최근 안모의 심미성이 중요시되면서 구순 돌출로 인한 비심미성을 해소하고자 내원하는 양악 치조골 전돌자의 수가 증가하는 추세이다. 그러나 이러한 환자에 대한 연구는 많지 않으며 특히 모형을 이용한 연구는 극히 적다. 모형을 이용하는 연구는 주로 손으로 직접 계측하거나 2차원 평면에서 컴퓨터를 이용하여 측정하였으나 근래에 3차원 레이저 스캐너가 도입되면서 모형을 이용한 연구를 좀 더 빠르고 간편하면서도 정확하게 할 수 있게 되었다. 이 연구는 3차원 디지털 모형을 이용하여 양악 치조골 전돌자의 치열궁 특성을 알아보기 위한 것이다. 양악 치조골 전돌자 20명과 정상교합자 20명을 대상으로 치아 크기, 치열궁 폭경 및 치열궁 길이, 치열궁과 구개의 형태를 비교하여 본 결과 양악 치조골 전돌자는 정상교합자보다 치아 크기가 크고, 하악 견치간 폭경 및 제1소구치간 폭경이 크며 치열궁 길이가 길었다. 그리고 치열궁 형태와 구개 형태에서 정상교합자와 차이를 보였다.
목적: 본 연구는 현재 시판되고 있는 구강스캐너를 사용하여 구강스캐너 정확도 평가를 위해 국제표준이 제안하고 있는 모형의 스캔이미지 획득이 가능한 지 분석하고, 이를 통해 표준모델이 가지고 있는 문제점을 파악하는 데 있다. 연구 재료 및 방법: ISO12836과 ANSI/ADA no.132에서 규정하는 국제표준을 참고하여 3D 프린터기를 이용하여 모델을 제작하였으며, 모델스캐너와 두 가지 구강스캐너를 이용하여 스캔을 하였다. 스캔이미지 획득 정도를 3등급으로 분류하여 스캐너의 성능을 비교하였으며, 모델 표면의 상태에 따른 이미지 획득 능력도 비교하였다. 결과: 모델 스캐너가 모든 모델에서 가장 우수한 이미지를 얻을 수 있었으며 TRIOS3는 둥근 형태의 구조물, CS3500은 각진 형태의 구조물에 대한 이미지 재현이 좋은 결과를 보였다. 표준 모델의 표면상태에 따른 스캔이미지 재현에서는 초경석고 모델이 스캐너 종류와 관계없이 가장 우수하였다. 3D 프린팅 모델의 경우, 표면에 파우더 처리를 한 모델에서 가장 우수한 스캔이미지를 얻을 수 있었다. 결론: ISO12836과 ANSI/ADA 132의 표준모델의 경우, 구강스캐너의 field of view (FOV)를 벗어나는 구조물을 스캔할 때 서로 다른 면인 것을 구분하는 기준점이 존재하지 않게 되면 연속적인 스캔 및 정합과정에서 정확한 이미지를 나타내지 못한다는 것을 알 수 있었다. 그러므로 단순한 패턴의 반복과 대칭구조를 가지지 않는 새로운 표준모델이 필요하다고 여겨진다.
To determine the dental arch length and width in Korean yaung adults, various points (shown on Fig 1) were measured by means of Boley gauge on the 124 plaster models which obtained from 2 young men. Despite advancements in therapeutic approaches, radiotherapy and cisplatin-based chemotherapy remain primary noninvasive treatments for patients with oral squamous cell carcinoma (OSCC). Moreover, the 5-year survival rate for patients with OSCC has remained almost unchanged for several decades, and many side effects of chemotherapy still exist. In this study, three-dimensional (3D) models of OSCC were established using fibroblasts, and the efficacy of various biological inhibitors was evaluated. A culture of epithelial cells with two types of fibroblasts (hTERT-hNOFs and cancer-associated fibroblasts) within a type I collagen matrix resulted in the formation of a continuous layer of tightly packed cells compared to models without fibroblasts. Furthermore, the effects of biological chemicals, including Y27632, latrunculin A, and verteporfin, on these models were investigated. The stratified formation of the epithelial layer and invasion in OSCC 3D-culture models were effectively inhibited by verteporfin, whereas invasion was weakly inhibited by Y27632 and latrunculin. Collectively, the developed OSCC 3D-culture models established with fibroblasts demonstrated the potential for drug screening, with verteporfin showing promising efficacy.
Nowadays, additive manufacturing (AM) technology is a promising process to fabricate complex shaped devices applied in medical and dental services. Among the AM processes, a DLP (digital light processing) type 3D printing process has some advantages, such as high precision, relatively low cost, etc. In this work, we propose a simple method to fabricate precise dental models using a DLP 3D printer. After 3D printing, a part is commonly post-cured using secondary UV-curing equipment for complete polymerization. However, some shrinkage occurs during the post-curing process, so we adaptively control the UV-exposure time on each layer for over- or under-curing to change the local shape-size of a part in the DLP process. From the results, the shrinkage amounts in the post-curing process vary due to the UV-dose in 3D printing. We believe that the proposed method can be utilized to fabricate dental models precisely, even with a change of the 3D CAD model.
Objectives : The purpose of this in vitro study was to evaluate the effect of firing cycles on the marginal and internal fit of metal ceramic crown. Methods : Ten same cases of stone models (abutment teeth 11) were manufactured. Ten cobalt-chrome cores were made per each models and the marginal and internal fit was evaluated through a silicone replica technique. The marginal and internal fit of specimens was measured twice. The first measurement was done after manufacturing cobalt-chrome alloy core and the second measurement was done after porcelain firing. T-test of paired sample for statistical analysis was executed with SPSS 12.0K for Windows (${\alpha}$=0.05). Results : Mean(SD) marginal and internal fit were 77.1(23.3) ${\mu}m$ for the cobalt-chrome alloy core group and 84.4(21.9) ${\mu}m$ for the metal-ceramic crown group. They were statistically significant differences between groups for marginal and internal fit (p<.05). Conclusions : All metal ceramic crowns showed marginal and internal fit ranged within the current clinical recommendations.
이 연구의 목적은 디지털 치열 모형에서 치아 폭경, 볼튼 비율, 수평 피개, 수직 피개를 포함한 교정적 측정의 타당성을 확인하는 것이다. 만 12 - 18세의 환자를 대상으로 세가지 형태의 치열 모형을 획득하였다. 기존의 석고 모형을 형성하였고, DOF freedom HD 모형 스캐너를 통해 디지털 모형으로 변환하였다. 그리고 CS3600 구강 스캐너로 디지털 모형을 형성하였다. 각 모형에서 측정 시행 후, 급내 상관 계수를 통해 계측의 신뢰성과 재현성을 확인하였으며, 대응 표본 t 검정을 사용하여 타당성을 평가하였다. 결과적으로 모든 군에서 급내 상관계수는 0.750을 초과하여 연구자 내 신뢰성과 연구자 간 재현성이 있음을 확인하였다. 모형 스캔한 군은 전체 및 전치 볼튼 비율, 수평 및 수직 피개에서 타당성을 보였다. 구강 스캔한 군은 전치 볼튼 비율, 수평 피개에서 타당성을 보였다. 구강 스캔한 디지털 모형을 이용한 계측은 인상 채득에 어려움이 있는 소아 청소년에게 고려할 수 있는 대안이다. 하지만 임상에서 이를 교정적 분석에 이용할 경우 오차를 고려한 적용이 필요하다.
Background: Mandibular motion tracking system (ManMoS) has been developed for orthognathic surgery. This article aimed to introduce the ManMoS and to examine the accuracy of this system. Methods: Skeletal and dental models are reconstructed in a virtual space from the DICOM data of three-dimensional computed tomography (3D-CT) recording and the STL data of 3D scanning, respectively. The ManMoS uniquely integrates the virtual dento-skeletal model with the real motion of the dental cast mounted on the simulator, using the reference splint. Positional change of the dental cast is tracked by using the 3D motion tracking equipment and reflects on the jaw position of the virtual model in real time, generating the mixed-reality surgical simulation. ManMoS was applied for two clinical cases having a facial asymmetry. In order to assess the accuracy of the ManMoS, the positional change of the lower dental arch was compared between the virtual and real models. Results: With the measurement data of the real lower dental cast as a reference, measurement error for the whole simulation system was less than 0.32 mm. In ManMoS, the skeletal and dental asymmetries were adequately diagnosed in three dimensions. Jaw repositioning was simulated with priority given to the skeletal correction rather than the occlusal correction. In two cases, facial asymmetry was successfully improved while a normal occlusal relationship was reconstructed. Positional change measured in the virtual model did not differ significantly from that in the real model. Conclusions: It was suggested that the accuracy of the ManMoS was good enough for a clinical use. This surgical simulation system appears to meet clinical demands well and is an important facilitator of communication between orthodontists and surgeons.
Purpose: The purpose of this study was to analyze the biomechanical properties of the dental implants on the supporting bone using three-dimensional finite element method when three different abutment materials were applied to the implant system. Methods: Three different dental implant models were fabricated by applying Ti, PEEK, and CRE-PEEK (60% carbon-reinforced PEEK) to abutment material. The abutment and connecting screw from the fixture was applied with a tightening torque of 20 Ncm. And then, total loads of 150 N were applied in an $30^{\circ}oblique$ direction (to the vertical). The structural stability of dental implants on the supporting bone was analyzed using Von Mises stress and principal stress values. Results: The maximum tensile stress of the cortical bone was highest at 12.6 MPa in the PEEK abutment (Model-B). Ti abutment (Model-A) and CRE-PEEK abutment (Model-C) showed similar stress distributions (10.6 and 10.3 MPa, respectively). And the maximum compressive principal stress was similar in all models. The Von Mises stress value delivered to the bone around the implant was highest at 16.5 MPa in Model-B. On the other hand, Model-A and C showed similar stress distributions (14.0 and 13.8 MPa, respectively). In addition, the maximum equivalent stress applied to the abutment was highest at 629.8 MPa in Model-A. The stress distribution in Model-C was 573.9 MPa. Whereas, Model-B showed the lowest value at 165.6 MPa. Conclusion : The dental implant supporting bone system using PEEK material seems to have the possibility of supporting bone fracture. It was found that the CRE-PEEK abutment can reduce the elastic deformation and reduce the stress value of the interfacial bone.
Objective: The purpose of this study was to compare the precision of three-dimensional (3D) images acquired using iTero$^{(R)}$(Align Technology Inc., San Jose, CA, USA) and Trios$^{(R)}$(3Shape Dental Systems, Copenhagen, Denmark) digital intraoral scanners, and to evaluate the effects of the severity of tooth irregularities and scanning sequence on precision. Methods: Dental arch models were fabricated with differing degrees of tooth irregularity and divided into 2 groups based on scanning sequence. To assess their precision, images were superimposed and an optimized superimposition algorithm was employed to measure any 3D deviation. The t-test, paired t-test, and one-way ANOVA were performed (p < 0.05) for statistical analysis. Results: The iTero$^{(R)}$ and Trios$^{(R)}$ systems showed no statistically significant difference in precision among models with differing degrees of tooth irregularity. However, there were statistically significant differences in the precision of the 2 scanners when the starting points of scanning were different. The iTero$^{(R)}$ scanner (mean deviation, $29.84{\pm}12.08{\mu}m$) proved to be less precise than the Trios$^{(R)}$ scanner ($22.17{\pm}4.47{\mu}m$). Conclusions: The precision of 3D images differed according to the degree of tooth irregularity, scanning sequence, and scanner type. However, from a clinical standpoint, both scanners were highly accurate regardless of the degree of tooth irregularity.
