Purpose: To determine the conversion coefficients (CCs) from the dose-area product (DAP) value to effective dose in cone-beam CT. Materials and Methods: A CBCT scanner with four fields of view (FOV) was used. Using two exposure settings of the adult standard and low dose exposure, DAP values were measured with a DAP meter in C mode ($200mm{\times}179mm$), P mode ($154mm{\times}154mm$), I mode ($102mm{\times}102mm$), and D mode ($51mm{\times}51mm$). The effective doses were also investigated at each mode using an adult male head and neck phantom and thermoluminescent chips. Linear regressive analysis of the DAP and effective dose values was used to calculate the CCs for each CBCT examination. Results: For the C mode, the P mode at the maxilla, and the P mode at the mandible, the CCs were 0.049 ${\mu}Sv/mGycm^2$, 0.067 ${\mu}Sv/mGycm^2$, and 0.064 ${\mu}Sv/mGycm^2$, respectively. For the I mode, the CCs at the maxilla and mandible were 0.076 ${\mu}Sv/mGycm^2$ and 0.095 ${\mu}Sv/mGycm^2$, respectively. For the D mode at the maxillary incisors, molars, and mandibular molars, the CCs were 0.038 ${\mu}Sv/mGycm^2$, 0.041 ${\mu}Sv/mGycm^2$, and 0.146 ${\mu}Sv/mGycm^2$, respectively. Conclusion: The CCs in one CBCT device with fixed 80 kV ranged from 0.038 ${\mu}Sv/mGycm^2$ to 0.146 ${\mu}Sv/mGycm^2$ according to the imaging modes and irradiated region and were highest for the D mode at the mandibular molar.

Maxillary canines have less anatomical diversities than other teeth. They usually have a single root and root canal. This report describes an endodontic treatment of a maxillary canine with two separated root canals which have not been reported through the demonstration of radiography and computerized tomography (CT). Even though appropriated endodontic treatment has been performed, the severe pain could happen due to lack of consideration of anatomical variations of the teeth. Therefore, the clinicians should be well aware of the possibility of anatomical variations in the root canal system during endodontic treatment even if the number of root canals is obvious such as in this case.

With rising prevalency of mouth breathing children caused by developing civilization and increasing pollution, there are many maxillary transverse discrepancy patients with undergrowth of maxilla. For improving this, maxillary mid-palatal suture splitting was often performed. The purpose of this study was to analyse the stress distribution on the craniofacial suture and cranium after rapid maxillary expansion by finite element model. The boy(13Y6M) was chosen for taking computed-tomography for finite element model. Three-dimensional model of maxilla, first premolar, first molar, buccal and lingual part of rapid maxillary expansion were constructed. 1. The alveolar bone adjacent to the first molar and the first premolar that was affected directly by rapid maxillary expansion was displaced laterally approximately 4.04mm at maximum. The force decreased toward anterior region and frontal alveolar bone displaced laterally about 3.18mm. 2. A forward maximum displacement was exhibited at zygomatic process middle region. 3. At maximum, maxillary median part experienced 0.973mm downward repositioning and 0.65mm upward repositioning at lateral alveolar bone. 4. Von mises stress was observed the largest stress distribution around teeth and zygomatic buttress. 5. The largest tensile force was observed around alveolar bone of teeth, while compression force was observed at zygomatic buttress.

Purpose: The aim of this study was to assess and compare the diagnostic performance of panoramic and occlusal radiographs in detecting submandibular sialoliths. Materials and Methods: A total of 40 patients (20 cases and 20 controls) were included in this retrospective study. Cases were defined as subjects with a submandibular sialolith confirmed by computed tomography (CT), whereas controls did not have any submandibular calcifications. Three observers with different expertise levels assessed panoramic and occlusal radiographs of all subjects for the presence of sialoliths. Intraobserver and interobserver agreement were assessed using the kappa test. Sensitivity, specificity, accuracy, positive and negative predictive values, and the diagnostic odds ratio of panoramic and occlusal radiographs in screening for submandibular sialoliths were calculated for each observer. Results: The sensitivity and specificity values for occlusal and panoramic radiographs all ranged from 80% to 100%. The lowest values of sensitivity and specificity observed among the observers were 82.6% and 80%, respectively (P=0.001). Intraobserver and interobserver agreement were higher for occlusal radiographs than for panoramic radiographs, although panoramic radiographs demonstrated a higher overall accuracy. Conclusion: Both panoramic and occlusal radiographic techniques displayed satisfactory diagnostic performance and should be considered before using a CT scan to detect submandibular sialoliths.
Purpose : To assess the width of the labial alveolar bone of the incisive canal and the width of the incisive canal on spiral computed tomographic images of the anterior portion of the maxilla. Materials and Methods : Study materials included 38 CT scans taken for preoperative planning of implant placement. Axial cross-sectioned image entirely showing the incisive canal was selected and scanned with 600 DPI resolution. The width of the labial alveolar bone of the incisive canal at an orifice to the oral cavity, middle portion, and an orifice to the nasal cavity and the diameter of the incisive canal at the middle portion were determined by two specialist using Digora for Windows 2.1 The statistical analyses were carried out using SPSS 12.0.1. Results : When the maxillary central incisors remained, the mean labial alveolar bone width were $6.81{\pm}1.41mm,\;6.46{\pm}1.33mm$, and $7.91{\pm}1.33mm$. When the maxillary central incisors were missed the mean width were $5.42{\pm}2.20mm,\;6.23{\pm}2.29mm$, and $7.89{\pm}2.13mm$. Conclusions : The labial alveolar bone width at middle portion and an orifice to the nasal cavity were of no statistical significant difference according to presence of the maxillary central incisors (P>0.05). The width between oral cavity and nasal cavity, middle portion and to nasal cavity revealed statistically significant difference (P<0.05).

PURPOSE. The purpose of this study was to propose finite element (FE) modeling methods for predicting stress distributions on teeth and mandible under chewing action. MATERIALS AND METHODS. For FE model generation, CT images of skull were translated into 3D FE models, and static analysis was performed considering linear material behaviors and nonlinear geometrical effect. To find out proper boundary and loading conditions, parametric studies were performed with various areas and directions of restraints and loading. The loading directions are prescribed to be same as direction of masseter muscle, which was referred from anatomy chart and CT image. From the analysis, strain and stress distributions of teeth and mandible were obtained and compared with experimental data for model validation. RESULTS. As a result of FE analysis, the optimized boundary condition was chosen such that 8 teeth were fixed in all directions and condyloid process was fixed in all directions except for forward and backward directions. Also, fixing a part of mandible in a lateral direction, where medial pterygoid muscle was attached, gave the more proper analytical results. Loading was prescribed in a same direction as masseter muscle. The tendency of strain distributions between the teeth predicted from the proposed model were compared with experimental results and showed good agreements. CONCLUSION. This study proposes cost efficient FE modeling method for predicting stress distributions on teeth and mandible under chewing action. The proposed modeling method is validated with experimental data and can further be used to evaluate structural safety of dental prosthesis.

Oral rehabilitation of a patient having severe periodontitis with alveolar bone resorption and periodontal inflammation presents a challenge to clinicians. However, if appropriate implant placement according to the bone shape is selected, unnecessary bone grafting or soft tissue surgery can be minimized. In recent years, using cone beam CT and software, it has become possible to operate the planned position with the surgical guide made with 3D printing technology. This case was a 70 years old female patient who required total extraction of teeth due to severe periodontitis and performed a full-mouth rehabilitation with an implant - supported fixed prosthesis. During the surgery, the implant was placed in a flapless manner through a surgical guide. Immediate loading of the temporary prosthesis made by CAD/CAM method before surgery was done. Since then, we have produced customized abutments and zirconia prostheses, and have reported satisfactory aesthetic and functional recovery.
Purpose: The prevalence of supernumerary teeth has been reported to be between 0.1% and 3.8%. The aim of this study was to determine the prevalence, clinical significance, and associated pathologies of fourth molars based on a retrospective study and a literature review. Materials and Methods: A 5-year retrospective prevalence study was conducted at the Department of Oral Diagnosis and Dentomaxillofacial Radiology of Okayama University Hospital, Okayama, Japan. The study involved extracting data from the digital records of patients from January 1, 2013 through December 31, 2017. The sampling frame included all patients who had panoramic radiographs, cone-beam computed tomography (CT), and multislice CT images during the period under review. Results: A total of 26,721 cases were reviewed and 87 fourth molars were identified. The prevalence of fourth molars in the 5-year study at Okayama was calculated as 0.32%. The mean age of patients with a fourth molar was 30.43 years, and the male-to-female ratio was 1:0.98. The vast majority of cases were in the maxilla (92%) and had normal shapes(89.7%); furthermore, 82.8% of cases were unerupted. Conclusion: The prevalence of fourth molars in the study population was found to be 0.32%, and fourth molars occurred with approximately equal frequency in males and females. Fourth molars were more common in the maxilla and were predominantly unerupted and small.

Purpose: This study aimed to evaluate periodontal and pulpal healing in replanted rat teeth, preserved under different storage conditions, with or without root conditioning using doxycycline. Methods: A total of 40 maxillary first molars extracted from 20 Sprague-Dawley rats were stored for different durations under different conditions (5 minutes in dry storage and 60 minutes in Hank's balanced salt solution [HBSS]) and subsequently replanted. The rats were divided into 2 groups based on the use of root surface treatment: the doxycycline group (root surface treated with doxycycline) and the control group (no doxycycline treatment). Eight weeks after replantation, the animals were sacrificed, and the teeth were evaluated using micro-computed tomography (micro-CT) and histomorphometric analysis. Results: In the micro-CT analysis, the doxycycline group showed the same rate of occurrence of periapical radiolucency as was observed in the control group, but a lower degree of root resorption in teeth replanted after 60 minutes of storage in HBSS (P<0.05). In the histomorphometric analysis, the doxycycline group exhibited no improvement in either pulpal or periodontal healing of the replanted tooth after 5 minutes of dry storage, but showed a lower grade of surface root resorption ($1.37{\pm}0.77$) and inflammatory resorption in the teeth stored for 60 minutes in HBSS ($1.33{\pm}0.71$). Conclusions: In conclusion, doxycycline improved the periodontal healing of replanted teeth stored for 60 minutes in HBSS, whereas doxycycline did not improve periodontal healing of replanted tooth after 5 minutes of dry storage. Within the limits of this study, doxycycline showed more favorable periodontal healing despite delayed replantation.
Purpose: This study was performed to assess and describe the imaging features of 40 cases of Stafne bone defects (SBDs) on computed tomographic (CT) examinations. Materials and Methods: This study collected data, including age and sex, from 40 patients with SBDs who underwent CT exams. The imaging features of the SBDs were assessed in terms of their location, average size, the relationship of their contour with the cortical plate of the lingual mandible, bone margins, degree of internal density, shape, topographic relationship between the defect and the mandibular edge, the distance from the SBD to the base of the mandible, and the Ariji classification (type I, II, and III). Results: The average age was 57.3 years(range, 28-78 years), and the patients were predominantly male (70%). In all cases (100%), the posterior unilateral lingual SBD variant was observed. Within the Ariji classification, type I was the most common (60%). Among the most frequently observed radiographic characteristics were thick sclerotic bone margin across the entire defect contour, completely hypointense internal content, an oval shape, and continuity with the mandibular base with discontinuity of the mandibular edge. Conclusion: This study showed that posterior SBDs could present with an oval or rounded shape, complete hypodensity, and thick sclerotic margins. Likewise, SBDs could appear almost anywhere, with minor differences from the classic SBD appearance. It is fundamental for dental practitioners to know the imaging features of SBDs, since they are diagnosed primarily based on imaging.
