Should We Recommend Ultrasonography for an Incidental Thyroid Nodule on Additional Cervicothoracic Sagittal T2-Weighted Image of Lumbar Spine MRI?

  • Purpose: To determine whether we should recommend ultrasonography (US) for an incidental thyroid nodule identified by additional cervicothoracic sagittal T2-weighted image (C-T sag T2WI) of lumbar spine magnetic resonance imaging (MRI). Materials and Methods: A retrospective study of 61 patients who underwent both lumbar spine MRI and thyroid US between December 2011 and April 2015 was conducted. For all US-found thyroid nodules > 1 cm, investigators evaluated whether there was any correlation between thyroid nodule detectability by C-T sag T2WI and US features such as echogenicity, composition, or suspicion of malignancy. Results: Solid hypoechoic (2/4; 50%) or mixed echoic nodules (4/8; 50%) appeared to be found relatively more easily by C-T sag T2WI than more benign-looking solid isoechoic (1/4; 25%) or spongiform nodules (0/6; 0%). Among six nodules with ultrasonographic suspicion for malignancy, only one nodule was detected by C-T sag T2WI. Conclusion: If an incidental thyroid nodule is seen by C-T sag T2WI, it would be better to recommend thyroid US for identifying malignancy.

Comparison of Polymer and BANGTM Gel Dosimeters to Use a Optical Computed Tomography Scanner (광학 전산화단층촬영 스캐너 사용을 위한 중합체 겔과 BANGTM 겔 선량계의 특성 비교)

  • The purpose of this study was to compare the basic radiological characteristics of in-house polymer gel and commercially-available gel (BANGTM) gel dosimeters with a spectro-photometer to use a optical computed tomography (CT) scanner. We investigated the radiological characteristics including dose linearity, absorbance spectrum, dose rate dependency and inter-and intra-reproducibility at wavelengths of 590, 600 and 630 nm. The optimal dose linearities of two gel dosimeters showed R2 value of 0.939 and 0.948 at wavelengths of 590 nm and 600 nm, respectively. For two polymer gel dosimeters, there is no peak sensitivity within the range of all wavelengths in absorbance spectrum. For in-house gel dosimeter, the dose rate dependency were within 5% for all wavelengths except for the dose rate of 100 MU/min. For BANGTM gel dosimeter, the dose rate dependency showed an error range of ±5% for all wavelengths. The inter-and intra-reproducibility of two gel dosimeters were within the range of 2.5%. We have confirmed that the two gel dosimeters was appropriate for use with a optical CT scanner.

Evaluation of the Applicability of PET/CT Phantom as a 3D Printing Material (PET/CT 팬텀의 3D 프린팅 소재 적용 가능성 평가)

  • The purpose of this study is to present objective information in applying 3D printing technology for PET/CT (Positron Emission Tomography/Computed Tomography) performance evaluation and use it as a basic research that can be applied to various purposes in the future. Phantoms were manufactured with step wedge of ABS(Acrylonitrile Butadiene Styrene) and ACR(Acrylic acid) material. The counts for each ROI(Region of Interest) were analyzed through image acquisition in PET/CT. And the variation rate of counts and CNR(Contrast Noise Ratio) was evaluated. In the counts analysis, the effect of thickness occurred. In addition, in the variation rate analysis, the thickness setting of steps wedge 4 to 5 levels should be considered first. These results minimize quantitative and qualitative changes in the phantom manufactured based on 3D printing, and enable more stable PET/CT performance evaluation. Based on 3D printing in PET/CT, various phantoms are expected to be produced in the future. If the characteristics of each material are considered and applied through the basic research such as this research, the result of the phantom manufactured through 3D printing can be more meaningful and will be used in a wide range.

  • Medical institutions are abundant in human resources and have a unique structure with diverse professions and various organizational cultures coexist. This organizational culture influences organizational effectiveness and influences the effectiveness of the organization as a factor of leadership and organizational culture. Leadership is an important factor in linking employee behavior to organizational commitment to achieve job satisfaction. The purpose of this study was to analyze the effect of organizational effectiveness on organizational culture, leadership perception, and job stress in a special organization called radiologist. As a result, organizational culture and transformational leadership showed high correlation with 0.627. Among the organizational effectiveness variables, organizational commitment showed the highest influence with transformational leadership(${\beta}=0.284$, p<0.001), and job satisfaction was also the highest with transformational leadership (${\beta}=0.440$, p<0.001). The results showed that the expectation of transformational leadership affects organizational commitment and job satisfaction. In order to expect a positive change in the effectiveness of the organization, the radiologist culture requires an organizational culture that can fully demonstrate autonomy and creativity, and transformational leadership is required for this.

Development of Liquid Crystal Optic Modulation Based X-ray Dosimeter by Using CdS Sensor (CdS 센서를 이용한 액정 광변조 X-선 검출 시스템 개발)

  • In this study, the liquid-crystal optical modulation X-ray detection system using a CdS which is a family of II-IV compound semiconductor was proposed. The system consist of the detector, the signal processing part, the liquid-crystal driving parts, microcontroller, and I/O parts, and was designed to be suitable for miniaturization and portable. In addition, the system can measure a wide range X-ray by using the detecting range selection. In order to evaluate the performance of the proposed system, the CdS sensor's output characteristics were confirmed in accordance with changes of dose, and excellent correlation was determined. And also, the optical penetration ratio was discussed in accordance with changes of the applied voltage by measuring the change of the liquid-crystal in accordance with changes of the applied voltage. Through these results, the characteristics of the liquid-crystal optical modulation system such as the excellent reproducibility and the noise immunity were confirmed. And we considered that the CdS cell-based liquid-crystal optical modulated portable X-ray detection system could be applied to compact, low-cost, portable system.

  • When most patients are diagnosed with the quiet progressed hepatoma which often would make the operation impossible, the Interventional Radiology hepatic artery embolization is an extremely useful method for such patients. An existence of the malfunction is evaluated by gaining a portal vein image as a delayed phase image after injecting a contrast media into the superior mesenteric artery. However, it is difficult to make a definite judgement due to the extended exposure time with the peristalsis and the intestine gas obstructing the sharpness of the image when the Patient exposure time increases and due to the increased usage of contrast media and its side effect. The portal vein can be evaluated by obtaining the MIP image after reconstructing a 3-dimensional personal computer setting using the 2-dimensional from an enhancement abdomen CT image that is almost a requisite in operation to a hepatoma patient. Such method nay prevent a decrease in the quality of image based upon the time delay and intestine gas; also, because the patient exposure dose and contrast media usage may be reduced, it is a new, valuable way to decide the operational matter of hepatic artery embolization on a pre-angiography.

Development and Validation of MRI-Based Radiomics Models for Diagnosing Juvenile Myoclonic Epilepsy

  • Objective: Radiomic modeling using multiple regions of interest in MRI of the brain to diagnose juvenile myoclonic epilepsy (JME) has not yet been investigated. This study aimed to develop and validate radiomics prediction models to distinguish patients with JME from healthy controls (HCs), and to evaluate the feasibility of a radiomics approach using MRI for diagnosing JME. Materials and Methods: A total of 97 JME patients (25.6 ± 8.5 years; female, 45.5%) and 32 HCs (28.9 ± 11.4 years; female, 50.0%) were randomly split (7:3 ratio) into a training (n = 90) and a test set (n = 39) group. Radiomic features were extracted from 22 regions of interest in the brain using the T1-weighted MRI based on clinical evidence. Predictive models were trained using seven modeling methods, including a light gradient boosting machine, support vector classifier, random forest, logistic regression, extreme gradient boosting, gradient boosting machine, and decision tree, with radiomics features in the training set. The performance of the models was validated and compared to the test set. The model with the highest area under the receiver operating curve (AUROC) was chosen, and important features in the model were identified. Results: The seven tested radiomics models, including light gradient boosting machine, support vector classifier, random forest, logistic regression, extreme gradient boosting, gradient boosting machine, and decision tree, showed AUROC values of 0.817, 0.807, 0.783, 0.779, 0.767, 0.762, and 0.672, respectively. The light gradient boosting machine with the highest AUROC, albeit without statistically significant differences from the other models in pairwise comparisons, had accuracy, precision, recall, and F1 scores of 0.795, 0.818, 0.931, and 0.871, respectively. Radiomic features, including the putamen and ventral diencephalon, were ranked as the most important for suggesting JME. Conclusion: Radiomic models using MRI were able to differentiate JME from HCs.

  • As an advancement of medical imaging modalities and analyzing software with multi-function, active researches to acquire high contrast and high resolution image being done. In recently, development of medical imaging modalities like as Computed Tomography (CT) and Magnetic Resonance Imaging (MRI) is aiming to display anatomical structure more accuracy and faster. Thus, one of the important areas in CT today is the use of CT scanner for the quantitative evaluation of 3-D reconstruction images from 2-D tomographic images. In CT system, the effective slice thickness and the quality of 3-D reconstructed image will be influenced by imaging acquisition parameters (e.g. pitch and scan mode). In diagnosis and surgical planning, the accurate distance measurements of 3-D anatomical structures play an important role and the accuracy of distance measurements will depend on the acquisition parameters such as slice thickness, pitch, and scan mode. The skull phantom was scanned with SDCT for various acquisition parameters and acquisition slice thicknesses were 3 and 5 mm, and reconstruction intervals were 1, 2, and 3 mm to each pitch. 3-D visualizations and distance measurements were performed with PC based 3-D rendering and analyzing software. Results showed that the image quality and the measurement accuracy of 3-D SDCT images are independent to the reconstruction intervals and pitches.

  • This study was purpose to assessment of the resolution characteristics by using ATS 535H Basic quality assurance (QA) phantom for ultrasound. The ultrasound equipment was used Logiq P6 (Ultrasound, GE Healthcare System, Chicago, IL, USA). And the ultrasound transducer were used Convex 4C (4~5.5 MHz), Linear 11L (10~13 MHz), Sector 3SP (3~5.5 MHz) probe. As for the noise power spectrum (NPS) comparison results by using ATS 535H Basic QA ultrasound phantom and Convex 4C, Linear 11L, Sector 3SP probe. The NPS value of the Convex 4C probe image was 0.0049, Linear 11L probe image was 0.0049, Sector 3SP probe image was 0.1422 when the frequency is 1.0 mm-1. The modulation transfer function (MTF) comparison results by using ATS 535H Basic QA ultrasound phantom and Linear 11L probe the MTF value of the 3 cm focus image was 0.7511 and 4 cm focus image was 0.9001 when the frequency is 1.0 mm-1. This study was presented characteristics of spatial resolution a quantitative evaluation methods by using ultrasound medical images for QA of ultrasound medical QA phantom. The quality control (QC) for equipment maintenance can be efficiently used in the clinic due to the quantitative evaluation of the NPS and MTF as the standard methods. It is meaningful in that it is applied mutatis mutandis and presented the results of physical resolution characteristics of the ultrasound medical image.

Correlation of Left Atrial Enlargement and Metabolic Syndrom Risk Factors in Echocardiography (심장초음파검사에서 좌심방 확장과 대사증후군 요인과의 상관관계)

  • This study investigated the relationship between metabolic syndrom risk factors to patients with left atrial enlargement according to the results of echocardiography. Male were 269(8.9%) and female were 46(1.5%) that of the total 315 patients (10.5%) with left atrial enlargement. In the obesity factors analysis, BMI, WC, and WHtR were associated with left atrial enlargement. Among them, WHtR included the many obese people with left atrial enlargement. In the analysis of blood pressure, The systolic blood pressure was the highest in the borderline hypertension group in the left atrial enlargement and the diastolic blood pressure was the highest in the normal range group in the left atrial enlargement. In the blood result analysis, there was no correlation with left atrial enlargement that total cholesterol, HDL, LDL cholesterol, triglyceride, and fasting blood glucose level were within the normal range. Therefore, obesity and systolic blood pressure were associated with the left atrial enlargement group. However, the systolic blood pressure did not show pathological condition due to borderline hypertension, and the distribution of the left atrial enlargement group was smaller than that of obesity. Obesity, which includes the largest number of left atrial enlargement group, has been identified to be the highest risk factor for left atrial enlargement.
