The opinions of health care workers on the inclusion of dental hygienists in the category of medical personnel (치과위생사의 의료인화에 대한 의료종사자들의 견해)

  • Objectives: The study investigated health professionals working in the metropolitan area and Jeolla-bukdo, South Korea, from April 30 to May 17, 2017 to understand their views on the issue of including dental hygienists in the scope of medical personnel of South Korea. Methods:A total of 270 surveys were analyzed in this study. The survey consists of 5 questions on general issues; 10 on the awareness of present work of dental hygienist; and 1 on opposition or approval about including dental hygienist in medical personnel. The collected data were analyzed using SPSS for Windows 18.0, as well as frequency analysis, cross analysis and logistic regression analysis. Statistical significance level (${\alpha}$) is 0.05. Results: 1. Broken down by gender, male subjects showed more oppositions against including dental hygienists in the category of medical personnel while female subjects showed more favorable opinions (p<0.05). In terms of academic background, those who had graduated from graduate school or higher showed a higher propensity for opposition while those who had graduated from a 3-year college showed a higher tendency for approval on the idea (p<0.05), In terms of occupational type, health professionals showed more opposing views whereas medical technologists showed more approvals than others (p<0.001). 2. With respect to the awareness of work specialty and proficiency of dental hygienist according to general characteristics, the higher the age, the higher the awareness level was. In terms of the occupational type, medical technologists were found to have higher awareness level than health professionals (p<0.001). 3. With respect to the relationship between general characteristics and view on including dental hygienists in medical personnel, the occupational type of health professional was found to have 6.33 times more oppositions than medical technologists. When the awareness level on proficiency of dental hygienist was low, opposition was 6.52 times more frequent (p<0.05). Conclusions: Based on the findings above, the inclusion of dental hygienists in medical personnel seems necessary in properly establishing the specialty and role of dental hygienist in the dentist medical environment of the country in order to enhance national oral health related preventive dental service and expand the demand. To this end, it is necessary to provide nationwide promotion, work to change the awareness of health professionals in other occupational types, and facilitate public promotion for legal ground establishment.

  • Park Myoung Jin (1903-1957) was a respectable leader who disseminated dental medical education to make our path as the leading dental medical education developing a new global history of dental medicine. Dr. Park was born in Seoul on 3 July 1903. He graduated Kyongseong dental medical school and studied at the pharmacology department achieving his M.D. In 1938, as the president representing the Hanseong dentists association equivalent to the Japanese dentists association, Dr. Park participated in various events. After liberation, Dr. Park tried his best to achieve Korean dental medical education as the pursuit of ideal ego with self-centered ego. He reorganized the Kyongseong dental medical school and incorporated it to the Seoul National University dental college. Even during the Korea war, Dr. Park still sincerely carried out his duties as the director of the Seoul National University dental medical college by recruiting university entrants and turned out graduates. In 1954, Dr. Park as the director of the Seoul National University dental medical college, he frontiered an opportunity to adapt the American dental medicine by sending school staffs to study overseas. On 25 June 1954, Dr. Park received 25 years of meritorius service award presented by Seoul National University Dental Medical College. Further, on 6 Aril 1954, Dr. Park became a member of an academic research committee. In April 1946, Dr. Park was elected as the president of the Chosun Dentists Association(Korean Dental Association). On 19 May 1947, Dr. Park was also appointed as the director of the Korea dental medicine association leading the general meetings and academic conferences from 2nd through the 8th sessions. On 30 November 1954, as the president of the Korea dental medicine association, Dr. Park also published the Korea dental association publications. In 1957, Dr. Park donated the school housing for the principle of the Kyongseong dental medical school establishing the basis for the Korea dentists association center. Dr. Park also participated in establishment of the oral hygiene campaigne, dental administration policy, organization of the specialized subject delegation board members and the dental materials association. On 10 December 1955, we can recognize Dr. Park's respective historical consciousness through his declaration 'history is a true record of historical traces of a national'. Dr. Park was a living witness of the Korean dental industry. Especially, he stated that the origin of the Korean dentists association was in the Hanseong dentists association. Dr. Park overcame the pressure and indignity during the Japanese colonization. The joy of liberation did not last long since he also had to experience the fraticidal tragedy of the Korea war. Dr. Park was a professional dental specialist and a leader researching dental medicine. He was a great leader who understood the dental medicine and dedicated for the dentist association and dental medicine association with compassion for the nation and national as a Korean.

The Model Predicting Unqualified Dental Practitioners Experience Pattern (치과 무면허 불법시술 경험 패턴 연구 : 2006 국민 구강건강 실태조사 자료)

  • In this study, the patterns of people who get dental services from unqualified dental practitioners were identified by using "2006 National Dental Health Investigation". There were 4,543 people in total and the group was divided into two groups - those who had experience of receiving dental services from unqualified practitioners and of those who had not. The most pattern model was CHAID. It was patterned that 39% of those who are more than 68 years would experience such unqualified dental service, and it was found that 3% of those who are less than 33 years experienced such a service. It was found that 45% of those who are 55 or 68 years and woman would experience such unqualified dental service, and 32% of those who are 55 or 68 years and man experienced such a service. With the increase in health awareness, the burden of medical bills is also rising. However, cases of unqualified dental treatments that could cause extremely dangerous symptoms are also rising recently. Therefore it is most important to anticipate those groups who are most exposed to unqualified dental services, and educate them with appropriate information and publicise about the danger.

  • As the level of the national income has been improved and the whole nation health insurance project has been spread, medical demand for dental health is also increasing. To cope with the increased medical demand, the need of dental health education and preventive dental care is recognized, and the number of dental hygienists is being increased with increase in dental clinics and hospitals. While more weight is actually given to treatment than prevention in hospitals, emphasis is placed on prevention in public health centers of state-run organizations and dental hygienists are performing the duty. According to this, dental hygienists shall be treated and recognized as professionals, who instruct people in dental health and assist the dentists in hospitals. Therefore to suggest vision to the students of department of dental hygiene, give them a solid motive, and inspire them with professional sense so that they can devote themselves to their duty with pride and self-confidence in the field can lead to contribution to dental health of each individual and the whole nation, in order to train them to be dental hygienists who are active and strive for change and development as professionals and capable of adapting themselves to the field of dental medicine. In this study, the professional sense and the recognition of the way of students belonging to department of dental hygiene were examined and based on this, a new direction for department of dental hygiene of three-year college was suggested.

Factors related to English communication skills in the dental health service process of clinical dental hygienists (임상 치과위생사의 치과의료 서비스 과정에서 영어 의사소통 능력 관련 요인)

  • Objectives: This study analyzes factors related to English communication skills in the dental health services of clinical dental hygienists who provide dental medical services to foreigners. Methods: Surveys were conducted to measure students' English communication skills. Participants comprised 195 clinical dental hygienists working at dental English study cafes or who provided dental medical services to foreigners. After analyzing the differences in English communication skills, hierarchical multiple regression analysis was performed on the factors related to English communication skills. Results: English communication skill of dental hygienists was 1.96 points out of 5 points. The factors related to the English communication skill of the clinical dental hygienists were foreign patient care, language training experience, overseas living experience, and certified English proficiency. The adjusted explanatory power of this model was 53.0%. Conclusions: Dental hygienists in charge of foreign patients have experience in language training and overseas residence, have a language qualification certificate, and have higher English communication skills. It is necessary to develop English language learning programs based on metaverse to develop the English communication skills of dental hygienists who provide dental health services to foreign patients and to operate a creative educational environment to increase interest in learning English.

The effect of medical service satisfaction of orthodontic treatment on life quality related to oral health(OHIP-14) (교정치료의 의료서비스 만족도가 구강건강관련 삶의 질(OHIP-14)에 미치는 영향)

  • Objectives: The purpose of this study is to investigate the relationship between perceived oral health, medical service satisfaction, and oral health impact profile (OHIP-14) and the factors affecting OHIP for orthodontic patients, and to provide basic data to improve the quality of life of orthodontic patients in accordance with oral health. Methods: The study explained the purpose and purport of this study to orthodontic patients who visit dental clinics located in Daejeon and Chungcheong province for about two months from July 01, 2019, and conducted a self‐reported questionnaire survey for 220 participants who agreed to participate in this study. of the collected questionnaires, data of 197 subjects were used for the final analysis except 23 that were inadequate. Results: As for orthodontic treatment satisfaction according to perceived oral health and OHIP, there was statistically significant difference between respondents who were 'very healthy' and 'not healthy' (p<0.05). The higher the supplementary service satisfaction, the higher the orthodontic treatment satisfaction, and the higher the perceived oral health, the OHIP was found to be the higher. The factors affecting OHIP were found to be significantly associated with orthodontic treatment satisfaction and perceived oral health. The explanatory power was 15.6%. Conclusions: Active and ongoing oral health education should be provided to dental hygienists to ensure that the patient's oral cavity is healthy, and although additional services need to be improved, it is more important to have management strategies for patients to improve orthodontic treatment satisfaction.

  • In patients with multiple missing molars, wear patterns or displacement of anterior teeth, collapsed occlusal plane, and reduction in vertical dimension may occur. Particularly, in case of a few remaining teeth on one side, a removable partial denture has biomechanical disadvantage. For this reason, an implant-assisted removable partial denture with a few implant surveyed crowns can be an alternative. In this case, due to the right mandibular posterior teeth loss, the anterior teeth were severely worn and the occlusal plane was collapsed. With minimal increasing vertical dimension, oral rehabilitation was performed using a maxillary fixed prosthesis and mandibular implant-assisted removable partial denture. As a result, functional and aesthetic clinical outcomes were obtained.

A study of hospital determinants and treatment satisfaction of patients in an orthodontic clinic, specifically targeting 20-year-old patients (교정치과 내원 환자들의 치료병원 결정요인 및 치료 만족도에 관한 연구-20대 연령층을 대상으로)

  • Objectives: The purpose of this study was to identify factors affecting choice of treatment hospital (i.e., determinants) and satisfaction with the treatment hospital, and to provide a more satisfactory and high-quality medical service for orthodontic patients. Methods: A questionnaire survey was conducted for approximately 1 month beginning in August 2017, involving patients who visited orthodontic dentistry clinics in Seoul and Gyeonggi-do. Multiple regression analysis was performed on the results to examine factors that affected satisfaction with orthodontic treatment. Results: The determinant of dental clinics was 3.90 points overall; hospital environment and facilities were highest at 4.05, followed by dental hygienist at 3.99, and dentist at 3.97. Factors influencing satisfaction with orthodontic treatment were positively influenced by dentists (p<0.01), medical procedures (p<0.01) and medical expenses (p<0.01). Conclusions: It is important to provide comfort and convenience to patients by simplifying hospital facility management, reception, and reservation procedures.

According to the dental hygienists care services of patient satisfaction (치과위생사의 의료서비스에 따른 내원환자의 만족도)

  • Objectives : The purpose of this study was to examine factors related to the satisfaction level of patients with dental hygienists services. Methods : Questionnaire survey was carried out targeting 157 dental hygienists. The collected data was performed frequency & percentage, independent t-test and Pearson's correlation coefficient analysis. Results : 1. The mean score of kind of dental hygienists was 3.07 out of a maximum 4 points and level medical treatment of dental hygienists 2.94, friendly of dental hygienists 2.75. 2. Regarding the correlation between kind, level medical treatment, friendly of dental hygienists had a statistically significant correlation of r=0.677 which was the highest correlation factor(p<0.01). 3. dental satisfaction, dental reuse intention between intention recommendation, they were statistically significant different with dental satisfaction(p<0.05) 4. Regarding the correlation between dental satisfaction, dental reuse intention, intention recommendation had a statistically significant correlation of r=0.705 which was the highest correlation factor(p<0.01). Conclusions : Dental care service, dental hygienists have significant and their to revisit and recommend.

Regional Difference of Chronic Periodontal Care Services in Korea (의료기관 종별 만성 치주염 진료의 지역 간 차이)

  • Objectives: This study aimed to investigate the regional difference of chronic periodontal care services in Korea by the analysis of 2010 raw data from Health Insurance Review and Assessment Service. Methods: The subjects were the chronic periodontitis patients over 35 years old from dental care facilities in Korea. The study population was 278,319 including 264,994 claims made by dental clinics, 8,084 by dental hospitals, 3,509 by general hospitals, and 1,732 by tertiary hospitals. Results: There was a significant difference in medical care cost benefit between the provinces(p<0.0001). The age groups showed a clear difference in the patient charge, cost of insurance, and medical care cost benefit(p<0.0001). In consideration of the first visit or revisit, there were differences in the rate of prescription, dental examination, and surgical procedures of the chronic periodontal patients from dental facilities. The radiographic use rate in the tertiary hospitals was 2.6 times higher than that of the dental clinic in the treatment of the new chronic periodontal patients. Conclusions: The use of dental services in the periodontitis is influenced by the types of medical services facility, cost of medical insurance, and patient charge. In consideration of cost benefit analysis, prevention is the most important care for the periodontitis. Regional difference in peridontitis is cause by the use of medical services and quality of treatment.
