The antrolith is a rare disease in the maxillary sinus. We experienced a 67-year-old female whose chief complaint was a intermittent dull pain of clinical, radiological and histological findings, we diagnosed it as antrolith in the left maxillary sinus and obtained results as follows: 1. Three radiopaque substances in the left maxillary sinus was revealed as 'Antroliths'. 2. These antroliths were movable in the left maxillary antrum and combined with maxillary sinusitis. 3. We supposed it internal origin of nidus in this case.
In recent years, many researchers and clinicians found interest in regenerative medicine using induced pluripotent stem cells (iPSCs) with biomaterials due to their pluripotency, which is able to differentiate into any type of cells without human embryo, which of use is ethically controversial. However, there are limitations to make iPSCs from adult somatic cells due to their low stemness and donor site morbidity. Recently, to overcome above drawbacks, dental tissue-derived iPSCs have been highlighted as a type of alternative sources for their high stemness, easy gathering, and their complex (ectomesenchymal) origin, which easily differentiate them to various cell types for nerve, vessel, and other dental tissue regeneration. In other part, utilizing biomaterials for regenerative medicine using cell is recently highlighted because they can modulate cell adhesion, proliferation and (de)differentiation. Therefore, this paper will convey the overview of advantages and drawbacks of dental tissue-derived iPSCs and their future application with biomaterials.
Bone is a complex tissue in which resorption and formation continue throughout life. The bone tissue contains various types of cells, of which the bone forming osteoblasts and bone resorbing osteoclasts are mainly responsible for bone remodeling. Periodontal disease represents example of abnormal bone remodeling. Osteoclasts are multinucleated cells present only in bone. It is believed that osteoclast progenitors are hematopoietic origin, and they are recruited from hematopoietic tissues such as bone marrow and circulating blood to bone. Cells present in the osteoclast microenvironment include marrow stromal cells, osteoblasts, macrophages, T-lymphocytes, and marrow cells. These cells produce cytokines that can affect osteoclast formation. In vitro model systems using bone marrow cultures have demonstrated that $IL-l{\beta},\;IL-3,\;TNF-{\alpha},$ bFGF can stimulate the formation of osteoclasts. In contrast, IL-4 inhibits osteoclast formation. Knowledge of cytokines and bFGF that affect osteoclast formation and their capacity to modulate the bone-resorbing process should provide critical insights into normal calcium homeostasis and disorders of bone turnover such as periodontal disease, osteoporosis and Paget's disease.
The proper management of mandibular fractures involves reduction, rigid fixation, and immobilization to allow bone healing. Nonunion or malunion at the fractured sites is a well-known complication of fracture when the treatments are inappropriate. We present a case of left mandibular fracture due to shrapnel during the Korean War. The patients did not receive appropriate treatment at that time, so nonunion and malunion developed. Sixty years after the accident, mandibular osteomyelitis on the fracture site developed due to dental-origin inflammation. The treatment was based on relatively conservative care, such as saucerization and administration of antibiotics. There was no complication during the short-term follow-up. We present the case with literature review.
Neuropathic pain is caused by functional abnonnalities of structural lesions in the peripheral or central nervous system, and occurs without peripheral nociceptor stimulation. Trigeminal neuropathy always pose differential location difficulties as multiple diseases are capablc of producing them: they can be the result of traumatism, tumors, or diseases of the connective tissue, infectious or demyelinating diseases, or may be of idiopathic origin. There are a number of mechanisms described as causing neuropathy. They can be described as ectopic nerve activity, neuroma, ephatic trasmission, change of sodium channel expression, sympathetic activity, central sensitization, and alteration in central inhibition systems. More than I mechanism may be active to create individual clinical presentations. In order to provide better pain control, the mechanism-based approach in treating neuropathic pain should be familiar to physicians.
The odontogenic myxoma is an benign, slow growing neoplasm which is of ectomesenchymal origin. This neoplasm occurs almost exclusively in the jaw bones and comprises 0.2% to 17.7% of odontogenic tumors. The odontogenic myxoma may show a wide spectrum of radiographic appearances, unilocular, multilocular radiolucency and a distinct or diffuse border, making the differential diagnosis difficult. We present a case of the odontogenic myxoma in the maxilla with conventional and recent image modalities. Occlusal film revealed a medially extended multilocular lesion with intralesional fine and straight trabeculations from the scalloped margin and buccal expansion and thinning of cortical bone. Computed tomogram revealed lesion showed equivalent density to the muscles in the left maxillary sinus with partial cortical discontinuity of medial wall and the tennis-racket pattern with internal straight trabeculations. MRI revealed intermediate signal intensity on Tl weighted image and high signal intensity on T2 weighted image. In Gd enhanced MR image, the peripheral portions of the lesion were enhanced.
Background: Myofascial pain dysfunction syndrome (MPDS), otherwise called myofascial pain is one of the most common temporomandibular disorders, which in turn is the most common cause of orofacial pain of non-dental origin. Its etiology is multifactorial and still poorly understood. Psychological factors have been shown to play a role in the etiology. The aim of the study was to evaluate the association between anxiety and salivary cortisol levels in patients with myofascial pain. Methods: Twenty patients suffering from myofascial pain were recruited as the study group. The same number of age and sex matched healthy individuals were taken as the control group. The salivary samples collected between 9-9:15 am from both groups were analyzed for cortisol levels with the competitive enzyme-linked immunosorbent assay method. Anxiety levels of 40 patients were measured using Hamilton's anxiety scale. Results: The mean serum cortisol level of the MPDS group showed a highly significant difference (P < 0.001) from the controls. The mean anxiety scores of the MPDS group showed a highly significant difference (P < 0.001) from the controls. A positive correlation was found between anxiety and the salivary cortisol levels in MPDS patients. Conclusions: These findings suggest that anxiety plays a vital role in the etio-pathogenesis of MPDS; thus, besides pharmacological treatment, psychological support is also needed.
Objective: The purpose of this study was to analyze the dental and basal arch forms in patients with normal occlusion using the computed tomography (CT) imaging method. Methods: CT images were taken from 27 normal occlusion subjects (male, 15; female, 12) and these images were reconstructed into three-dimensional models. A 3D-coordinate system was formed by setting the middle of the facial axis (FA) point of the maxillary central incisors as the origin. The morphology of the maxilla and mandibular dental and basal arches were analyzed by sectioning parallel to the maxillary occlusal plane. Results: There was no significant difference between A point and B point and between the maxillary 1st molars in both sides of the maxillary and mandibular basal bone. The dental arch was located more labially than the basal arch in the anterior portion. The bucco-lingual crossover of the dental arch and basal arch was formed at the molar region in the maxilla, and at the premolar region in the mandible. Conclusions: This study will help provide three-dimensional diagnostic information about the relationship between basal and dental arches in patients who have severe dental compensation and inappropriate jaw relationships.

Odontomas, hamartomas of odontogenic origin, are composed of all the structures that make up teeth. The WHO distinguishes odontoma into two types. The complex odontoma is defined as "a malformation in which all the dental tissues are represented, individual tissues being mainly well-formed but occurring in a more or less disorderly pattern." The compound odontoma is defined as "a malformation in which all the dental tissues are represented in a more orderly pattern than in the complex odontoma, so that the lesion consists of many toothlike structures. Most of these structures do not resemble morphologically the teeth of normal dentition, but in each one enamel, dentine, cementum, and pulp are arranged as in the normal tooth." Almost all odontomas are located intraosseously, but they have occasionally been reported in extrabony location. Peripheral or soft tissue odontomas, those arising outside of the alveolar bone, are very rare. Peripheral or soft tissue odontoma are defined as tumors that demonstrate the histologic characteristics of their intraosseous counterparts but occur solely in the soft tissue covering the tooth-bearing portion of the mandible and maxilla. When they mature, they appear as a radiopaque mass without the peripheral halo. The final diagnosis should be confirmed by biopsy. The origin of peripheral odontoma is probably related to remnants of the dental lamina in the gingiva. The treatment of choice is complete surgical excision, similarly to intraosseous odontoma and it does not tend to recur. This report presents a case of 5-year-old boy with swelling on labial gingiva of primary central incisor. And it was diagnosed as peripheral odontoma by excisional biopsy.
Purpose: The odontogenic sinus and fistulous tracts is the most common draining sinus and fistulous tract of the head and neck region. These are often misdiagnoses by clinicions who are not familiar with cutaneous sinus, since most of patients do not have dental symptoms. Here we present two cases of odontogenic cutaneous sinus tract which have been diagnosed after excision of epidermal cyst. Methods: Two patients who presented with an odontogenic sinus tract draining to the skin at our institusion during the two years were enrolled in this study. We reviewed all the medical records of the patients and literature about odontogenic cutaneous sinus tract. Results: Odontogenic cutaneous sinus tracts of our cases were healed after treatment of periapical odontitis and extraction of the carious tooth. Conclusion: The cutaneous sinus tract of dental origin is well documented condition. But its diagnosis is not always easy unless the clinicians consider the possibility of its dental origin. An understanding of the pathogenesis of odontogenic cutaneous sinus tract will lead to early correct diagnosis and proper treatment without unnecessary surgery.
