Assessment of Clinical Applicability of a New Plaque Scoring System Using Quantitative Light-Induced Fluorescence-Digital (Quantitative Light-Induced Fluorescence-Digital을 이용한 치면세균막 검사법의 임상적 활용 가능성 평가)

  • The aim of this study was to suggest a convenient method of monitoring the gingival state through plaque detection. Quantitative Light-Induced Fluorescence-Digital (QLF-D), which can assess mature plaque, can be used to assess the oral hygiene status of individuals and to establish an adequate intervention plans for them. The subjects of the study participated in the clinical training at Department of Dental Hygiene, N University. The subjects completed questionnaires on general characteristics and oral hygiene methods. Then, photographs of maxillary and mandibular anterior teeth of the subjects were taken using the QLF-D. After the oral examinations, gingival state was recorded according to the $L{\ddot{o}}e$ & Silness's Gingival Index (GI). In addition, a plaque control record was calculated in percentage using disclosing agent. The analysis of the relation between the plaque control record and the QLF-D scores showed positive correlation (r=0.638, p<0.001), and the analysis of the relation between the QLF-D scores and the gingival bleeding index scores also showed positive correlation (r=0.562, p<0.001). Besides, the study classified the participants into healthy gingiva group and the gingivitis group according to the classification criteria of GI, and when the difference of QLF-D scores between the groups was analyzed, the QLF-D scores were statistically significantly higher (t=-2.785, p=0.007) in the gingivitis group ($1.71{\pm}1.545$) than the healthy gingiva group ($0.74{\pm}1.290$). When the differences in mean values of the QLF-D scores were analyzed against and the gingival bleeding index scores, the QLF-D scores of 0 and 1 belonged to the category of normal gingival state while the QLF-D scores of 2, 3 and 4 belonged to the category of gingivitis state. Therefore, it is deemed that the red fluorescence detected by the QLF-D from the mature supragingival plaque can be used for monitoring the state of gingivitis.

A comparative evaluation of $CO_2$ and erbium-doped yttrium aluminium garnet laser therapy in the management of dentin hypersensitivity and assessment of mineral content

  • Purpose: Dentin hypersensitivity is a potential threat to oral health. Laser irradiation may provide reliable and reproducible treatment but remains controversial. The present study aimed to evaluate the effects of $CO_2$ or erbium-doped yttrium aluminium garnet (Er:YAG) laser therapy, and to assess mineral content. Methods: Eighteen human single-rooted teeth affected with advanced periodontitis were obtained. Buccal and lingual surfaces were planed to form 36 specimens. Ethylenediaminetetraacetic acid gel (24%) was applied to remove the smear layer and simulate hypersensitive teeth. The experimental groups were: group 1, control (no irradiation); group 2, $CO_2$ laser (repetitive pulsed mode, 2 W, $2.7J/cm^2$); and group 3, Er:YAG laser (slight contact mode, 40 mJ/pulse and 10 Hz). To evaluate dentinal tubule occlusion, six specimens per group (2-mm thickness) were prepared and observed using scanning electron microscopy (SEM) for calculation of the occlusion percentage. To evaluate the mineral content, six specimens per group (0.6-mm thickness) were used, and then the levels of Ca, K, Mg, Na, and P were measured by inductively coupled plasma-atomic emission spectrometry. In addition, the surface temperature of the specimens during laser irradiation was analyzed by a thermograph. Results: The SEM photomicrographs indicated melted areas around exposed dentinal tubules and a significantly greater percentage of tubular occlusion in the $CO_2$ and Er:YAG laser groups than the control, and in the Er:YAG group than the $CO_2$ laser group. In addition, no significant differences were noted among the experimental groups for the mineral elements analyzed. The $CO_2$ laser group showed an evident thermal effect compared to the Er:YAG group. Conclusions: $CO_2$ and Er:YAG laser are effective in treating dentin hypersensitivity and reducing its symptoms. However, the Er:YAG laser has a more significant effect; thus, it may constitute a useful conditioning item. Furthermore, neither $CO_2$ nor Er:YAG lasers affected the compositional structure of the mineral content.

A Study on the Level of Self-assessed Oral Health by the Adults (성인의 자기평가 구강건강수준에 관한 연구)

  • The purpose of this study is to compare oral hygiene status between young adult group and older adult group and to suggest a pathway model of oral health level in self-assessed. Subjects were totally 80 people with young adult group and the older adult group. The data, which was collected by surveying for one month from July 1, 2010 to July 30, was analyzed with SPSS VERSION 19.0 and AMOS 19.0. Oral hygiene test and questionnaire survey were carried out from targeting. 1. The young adult group and older adult group had the same quality in oral health level score and living habit. 2. The oral hygiene status in young adult and older adult group was same in 7 items(58.33%) out of 12 items. 3. Oral health level score in self-assessed of young adult group could be explained with 34% by 3 variables. Older adult group could be explained with 26% by 2 variables. 4. In young adult, the fitness of path model was very good with 0.96 in CFI, 0.94 in TLI, and 0.00 in RMSEA. In older adults, the fitness of path model was very good with 1.00 in CFI, 1.15 in TLI, and 0.00 in RMSEA. This might imply if TMJ disorder level and number of infection is neglected in young aldults, mastication function will be significantly effected in older adults. Thus, development and management of health education program is needed based on self-assessment oral health level by young adults group and older adults group.

Assessment of the Caries Detection Ability of Quantitative Light-induced Fluorescence (QLF) in Primary Teeth in vitro (유치의 치아우식증 확인을 위한 정량형광분석(QLF) 검사법의 우식탐지능력 평가)

  • The aim of this study was to evaluate QLF (quantitative light-induced fluorescence) technology for the caries detection in primary teeth and validate the relationships between the cavity volume of carious lesions and QLF analysis results. Total 125 tooth surfaces include 53 occlusal surfaces, and 72 proximal surfaces were investigated with the portable QLF device for detection of dental caries in primary molars. Micro-CT radiograph was also performed to classify carious lesions and calculate the cavity volume. QLF showed good accuracy and reliability (sensitivity 0.75 - 0.94, specificity 0.82 - 0.95, and AUROC 0.88 - 0.98) for the caries detection in primary teeth except 𝚫R average results of proximal caries which showed relatively low values. Statistically significant relationships were found between 𝚫F average, QS-Index and the cavity volume according to Spearman rank-order correlation coefficients (r = 0.805 - 0.832, p < 0.001). QLF detection method would be a harmless and reliable way for children to diagnose dental caries without the concern about radiation exposure.

Implant-assisted removable partial denture restoration in small number of residual teeth in mandible: A case report (하악 소수 잔존치 환자에서 임플란트 보조 국소의치 수복 증례)

  • The patient in this case presented with a desire to have new dentures due to discomfort with existing ones. At the initial visit, all of teeth were missing except for the mandibular left second molar. As the patient was 65 years old, treatment with dentures and implant-supported prostheses was possible under the national health insurance system, and the patient opted for the mandibular denture using implant. Temporary prostheses were initially provided for patient adaptation, and following successful adaptation period, the treatment progressed. A maxillary complete denture and a mandibular implant-supported denture using two implants in the canines were fabricated. The mandibular denture is a Kennedy Class II removable partial denture which consists of a six-unit porcelain fused to metal fixed dental prostheses supported by the implant in the canines on both sides and left second molar serving as the abutments. Despite severe bone resorption and insufficient abutment teeth, the patient expressed satisfaction with the treatment results. In cases with economic and anatomical constraints affecting the feasibility of complete denture, implant-supported overdenture, and implant-supported fixed dental prostheses, an implant-assisted removable partial denture using implant surveyed crowns proves to be a viable and effective alternative treatment option. Nevertheless, the current dearth of scientifically rigorous studies underscores the necessity for meticulous regular check-up and occlusal assessment.

The Relationship Between Chewing Ability and Health Status in the Long-lived Elderly of Kyungpook Area (경북지역 장수노인의 저작능력과 건강상태)

  • Background: The objective of this study is to evaluate the effect of the dental and general health in relation to the state of dentition and chewing ability by surveying oral condition and anthropometric measure in order to provide primary statistics for the development of a program which may lead to an improvement in the long-lived elderly health status in a rural community. Materials and Methods: The subjects of this study were 97 rural long-lived elderly(27 males and 70 females) who were over 85 years-old (average age of subjects are $88.14{\pm}3.20$ year old) in Sungju-Gun, Kyungpook Province. Data were collected by using questionnaires and direct measurement of anthropometrics, and oral examination from all 97 subjects on July, 1999. Results: The following results were obtained: 1. 53.6% of all subjects believe that they are healthy. The average values of height, weight, BMI, body fat, lean body fat and total water were $148.8{\pm}11.2cm$, $46.9{\pm}10.5kg$, $21.2{\pm}3.5kg/m^2$, $26.7{\pm}6.9%$, $73.0{\pm}7.1%$, and $53.4{\pm}5.2%$, respectively. 2. The average number of teeth remaining in the subjects were $3.50{\pm}5.71$; the number of maxillary teeth remaining were $1.08{\pm}2.88$; and the number of mandibular teeth remaining were $2.41{\pm}3.76$. The maximum number of teeth remaining among subjects were 22 teeth, and the fully edentulous(no natural teeth) people were 76.3%. The oral conditions of the subjects were 52.6% using denture, 23.7% using natural teeth, and 23.7% masticating edentulous ridge without denture. 3. In terms of oral condition in self-assessment of health, digestive ability, and chewing ability ; On self-assessment of health, 47.1% of those wearing denture group responded as feeling good, 56.5% of those in the group of edentulous without denture, and 65.2% in group of natural teeth only. On self-assessment of digestive ability, 82.4% of those in group of denture responded as feeling good, 65.2% of those in group of no teeth and no denture, and 73.9% of those in group of natural teeth only. On self-assessment of chewing ability, 90.2% of those in the group wearing a denture, 60. 9% of those in the group of no teeth and no denture, and 65.2% of those in the group of natural teeth only. 4. In terms of oral condition in anthropometric measurements; The height, weight, body fat, lean body mass, and total water according to oral conditions were $150.0{\pm}10.7cm$, $49.0{\pm}10.9kg$, $26.9{\pm}6.6%$, $72.7{\pm}7.0%$, $53.2{\pm}5.1%$, respectively, in group wearing a denture, $142.7{\pm}6.0cm$, $43.2{\pm}5.5kg$, $29.5{\pm}7.2%$, $70.8{\pm}6.9%$, $51.8{\pm}5.0%$, respectively, in the group of no teeth and no denture, and $152.3{\pm}14.1cm$, $45.9{\pm}12.6kg$, $23.4{\pm}6.0%$, $75.9{\pm}6.9%$, $55.6{\pm}5.1%$, respectively, in the group of natural teeth only. Conclusion: The subjective measurements of good health were higher denture user, and natural teeth.

Association between consumption of milk and dairy products, calcium and riboflavin, and periodontitis in Korean adults: Using the 2007-2010 Korea National Health and Nutrition Examination Surveys (한국 성인의 우유 및 유제품과 칼슘 및 리보플라빈 섭취량과 치주염 간의 연관성: 2007~2010년 국민건강영양조사 자료를 이용하여)

  • Purpose: The current study was designed to investigate the relationship of dietary calcium and riboflavin and their main dietary source (milk and dairy products) with the risk of periodontitis using data from 2007 to 2010 Korea National Health and Nutrition Examination Surveys. Methods: A total of 1,690 adults aged ${\geq}40$ years were included. We used results of dental examination regarding all sextant information on probing depth of at least two index teeth, nutritional assessment by a single 24-hour dietary recall, and demographic and medical information. The periodontitis group was defined as those who had 3-4 points, and the normal group was defined as those who had 0 points of Community Periodontal Index at all locations of six examination sites using a probe. Results: Using multiple logistic regression analysis, after adjustment for age, body mass index, energy intake, income, smoking, and alcohol drinking, we found an inverse relationship between consumption of dairy products and risk for periodontitis (OR: 0.465, 95% CI: 0.224-0.964) and between dietary riboflavin intake more than the estimated average requirements and risk for periodontitis (OR: 0.535, 95% CI: 0.300-0.954) in males. Conclusion: Adequate intake of milk dairy products and riboflavin may be recommended for prevention of periodontitis in the Korean male population.

Effects of Special Mouth Care with an Aroma Solution on Oral Status and Oral Cavity Microorganism Growth in Elderly Stroke Patients (아로마 용액을 이용한 구강간호가 뇌졸중 노인의 구강상태와 구강 세균 집락에 미치는 효과)

  • Purpose: This study was conducted to examine the effect of oral care with an aroma solution on oral status and oral cavity microorganism growth in elderly patients with stroke. Methods: A non-equivalent control group, with a pretest-posttest design was used in this study. The participants were assigned to the experimental group (n=30) that received oral care with an aroma solution or the control group (n=31) that received 0.9% saline solution. To identify the effect of the experimental treatments, objective/subjective assessments of oral status and oral cavity microorganism growth were performed using the oral assessment guide, oral perception guide, and oral swab culture. Data were analyzed using Chi-square test, Fisher's exact test, and t-test with the SPSS version 21.0 program. Results: The objective oral status was significantly lower in the experimental group than in the control group (t= -3.64, p<.001). There was no significant difference between the subjective oral status of the experimental group and control groups (t= -1.24, p=.109). Oral microorganism growth was significantly lower in the experimental group than in the control group (t= -7.39, p<.001). Conclusion: These findings indicate that special mouth care using an aroma solution could be an effective oral health nursing intervention for elderly patients with stroke.

A Study on the Distribution of VOC Concentrations by Attachment Position of Air Samplers for Working Environment Measurement (작업환경측정 시료 채취기의 부착 위치에 따른 유기화합물의 농도 분포에 관한 연구)

  • Objectives: The purpose of this study is to investigate differences in concentration according to the position at the left or right shoulder within a 30 cm of radius of workers' respirators and provide basic data for the establishment of an industrial health policy. Methods: Personal samples were collected from a total of 65 workers from 27 manufacturing firms in South Gyeongsang-do Province from November 5, 2011 to December 30, 2012 after classifying the laborers into left- and right-side groups. The organic compound samples were collected and analyzed in accordance with the NIOSH Manual of Analytical Methods (NMAM) 1501. Results: In terms of the concentration of organic compounds collected from both left and right shoulders at the position of workers' respirators, isobutyl acetate was the highest with 145 ppm at the left shoulder, followed by ethyl acetate (133.5 ppm) and toluene (38.13 ppm). At the right shoulder, on the contrary, ethyl acetate (149.3 ppm) was the highest, followed by toluene (46.26 ppm), xylene (29.63ppm) and isopropyl alcohol (28.06 ppm). Overall, the right shoulder was higher than the left shoulder in terms of concentrations. Conclusions: For the measurement of the working environment, workers' personal samples should be collected at the place closest to the respirator. In terms of the reduction of error, the attachment of two sample media is expected to reduce errors in exposure assessment.

Assessment of MMP-1, MMP-8 and TIMP-2 in experimental periodontitis treated with kaempferol

  • Purpose: The objective of this study was to investigate the effect of a dietary flavonoid, kaempferol, which has been shown to possess antiallergic, anti-inflammatory, anticarcinogenic, and antioxidant activities on the periodontium by histomorphometric analysis and on gingival tissue matrix metalloproteinase-1 (MMP-1), MMP-8, and tissue inhibitor of metalloproteinase-2 (TIMP-2) by biochemical analysis of rats after experimental periodontitis induction. Methods: Sixty Wistar rats were randomly divided into six groups of ten rats each, and silk ligatures were placed around the cervical area of the mandibular first molars for 15 days, except in the healthy control rats. In the experimental periodontitis groups, systemic kaempferol (10 mg/kg/2d) and saline were administered by oral gavage at two different periods (with and without the presence of dental biofilm) to all rats except for the ten non-medicated rats. Alveolar bone area, alveolar bone level, and attachment level were determined by histomorphometric analysis, and gingival tissue levels of MMP-1, MMP-8, and TIMP-2 were detected by biochemical analysis. Results: Significantly greater bone area and significantly less alveolar bone and attachment loss were observed in the kaempferol application groups compared to the control groups (P<0.05). In addition, gingival tissue MMP-1 and -8 levels were significantly lower in the kaempferol application groups compared to the control groups and the periodontitis group (P<0.001). There were no statistically significant differences in TIMP-2 levels between the kaempferol and saline application groups (P>0.05). Conclusions: Kaempferol application may be useful in decreasing alveolar bone resorption, attachment loss, and MMP-1 and -8 production in experimental periodontitis.
