Paresthesia diagnosed using cone-beam computed tomography: a case report

  • Various dental procedures, such as injection administration, surgical treatment, and endodontic treatment, can cause injury to the nerves. The most commonly injured nerves are the inferior alveolar and lingual nerves. This can manifest as altered sensation to the area of innervation of the injured nerve, such as the lower lip, chin, teeth, tongue, and mucosa. Altered sensations or loss of sensation are relatively infrequent complications in daily dental practice. Here, we report an uncommon case of altered sensation in the midfacial region caused by an endodontic procedure and discuss the need to consider local dental causes in the differential diagnosis of numbness in the facial region.

  • Patients with severe mental challenges are often subjected to extensive dental problems. Their impaired communication and lack of cooperation unable to receive timely dental treatments, which deteriorates their adverse oral conditions. In spite of a limited time frame, well-planned comprehensive treatments can be delivered in an one-day operation setting under general anesthesia. This multidisciplinary treatment regimen primarily comprises of single-visit endodontics, direct bonded restoration, periodontal treatment, and surgical extraction. Further, one or two additional visits for prosthetic procedures are able to accomplish functional reestablishment and esthetic enhancement. This article discusses on clinical managements for severely mentally challenged patients under a single-day operation.

치과적 중증 장애인 환자를 위한 치료 계획 수립 (How to establish dental treatment plans for the patients with severe disabilities)

  • This article discusses how to conduct treatment planning and decision making in special needs dentistry. Special needs patients often lack cooperative ability during dental treatment and have a deteriorated oral health status. To overcome the limitation in communication with special needs patients and solve their complicated dental problems, dentists need to have extensive preoperative information about the patients and their caregivers. Treatment procedures should be organized in a patient-centered and cost-effective manner. Additionally, clinical outcomes need to be predicted taking into consideration of the patients' condition. The clinical experience of committed dentists is another factor that enhances the benefits of extensive treatment in special needs patients with many limitations. The insightful treatment decision-making of dentists will contribute to improving the oral health of special needs patients despite the various obstacles.

Disappearance of a dental implant after migration into the maxillary sinus: an unusual case

  • Migration of dental implants into the maxillary sinus is uncommon. However, poor bone quality and quantity in the posterior maxilla can increase the potential for this complication to arise during implant placement procedures. The aim of this report is to present a dental implant that migrated into the maxillary sinus and disappeared. A 53-year-old male patient was referred to us by his dentist after a dental implant migrated into his maxillary sinus. The displaced implant was discovered on a panoramic radiograph taken five days before his referral. Using computed tomography, we determined that the displaced dental implant was not in the antrum. There was also no sign of oroantral fistula. Because of the small size of the displaced implant, we think that the implant may have left the maxillary sinus via the ostium.

  • 치과의사는 진료실에서 불안이나 치과공포증을 가지는 환자들을 자주 만나게 된다. 치과의사들은 환자의 불안과 공포를 해결하기 위하여 여러 가지 비약물적인 방법을 먼저 고려하게 된다. 그러나 약물을 사용하는 다양한 치과 내 진정요법이 치과공포증 환자에게 더 효과적일 수 있다. 저자는 4명의 치료 증례를 통해 가벼운 아산화질소 흡입진정으로부터 전신마취 등의 방법으로 성인 치과공포증 환자에 대처하였던 과정을 소개하고자 한다.

치과계 변해야한다 (Executive Strategies for CPD Program Enhancement of Korean Dentist)

  • We had our share of success in the dental industry in 2009, but we also had our bits of problems. I regret that our still unsolved bits of problems were some of the more important issues in dentistry. We have tried our best with passion, but we faced many limitations. I hope in expectation, that we analyze these problems so that we may overcome them. I hope that the Korean Dental Association be equipped with valid evidence in decision making procedures corresponding to social justice, heighten our dentists pride by respecting the law and fulfill our duty as national oral health caretakers. The Korean Dental Association claims the goal as ${\AE}$Your Dentist, World Best, World Best Dentist with the people$^{\circ}\phi$ and the association should establish related policies and should be able to execute them. Also the association should be able to manage and execute the duties presented in the association regulations, thus, the importance of duty division according to profession must be emphasized.

  • The number of geriatric patients seeking dental service is ever-rising because of increased life expectancy, also with problem of increased chronic medical conditions. One of them are patients on anti-thrombotic medication. Bleeding complication after minor oral surgery by anti-thrombotic agents is of concerns to dentists on dental management of these patients. Risk and benefit of the anti-thrombotic agents must be weighed before initiating dental procedures, which should be established as a treatment guideline. Purpose of the paper is to optimize the management of the dental patients on antithrombotic medication via standardization of treatment protocol of such a patient.

  • Despite all preventive efforts, aspiration of foreign body during clinical dental procedures occurs occasionally. Careful clinical physical examination, proper assessment of radiography and prompt emergency treatment can prevent any potentially serious consequences. In this report, we will report serial cases of foreign body in different situation from mild cases to severe cases and suggest some clinical guidelines to dental health care providers.

덱스메데토미딘의 임상적인 사용 (Dexmedetomidine: Clinical use)

  • Dexmedetomidine is a potent alpha-2-adrenergic agonist, more selective than clonidine, with widespread actions on the mammalian brain. A large body of recent work supports its analgesia and sympatholytic properties. Dexmedetomidine is a useful medication with many clinical applications. The medication has shown efficacy in decreasing the need for opioids, benzodiazepines, propofol, and other sedative medications. Dexmedetomidine has been used effectively for sedation during invasive procedures and in the ICU. Short-term sedation has been shown to be safe in studies, although hypotension and bradycardia are the most significant side effects. Dexmedetomidine is emerging as an effective therapeutic agent in the management of a wide range of clinical conditions with an efficacious, safe profile.

Effectiveness of dental implantation with the partial split-flap technique on vertical guided bone regeneration: a retrospective study

  • Purpose: This study aimed to evaluate the effectiveness of the partial split-flap technique with a K-incision on vertical guided bone regeneration (vGBR) and to retrospectively analyze the clinical and radiographic outcomes of dental implantation using this approach. Methods: In total, 78 patients who received 104 dental implants with vGBR, categorized as (1) pre-GBR and post-implantation and (2) simultaneous GBR and implantation, were enrolled. Data analysis was based on periapical radiographs, clinical photos, and dental records. The 2-sample t-test was used to compare the 2 surgical procedures. Results: The baseline vertical bone level, augmented bone height (ABH), and treatment duration were significantly higher in the pre-GBR procedure group. The survival rates of the implants were 96.1% and 94.8% in implant- and patient-based analyses, respectively. In Cox regression analysis, high rates of implant failure were found in the presence of ABH of ≥4 mm, smoking, and diabetes. Conclusions: Within the limitations of this retrospective study, the partial split-flap technique using a K-incision for vGBR showed stable clinical outcomes and favorable dental implant survival.
