Impact of chronic disease on oral health behavior (만성질환이 구강건강 행동에 미치는 영향)

  • Objectives: The purpose of this study was to examine the impact of chronic disease on oral health behavior. Methods: The subjects were 317 adults over 30-years old living in urban and rural areas. They were selected by convenience sampling method and filled out the self-reported questionnaire. The questionnaire consisted of general characteristics, dental treatment, chronic disease, and oral health behavior including oral health self-care behavior and professional oral health care. Results: The self-care oral health behavior and the professional oral health care had a negative correlation with the chronic diseases. Especially, the self-care oral health behavior and the professional oral health care had a statistically significant negative correlation with hypertension and osteoporosis. Multiple regression analysis was performed after including general characteristics, dental treatment, chronic disease. Meanwhile the presence of chronic disease had a significant influence on the self-care oral health behavior and the professional oral health care. Hypertension and Osteoporosis were the most influential factors of chronic diseases and had a significant influence on the oral health behavior. In conclusion, the chronic diseases aggravated the oral health behavior practice. Conclusions: presence of chronic disease affects oral health behavior. Therefore, the effective intervention and education programs related to oral health care are necessary to enhance adult's oral health behavior and total health. The continuous follow-up study will determine the causal relationship between oral health behavior and the presence of chronic disease.

The effect of self-efficacy and job satisfaction of Dental Hygienists' on the organizational commitment and turnover intention (치과위생사의 자기효능감과 직무만족이 조직몰입과 이직의도에 미치는 영향)

  • Objectives : The purpose of this study is to identify relationships between turnover intention and variable factors among dental hygienists. Methods : Each factor of validity and reliability were investigated using factor analysis and Cronbach's ${\alpha}$. The hypothesis of the data was proven and analyzed using the pearson correlation, regression analysis, and analysis of variance (ANOVA). Results : The independent variables were based on self-efficacy, the dependent variables were based on job satisfaction. The results of the regression analysis were that self-efficacy is as high as job satisfaction. Independent variables based on self-efficacy and job satisfaction. The dependent variable is organizational commitment multiple regression analysis was used. The results showed that self-efficacy and job satisfaction are as high as organizational commitment. Independent variables based on organizational commitment. The dependent variable is the turnover-intention. Simple regression analysis showed that organizational commitment is as low as turnover-intention. Self-efficacy, job satisfaction and organizational commitment are different depending on individual traits and personal characteristics. Self-efficacy has been shown to have a positive relationship with job experiences, public work and turnover intention. Organizational commitment is also related with job experiences. However, there was no significant difference between job satisfaction, turnover intention and personal characteristics. Conclusions : According to the factor analysis of turnover intention, self-efficacy, job satisfaction, organizational commitment and turnover intention were correlated to each other. Self-efficacy, job satisfaction, and organizational commitment has a negative association with turnover intention.

Relationship between the oral contraceptive pill and periodontal disease in Korean women: The 5th Korean National Health and Nutrition Survey data analysis phase3(2012) (경구피임약을 복용하는 한국여성과 치주질환의 관련성 연구: 제5기 3차년도(2012) 국민건강영양조사 자료 분석)

  • Objectives: The purpose of the study was to investigate the relationship between the oral contraceptive pill and periodontal disease in Korean women aged from 19 to 50 years old. Methods: This study selected 1,579 women of childbearing age from 19 to 50 years old from the 5th National Health and Nutrition Survey data analysis(phase 3, 2012) taking the oral examination. The questionnaire consisted of socioeconomic demographic characteristics, health behavior, use of oral contraceptive pills, and periodontal disease. Socioeconomic demographic characteristics included age, marital status, education, monthly income, and vocation. The health behavior included obesity, stress, smoking, subjective oral health status, use of dental floss, tooth brushing, and diabetes mellitus by fasting blood sugar level. Use of oral contraceptive pills was recorded by monthly use and duration. The periodontal disease was documented by yes or no and selected as dependent variable by logistic regression analysis. Results: After revising the taking period of oral contraceptive pill for this study, there was the correlation between the prevalence of periodontal disease and odds ratio(95% CI) 1.288(1.027-1.617). Conclusions: This study will contribute to the direction of policy for an oral contraceptive pill and provide the basic data for counseling for the oral health and the side effects of oral contraceptive pills.

The research regarding the self concept of part dental hygienes students (일부 치위생과 학생의 자아개념에 관한 연구)

  • The research which it sees against the self concept of part dental hygienes students from the hazard which it examines dental hygienes which is located on Kyoung-Buk and students 318 people in the object un-signature it enforced a research with the entry method which it got a same result. 1. From general characteristics of part hygiene students "the employment went well from the motive which selects hygiene" and it appeared most highly. The employment will go well and the social advance will do and ease when seeing specially with characteristics of college, it has an corresponding with the fact that is a possibility of employment.. 2. The average score against the self concept of part hygiene students "me it is an existence which is isolated from this world and nothing to do with the others". It appeared most highly with 3.52. The negation self concept appeared more highly affirmative self concept than. 3. To self concept and grade, religion, health condition, hygiene selection motive, subject record and native high school of part hygiene students the difference which considers statistically did not appear(p>0.05). However it tries to observe the self concept which it follows subject satisfactory degree, it is very unsatisfactorily(3.37) with it is unsatisfactorily(3.16) gaps it was not visible the difference which considers statistically, it is very satisfied(3.37) with the difference which considers statistically it was visible(p<0.05). Students the affirmative reaction is correct phenomenon in order to establish a specialty intuition. 4. When self concept and record and the subject satisfaction sees a interrelation for, Between self concept subject satisfaction is appeare big affirmative interrelation(r=0.331) and the subject record and subject satisfaction just interrelation appeared as well(r=0.282) all they considered statistically and appeared.(p=0.000) but to self concept and subject record for the interrelation did not appear. Negation self concept of part hygiene students appears more strongly than affirmative one, And studying for self concept of the students, it leads through the program which is the possibility to pull affirmatively is demanded.

Prevention of Mother-to-child Transmission of Streptococcus mutans (치아우식증 유발세균의 모자감염)

  • Objective: Transmission of S. mutans, a major dental caries pathogen, occurs mainly during the first 2.5 years of age. Children appear to acquire S. mutans mostly from their mothers, but few studies have investigated preventive effect of xylitol to S. mutans transmission from mother to child. The aim of this study was to perform a follow-up evaluation the preventive effect of xylitol chewing gum of the S. mutans of children's oral cavities, which included the characteristics of vertical transmission from mother to child. Methods: The mothers voluntarily participating in a women's oral health prevention program were divided into two groups (a control and a xylitol group). The subjects were 20 mother-child pairs, who were monitored for 30 months. Xylitol chewing gum group had consumed 2 gum pellets, 3 times a day for 24 months, and then they were followed until 30 months. At baseline, 24 and 30 months whole stimulated saliva samples were collected from the mothers. Children were also recruited from 6 months to 30 months after birth and were collected their dental plaque samples. After isolation and identification, the analysis of the colony count, transmission electron microscopy and real-time RT-PCR were performed to analyze the characteristics of S. mutans. Results: The S. mutans counts decreased steadily in the xylitol group at 24 months, but increased at 30 months. The similar results were showed at their children. While the glucan synthesis was decreased at xylitol group both mother and child. The expression of gtfB, gtfD and ftf were significantly reduced in the xylitol group both mother and child (p<0.05). Conclusions: These findings indicate that chewing xylitol gum over a long period may decrease the expression of the genes associated virulence and reduced the glucan synthesis of S. mutans, which can result the preventing the mother-to-child transmission of S. mutans.

A Study on Factors Affecting Dental Hygienists' Job Stress (치과위생사의 직무 스트레스에 영향을 미치는 요인 분석)

  • The purpose of this study is to find options which reduce the job stress and to enhance morale through a variable analysis, and also apply this data in the legal and administration fields. The data for the research was obtained from 217 dental hygienists working in the dental clinics, dental hospitals and university's dental hospitals' in Seoul. The tool used by this study was a Questionnaire measuring on-the-job stress of dental hygienists, was a modified and revised. The Questionnaire version used to measure bank's job stress and used to suggest options by Dae-ha Yoon(1991). Classification of items for measuring job stress was based on Theoretical Study on Job Stress by Dae-hyon Song(1986). The scale used in study was the two points (yes or no in the job stress) scale measuring for variables. The period of data collection was 30 days from December 4, 1995 to January 5, 1996. The methods of data collection were self-writing, direct visit, and postal Questionnaire answering, 224 copies of Questionnaire data were collected, but only 217 copies were used. 7 copies could not be analyzed, were not used for this study. The data analysis was conducted by SPSS after coding the collected raw data. The general characteristics was obtained from real digits and percentages. In order to analyze the difference of sub-variables against the job stress based on general characteristics. Mean, Standard Deviation, and F test (ANDVA) were conducted. The following were the results of job stress variables: 1. Meaningful variable affecting the working organization, is compensation system(p〈0.03). 2. Meaningful variable affecting the working period, is work overload(p〈0.02). 3. Meaningful variable affecting average patients per day, is the career management and payment(p〈0.04, p〈0.01). 4. Meaningful variable affecting number of staff, is the comrade relationship, role conflict among patients, work overload, and job overload(p〈0.000, p〈0.05, p〈0.04, p〈0.01). The comrade relationship is most affected to the number of staffs aides. 5. Meaningful variables affecting job diversion, are the desire and value, the non-role play, and the environment(p〈0.003, p〈0.02, p〈0.005).

A comparison of surface hardness and microstructural characteristics between a type IV stone with and without die hardening treatment, and a polyurethane resin die material (IV형 경석고와 경화처리된 IV형 경석고 그리고 폴리우레탄 수지 치형재의 표면경도와 미세구조 특성의 비교)

  • Purpose: This study compared the surface hardness (Vickers) and microstructural characteristics between a type IV stone with and without die hardening treatment, a polyurethane die material. Methods: Materials used were a type IV stone(MG Crystal Rock), two die hardeners (Hardening bath, Epox-it), and a polyurethane resin material(Polyluck). Six specimens per group were prepared according to manufacturer's directions. The prepared specimens were tested by means of hardness test, one-way ANOVA analysis, scanning electron microscopic(SEM) observations and energy dispersive spectroscopic(EDS) analysis. Results: In the hardness test and its statistical analysis, there was no significant difference in the surface hardness between a type IV stone and type IV stone with die hardener coating, type IV stone mixed with an epoxy like material instead of water. In contrast, polyurethane resin material exhibited significantly greater surface hardness than other specimen groups (p<0.05). Conclusion: By considering the results of the hardness test, SEM observations and EDS analysis, although the die hardeners on type IV stone did not show remarkable improvement in surface hardness, the die hardener coating on the surface of type IV stone material did show decrease of microporous and improvement of surface defects.

Dental trauma trends in emergency care: a comparative analysis before, during, and after COVID-19

  • Objectives: This analysis details the characteristics of dental trauma in South Korea during the coronavirus disease 2019 (COVID-19) (DC) pandemic and compares them in patients before and after COVID-19 (BC and AC, respectively). Materials and Methods: Data were collected from medical records of patients who visited Seoul National University Bundang Hospital's Emergency Dental Care Center during three 12-month periods: BC, DC, and AC (BC from March 1, 2019 to February 29, 2020; DC from March 1, 2020 to February 28, 2021; AC from March 1, 2022 to February 28, 2023). A retrospective review was conducted to investigate patient age, sex, time of visit, cause, and diagnosis. The study included 1,544 patients: 660 BC, 374 DC, and 510 AC. Results: Significant difference in age and sex was not observed among the three periods; 1-9 years of age was the largest group (38.3% in BC, 29.6% in DC, and 27.8% in AC), and the percentage of male patients was greater than of female patients (male proportion as 63.5% in BC, 67.4% in DC, and 64.9% in AC). The number of patients generally peaked at a Saturday night in spring (for BC: May, Saturday, 18:00-19:59; for DC: March, Saturday, 18:00-19:59; for AC: April as the second most (October as the most peaked), Saturday, 20:00-21:59). The primary etiology of the dental trauma was identical in the three periods: falls, followed by sports. The most frequent diagnosis was laceration, followed by tooth avulsion and jaw fracture. Conclusion: Significant differences were not found between the characteristics and patterns of dental trauma in the BC, DC, and AC periods. However, due to the pandemic and social distancing, activities decreased and associated dental trauma-related incidents declined.

ANALYSIS OF DENTAL FEAR AND ITS RELATED FACTORS USING DENTAL FEAR SURVEY AMONG 13 TO 18 YEAR OLDS (13-18세 청소년을 대상으로 Dental Fear Survey 척도를 이용한 치과공포도 및 그 관련요인 분석)

  • Dental fear is one of the main barriers to the use of dental services, leading patients to avoid periodical dental check-ups or treatments, thus making oral health worse, and sometimes becoming the reason that dental professionals fail control the behavior of patients. Therefore, a dental fear must be controlled carefully in order to promote oral health and effective dental treatment. This study was taken from 313 people 13 to 18 year olds to measure their levels of dental fear. After analyzing the characteristics of dental fear and its related factors, as well as other factors which influence dental fear, we acquired the following results. 1. The level of dental fear was high, compared with advanced countries with relatively good oral health. 2. The strongest physiological response experienced during a dental treatment was the tension of muscles. These dental fears were mainly related to anesthetic needles and drills. 3. Levels of dental fear became higher, the number of times for the dental services utilization had reduced, avoid regular dental examination and perceived oral disease symptoms increased. 4. One of the biggest influences on dental fears turns out to be direct painful experiences and beliefs about dentists.

A study on the knowledge and attitude of dental hygienists for infection control in dental clinic (치과진료실 감염 예방에 관한 치과위생사의 지식 및 태도 연구)

  • Objectives : The purpose of this study was to examine the knowledge of dental hygienists on infection control in dental office and their attitude toward that in a bid to provide some information on ways of enhancing the level of infection control in dental office. Methods : The subjects in this study were 220 dental hygienists who worked in dental hospitals and clinics in Busan and South Gyeongsang Province. A survey was conducted from May 17 to June 17, 2010, and the answer sheets from 183 respondents were analyzed with a SPSS WIN 12.0 program. Statistical data on frequency, percentage and mean were obtained, and t-test, Pearson correlation coefficient and one-way ANOVA were utilized. Results : They got a mean of $4.59{\pm}.68$ in six categories of infection control knowledge. They had the best knowledge on dental waste disposal, followed by hand washing, post-sterilization management, instrument disinfection and sterilization, surface management of dental equipment and wearing personal protective equipment. They got a mean of $3.99{\pm}.54$ in attitude, and they scored lowest in practice of surface management of dental equipment. Overall, they scored higher in every aspect of knowledge than in attitude(t=11.410, p=.0.000). There was the greatest gap between their knowledge and practice in surface management of dental equipment (t=13.885, p=0.000), and there was the smallest gap between their knowledge and practice in hand washing(t=5.460, p=0.000). And a positive correlation was found between knowledge and attitude, as better knowledge of infection control led to better attitude toward that(p<.001). Finally, concerning infection control knowledge and attitude by general characteristics, the presence or absence of infection control guidelines made differences to infection control knowledge, and infection control attitude was statistically significantly different according to infection control education experiences(t=6.501, p=.012) and the presence or absence of infection control guidelines(t=22.836, p=.000). Conclusions : In order to bolster infection control in dental office, the related system should be improved to legally require dental personnels to implement infection control. Every dental office must be furnished with infection control guidelines, and sustained education should be provided for dental hygienists to carry out infection control.
