The factors to impact on the satisfaction of field practice among dental hygiene students (일부 치위생(학)과 학생들의 현장임상실습 만족도의 영향을 미치는 요인)

  • Objective: The purpose is to study the factors to impact on the satisfaction of field practice among dental hygiene students and to provide the basic data for the quality improvement of the field practices for dental hygiene students. Methods: A self-reported questionnaires were filled out by 108 dental hygiene students in C city. Questionnaires were composed with the general characteristics, satisfaction to major, and satisfaction to field clinical practice. The data was analyzed by chi-square statistics using SPSS 20.0 program. Results: The higher satisfaction to communication with professor or seniors in department was associated with the higher satisfaction to field clinical practice (p<0.05). The higher satisfaction to field clinical practice hours per week was associated with the higher satisfaction to field clinical practice (p<0.05). The higher interest to major subjects and the satisfaction to the laboratory environment were associated with the higher satisfaction to field clinical practice (p<0.05). Conclusions: The higher satisfaction to major was associated with the higher satisfaction to field clinical practice. Therefore, the systematic curriculum development is needed to improve the satisfaction with major and clinical practice.

A study on the preschool children dental health awareness and behavior of the educators at the nurseries and kindergartens in Chungnam region (충남지역 어린이집 및 유치원 교육담당자의 유아 구강보건의식과 행태에 관한 연구)

  • Objectives : The oral care during early childhood plays an important role to maintain sound oral health during adulthood. As the number of children's using child care facilities is on the rise recently, the children's oral health awareness and behavior of the educators at nurseries and kindergartens are to be examined. Methods : A self-recording survey was conducted on 194 educators at the nurseries and kindergartens in Chungnam region. Results : The most frequently given snack was milk, 91.8 percent of the facilities had children brush their teeth, 63.4 percent made them apply fluoride and 56.2 percent kept children's toothbrushes in an ultraviolet rays sterilizer. There was a meaningful difference in the awareness of the necessity to educate children about oral health according to the offer of the opportunity to apply fluoride (p<0.05). A meaningful difference was found in the recognition of the necessity to educate children according to their grade (p<0.05) and to train teachers themselves (p<0.05). The most desirable persons in charge of oral health education were dentists in health centers (46.9%) in order. The more experience in teaching they have had (p<0.05), when they're married (50.5%) (p<0.05) and when they're not homeroom teachers (52.6 percent), the more regular checkups they have had (p<0.05). As for the importance of oral health and the results of regular checkups, those who had answered 'very important'(42.4%) showed higher rate of regular checkup (p<0.05) than those who had answered 'important'(23.9%). When teachers have the experience to get trained about oral care (96.3%), the practice frequency of brushing teeth was proved to become higher (p<0.05). Conclusions : The educators for children should recognize the importance of oral health education, educate children to practice oral health care, and the environment and systematic foundation should be established which the educators manage effectively.

Influence on Job Satisfaction of Emotional Labor Accordance to Working Properties and Patients Type of Dental Hygienist (치과위생사의 근무특성과 환자유형에 따른 감정노동이 직무만족도에 미치는 영향)

  • In this study, the impact of the emotional labor on the job satisfaction according to the service characteristics and the patient type was investigated in the 269 dental hygienist. The average score of the emotional labor according to the general characteristics was 2.85 and it was higher in case of the male and in case of the drinkers. The average score of the job satisfaction according to the general characteristics was 3.02 and the female was higher than the male, the older professionals than the younger professionals and the married was higher than the singles. In the emotional labor according to the service characteristics, the dental hygienist was highest than the other professionals and in the job satisfaction according to the service characteristics, it was higher the higher the career and the higher the income. In the emotional labor by patient type, it was highest when the patient was male, when ages were between 20 and 64, and when it was after 7 PM. The job satisfaction by patient type was higher when the patient age was 65 years old or older, when the type of treatment was dental caries and periodontal diseases, when the consultation hour was after 7 PM and when the economic level of patient was high. The job satisfaction was increased when the emotional labor was 'average' but was analyzed as the lowest when the emotional labor was high. A hierarchical regression analysis was conducted to examine the effects of work characteristics, patient type, and emotional labor on job satisfaction(t = 1.994, p = .047), which had high monthly incomes Patients with older patients (t = 3.027, p = .003), the lower the emotional labor (t = 13.858, p = .000), the higher the job satisfaction. That is, to increase the job satisfaction of the dental health professionals, the effort to make the pleasant working environment should be made considering the emotional labor or the characteristics of patient.

Study of On- and Off-Campus Clinical Practice Satisfaction and Major Satisfaction among Dental Hygiene Students in Some Regions (일부지역 치위생(학)과 학생들의 교내·외 임상실습만족도와 전공만족도에 관한 조사연구)

  • A self-administered survey was conducted on dental hygiene students at colleges or universities in Chungcheong Province to increase the levels of practice for them and provide basic data that could improve their major satisfaction after practice. The data was collected from December 2012 to April 2013 and 766 questionnaires, except for 13 containing insincere responses, were finally analyzed using SPSS/Win18.0. The on- and off-campus practice groups formed 59.7% and 57.4%, respectively. The most frequent place for practice was university dental hospitals, followed in order by hospitals/clinics and health centers. University students were more satisfied with on-campus practice in terms of factors, such as practice contents, practice time, and practice performance than those at three-year colleges. The students in the higher grades were more satisfied with the practice time. The university students were also more satisfied with off-campus practice in terms of all the factors than those at three-year colleges. The students in higher grades were more satisfied with the practice time and practice performance. The on- or off-campus practice group was generally more satisfied with their major than the non-practice group, and each of the factors-practice performance, practice time, practice environment, and practice contents-significantly affected the major satisfaction. The results highlight the need to plan and provide satisfactory education based on an understanding and awareness of students so that they can take pride in their major through on- and off-campus practice. In addition, it is also necessary to develop educational guidelines for clinical practice and efficient programs so that clinical practice can create the optimal learning situation.

Annealing of Co-Cr dental alloy: effects on nanostructure and Rockwell hardness

  • PURPOSE. The aim of the study was to evaluate the effect of annealing on the nanostructure and hardness of Co-Cr metal ceramic samples that were fabricated with a direct metal laser sintering (DMLS) technique. MATERIALS AND METHODS. Five groups of Co-Cr dental alloy samples were manufactured in a rectangular form measuring $4{\times}2{\times}2$ mm. Samples fabricated by a conventional casting technique (Group I) and prefabricated milling blanks (Group II) were examined as conventional technique groups. The DMLS samples were randomly divided into three groups as not annealed (Group III), annealed in argon atmosphere (Group IV), or annealed in oxygen atmosphere (Group V). The nanostructure was examined with the small-angle X-ray scattering method. The Rockwell hardness test was used to measure the hardness changes in each group, and the means and standard deviations were statistically analyzed by one-way ANOVA for comparison of continuous variables and Tukey's HSD test was used for post hoc analysis. P values of <.05 were accepted as statistically significant. RESULTS. The general nanostructures of the samples were composed of small spherical entities stacked atop one another in dendritic form. All groups also displayed different hardness values depending on the manufacturing technique. The annealing procedure and environment directly affected both the nanostructure and hardness of the Co-Cr alloy. Group III exhibited a non-homogeneous structure and increased hardness ($48.16{\pm}3.02$ HRC) because the annealing process was incomplete and the inner stress was not relieved. Annealing in argon atmosphere of Group IV not only relieved the inner stresses but also decreased the hardness ($27.40{\pm}3.98$ HRC). The results of fitting function presented that Group IV was the most homogeneous product as the minimum bilayer thickness was measured (7.11 ${\AA}$). CONCLUSION. After the manufacturing with DMLS technique, annealing in argon atmosphere is an essential process for Co-Cr metal ceramic substructures. The dentists should be familiar with the materials that are used in clinic for prosthodontics treatments.

Corrosion behaviors of Cp-Ti and Ti-6Al-4V alloys by TiN coating (TiN 코팅된 Ti 및 Ti-6Al-4V합금의 부식거동)

  • Cp-Ti and Ti-6Al-4V alloys commonly used dental implant materials, particularly for orthopaedic and osteosynthesis because of its suitable mechanical properties and excellent biocompatibility. This alloys have excellent corrosion behavior in the clinical environment. The first factor to decide the success of dental implantation is sufficient osseointegration and high corrosion resistance between on implant fixture and its surrounding bone tissue. In this study, in order to increase corrosion resistance and biocompatibility of Cp-Ti and Ti-6Al-4V alloy that surface of manufactured alloy was coated with TiN by RF-magnetron sputtering method. The electrochemical behavior of TiN coated Cp-Ti and Ti-6Al-4V alloy were investigated using potentiodynamic (EG&G Co, PARSTAT 2273. USA) and potentiostatic test (250mV) in 0.9% NaCl solution at 36.5 $\pm$ 1$^{\circ}C$. These results are as follows : 1. From the microstructure analysis, Cp-Ti showed the acicular structure of $\alpha$-phase and Ti-6Al-4V showed the micro-acicular structure of ${\alpha}+{\beta}$ phase. 2. From the potentiodynamic test, Ecorr value of Cp-Ti and Ti-6Al-4V alloys showed -702.48mV and -319.87mV, respectively. Ti-6Al-4V alloy value was higher than Cp-Ti alloy. 3. From the analysis of TiN and coated layer, TIN coated surface showed columnar structure with 800 nm thickness. 4. The corrosion resistance of TiN coated Cp-Ti and Ti-6Al-4V alloys were higher than those of the non-coated Ti alloys in 0.9% NaCl solution from potentiodynamic test, indicating better protective effect. 5. The passivation current density of TiN coated Cp-Ti and Ti-6Al-4V alloys were smaller than that of the noncoated implant fixture in 0.9% NaCl solution, indicating the good protective effect resulting from more compact and homogeneous layer formation.

Clinical outcome of conservative treatment of injured inferior alveolar nerve during dental implant placement

  • Objectives: Infererior alveolar nerve (IAN) damage may be one of the distressing complications occurring during implant placement. Because of nature of closed injury, a large proportion is approached non-invasively. The purpose of this study was to analyze the outcomes of conservative management of the injured nerve during dental implant procedure. Materials and Methods: Sixty-four patients of implant related IAN injury, who were managed by medication or observation from January 1997 to March 2007 at the Department of Oral and Maxillofacial Surgery, Seoul National University Dental Hospital, were retrospectively investigated. The objective tests and subjective evaluations were performed to evaluate the degree of damage and duration of sensory disturbance recovery. Tests were performed on the day of the first visit and every two months afterward. Patient's initial symptoms, proximity of the implant to the IAN, time interval between implant surgery and the first visit to our clinic, and treatment after implant surgery were analyzed to determine whether these factors affected the final outcomes. Results: Among the 64 patients, 23 had a chief complaint of sensory disturbance and others with dysesthesia. The mean time until first visit to our hospital after the injury was 10.9 months.One year after nerve injury, the sensation was improved in 9 patients, whereas not improved in 38 patients, even 4 patients experienced deterioration. Better prognosis was observed in the group of patients with early visits and with implants placed or managed not too close to the IAN. Conclusion: Nearly 70% of patients with IAN injury during implant placement showed no improvement in sensation or dysesthesia with the conservative management. Earlier decision for active treatment needs to be considered because of possibility of deterioration of symptoms and unsatisfactory recovery.

Accuracy of computer-aided template-guided oral implant placement: a prospective clinical study

  • Purpose: The aim of the present study was to evaluate the in vivo accuracy of flapless, computer-aided implant placement by comparing the three-dimensional (3D) position of planned and placed implants through an analysis of linear and angular deviations. Methods: Implant position was virtually planned using 3D planning software based on the functional and aesthetic requirements of the final restorations. Computer-aided design/computer-assisted manufacture technology was used to transfer the virtual plan to the surgical environment. The 3D position of the planned and placed implants, in terms of the linear deviations of the implant head and apex and the angular deviations of the implant axis, was compared by overlapping the pre- and postoperative computed tomography scans using dedicated software. Results: The comparison of 14 implants showed a mean linear deviation of the implant head of 0.56 mm (standard deviation [SD], 0.23), a mean linear deviation of the implant apex of 0.64 mm (SD, 0.29), and a mean angular deviation of the long axis of $2.42^{\circ}$ (SD, 1.02). Conclusions: In the present study, computer-aided flapless implant surgery seemed to provide several advantages to the clinicians as compared to the standard procedure; however, linear and angular deviations are to be expected. Therefore, accurate presurgical planning taking into account anatomical limitations and prosthetic demands is mandatory to ensure a predictable treatment, without incurring possible intra- and postoperative complications.

A Study on the Change of Dental Scaling Experience in Some Areas after Applying Scaling Insurance (스케일링 보험적용에 따른 일부지역의 스케일링 경험 변화 연구)

  • The purpose of this study is to investigate the regional change of dental scaling experience by scaling insurance coverage in July, 2013. The data were used in the "Community Health Survey" of the 2012 and 2014. The results of the study are as follows; 1) The subjective oral health status and brushing of lunch was highest in Gangnam-gu in both 2012 and 2014(p<0.001). 2) Regular dental check-up was high in Gangnam-gu in both 2012 and 2014(p<0.001). 3) The Scaling experience rate increased in all three regions(p<0.001). 4) The socio-demographic characteristics and scaling experience were higher in 2012 and 2014(p<0.001). The scaling experience was higher when there were office workers and spouses(p<0.001). From the policy perspective, it seems necessary to take measures to reduce the gap in scaling experience rate due to differences in income and unequal medical environment.
