치매의 FDG PET 영상 (FDG PET Imaging For Dementia)

  • Dementia is a major burden for many countries including South Korea, where life expectancy is continuously growing and the proportion of aged people is rapidly growing. Neurodegenerative disorders, such as, Alzheimer disease, dementia with Lewy bodies, frontotemporal dementia, Parkinson disease, progressive supranuclear palsy, corticobasal degeneration, Huntington disease, can cause dementia, and cerebrovascular disease also can cause dementia. Depression or hypothyroidism also can cause cognitive deficits, but they are reversible by management of underlying cause unlike the forementioned dementias. Therefore these are called pseudodementia. We are entering an era of dementia care that will be based upon the identification of potentially modifiable risk factors and early disease markers, and the application of new drugs postpone progression of dementias or target specific proteins that cause dementia. Efficient pharmacologic treatment of dementia needs not only to distinguish underlying causes of dementia but also to be installed as soon as possible. Therefore, differential diagnosis and early diagnosis of dementia are utmost importance. F-18 FDG PET is useful for clarifying dementing diseases and is also useful for early detection of the diseases. Purpose of this article is to review the current value of FDG PET for dementing diseases including differential diagnosis of dementia and prediction of evolving dementia.

한국 치매 환자의 건강보험 의·한의 진료 비용에 관한 연구 (A Study on Medical Expenses of Modern and Korean Medicine for Dementia Patients Under National Health Care)

  • Objectives: To identify the cost effectiveness of early dementia diagnosis using the 2014 Health Insurance Review & Assessment Service-National Patients Sample (HIRA-NPS). Methods: The medical costs of dementia between Western medical care and Korean medical care were compared through the reimbursement and non-reimbursement item code for dementia examination. In addition, the medical expenses of patients with dementia and mild cognitive impairment were compared and analyzed with respect to Western and Korean medical care. Results: There were 87,434 claims, of which 16,101 patients were diagnosed with dementia and 38,680,789,560 won was found to be the medical expenses. 12,881 patients (80.0%) with dementia, 3,144 patients (19.5%) with mild cognitive impairment, and 76 patients (0.5%) progressing from mild cognitive impairment to dementia. The proportion of medical expenses was 97.6% for dementia patients, 2.3% for mild cognitive impairment, and 1% for patients progressing from mild cognitive impairment to dementia. 86,070 claims (98.4%) were from Western medical care, with16,824 patients (98.2%), and the medical expenses was 38,546,895,400 won (99.7%). 1,361 claims (1.6%) were from Korean medical care, with 303 patients (1.8%), and the medical expenses was 133,894,160 won (0.3%). Conclusions: This study compared and analyzed the medical costs of dementia patients and the diagnosis of both Korean and Western medical care. The results of this study are expected to be used as basic research data for investigating cost effectiveness of developing early diagnosis of dementia.

치매의 진단, 예방 및 완화를 위한 스마트폰용 게임 애플리케이션 개발 (Integration of Application Program for Dementia Diagnosis using Biometric Sensor and Oxygen Chamber)

  • 치매는 한번 발병하면 치유가 어려운 병이지만 조기에 발견하면 치료효과를 극대화 할 수 있다.그러므로 본 연구에서는 스마트폰과 생체센서를 이용하여 치매를 조기에 진단 및 관리 할 수 있는 앱을 개발하였다. 그리고 치매진단의 결과에 따라 치매를 사전에 예방하고 치매를 지연 및 완화 할 수 있는 기능성 게임을 개발하여 적용함으로써 치매의 진단, 예방 및 완화 등의 기능을 한 번에 제공하는 통합 치매관리 도구를 제공하였다. 특히 제안한 치매완화용 게임은 단순한 화살표 조작인 아닌 양팔을 이용한 공간감각과 평형감각을 요구하므로, 치매의 치료 효과가 배가된다. 만약 제안하는 치매 관리용 스마트폰용 앱을 이용하여 꾸준히 치매를 진단하고 예방하며, 게임을 통해 완화시킨다면 치매로 인한 사회적 손실을 최소화 할 수 있을 것이다.

  • 본 연구의 목적은 생체센서와 자체진단 프로그램의 연동을 통하여 치매를 조기에 진단할 수 있는 스마트폰용 어플리케이션을 개발하는 것이다. 생체센서로부터 산소포화도와 혈압 데이터를 블루투스를 통해 수신 받고 스마트폰에 내장된 치매진단 설문지와 기억력 테스트 게임 등의 점수를 합산하여 스마트폰은 최종 치매지수를 도출해낸다. 간단한 임상실험 통해 치매 판단용 임계값을 결정하고 치매지수와 비교하여 치매를 진단하였다. 60세 이상의 노인에게 제안 어플리케이션을 적용한 결과, 초기 치매를 쉽고 빠르게 진단 가능함을 알 수 있었다. 추가적으로 산소챔버와 제안하는 치매진단용 어플리케이션을 융합함으로써 초기 치매를 치료하는 의료기기로서의 가능성을 제시하였다.

Strategies for Deriving Strategic Products to Enter the Dementia Industry on SMEs-Focused on AHP Analysis

  • Many SMEs are interested in entering the dementia industry as the national dementia responsibility system begins in earnest. However, it is ambiguous about which technology and field to enter, and accordingly, it is impossible to make a quick judgment and misses the time to enter the market. Therefore, the purpose of this study is to provide a simple but clear information to many SMEs in this situation, which products and related technologies will be able to help product development and market entry in the dementia prevention and diagnosis technology market more easily. In this regard, I would like to suggest the direction through hierarchical analysis (AHP) through conducting with a group of experts who can make professional judgments about the development of dementia medical technology, including the four-year university senior welfare department, nurses, directors and directors of long-term care institutions, medical device workers and experts' opinions on what sectors SMEs can most effectively apply to product development to enter the dementia market.

  • 본 연구는 알츠하이머 치매 환자의 MMSE-K, SGDS, NPI-Q, BADL IADL CDR 평가도구 결과를 분석하여 치매평가 도구들 간의 상관관계와 치매중등도 판정에 미치는 영향정도를 확인하고자 하였다. 본 연구는 일개 병원의 알츠하이머대상자의 치매검사자료인 2차 자료를 분석한 서술적 조사 연구이다. 본 연구는 2015년 1월부터 2017년 4월까지 일개 치매검진병원에서 등록 관리되고 있는 알츠하이머 치매 환자 총 617명의 자료를 분석하였으며, 수집된 자료는 SPSS 25.0 통계 프로그램을 이용하여 분석하였다. 치매평가도구 중 치매중등도 판정에 영향력이 큰 유의한 도구로는 CDR, ADL, MMSE-K, SGDS 도구이며, 유의한 영향을 미치지 못하는 도구로는 NPI-Q, IADL로 나타났다.

  • 인구 고령화에 따라 치매환자가 크게 늘어나고 있으며, 이에 따른 사회 경제적 부담이 커지고 있다. 현재 치매환자를 효과적으로 관리하는 방법은 조기에 치매 초기 환자를 파악하는 것이다. 그러나 의료진이 부족한 농촌과 섬 지역에서는 병원을 방문해서 조기 검사를 받기 어려운 문제가 있다. 이 문제를 해결하는 방법으로 본 논문에서는 원격 치매 조기 검사 시스템을 제안한다. 원격 치매 조기 검사 시스템은 환자가 실시간 화상 통신을 기반으로 한 원격 의료 기술을 활용하여 원격의 치매 전문가에게 검사 및 진료를 받을 수 있는 시스템이다. 치매 원격 조기 검진 시스템은 섬 지역의 보건지소 의료진이 사용하는 로컬 클라이언트 시스템, 영상을 전송 및 저장/관리하는 화상 서버, 원격 치매 전문가가 활용하는 전문가 클라이언트로 구성된다. 로컬 클라이언트는 환자가 보건지소의 의료진과 같이 사용할 수 있도록 함으로써 현 의료법의 원격협진 기준을 만족시킨다. 또한 전문가 클라이언트는 치매 전문가가 사용하며, 환자의 정보를 저장/관리하고, 환자의 이력 정보를 분석하고 향후 치매 진행 정도를 예측할 수 있다.

The Seoul Neuropsychological Screening Battery (SNSB) for Comprehensive Neuropsychological Assessment

  • The Seoul Neuropsychological Screening Battery (SNSB) is known as a representative comprehensive neuropsychological evaluation tool in Korea since its first standardization in 2003. It was the main neuropsychological evaluation tool in the Clinical Research Center for Dementia of South Korea, a large-scale multi-center cohort study in Korea that was started in 2005. Since then, it has been widely used by dementia clinicians, and further solidified its status as a representative dementia evaluation tool in Korea. Many research results related to the SNSB have been used as a basis for the diagnosis and evaluation of patients in various clinical settings, especially, in many areas of cognitive assessment, including dementia evaluation. The SNSB version that was updated in 2012 provides psychometrically improved norms and indicators through a model-based standardization procedure based on a theoretical probability distribution in the norm's development. By providing a score for each cognitive domain, it is easier to compare cognitive abilities between domains and to identify changes in cognitive domain functions over time. Through the development of the SNSB-Core, a short form composed of core tests, which also give a composite score was provided. The SNSB is a useful test battery that provides key information on the evaluation of early cognitive decline, analysis of cognitive decline patterns, judging the severity of dementia, and differential diagnosis of dementia. This review will provide a broad understanding of the SNSB by describing the test composition, contents of individual subtests, characteristics of standardization, analysis of the changed standard score, and related studies.

  • Evaluation of dementia in patients with early symptoms of cognitive decline is clinically challenging, but the need for early, accurate diagnosis has become more crucial, since several medication for the treatment of mild to moderate Alzheimer' disease are available. Many neurodegenerative diseases produce significant brain function alteration even when structural imaging (CT or MRI) reveal no specific abnormalities. The role of PET and SPECT brain imaging in the initial assessment and differential diagnosis of dementia is beginning to evolve vapidly and growing evidence indicates that appropriate incorporation of PET into the clinical work up can improve diagnostic and prognostic accuracy with respect to Alzheimer's disease, the most common cause of dementia in the geriatric population. in the fast few years, studios comparing neuropathologic examination with PET have established reliable and consistent accuracy for diagnostic evaluations using PET - accuracies substantially exceeding those of comparable studies of diagnostic value of SPECT or of both modalities assessed side by side, or of clinical evaluations done without nuclear imaging. This review deals the role of functional brain imaging techniques in the evaluation of dementias and the role of nuclear neuroimaging in the early detection and diagnosis of Alzheimer's disease.

  • Purpose : This study is a descriptive research study to establish basic data for stable operation of dementia relief center as a part of national responsibility system for dementia and to establish the role of the professional workforce. Methods : This study is a survey study involving 126 nurses working at 19 dementia relief centers. The collected data were analyzed using the SPSS/WIN 23.0. Results : The importance tasks that nurses think of were Dementia Diagnosis, Counseling, Register and Classification and Dementia Preventive Projects. In addition, tasks that showed high performance were in the order of Dementia Diagnosis, Counseling, Register and Classification, Management of Dementia subjects. As a result, the final items of work areas and contents included 45 subcategories of work content for eight work areas. Conclusion : Through this study, it was possible to investigate the overall work area and content of nurses at the Dementia relief Center. It is necessary to develop a work description suitable for nurses who are medical personnel specializing in dementia safety centers.
