and molecular mechanisms of glial cells in the pathology of mood disorder and new research on the pathophysiology of depression will be discussed.
Objective : This study was performed to confirm the hypothesis that the more one applies positive thinking, the less severe the symptoms of stress and the better the therapeutic responsein panic disorder and major depressive disorders. Methods : The study included 50 subjects with confirmed diagnoses of panic disorder or major depressive disorders. Positive thinking was assessed using Positive thinking scale. Beck Depression Inventory was used as a subjective measure for depression, and to ensure an objective measure for depression and anxiety, the Hamilton Depression and Hamilton Anxiety rating scales were implemented. Results : The positive thinking scale measured at the initial visit had shown a strong negative correlation with objective depression. Although patients with a high level of positive thinking had shown a tendency to respond better to the treatment, as compared with those with a lower level, the differences were not statistically significant. Conclusion : Positive thinking is likely to ameliorate major depressive disorder, panic disorder-induced depression, and anxiety. Nevertheless, it was not possible to confirm the effects of positive thinking on the patients' treatment responses.
Purpose: This study was conducted in order to analyze the factors that influence alcohol use disorder in elders in KyongBuk province. The results of the study will provide basic information for the development of nursing interventions to promote proper alcohol use in elders. Methods: The subjects were 626 elderly individuals. Alcohol use disorder was defined as a score of more than 15 point on the AUDIT-K(Korean version of Alcohol Use Identification Test). Descriptive statistics, chi-square test, ANOVA and logistic regression were utilized to analyze the data. Results: Overall incidence of alcohol use disorder level; normal 73.2%, problem drinking 7.5% and alcohol use disorder 19.3%. Significant factors affecting alcohol use disorder were sex (OR=6.897), religious belief (OR=1.836), smoking (OR=2.948), liver disease (OR=4.753) and depression (OR=1.779). Conclusion: Community health care nurses perform a crucial function in the screening of elderly alcoholics. Early detection and treatment of depression in elders may help to prevent alcoholism.
Objective : The aims of this study were to investigate the prevalence of co-morbid depression and related factors of depression in mild cognitive impairment (MCI) patients. Methods : Nine hundred and six MCI individuals were included in this study. Depression was defined as major and minor depressive disorder according to the Diagnostic and Statistical Manual of Mental Disorders, fourth edition (DSM-IV). Depression-related demographic and clinical factors were also explored through multivariate logistic regression analysis. Results : In MCI patients, the prevalence of depression as defined according to the DSM-IV diagnostic criteria was 1.8% for major depressive disorder, 11.4% for minor depressive disorder, and overall 13.2% for both. Multivariate logistic regression showed that increased prevalence of overall depression was associated with female gender and non-amnestic subtype of MCI. Conclusion : Our findings based on a large number of MCI subjects who visited memory clinic indicated that more than ten percent of MCI patients have comorbid depression. In addition, female and non-amnestic subtype of MCI patients seems to be more vulnerable to depression.
