근관 밀폐도에 미치는 도말층 및 상아질 접착강화제의 영향 (EFFECTS OF SMEAR LAYER AND DENTIN PRIMERS ON THE SEALING ABILITY OF ROOT CANAL)

  • The purpose of this study was to evaluate effects of smear layer and dentin primers on the sealing ability of root canals. 126 extracted human teeth with single, straight canals and mature apices were used. The Samples were first classified into six groups as follows: presence of smear layer; absence of smear layer; Scotchbond Bond Multi-Purpose; All Bond 2; Mac Bond 2; Clearfil Liner Bond 2. A Positive control was also established. All teeth except the control group were then obturated with thermoplasticized gutta-percha and AH26. Electrochemical and dye penetration technique were later used to evaluate the degree of micro leakage through the root canal. Seventy teeth were then immersed in a 1% potassium chloride solution and An external power supply(DC 10 V) was then applied to the circuit for the electrochemical microleakage test. The degree of Microleakage was determined over period of 28 days before being evaluated. In total, 48 teeth were submitted to the dye infiltration technique. All specimen were suspended in 2% methylene blue dye for 1 week before being longitudinally split. The degree of dye infiltration was measured under a stereo microscope at ${\times}10$ magnification and evaluated. The results were as follows: 1. Apical microleakage increased throughout the test period in all group and one group having a smear layer showed a dramatic increase under electrochemical test (p<0.05). In the group having smear layer, the degree of apical microleakage was the highest, and the micro leakage was much higher than in the smear layer removed group in electrochemical test (p<0.05). Scotchbond Multi-Purpose, All Bond 2, Mac Bond 2 and Clearfil Liner Bond 2 showed lower micro leakage than one group having smear layer. The All Bond 2 and Clearfil Liner Bond 2 treated groups showed the lowest microleakage in electrochemical test (p<0.05). 2. There was no significant difference between the experimental groups in dye penetration technique. These results suggested that the removal of the smear layer from root canal and concomitantly the application of dentin primer into root canal could improve the sealing ability of root canal obturation.

접착시스템의 소수성이 Low-shrinkage silorane resin과 상아질의 미세인장강도에 미치는 영향 (Effect of adhesive hydrophobicity on microtensile bond strength of low-shrinkage silorane resin to dentin)

  • 연구목적: 본 연구의 목적은 다양한 소수성을 지닌 최신 상아질 접착시스템과 저수축 silorane 레진의 미세인장결합강도를 평가하는 것이다. 연구 재료 및 방법: 36개의 갓 발치된 제3대구치를 이용했다. Low-speed diamond saw를 사용하여 교합면에 평행하게 치관을 잘라 middle dentin을 노출시켰다. 치아를 무작위로 9 group으로 나눴다. Silorane self-etch adhesives (SS), SS + phosphoric acid etching (SS + pa), Adper Easy bond (AE), AE + Silorane system bonding (AE + SSb), Clearfil SE bond (CSE), CSE + SSb, All-Bond 2 (AB2), AB2 + SSb, All-Bond 3 (AB3). 접착제를 적용한 후에 Filtek LS (3M ESPE)를 2 mm씩 3회 적층충전하였다. 각 층은 40s씩 광중합하였다. 0.8 mm ${\times}$ 0.8 mm stick을 Micro Tensile Tester로 1 mm/min cross-head speed의 인장력을 가하였다. 파절양상를 관찰하기 위해 광학현미경을 이용하였다. 5가지 접착제의 소수성정도를 결정하기위해 water sorption test하였다. 결과: silorane 레진과 5가지 접착제의 ${\mu}TBS$: SS, 23.2 ${\pm}$ 6.9 MPa; CSE, 19.4 ${\pm}$ 4.4 MPa; AB3, 30.3 ${\pm}$ 4.0 MPa; AB2와 AE, no bond. Additional layering of SSb: CSE + SSb, 26.2 ${\pm}$ 10.3 MPa; AB2 + SSb, 33.9 ${\pm}$ 7.3 MPa; AE + SSb, no bond. 높은 ${\mu}TBS$는 cohesive failure와 관련있었다. SS는 낮은 가장 낮은 water sorption을 보였고 다음으로 AB3, AE, CSE, AB2 순서였다. AE는 가장 높은 용해도를 나타냈고 다음으로 CSE, AB2였다. 결론: 접착제의 소수성이 증가할수록, silorane 레진의 접착강도도 증가하였다. 비전용접착제 위에 silorane adhesive bonding을 layering하는 것은 AB2 + SSb 그룹에서만 결합강도를 유의하게 증가시켰다. AB3는 SS와 유사한 ${\mu}TBS$ & water sorption을 나타냈다. 따라서 AB3는 siloran resin을 접착시키는데 SS를 대체할만한 경쟁력있는 접착제이다.

The effects of dentin bonding agent formulas on their polymerization quality, and together with tooth tissues on their microleakage and shear bond strength: an explorative 3-step experiment

  • PURPOSE. Bonding agents (BA) are the crucial weak link of composite restorations. Since the commercial materials' compositions are not disclosed, studies to formulize the optimum ratios of different components are of value. The aim of this study was to find a proper formula of BAs. MATERIALS AND METHODS. This explorative experimental in vitro study was composed of 4 different sets of extensive experiments. A commercial BA and 7 experimental formulas were compared in terms of degree of conversion (5 experimental formulas), shear bond strength, mode of failure, and microleakage (3 experimental formulas). Statistical analyses were performed (${\alpha}$=.05). The DC of selected formula was tested one year later. RESULTS. The two-way ANOVA indicated a significant difference between the shear bond strength (SBS) of two tissues (dentin vs. enamel, P=.0001) in a way that dentinal bonds were weaker. However, there was no difference between the four materials (P=.283). The adhesive mode of failure was predominant in all groups. No differences between the microleakage of the four materials at occlusal (P=.788) or gingival (P=.508) sites were detected (Kruskal-Wallis). The Mann-Whitney U test showed a significant difference between the microleakage of all materials (3 experimental formulas and a commercial material) together at the occlusal site versus the gingival site (P=.041). CONCLUSION. A formula with 62% bisphenol A-glycidyl methacrylate (Bis-GMA), 37% hydroxy ethyl methacrylate (HEMA), 0.3% camphorquinone (CQ), and 0.7% dimethyl-para-toluidine (DMPT) seems a proper formula for mass production. The microleakage and SBS might be respectively higher and lower on dentin compared to enamel.

와동 세척제가 상아질 결합제의 결합에 미치는 영향 (EFFECT OF CAVITY DISINFECTANT ON THE BOND STRENGTH AND MICROLEAKAGE OF DENTIN BONDING AGENTS)

  • 와동 형성시 박테리아에 감염된 상아질이나 법랑질을 완전히 제거하지 않는 것은 수복학적 측면에서 볼 때 잠재적 문제점이 된다. 수복물 하방에서의 박테리아의 활동에 의해 술후 과민반응, 치수의 염증. 이차 우식 등이 진행될 수 있다. 와동세척제는 와동 형성 후 잔존 박테리아의 제거를 위해 효과적으로 사용될 수 있지만 상아세관에 잔유물을 남겨 상아질 결합제와 치질간의 긴밀한 결합을 방해하여 미세누출을 증가시키고 결합강도를 약화시킬 가능성이 있다. 본 연구는 2% 클로르헥시딘을 함유하는 와동 세척제인 $Consepsis^{(R)}$(Ultradent, USA)가 $Adper^{TM}$ $Scotchbond^{TM}$ Multi-Purpose(3M ESPE, USA), $Adper^{TM}$ Single Bond(3M ESPE, USA), $Adper^{TM}$ $Prompt^{TM}$ $L-Pop^{TM}$(3M ESPE, USA)의 결합에 미치는 영향을 평가하기 위하여 발거된 제 3대구치 120개를 이용하여 $Consepsis^{(R)}$ 와동 세척제를 사용한 군과 사용하지 않은 군으로 나누어 전단결합강토 측정과 수복물 변연에 나타나는 미세누출 정도를 색소 침투 평가 방법으로 분석함으로써 다음과 같은 결론을 얻었다. 1. $Consepsis^{(R)}$ 와동 세척제의 사용 유무는 $Adper^{TM}$ $Scotchbond^{TM}$ Multi-Purpose, $Adper^{TM}$ Single Bond, $Adper^{TM}$ $Prompt^{TM}$ L-Pop의 전단결합강토에 있어 통계학적으로 유의한 차이를 보이지 않았다(p>0.05). 2. $Consepsis^{(R)}$와동 세척제의 사용 유무는 $Adper^{TM}$ $Scotchbond^{TM}$ Multi-Purpose, $Adper^{TM}$ Single Bond, $Adper^{TM}$ $L-Pop^{TM}$ $L-Pop^{TM}$의 미세누출 양상에 있어 통계학적으로 유의한 차이를 보이지 않았다(p>0.05).

Flowable Composite Resin의 미세변연누출 및 전단결합강도 (MICROLEAKAGE AND SHEAR BOND STRENGTH OF FLOWABLE COMPOSITE RESIN)

  • Flowable composite resin has lower filler content, increased flow, and lower modules of elasticity. It is suggested that flowable composite resin can be bonded to the tooth structure intimately and absorb or dissipate the stress. Therefore, it may be advantageous to use flowable composite resin for the base material of class II restoration and for the class V restoraton. The purpose of this study was to evaluate the microleakage and shear bond strength of four flowable composite resins (Aeliteflo, Flow-It, Revolution, Ultraseal XT Plus) compared to Z100 using Scotchbond Multi Purpose dentin bonding system. To evaluate the microleakage, notch-shaped class V cavities were prepared on buccal and lingual surfaces of 80 extracted human premolars and molars on cementum margin. The teeth were randomly divided into non-thermocycling group (group 1) and thermocycling group (group 2) of 40 teeth each. The experimental teeth of each group were randomly divided onto five subgroups of eight samples (sixteen surfaces). The Scotchbond Multi-Purpose and composite resin were applied for each group following the manufacturer's instructions. the teeth of group 2 were thermocycled five hundred times between 5$^{\circ}C$ and 55$^{\circ}C$. The teeth of group 2 were placed in 2% methylene blue dye for 24 hours, then rinsed with tab water. The specimens were embedded in clear resin, and sectioned longitudinally with a diamond saw. The dye penetration on each of the specimen were observed with a stereomicioscope at $\times$20 magnification. To evaluate the shear bond strength, 60 teeth were divided into five groups of twelve teeth each. The experimental teeth were ground horizontally below the dentinoenamel junction, so that no enamel remained. After applying Scotchbond Multi-Purpose on the dentin surface, composite resin was applied in the shape of cylinder. The cylinder was 4mm in diameter and 2mm in thickness. Shear bond strength was measured using Instron with a cross-head speed of 0.5mm/min. After shear bond strength measurement, mode of failure was evaluated with a stereomicroscope at $\times$30 magnification. All data were statistically analyzed by One Way ANOVA and Student-Newman-Keuls method. The correlation between microleakage and shear bond strength was analyzed by linear regression. The results of this study were as follows ; 1. In non-thermocycling group, the leakage value of Z100 was significantly lower than those of flowable composite resins at the enamel and dentin margin, margin, except that Revolution showed the lower leakage value than that of Z100 at the dentin margin (p<0.05). 2. In thermocycling group, the leakage values of Z100 and Ultraseal XT Plus were lower than those of other subgroup at the enamel and dentin margin, except that Flow-It showed the lower leakage value than that of Ultraseal XT Plus at the dentin margin (p<0.05). 3. The leakage value of Z100 and Ultraseal XT Plus in thermocycling group were not higher than that in non-thermocycling group at the enamel margin. The leakage value of Z100 in thermocycling group was not higher than that in non-thermocycling group at the dentin margin (p<0.05). 4. As for the shear bond strength measurement, there were no statistically significant differences among groups (p<0.05). The shear bond strengths given in descending order were as follows: Z100(16.81$\pm$2.98 MPa), Flow-It(14.8$\pm$4.43 MPa), Aeliteflo(14.34$\pm$3.69 MPa), Revolution(13.46$\pm$4.23 MPa), Ultraseal XT Plus(12.83$\pm$3.16 MPa). 5. Failure modes of all specimens were adhesive failures. 6. There was no correlation between microleakage and shear bond strength.

상아질 표면처리가 글라스 아이오노머 시멘트의 결합강도에 미치는 영향에 관한 연구 (AN EXPERIMENTAL STUDY ON SHEAR BOND STRENGTH OF GLASS IONOMER CEMENT TO DENTIN SURFACE FOLLOWING SURFACE CONTIONING)

  • The purpose of this study was to evaluate the shear bond strength of glass ionomer cement(Ketacfil, ESPE, Co.) against dentin surface which had been treated with surface conditioning agents(distilled water, 5% sodium hypochlorite solution, Ketac - conditioner, 40% polyacrylic acid). In this study, 60 human molars with sound and healthy crown portion which were previously extracted for orthodontic or periodontal problem. The dentin surfaces of these teeth were exposed with wet trimmer and polished with 150 - grit and 600 - grit silicon carbide paper and the teeth were divided into four groups(15 teeth per group) according to the following surface conditioning methods. Group I : Surface treatment with distilled water as control group. Group II : Surface conditioning with 5% sodium hypochlorite solution. Group III : Surface conditioning with Ketac conditioner. Group IV : Surface conditioning with 40% polyacrylic acid. The shear bond strengths were measured by Autograph(Shimatzu Co. Japan). The result of the evaluations were then subjected to statistical analysis using one - way analysis of variance and Duncan test and the results were as follows : 1. The shear bond strength accrding to the dentin surface conditioning conditions was highest in Ketac conditioner group, with measurements of $44.44{\pm}0.74(kg/cm^2)$ and lowest in the distilled water group, with measurements of $28.84{\pm}0.88(kg/cm^2)$. 2. Statistically significant differences were found between surface conditioning with 5% sodium hypochlorite solution group or Ketac conditioner group and distilled water group(P<0.01). 3. Also, statistically significant difference was found between surface conditioning with distilled water group and 40% polyacrylic acid group(P<0.05). 4. Overall difference in statistical significance between the groups was not found (P<0.05). 5. Fractured dentin surface treated with conditioning solutions showed cohesive fracture. 6. Distilled water group and 5% sodium hypochlorite solution group removed the smear layer less effectively. 7. Conditioning dentin with Ketac conditioner and 40% polyacrylic acid resulted in the removal of a significant amount of the smear layer without removing the tubular plugs and dissolving the peritubular dentin.

  • The purpose of this experiment was to determine the effects of etching with a Nd : YAG Laser on dentin, or porcelain surface on the bond strength with composite resin. The dentin specimens were devided into the following 4 groups. D1 : No treatment D2 : Etched with 10% phosphoric acid D3 : Laser etchded with 1W, 20PPs D4 : Laser etched with 2W, 20PPS The procelain specimens were devided into the following 4 groups. P1 : diamond roughened P2 : etched with HF acid P3 : Laser etched with 2W, 20PPS P4 : Laser etched with 3W, 20PPS All specimens were veneered with resin. One half of the specimens were stored in $37^{\circ}C$ water for one day and the other half were thermocycled 1000 times at temperature of $5^{\circ}C\;to\;55^{\circ}C$ at 20 seconds intervals. After that, the bonding strength of composite resin to the dentin and porcelain was measured. The surface treated state and fractured state were observed with SEM. The following results were obtained. 1. In the dentin specimens, the bond strength of group D2 was highter than that of groups D1 and D3 in the case of the specimens stored in $37^{\circ}C$ water for one day, there was a statistically significant difference between group D2 and D1, D3 (P<0.05). The bonding strength of the specimens that were thermocycled decreased in the following order : group D2,D4,D3 and then D1. 2. In the porcelain specimens, the bonding strength of groups P1,P2 were higher than that of group P3 in the case of the specimens stored in $37^{\circ}C$ water for one day (P<0.05). The bonding strength of the specimens of being thermocycled decreased in the following order : group P2,P1,P4 and then P3. 3. The groups of high bond strength had a rougher surface and a high level of microporosity with SEM findings.

공기건조된 상아질에 대한 수분함유 primer의 재습윤효과 (REWETTING EFFECT OF WATER-BASED PRIMER ON THE AIR-DRIED DENTIN)

  • The purpose of this study was to evaluate the rewetting effect of water-based primer on the air-dried dentin. In this in vitro study, freshly extracted non-caries human molars and three-step adhesive system(SBMP) were used. Freshly extracted non-caries human molars and three-step adhesive system(SBMP) were used. Flat occlusal dentin surface were prepared using low-speed diamond saw, Prepared teeth were randomly divided into three groups. Group 1.(W): etched(35% phosphoric acid for 15s) and blot-dried, Group 2.(5D): 5s air-dried, Group 3.(30D): 30s ail-dried, To obtain color contrast in CLSM observation, primer was mixed with rhodamine B and bonding resin was mixed with fluorescein. Microscopic sample of each group were obtained after longitudinal section. Morphological investigation of resin-dentin interface and thickness of hybrid layer measurement using CLSM were done. Microtensile bond strength for each specimen was measured. Specimen were observed under microscope to examine the failure patterns of interface between resin and dentin. The results of this study were as follows: 1. The results(mean) of Thickness of hybrid layer were W:19.67, 5D:20.9, 30D:10$\mu\textrm{m}$. Only 30D had statistically significant differences to Wand 5D(P<0.05). 2. The results(mean) of Microtensile bond strength were W:16.02, 5D:14.69, 30D:11.14MPa. Only 30D had statistically significant differences to Wand 5D(P<0.05). 3. There were positive correlation between Thickness of hybrid layer and microtensile bond strength(P<0.05).

타액에 의한 오염이 상아질 접착제의 미세전단결합강도에 미치는 영향 (EFFECT OF SALIVARY CONTAMINATION OF TEETH ON MICROTENSILE BOND STRENGTH OF VAR10US DENTIN BONDING SYSTEMS.)

  • The purpose of this study was to evaluate the effect of salivary contamination of teeth on bonding efficacy of self-priming and self-etching DBSs. The materials used were Single Bond(SB, self-priming system, 3M), Unifil Bond(UB, self-etching system, GC), and Scotchbond Multi-Purpose Plus(SM, 3M) as control. Forty five human molars randomly allocated to three groups as dentin bonding systems tested and embedded in epoxy resin. Then the specimens were wet-ground to expose flat buccal enamel surface or flat occlusal dentin surface and cut bucco-lingually to form two halves with slow speed diamond saw. One of them was used under non-contamination, other under contamination with saliva. The bonding procedure was according to the manufacturer's directions and resin composite(Z-100, 3M Dental Products, St. Paul, MN) was built-up on the bonded surface 5mm high. The specimens were ground carefully at the enamel-composite interface with fine finishing round diamond bur to create an hour-glass shape yielding bonded surface areas of $1.5{\pm}0.1\textrm{mm}^2$. The specimens were bonded to the modified microtensile testing apparatus with cyanoacrylate, attached to the universal testing machine and stressed in tension at a CHS of 1mm/min. The tensile force at failure was recorded and converted to a tensile stress(MPa). Mean values and standard deviations of the bond strength are listed in table. One-way ANOVA was used to determine significant difference at the 95% level. The bond strength of SBMP and SB were not affected by salivary contamination, but that of UB was significantly affected by salivary contamination. These results indicate that DBSs with total etch technique seems less likely affected by salivary contamination in bonding procedure.
