Horizontal root fracture (HRF) is a result of trauma to teeth and periodontium, which implies severe injury to cementum, dentin, and pulp. This is a rare case of HRF in the maxillary lateral incisor of a 62-year-old male who only presented persistent gingival swelling, fistula, and dull pain at first. An apical radiolucency of unknown origin turned out to be a result of hidden HRF at the coronal third level that was later visualized radiographically during endodontic treatment. The tooth was scheduled to be extracted upon the patient's agreement. The purpose of this report is to alert clinicians about the importance of diagnosing HRF through thorough clinical and radiographic examinations. Where there is persistent fistula without proper cause, HRF should be considered as a causative factor, and the diagnosis could be effective with aid of cone beam computed tomography, electronic root apex locator, as well as other clinical signs.
Purpose: This study was performed to analyze the clinical and imaging features of contemporary osteomyelitis(OM) and to investigate differences in these features on panoramic radiography according to patients' history of use of medication affecting bone metabolism. Materials and Methods: The records of 364 patients(241 female and 123 male, average age 66.8±14.9 years) with OM were retrospectively reviewed. Panoramic imaging features were analyzed and compared between patients with medication-related OM(m-OM) and those with conventional, medication-unrelated OM(c-OM). Results: The age of onset of OM tended to be high, with the largest number of patients experiencing onset in their 70s. The 2 most frequent presumed causes were antiresorptive medication use (44.2%) and odontogenic origin (34.6%). On panoramic radiographs, a mix of osteolysis and sclerosis was the most common lesion pattern observed (68.6%). Sequestrum, extraction socket, and periosteal new bone formation were found in 143(42.1%), 79 (23.2%), and 24 (7.1%) cases, respectively. The m-OM group exhibited sequestrum and extraction socket more frequently and displayed significantly higher mandibular cortical index values than the c-OM group. Conclusion: We observed some differences in imaging features as shown on panoramic radiography according to the history of antiresorptive medication use. This study may help elucidate the predictive imaging features of medication-related osteonecrosis of the jaw.
Purpose: The research comprehends learning attitude level of Dental Technology Department students, searches variable element to affect and is trying to provide basic material of method arrangement to induce change of positive learning attitude. Methods: It was conducted survey 410 students, who understand purpose of the research, consentient to involve research, in universities which is located in Daegu area, Daejeon City, Gangwon province from Aug., 1stto Nov.,10th2017, Questionnaires were distributed to 410 students totally. it was finally analyzed 374 questionnaires except 36 questionnaires that answer was not sufficient. Results: The positive answer over 'Average (3)' among study attitude factors was presented highly in order of 'Targeting & Execution' (3.351). 'Effective Content Summary' (3.307), 'Study Plan' (3.173), 'Priority and Self-Management' (3.116), and 'Study Habit & Enhancement' (2.925) showed negative trend under average. The five sub-factors of learning attitude were examined according to general characteristics and there were statistically significant differences according to gender, grade, high school of origin, academic aptitude(p<0.01, p<0.001). Conclusion : As male students show more positive learning attitude than female students, we need to develop more special programs to encourage them. Meanwhile we also need to find out a motive to bring positive effect to change the academic aptitude and learning attitude of female students, or methods to improve their learning attitude in a complementary way by their high school, gender and grade.
Intraoperative airway obstruction is perplexing to anesthesiologists because the patient may fall into danger rapidly. A 74-year-old woman underwent an emergency incision and drainage for a deep neck infection of dental origin. She was orally intubated with a 6. 0 mm internal diameter reinforced endotracheal tube by video laryngoscope using volatile induction and maintenance anesthesia (VIMA) with sevoflurane, fentanyl ($100{\mu}g$), and succinylcholine (75 mg). During surgery, peak inspiratory pressure increased from 22 to $38cmH_2O$ and plateau pressure increased from 20 to $28cmH_2O$. We maintained anesthesia because we were unable to access the airway, which was covered with surgical drapes, and tidal volume was delivered. At the end of surgery, we found a longitudinal fold inside the tube with a fiberoptic bronchoscope. The patient was reintubated with another tube and ventilation immediately improved. We recognized that the tube was obstructed due to dissection of the inner layer.

Cavernous sinus thrombosis not only presents with constitutional symptoms including fever, pain and swelling but also with specific findings such as proptosis, chemosis, periorbital swelling, and cranial nerve palsies. It is known to occur secondary to the spread of paranasal sinus infections in the nose, ethmoidal and sphenoidal sinuses. However, paranasal sinus infection of dental origin is rare. The following is a case of cavernous sinus thrombosis due to the spread of an abscess in the buccal and pterygomandibular spaces via buccal mucosal laceration.

The supernumerary tooth which is extra tooth in comparison to normal dentition is one of the developmental problems. The most common type of supernumerary tooth is mesiodens which may cause several complications like delayed eruption, crowding, spacing et al. Moral Incisor Hypomineralization (MIH) describes the clinical appearance of enamel hypomineralization of systemic origin affecting one or more permanent first molars that associated frequently with affected incisors. We report a case of a 6 - year - old boy who visited our clinic for removal of mesiodentes. The patient was diagnosed by mesiodentes and MIH by clinical examination and radiographic examination. Under local anesthesia, Mesiodentes were removed surgically. The demarcated opacities, a feature of MIH, were observed in the removed mesiodentes. After removal of mesiodentes, the maxillary central incisors erupted normally and in order to manage the teeth affected MIH, follow-up and fluoride varnish application were done every 3 months.

Individuals with human immunodeficiency virus (HIV) infection present with unique intraoral manifestations of various neoplasms. Intraoral HIV-associated Burkitt's lymphoma is a rare presentation, especially in patients of Indian origin and may present as an initial sign of HIV. The objective of this paper is to report a rare case of Burkitt's lymphoma in an HIV-positive Indian patient along with a special emphasis on differential diagnosis. A 30-year-old Indian female presented with a solitary, well-defined, exophytic mass extending anteroposteriorly and buccolingually from the 35th to 38th regions with no evidence of intraosseous extension. An incisional biopsy was performed, and histopathology showed sheets of neoplastic lymphoid cells with numerous tingible body macrophages with clear cytoplasm, presenting a starry sky appearance, suggesting a diagnosis of BL. The tumor cells were positive for CD10, CD20, c-myc, and Epstein-Barr virus, with a nearly 100% Ki-67 proliferative index. The patient tested positive for HIV. This report indicates the importance of immunohistochemical analysis to differentiate Burkitt's lymphoma from other similar lesions like diffuse large B-cell lymphoma. Thorough knowledge of the clinical presentation, etiopathogenesis, histopathology, and immunoprofile of intraoral HIV-associated Burkitt's lymphoma is essential among clinicians and pathologists.

Cervicofacial subcutaneous emphysema is defined as the abnormal introduction of air into the subcutaneous tissues of the head and neck. It is mainly iatrogenic and traumatic in origin. Our two case reports are also due to the same cause, but the features of the trauma and the site of the dental treatment are different from the existing reports. A 29-year-old man visited our hospital with facial swelling and pain after experiencing facial trauma in a soccer game. Another 55-year-old woman visited with similar symptoms after replacement of her maxillary anterior fixed prosthesis. In the two cases presented, subcutaneous emphysema was gradually treated with no complications during antibiotic prophylaxis and supportive care. In this paper, we report two cases of traumatic and iatrogenic subcutaneous emphysema and their diagnoses, etiologies, complications, and treatments based on a literature review.

Ectopic eruption should be understood as a change in the course of the normal eruption path of a dental bud at any moment in its origin. Transposition is a unique and extreme form of ectopic eruption. The treatment for ectopic eruption and transposition is various from simple observation to surgical exposure and orthodontic traction, according to direction of erupting tooth degree of developing root apex and eruption space etc. Autotransplantation is transplantation of tooth from one area of the mouth to another in the same individual or is moving a eruption tooth into extraction socket or surgically prepared socket, and autotransplantation is considered as a treatment of choice for the ectopic eruption when orthodontic traction is unable or when tooth movement is limited. These cases which were treated with autotransplantation of maxillary lateral incisor and maxillary canine were reported, and good esthetic and functional result were induced.

Over the past few years, a large number of perforator flaps have been revised by several microsurgeons in the USA, France, Canada and Japan. A perforator flap is a flap of skin or subcutaneous tissue that is based on the dissection of a perforating vessel, which is a perforator. In short, a perforator is a vessel that has its origin in one of the axial vessels of the human body. By reducing any muscle harvesting and trauma to a minimum, perforator flaps aim to minimize donor site morbidity, and by avoiding the transfer of dennervated muscle, the long-term bulk of the free tissue transfer becomes more predictable. There are a finite number of potential perforator flaps in the body, which are based on the named source arteries. The most commonly used perforator flaps are deep inferior epigastric perforator, superior gluteal artery perforator, thoraco dorsal artery perforator, medial sural artery perforator, and anterolateral thigh perforator flap. For a better understanding of perforators as a routine reconstructive procedure in oral and maxillofacial surgery, the definition with nomenclature, classifications with special characteristics, and review points for their individual applications must be learned and memorized by the young doctors in the course regarding the special curriculum periods for the Korean national board of oral and maxillofacial surgery. Perforator flaps have been known to have many advantages, so this review article summarized their applications to the maxillofacial reconstruction in the Korean language.
