  • The purpose of this study was to compare the distributing pattern of stress on the finite element models of two units implant prosthesis with one angulated placement of two implant fixtures. The two unit implant crowns simulated to mandibular first and second molars were made. The two kinds of finite element models were designed according to angulation of fixture ($4.0mm{\times}11.5mm$) : Model 1($15^{\circ}$ buccally angulated placement of one fixture on second molar area), Model 2($15^{\circ}$ lingually angulated placement of one fixture on second molar area). Axial loads of 200N were applied to the center of central fossa and to distance of 2mm and 4mm apart from the center of central fossa. Von-Mises stresses were recorded and compared in the fixtures, and buccolingual section of implants. The results were as follows: 1. Under axial loading at the central fossa, the stress was distributed along the straight fixture except apical portion, while on buccally or lingually angulated placement, the highest stresses were concentrated in the neck portion on the opposite side of the angulated fixture. 2. With offset distance increasing, the stresses were concentrated greater in buccal neck of lingually angulated fixture than in lingual neck of buccally angulated fixture. From the above results, in designing of the occlusal scheme for implant prosthesis with the angulated fixture, occlusal contacts should be placed to distribute stress axially in maximum intercuspation and to avoid offset force during eccentric movements.

  • STATEMENT OF PROBLEM: Recently, various all-ceramic crowns fabricated with CAD/CAM systems have come into wide use in dental clinic. However, there are only few domestic studies on CAD/CAM restorations. PURPOSE: Purpose of this study was to compare the fidelity (absolute marginal discrepancy and internal gap) between various cores fabricated with different CAD/CAM systems (Procera system, Lava system, Cerec inLab system) and conventional metal cast core. MATERIALS AND METHODS: 10 cores per each system were fabricated. The absolute marginal discrepancies were measured using measuring microscope and digital counter. The internal gaps were calculated using a silicone paste. The results were statistically analyzed using the one-way ANOVA test and Tukey's HSD test. RESULTS: Within the limits of this study the results were as follows. 1. The absolute marginal discrepancies were $32.5{\pm}3.7\;{\mu}m$ for metal cast core, $72.2{\pm}7.0\;{\mu}m$ for Procera core, $40.8{\pm}5.4\;{\mu}m$ for Lava core, and $55.3{\pm}8.7\;{\mu}m$ for Cerec inLab core. The internal gaps were $38.4{\pm}5.7\;{\mu}m$ for metal cast core, $71.4{\pm}5.3\;{\mu}m$ for Procera core, $45.9{\pm}7.3\;{\mu}m$ for Lava core, and $51.8{\pm}6.2\;{\mu}m$ for Cerec inLab core. 2. The fidelity of metal cast core showed the smallest gaps, followed by Lava core, Cerec inLab core, and Procera core. CONCLUSION: The fidelities of 4 core groups were all within the clinically acceptable range ($120\;{\mu}m$).

  • Purpose: The aim of this retrospective chart review was to evaluate the four-year survival rate of a titanium implant system. Methods: A total of 352 sand-blasted, thermally acid-etched titanium implants were inserted into 181 partially or completely edentulous patients. Their cumulative survival rate was evaluated retrospectively. Associated factors, such as the implant distribution and treatment type were included in the evaluation. Results: The implants were equally distributed between the maxilla (52.3%) and the mandible (47.7%). 48 implants (13.6%) were placed in the anterior region and 304 implants (86.4%) in the posterior region. The majority of the implants were inserted into bone of type II and III quality (89.8%) and volume (quantity B and C, 87.2%). Most of the implants (70.7%) were restored as single crowns; 28.7% supported a bridge construction and 0.6% a full denture. Only one implant failed, resulting in a four-year cumulative survival rate of 99.7%. Conclusions: The implant system showed an excellent four-year survival rate. It proved to be a safe and predictable means for restoration of the dentition in partially or completely edentulous patients.

  • An in vitro study was conducted to compare the bond strength of cements between Verabond coping and various cores. Fifty-four idential cores simulating maxillary central incisor prepared for PFM crowns were made. Eighteen samples were made with 20K cast gold, eighteen with Verabond, and eighteen with Adaptic. Samples were randomly divided into three groups, each consisting of six 20K cast gold, six verabond, and six Adaptic samples. The first group was cemented with zinc phosphate cement, the second group with poly-carboxylate cement, and the third group with glass ionomer cement. Constant finger pressure was applied for cementation. The sample were then stored at $37^{\circ}C$ in distilled water bath for 24 hours. The tensile strength test was performed on an Instron Universal test machine with crosshead speed of 0.05cm/min and the results compared statistically. Results of the study showed that: 1. A significant difference of bond strength was observed with different types of dental cements and core materials. 2. With gold core, zinc phosphate cement was stronger than both the polycarboxylate cement and glass ionomer cement, which did not differ in bond strength. 3. With base-metal core, zinc phosphate cement showed the highest bond strength and was followed by polycarboxylate cement and glass ionomer cement. 4. With composite resin core, zinc phosphate cement showed the highest bond strength and was followed by glass ionomer cement and polycarboxylate cement. 5. The base-metal core (Verabond core) privided the highest retention of all core materials.

  • Statement of problem: A tenderness of the dentin after tapering of teeth for dental prosthesis is a common phenomenon. In practice, the alternative desensitizer may be used for minor pain after tapering of teeth. Purpose: In this study, the desensitizers were used to investigate the affect decreasing of shear bond strength according to the use of various cement, such as resin, Glass Ionomer, and phosphate cement. Material and method: Three different desensitizers were used on this study Compositions of two dentin desensitizers were HEMA(hydroxyethylmethacrylate) and glutaraldehyde. The other one is oxalic acid. Three dentin desensitizers applied on 12 degrees taper teeth. Then, Ni-Cr crowns were bonded with Resin cement, Zinc Phosphate (ZPC) cement and Glass Ionomer (GIC) cement. 120 human premolar teeth were used for specimens. The specimens were divided into four group as the reference and the empirical each with thirty specimens, then further divided into 12 group according to type of desensitizers and cement types. The shear bond strength were measured by Instron multi task instrument. Results: According to the result, the measured shear bond strength in order from the weakest to the strongest in general was ZPC, Resin, and GIC. And it is found that the application of desensitizers on dentin surface does not affect the shear bond strength. Conclusion: Dentin desensitizers that alleviate or prevent a dentin tenderness, usually contains HEMA and glutaraldehyde compounds. Such desensitizers are widely used in clinical studies. By applying the dentin desensitizer on the exposed dentin surface, the dentin capillary are blocked and periodontal membrane and cementum can not be drawn in pulp cavity. Since HEMA and glutaraldehyde may cause harm to the pulp cavity, an alternative desensitizer was developed.
