The purpose of this study was to make and ascertain a decision making process on the base of patient-oriented utilitarianism in the treatment of patients of chronic adult periodontitis. Fifty subjects were chosen in Yonsei Dental hospital and the other fifty were chosen in Severance dental hospital according to the selection criteria. Fifty four patients agreed in this study. NS group(N=32) was treated with scaling and root planing without any surgical intervention, the other S group(N=22) done with flap operation. During the active treatment and healing time, all patients of both groups were educated about the importance of oral hygiene and controlled every visit to the hospital. When periodontal treatment needed according to the diagnostic results, some patients were subjected to professional tooth cleaning and scaling once every 3 months according to an individually designed oral hygienic protocol. Probing depth was recorded on baseline and 18 months after treatments. A questionnaire composed of 6 kinds(hygienic easiness, hypersensitivity, post treatment comfort, complication, functional comfort, compliance) of questions was delivered to each patient to obtain the subjective evaluation regarding the results of therapy. The decision tree for the treatment of adult periodontal disease was made on the result of 2 kinds of periodontal treatment and patient's ubjective evaluation. The optimal path was calculated by using the success rate of the results as the probability and utility according to relative value and the economic value in the insurance system. The success rate to achieve the diagnostic goal of periodontal treatment as the remaining pocket depth less than 3mm and without BOP was $0.83{\pm}0.12$ by non surgical treatment and $0.82{\pm}0.14$ by surgical treatment without any statistically significant difference. The moderate success rate of more than 4mm probing pocket depth were 0.17 together. The utilities of non-surgical treatment results were 100 for a result with less than 3mm probing pocket depth, 80 for the other results with more than 4mm probing pocket depth, 0 for the extraction. Those of surgical treatment results were the same except 75 for the results with more than 4mm. The pooling results of subjective evaluation by using a questionnaire were 60% for satisfaction level and 40% for no satisfaction level in the patient group receiving nonsurgical treatment and 33% and 67% in the other group receiving surgical treatment. The utilities for 4 satisfaction levels were 100, 75, 60, 50 on the base of that the patient would express the satisfaction level with normal distribution. The optimal path of periodontal treatment was rolled back by timing the utility on terminal node and the success rate, the distributed ratio of patient's satisfaction level. Both results of the calculation was non surgical treatment. Therefore, it can be said that non-surgical treatment may be the optimal path for this decision tree of treatment protocol if the goal of the periodontal treatment is to achieve the remaining probing pocket depth of less than 3mm for adult chronic periodontitis and if the utilitarian philosophy to maximise the expected utility for the patients is advocated.

Facial asymmetry has been found with a higher frequency (70~84%) in skeletal class III malocclusion patients. Anticipating the poor prognosis of prosthesis due to malocclusion, occlusal stability must be obtained by orthodontic treatment. Moreover, orthodontic surgery would be needed in some severe cases for better functional and esthetic results. The orthognathic surgery is performed on one jaw or two jaw depending on the results of facial diagnosis. Genioplasty may change the vertical, horizontal, sagittal position of chin by osteotomy or augmentation using implants, also. This case is about a 24 year-old male patient who visited our clinic to solve the facial asymmetry and mandibular prognathism. Skeletal class III malocclusion, maxillary canting and menton deviation to left by 13 mm were detected. Multiple ill-fitting prostheses, unesthetic maxillary anterior prostheses, and several dental caries were found. After pre-operative orthodontic treatment, Le-Fort I osteotomy, sagittal split ramus osteotomy, genioplasty, right mandibular angle augmentation were done for the correction of jaw relation and asymmetry. By diagnostic wax-up after post-operative orthodontic treatment, maxillary full mouth rehabilitation and mandibular posterior restorations were planned out. For better result, clinical crown lengthening procedure was done on #11, 12 and implant was placed on left mandibular first molar area. The patient was satisfied with the final prostheses. Because of his high caries risk, long-term prognosis will depend on the consistent maintenance of oral hygiene and periodic follow-up.
Autophagy plays an important role in cellular homeostasis and survival for cell recycling and various stresses within the cell. Recent studies have shown that autophagy activity modulates the expression of oncogene and tumor suppressor genes, leading to the development or suppression of cancer. Induction of autophagy is involved in preventing cancer development in normal cells and plays an important role in prompting a specific cell death mechanism in cancer cells with damaged cell death function. It is also known that autophagy inhibition increases the therapeutic efficacy by sensitizing cancer cells that are resistant to chemotherapy. However, the role of autophagy has not yet been fully understood in cancer treatment. Oral squamous cell carcinoma accounts for more than 90% of oral cancer and is the sixth most common cancer in the world. The incidence of oral cancer has increased by 50% over the last 20 years and the mortality rate is over 40% within 5 years after the onset. In oral cancers, the role of autophagy are described to look for tumor inhibitory in the early stages of tumor formation, like other cancers, indicating the dual functions involved in tumor cell survival include tumor progression stages. This review summarizes the various roles of autophagy in cancer cells and suggests the possibility of autophagy as a promising target for effective oral cancer therapy.

Ginsenoside Rd is a primary constituent of the ginseng rhizome and has been shown to participate in the regulation of diabetes and in tumor formation. Reports also show that ginsenoside Rd exerts anti-oxidative effects by activating anti-oxidant enzymes. Treatment with ginsenoside Rd decreased nitric oxide and prostaglandin $E_2$ ($PGE_2$) in lipopolysaccharides (LPS)-challenged RAW264.7 cells and in ICR mouse livers (5 mg/kg LPS; LPS + ginsenoside Rd [2, 10, and 50 mg/kg]). Furthermore, these decreases were associated with the down-regulations of inducible nitric oxide synthase (iNOS) and cyclooxygenase (COX)-2 and of nuclear factor (NF)-${\kappa}B$ activity in vitro and in vivo. Our results indicate that ginsenoside Rd treatment decreases; 1) nitric oxide production (40% inhibition); 2) $PGE_2$ synthesis (69% to 93% inhibition); 3) NF-${\kappa}B$ activity; and 4) the NF-${\kappa}B$-regulated expressions of iNOS and COX-2. Taken together, our results suggest that the anti-inflammatory effects of ginsenoside Rd are due to the down-regulation of NF-${\kappa}B$ and the consequent expressional suppressions of iNOS and COX-2.

This study is a descriptive study to analyze the hand washing practice of nurses working in clinic applying health belief model. This research involved 162 clinical nurse at G city. The data were collected from Sep 1, 2017 to Oct 31, 2017. The collected data were analyzed using an independent t-test, 1-way ANOVA, pearson's correlation coefficient and multiple regression analysis, where p-values of <0.05 were considered statistically significant by using SPSS 20.0. The overall practice of hand washing by general subjects were high 3.1 in all subjects, but there was no statistically significant difference. Health beliefs about hand washing practice according to general characteristics showed significant difference in perceived benefits (p<0.05), and there was a significant difference in perceived benefits and perceived barriers in age(p<0.05). In regard to the correlation among the subfactor of health beliefs, benefits had a statistically significant negative correlation to barriers and, positively correlated to cues to action. The results of this study suggest that continuing education of infection management in hospitals considering health beliefs about proper hand washing training will enhance hand washing practice.

This study was conducted to investigate the mediating effects of walking days on body mass index (BMI) according to the public transportation satisfaction of middle aged Koreans aged 40-59 in Korea using the 2015 community health survey data. The purpose of this study was to provide basic data for provision of the neighborhood environment and support programs for the walking activity and promotion of daily walking activities among middle aged people. Among 228,558 individuals, 85,344 middle aged people aged 40-59 years were selected as final subjects for analysis. The data were analyzed with the open source statistics program R 3.4.1 to determine whether the number of walking days had a mediating effect on body mass index (BMI) as an independent variable. The Sobel test revealed that the number of walking days increased by B=0.010(p=.010), and that when the satisfaction with public transportation increased, B=-0.052 (p=.021), the number of walking days decreased by B=-0.038 (p=.001). To increase the number of walking days and decrease the body mass index by increasing public transportation satisfaction by increasing the use of public transportation, public transportation fare adequacy and access convenience among the public transportation satisfaction mentioned above should be improved more than the current level. It is not easy for individuals that live in small cities to reach their destination by public transportation after leaving the metropolitan area; therefore, improvement of public transportation systems is necessary to improve health.

When a homothallic ascomycete Aspergillus nidulans is exposed to visible light, cleistothecial development is inhibited. The light response of development in A. nidulans implies the existence of delicate regulation process including reception and translocation of light signaling and determination of development. Previously, mutants that could develop cleistothecia even in the presence of relatively intensive visible light were isolated and several complementation groups were identified. A gene that was able to complement the silA98 mutation, which was responsible for preferred cleistothecia development under visible light, was isolated from AMA-NotI genomic library. The silA gene retained in the 4.3 kb recovered genomic library DNA has an open reading frame (ORF) consisted of 2,388 bp nucleotides, interrupted by 3 introns and consequently encoding 795 amino acids. The putative SilA carries a ${Zn_2}{Cys_6}$ binuclear cluster motif at N terminus and shows high amino acid sequence similarity to Aro80p of Saccharomyces cerevisiae. Deletion mutants of silA showed a strong induction of sexual development under visible light, indicating that SilA is involved in the negative regulation of sexual development in response to the light.
OBJECTIVES: This study was performed to find out static two-point discrimination (TPD) in fingertips. METHODS: This was a cross-section, measure study of static two-point discrimination involving healthy young adults. Measure was completed by 48 college students in Andong Science College from June 1 to 12, 2004. The minimal distance at which two-points could be discriminated was measured from thumb to little finger. RESULTS: For dermatomal regions of the fingertip, mean values ranged from 3.3mm to 4.9mm (thumb 3.6mm, index finger 3.3mm, middle finger 3.8mm, ring finger 4.2mm, and little finger 4.7mm in the left hand; thumb 3.7mm, index 3.5mm, middle 4.0mm, ring 4.3mm, and little 4.9mm in the right hand). A significant difference in discrimination ability was found between men and women, 3.5mm for women showed a greater sensitivity than 4.1mm for men in the left middle fingertip(p=0.0109), also 3.9mm for women showed a greater accuracy than 4.5mm for men in the left ring fingertip(p=0.0388). In the right index fingertip, women (3.1mm) have a narrow distance than men (3.6mm)(p=0.0329). The minimal distance of TPD was found a significant difference between 20 and 30 years in age. 4mm for 30 years showed a greater distance than 3.5mm for 20 years in the left thumb fingertip(p=0.0354), also, 3.8mm for 30 years showed a greater distance than 3.2mm for 20 years in the left index fingertip(p=0.0174), and 4.3mm for 30 years showed a greater distance than 3.7mm for 20 years in the left middle fingertip(p=0.0444). In the right index fingertip, 20 years (3.2mm) had also a narrow distance than 30 years (4.1mm)(p=0.0020), 20 years (3.9mm) showed a narrow distance than 30 years (4.6mm) in the right middle fingertip(p=0.0124), and 20 years (4.1mm) showed a greater sensitivity than 30 years (5.0mm) in the right ring fingertip(p=0.0070). CONCLUSIONS: Our results suggest that distance of TPD in the both index fingertips for 20 years women was significantly narrowed.
Previous studies have shown that five representative behaviors affect the health of adults in everyday life : smoking, drinking, exercise, nutrition, and stress. This study focuses on these five behaviors in an attempt not only to develop a program that promotes health, but also to suggest ways that such a program may be implemented to reflect diverse lifestyles. Our aim is to show how individual subjectivity influences behavior when making lifestyle choices that affect health. By analyzing individual characteristics, we tried to group people according to their common attitude and behaviors to promote health. We hope that this study may provide the fundamental data which may be used to assist professionals in promoting healthy behaviors among adults. In order to examine how subjectivity(e.g, personal opinions or attitudes) influences behavior, we prepared Q-statements which were composed of Q-samples for the study of human subjectivity. We polled forty-two adults and then analyzed the results using a PC qunal program. As a result, the respondents were classed according to six different types. Type One includes people who think managing stress is the most effective way to live a healthy life. They regard smoking and drinking as harmful behaviors. Type Two subjects, on the other hand, regard smoking and drinking as a matter of taste with few harmful effects. Although they recognize the importance of managing stress, they have a positive attitude towards smoking and regard nutrition as a minor factor in promoting their healthy lifestyle. Those classed as Type Three emphasize the necessity of exercise. They perceive drinking, smoking, and stress as harmful. People in this category seek to increase physical strength and to regulate all five health behaviors by keeping them in a normal range. Type Four consists of people who are indifferent to their health. While they seem to recognize the importance of regular exercise, they pay little attention to nutrition or to the harmful effects of smoking, drinking, and stress. They believe that exercise alone is sufficient to maintain their health. Type Five subjects believe drinking is not a harmful behavior at all. Rather. it has a positive effect on their mental health. They are characterized by this positive attitude towards drinking as well as by indifference toward nutrition (although they readily admit that good nutrition is important). Finally. those classed as Type Six have a positive attitude toward moderate drinking as a means to relieve stress. However. they also recognize the harmful effects of excessive drinking. They regard good nutrition as an important lifestyle choice but are indifferent toward exercise. This type is engaged in passive health management. This study devised six types or categories that reflect different attitudes toward promoting health in everyday life. It further went on to analyze the characteristics of each type. This study shows that programs designed to promote health must be modified to reflect the diversity of individual attitudes and patterns of behaviors.
