Evaluation of Adult Lung CT Image for Ultra-Low-Dose CT Using Deep Learning Based Reconstruction

  • Although CT has an advantage in describing the three-dimensional anatomical structure of the human body, it also has a disadvantage in that high doses are exposed to the patient. Recently, a deep learning-based image reconstruction method has been used to reduce patient dose. The purpose of this study is to analyze the dose reduction and image quality improvement of deep learning-based reconstruction (DLR) on the adult's chest CT examination. Adult lung phantom was used for image acquisition and analysis. Lung phantom was scanned at ultra-low-dose (ULD), low-dose (LD), and standard dose (SD) modes, and images were reconstructed using FBP (Filtered back projection), IR (Iterative reconstruction), DLR (Deep learning reconstruction) algorithms. Image quality variations with respect to varying imaging doses were evaluated using noise and SNR. At ULD mode, the noise of the DLR image was reduced by 62.42% compared to the FBP image, and at SD mode, the SNR of the DLR image was increased by 159.60% compared to the SNR of the FBP image. Based on this study, it is anticipated that the DLR will not only substantially reduce the chest CT dose but also drastic improvement of the image quality.

Balloon Bronchoplasty for the Treatment of Bronchial Stenosis After Lung Transplantation: A Single-Center 10-Year Experience

  • Objective: To assess the safety and efficacy of balloon dilatation under dual guidance using fluoroscopy and bronchoscopy for treating bronchial stenosis following lung transplantation (LT), and to elucidate the factors associated with patency after the procedure. Materials and Methods: From September, 2012, to April, 2021, 50 patients (mean age ± standard deviation, 54.4 ± 12.2 years) with bronchial stenosis among 361 recipients of LT were retrospectively analyzed. The safety of balloon dilatation was assessed by evaluating procedure-related complications. Efficacy was assessed by evaluating the technical success, primary patency, and secondary patency. Primary and secondary cumulative patency rates were calculated using the Kaplan-Meier method. The factors associated with patency after the procedure were evaluated using multivariable Cox hazard proportional regression analysis. Results: In total, 65 bronchi were treated with balloon dilatation in 50 patients. The total number of treatment sessions was 277 and the technical success rate was 99.3% (275/277 sessions). No major procedure-related complications were noted. During the mean follow-up period of 34.6 ± 30.8 months, primary patency was achieved in 12 of 65 bronchi (18.5%). However, the patency rate improved to 76.9% (50 of 65 bronchi) after repeated balloon dilatation (secondary patency). The 6-month, 1-year, 3-year, and 5-year secondary patency rates were 95.4%, 90.8%, 83.1%, and 78.5%, respectively. The presence of clinical symptoms was a significant prognostic factor associated with reduced primary patency (adjusted hazard ratio [HR], 0.465; 95% confidence interval [CI], 0.220-0.987). Early-stage treatment ≤ 6 months (adjusted HR, 3.588; 95% CI, 1.093-11.780) and prolonged balloon dilatation > 5 min (adjusted HR, 3.285; 95% CI, 1.018-10.598) were associated with significantly higher secondary patency. Conclusion: Repeated balloon dilatation was determined to be safe and effective for treating bronchial stenosis following LT. Early-stage treatment and prolonged balloon dilatation could significantly promote long-term patency.

대한민국 영남지역 해수욕장의 방사능 농도 분석 (Analysis of Radioactivity Concentration at Beaches in the Yeongnam Region, Republic of Korea)

  • 본 연구에서는 영남지역 해수욕장의 모래 시료 중에 함유되어 있는 방사능 농도를 측정하였다. 해수욕장에서 채취한 모래 표본은 영남지역의 유명해수욕장 8곳을 대상으로 선정하였다. 모래 표본을 반도체검출기인 고순도게르마늄검출기(HPGe)에 넣어 다채널분석기(MCA)를 이용한 정밀 감마 방사성핵종 분석을 실시했다. 방사선핵종의 농도를 측정하기 위해 측정 시간을 8,000 초로 설정하여 스펙트럼 파일을 얻어서 감마 방사성핵종의 농도를 분석하였다. 연구결과 8곳 해변 모두 TI-208의 방사능이 가장 높았다. 결론적으로 영남지역 해수욕장 모래표본에서 검출된 방사성핵종은 자연 방사성핵종이지만 인체 내부에 영향을 미칠 수 있다. 따라서 지속적으로 조사할 필요가 있다.

Antioxidant Effect of Homogentisic Acid on Hydrogen Peroxide Induced Oxidative Stress in Human Lung Fibroblast Cells

  • Homogentisic acid was found to scavenge intracellular reactive oxygen species (ROS), and 1,1-diphenyl-2-picrylhydrazyl (DPPH) radicals and thus prevented lipid peroxidation in human fibroblast (Wl 38) cells. The radical scavenging activity of homogentisic acid was found to protect Wl 38 cells against hydrogen peroxide $(H_2O_2)$ induced oxidative stress, via the activation of extracellular signal regulated kinase (ERK) protein. Homogentisic acid increased the activity of catalase. Hence, from the present study, it is suggested that homogentisic acid protects Wl 38 cells against $H_2O_2$ damage by enhancing the intracellular antioxidative activity.

Prognostic Value of Dual-Energy CT-Based Iodine Quantification versus Conventional CT in Acute Pulmonary Embolism: A Propensity-Match Analysis

  • Objective: The present study aimed to investigate whether quantitative dual-energy computed tomography (DECT) parameters offer an incremental risk stratification benefit over the CT ventricular diameter ratio in patients with acute pulmonary embolism (PE) by using propensity score analysis. Materials and Methods: This study was conducted on 480 patients with acute PE who underwent CT pulmonary angiography (CTPA) or DECT pulmonary angiography (DE CT-PA). This propensity-matched study population included 240 patients with acute PE each in the CTPA and DECT groups. Altogether, 260 (54.1%) patients were men, and the mean age was 64.9 years (64.9 ± 13.5 years). The primary endpoint was all-cause death within 30 days. The Cox proportional hazards regression model was used to identify associations between CT parameters and outcomes and to identify potential predictors. Concordance (C) statistics were used to compare the prognoses between the two groups. Results: In both CTPA and DECT groups, right to left ventricle diameter ratio ≥ 1 was associated with an increased risk of all-cause death within 30 days (hazard ratio: 3.707, p < 0.001 and 5.573, p < 0.001, respectively). However, C-statistics showed no statistically significant difference between the CTPA and DECT groups for predicting death within 30 days (C-statistics: 0.759 vs. 0.819, p = 0.117). Conclusion: Quantitative measurement of lung perfusion defect volume by DECT had no added benefit over CT ventricular diameter ratio for predicting all-cause death within 30 days.

  • As the demand for bone mineral density testing increases in Korea, which is close to an aging society, it is necessary to evaluate the repeatability of equipment such as femur phantom other than l-spine for more accurate diagnosis. However, in clinical practice, it is often not possible to proceed such evaluation due to insufficient quality control conditions. Therefore, this study is to evaluate the usefulness of the femur phantom after fabricating the same using 3D printing technology. The femur phantom was output using GlowFill filament and FDM 3D printing type. Each phantom was repeatedly scaned 20 times to compare whether the existing l-spine phantom and the fabricated femur phantom were suitable as a phantom for quality control. Each time the seven researchers took three times, the location of the femur phantom was readjusted, and then scanned to confirm the error between the researchers. As a result of conducting repeatability evaluation using femur phantom, the coefficient of variation rate was 2%, which was within the minimum precision tolerance of 2.5%. The reproducibility between the researcher was also found to be suitable as the average coefficient of variation was 0.031 and the coefficient of variation rate was 3.1%, which was within the minimum precision error range of 5%. In conclusion, it is considered that the prospective attitude and usefulness of the femur phantom fabricated by 3D printing in clinical practice will be sufficient.

  • Telomerase activation is detected in most cancerous cells; hence, telomerase is a highly selective target for cancer therapy, which plays an important role in the apoptotic process. We have previously reported that the ginseng saponin metabolite, Compound K (20-O-D-glucopyranosyl-20(S)-protopanaxadiol, IH901), inhibits cell proliferation by inducing apoptosis and cell cycle arrest at the $G_1$ phase. The present study investigated the regulation of telomerase activity in Compound K treated U937 cells. Compound K treatment caused a reduction in telomerase activity and down-regulated the human telomerase reverse transcriptase (hTERT) gene, resulting in the decreased expressions of its protein, and of the c-Myc and Spl proteins (transcription factors of hTERT). These results indicate that the anticancer activity of Compound K could be mediated by inhibition of the telomerase activity.

전산화단층촬영검사실 방사선사의 방사선피폭 방어행위에 영향을 미치는 요인 분석 (Factors Influencing Protective Behavior against Radiation Exposure of Radiological Technologist in Computed Tomography Examination Room)

  • This study was conducted to analyze factors Influencing Protective Behavior against Radiation Exposure using questionnaires for 231 radiological technologists working in Computed Tomography(CT) examination room with high radiation dose in diagnostic radiology field. Statistical analysis of the collected data revealed that the reasons for partially shielding the examination part in the CT scan were the lack of protective equipment, securing of radiation justification, being annoying and maybe not being harm to adults in order. It was also revealed that the variables influencing the protective behavior were protective behavior against radiation harm, self-efficacy, protective environment, organization culture, protective knowledge and protective instrument in order. The higher the radiological protective environment(${\beta}=0.245$) and the lower the radiological protective knowledge(${\beta}=-0.034$), the more influential the protective behavior against radiation harm was. In this study, it was shown that non examination parts were not shielded in the CT scan. Therefore, it is necessary to improve the level of protective environment, to cultivate knowledge to improve the protective behavior against radiation harm and to have an intervention strategy for concrete action.
