Radiographic evaluation of the maxillary sinus prior to dental implant therapy: A comparison between two-dimensional and three-dimensional radiographic imaging

  • Purpose: This study was performed to evaluate the diagnostic efficacy of panoramic radiography and cone-beam computed tomography (CBCT) in detecting sinus pathology. Materials and Methods: This study was based on a retrospective evaluation of patients who had undergone both a panoramic radiograph and a CBCT exam. A total of 100 maxillary sinuses were evaluated. Four examiners with various levels of expertise evaluated the images using a five-point scoring system. Receiver operating characteristic (ROC) curve analysis was performed to evaluate the diagnostic efficacy of the two modalities. The image analysis was repeated twice, with at least two weeks between the evaluation sessions. Interobserver reliability was assessed using Cronbach's alpha, and intraobserver reliability was assessed using Cohen's kappa. Results: Maxillary sinus pathology was detected in 72% of the patients. High interobserver and intraobserver reliability were observed for both imaging modalities and among the four examiners. Statistical analyses using ROC curves demonstrated that the CBCT images had a larger area under the curve (0.940) than the panoramic radiographs (0.579). Conclusion: Three-dimensional evaluation of the sinus with CBCT was significantly more reliable in detecting pathology than panoramic imaging.

  • The purpose of this study was to investigate the changes of the shape of the panoramic innominate line and also to examine the relation of the panoramic innominate line to maxillary posterior wall according to positional changes in panoramic radiographs of the patients who had no pathoses in the maxillary sinus and adjacent bony structures. Thus these panoramic radiographs were obtained and evaluated in standard, forward 10mm, backward 10mm, chin down 10°, and chin up 10° positions. And the following results were obtained : 1. Panoramic innominate lines were not changed in shape according to various positional changes in panoramic radiography. 2. The panoramic innominate line was observed to be more clear images and decreased distance to the posterior wall of maxillary sinus at forward 10mm and chin down 10° positions. And the panoramic innominate line was observed more blurred images and increased distance to the posterior wall of maxillary sinus at backward 10mm and chin up 10° positions.

  • Purpose: Several panoramic indices have been suggested to assess bone quality from the morphology and width of mandibular cortex on panoramic radiography. The purpose of this study was to compare dental implant failure group with control group in panoramic mandibular index (PMI), mandibular cortical index (MCI), and gonion index (GI) and to determine the effect of these panoramic indices on dental implant failure. Materials and Methods: A case-control study was designed. Test group (n=42) consisted of the patients who had their implants extracted because of peri-implantitis. Control group (n=139) consisted of the patients who retained their implants over one year without any pathologic changes and had been followed up periodically. They had dental implants installed in their mandibles without bone augmentation surgery from 1991 to 2001. The following measures were collected for each patients: 1) PMI, MCI, and GI were measured twice at one-week interval on preoperative panoramic views; and 2) age, sex, implant length, implant type, installed location, occluding dentition state, and complication were investigated from the chart record. Results: The PMI showed moderate level of repeatability. The intra-observer agreement of MCI and GI were good. There was statistically significant difference in PMI between two groups. There were significant different patterns of distribution of MCI and GI between two groups. Among the panoramic indices, PMI and MCI showed significant correlation with dental implant failure. Conclusion: Panoramic indices can be used as reference data in estimating bone quality of edentulous patients who are to have implants installed in their mandibles.

6세 아동을 위한 파노라마방사선사진 상층의 연구 (A study of panoramic focal trough for the six-year-old child)

  • Purpose: To make a focal trough (image layer) for an average maxillary dental arch of 6-year-old korean in panoramic radiography. Materials and Methods : Phantom for the maxillary dental arch was designed using intercanine width, intermolar width, tooth size, and interdental spacing to record the data of 6-year-old child. The characteristics of pre-corrected panoramic machine (for adult) was evaluated using the phantom, resolution test pattern for margin of the image layer, and metal ball for the center of the image layer. Panoramic image layer of the child was developed by means of decreasing the speed of film-cassette and positioning the phantom backwards, and then the characteristics of post-corrected panoramic machine (for child) were reevaluated. Results: At post-corrected panoramic image layer, beam projection angles at all interdental areas increased for about 2.6-3.8°, the position of the image layer was shifted toward the rotation center for about 2.5 mm at the deciduous central incisior area. The width of image layer decreased at all areas. Conclusion : Increased beam projection angle will reduce the disadvantage of tooth overlap, and the same form between the center of the image layer and dental arch will improve image resolution.

상악 정중과잉치 진단을 위한 cone beam CT의 유효선량 (EFFECTIVE DOSE FROM CONE BEAM CT FOR IMAGING OF MESIODENS)

  • 목적: 상악 정중과잉치의 영상진단 시 시행되는 콘빔 전산화단층촬영술에 대한 환자의 방사선 피폭을 유효선량으로 평가하고, 치근단 및 파노라마방사선촬영술의 방사선피폭과 비교하고자 하였다. 재료 및 방법: 선량 측정용 두경부 마네킨의 23부위에 열형광선량계 소자를 위치시키고 해당 방사선촬영술을 시행하였다. 열형광선량계 판독기로 흡수선량를 측정하고 방사선 조사된 조직의 비율을 곱하여 방사선 가중선량을 구한 후, 국제방사선방호위원회에서 2007년에 공지한 조직 가중계수를 이용하여 유효선량을 구하였다. 결과: 조직 및 기관의 흡수선량은 콘빔 전산화단층촬영술, 치근단방사선촬영술 그리고 파노라마방사선촬영술에서 뺨, 하악체, 이하선에서 가장 높았다. 유효선량은 콘빔 전산화단층촬영에서는 48 ${\mu}Sv$, 치근단방사선촬영술에서는 2 ${\mu}Sv$ 그리고 파노라마방사선촬영술에서는 18 ${\mu}Sv$였다. 결론: 상악 정중과잉치 진단 시, 추가적인 진단학적 정보를 제공하지만, 콘빔 전산화단층촬영술은 일반 치근단 및 파노라마 방사선촬영술보다 방사선피폭이 크다.

Craniometaphyseal dysplasia: Report of 2 cases with an emphasis on panoramic imaging features

  • Craniometaphyseal dysplasia (CMD) is a rare hereditary disorder characterized by hyperostosis of the craniofacial bones and flared metaphyses of the long bones. Although some reports have described the dentomaxillofacial characteristics of CMD, including increased density of the jaw, malocclusion, and delayed eruption of the permanent teeth, only a few studies have reported the distinct imaging features of CMD on panoramic radiography. This report presents 2 cases of confirmed CMD patients with an emphasis on panoramic imaging features. The patients' images revealed hyperostosis and sclerosis of the maxilla and mandibular alveolar bone, but there was no change in the mandibular basal bone. In both cases, the mandibular condyle heads exhibited a short clubbed shape with hyperplasia of the coronoid process. For patients without clear otorhinolaryngological symptoms, common radiologic features of CMD could be visualized by routinely-taken panoramic radiographs, and further medical examinations and treatment can be recommended.

Perceptions and attitudes of dental hygienists toward radiation safety and protection in the Republic of Korea

  • To investigate the perceptions and attitudes of dental hygienists toward radiation safety management in Korea. A total of 800 dental hygienists were randomly selected for an anonymous survey, and 203 of them participated. The questionnaire items included the following: sex, career period, type of installed radiographic equipment, recognition of the diagnostic reference level (DRL), participation in radiation safety education, and attitudes toward radiation protection for both patients and dental hygienists. The participants were divided into two groups according to their years of experience (< 10 years versus ≥ 10 years). The difference between the groups was investigated according to frequency distribution. Fisher's exact test or Pearson's chi-square (𝛘2) test was used as appropriate. A regression analysis was performed to investigate the impact of wearing a thyroid collar for personnel protection during patient radiation exposure. The types of installed radiographic equipment included panoramic radiography (96.1%), cephalometric radiography (76.9%), intraoral radiography (72.9%), and cone-beam computed tomography (69.5%). Significant differences were observed in the learning pathway for the DRL (Fisher's exact test, p < 0.05), satisfaction with radiation safety education (Pearson's 𝛘2 test = 5.3975, Pr = 0.02), and use of personnel radiation monitoring systems (Pearson's 𝛘2 test = 18.1233, Pr = 0.000) between the groups. Significant differences were also observed in personnel protection using a thyroid collar and patient protection during panoramic radiography (odds ratio = 14.2). Dental hygienists with more than 10 years of experience were more satisfied with radiation safety education and more interested in radiation monitoring. Considering career experience, customized, continuous, and effective radiation safety management education should be provided.

Dental Radiography for Age Estimation: A Scoping Review

  • Purpose: This study was to investigate the types of imaging modalities, analytical methods for age estimation, and the age of the subjects in research on age estimation using dental radiography through a scoping review, and to investigate the overall trends in age estimation studies. Materials and Methods: A scoping review was designed according to the Arksey and O'Malley guidelines and the Preferred Reporting Items for Systematic Review and Meta-analysis (PRISMA) statement. Three electronic databases were used as search sources (Medline, Embase, and Cochrane Library). Studies were classified according to the three main components of the research question. "What are the imaging modalities, analytical methods, and target age in dental imaging-based age estimation studies?" Result: The final 198 studies were selected by two reviewers. The most common imaging modality used in studies was panoramic radiography (69.7%), and studies using cone-beam computed tomography have increased over time. Analytical methods for age estimation were 62.6% in studies based on tooth development and 26.3% in studies using pulp/tooth ratio. The subject age was 27.8% for children and 27.3% for adults. Studies conducted in all age groups comprised the smallest category (5.2%). Conclusion: Panoramic radiography has been the most used types of imaging modalities for age estimation, and the most common analytical method was analysis of tooth development. Most studies targeted specific ages, and very few involved all age groups. Dental age estimation studies should be carried out with appropriate consideration of the imaging modality that is analyzed, the methods that are used, and the age that is targeted.

Comparative Study of the Effective Dose from Panoramic Radiography in Dentistry Measured Using a Radiophotoluminescent Glass Dosimeter and an Optically Stimulated Luminescence Detector

  • Accurate measurement of the absorbed dose and the effective dose is required in dental panoramic radiography involving relatively low energy with a rotational X-ray tube system using long exposures. To determine the effectiveness of measuring the irradiation by using passive dosimetry, we compared the entrance skin doses by using a radiophotoluminescent glass dosimeter (RPL) and an optically stimulated luminescence detector (OSL) in a phantom model consisting of nine and 31 transverse sections. The parameters of the panoramic device were set to 80 kV, 4 mA, and 12 s in the standard program mode. The X-ray spectrum was applied in the same manner as the panoramic dose by using the SpekCalc Software. The results indicated a mass attenuation coefficient of $0.008226cm^2/g$, and an effective energy of 34 keV. The equivalent dose between the RPL and the OSL was calculated based on a product of the absorbed doses. The density of the aluminum attenuators was $2.699g/cm^3$. During the panoramic examination, tissue absorption doses with regard to the RPL were a surface dose of $75.33{\mu}Gy$ and a depth dose of $71.77{\mu}Gy$, those with regard to the OSL were surface dose of $9.2{\mu}Gy$ a depth dose of $70.39{\mu}Gy$ and a mean dose of $74.79{\mu}Gy$. The effective dose based on the International Commission on Radiological Protection Publication 103 tissue weighting factor for the RPL were $0.742{\mu}Sv$, $8.9{\mu}Sv$, $2.96{\mu}Sv$ and those for the OSL were $0.754{\mu}Sv$, $9.05{\mu}Sv$, and $3.018{\mu}Sv$ in the parotid and sublingual glands, orbit, and thyroid gland, respectively. The RPL was more effective than the OSL for measuring the absorbed radiation dose in low-energy systems with a rotational X-ray tube.

Residual bone height measured by panoramic radiography in older edentulous Korean patients

  • PURPOSE. The aim of this study was to obtain statistical data on the residual bone height at different natural tooth positions by panoramic radiography in edentulous Korean patients aged 60-90 years. MATERIALS AND METHODS. The study included the diagnostic panoramic radiographs of 180 randomly selected edentulous patients without systemic diseases affecting bone. The radiographic selection criteria included absence of obvious facial asymmetry, clearly visible anatomic structures, and no surgical and fracture history. The panoramic radiographs of 79 patients met these criteria and were used in the analysis. The same researcher processed all the radiographs by using a standardized method. The height of the residual bone was measured at 18 predetermined sites (7 in the maxilla and 11 in the mandible) on digitized and printed radiographs by using a Digimatic caliper, triangle, and ruler. Gender- and age-related differences were statistically analyzed by using the t-test and rank-sum test (${\alpha}=0.05$). RESULTS. The maxillary residual bone height did not show significant gender-related differences, but male patients had significantly higher residual bone in the mandible(P<.05). No significant height differences at the measured sites were noted among the 60s, 70s, and 80s age groups. CONCLUSION. Dentists should pay greater attention to older female edentulous patients because they are more prone to rapid residual bone resorption. Residual bone resorption may not be affected by age.
