Delayed paresthesia of inferior alveolar nerve after dental surgery: case report and related pathophysiology

  • Paresthesia is an altered sensation of the skin, manifesting as numbness, partial loss of local sensitivity, burning, or tingling. The inferior alveolar nerve (IAN) is the third branch of the trigeminal nerve and is very important in dental treatment. IAN paresthesia may occur after various dental procedures such as simple anesthetic injections, surgical procedures, and endodontic treatment, and is reported to range from 0.35% to 8.4%. The altered sensation usually follows immediately after the procedure, and reports of late onset of nerve involvement are rare. This report presents a rare case of delayed paresthesia after dental surgery and discusses the pathophysiology of IAN delayed paresthesia.

Early Infant Feeding Practices May Influence the Onset of Symptomatic Celiac Disease

  • Purpose: To study whether breastfeeding and breastfeeding status during gluten introduction influences the age at diagnosis of celiac disease (CD). In addition to study, whether the timing of gluten introduction influences the age at diagnosis of CD. Methods: It was a hospital based observational study. Total 198 patients diagnosed with CD as per modified European Society of Pediatric Gastroenterology, Hepatology and Nutrition (2012) criteria, aged between 6 months to 6 years were included. Detail history taken with special emphasis on breastfeeding and age of gluten introduction. Standard statistical methods used to analyze the data. Results: $Mean{\pm}standard$ deviation age of onset and diagnosis of CD in breastfed cases was $2.81{\pm}1.42$ years and $3.68{\pm}1.55$ years respectively as compared to $1.84{\pm}1.36$ years and $2.70{\pm}1.65$ years respectively in not breastfed cases (p<0.05). Those who had continued breastfeeding during gluten introduction and of longer duration had significantly delayed onset of disease. The age at onset of CD was under one year in 40.42% of the cases, who had started gluten before 6 months of age compared to only 12.58% of those who had started gluten later (p<0.001). The proposed statistical model showed that two variables, i.e., breast feeding status during gluten introduction and age at gluten introduction positively influencing the age at diagnosis of CD. Conclusion: Delayed gluten introduction to infant's diet along with continuing breastfeeding, delays symptomatic CD. However, it is not clear from our study that these infant feeding practices provide permanent protection against the disease or merely delays the symptoms.

Neural Tissue-Specific Epidermal Growth Factor (EGF)-like Domain Containing Protein, NELL2, Plays on Important Role in the Control Regulation of Puberty Onset in the Female Rat Hypothalamus

  • In the present study we determined if NELL2, a neural tissue-specific protein containing 6 epidermal growth factor (EGF)-like repeat domains, plays an important role in the regulation of puberty initiation in the rat hypothalamus. We origin811y found that NELL2 is a new estrogen-responsive gene in hypothalami derived from estrogen-sterilized and control rats using a PCR differential display. In the 40-day-old female rat hypothalamus, NELL2 was up-regulated by neonatal estrogen treatment. In situ hybridization histochemistry showed that NELL2 is very abundant in the ventromedial hypothalamic nucleus that is responsible for the control of sex behavior. NELL2 mRNA level in the medial basal hypothalamus showed a dramatic increase before female puberty onset, which suggests that NELL2 may be involved in the process regulating female puberty onset. We attemped to block NELL2 synthesis with intracerebroventricular injection of an antisense oligodeoxynucleotide (ODN) to the NELL2 mRNA, and examined its effect on the puberty onset of the female rat. The antisense ODN significantly delayed puberty initiation determined by vaginal opening. In summary, NELL2 may play an important role in the regulation of female puberty onset.

Open-Loop-Onset-Point PIO 해석의 변위한계 (Handling Deflection Limit in Open-Loop-Onset-Point PIO Analysis)

  • 항공기 조종면의 시간당 변화율 한계에 의한 PIO 현상을 해석하는 기법으로 Open-Loop-Onset-Point (OLOP) 방법이 널리 이용된다. OLOP은 조종면의 변화율 한계에 의한 PIO 현상을 주원인으로 포함하고 더 나아가 조종면 구동기의 변위한계도 고려하는 데, 여기서는 이 변위 한계를 고려하는 새로운 방법을 제시하고자 한다. 새로운 기법은 독립된 구동기의 주파수 응답을 고려할 때, 기존의 OLOP 방법이 제시한 바와 달리, 변화율 한계가 시작되는 주파수가 변위 한계에 의해 더 높은 주파수로 지연되지 않는다는 사실을 인지함으로써 그 동기가 되었다. 실제로 피드백 제어 환경에서는 변화율 한계의 시작점이 오히려 더 낮은 주파수에서 일어나는데, 이는 변위 한계의 영향에 의한 제어력 감소분을 극복하기위해 제어기가 더 큰 제어 입력을 발생하기 때문이다. 본 연구는 이렇게 찾아진 변화율 시작 주파수와 변위 한계를 고려하여 OLOP 해석의 최종 단계에서 Nichols Chart 위에 변화율 시작점을 찾는 새로운 방법을 제시한다. 마지막으로 기존의 방법으로는 PIO 예측에 있어서 위험한 판단에 도달할 수 있으나, 제시된 새로운 방법으로는 올바른 예측을 할 수 있음을 보여준다.

복부근력이 약한 여성과 정상 여성에게 복부 드로우-인 운동 후 엉덩관절 폄 시 허리폄근의 근수축 개시시간과 골반각도의 변화 (Changes in Onset Time of Lumbar Extensor Muscles and Pelvic Angle during Prone Hip Extension after the Abdominal Draw-in Exercise in Normal Women and Women with Weak Abdominal Muscles)

  • Purpose : The aim of this study was to investigate the effects of the abdominal draw-in (ADI) exercise on the onset times of the gluteus maximus (GM) and erector spinae (ES) as well as the pelvic anterior tilt angle during prone hip extension (PHE). Methods : A total of 24 female adults were divided into two groups: those with normal abdominal muscles (n=12) and those with weak abdominal muscles (WAM; n=12). Before the intervention, the onset times of the GM and ES along with the pelvic angle during PHE were measured. Subsequently, the participants conducted the ADI exercise for 10 minutes. After conducting the ADI exercise, the onset times and the pelvic angle were re-measured. Results : In the pre-intervention comparison between the two groups, the WAM group showed faster ES onset times and higher pelvic angle than the normal group (p<0.05). In the WAM group, the ES onset times were significantly delayed after the ADI exercise (p<0.05). In both groups, the pelvic angle was significantly decreased after the ADI exercise (p<0.05). The decrease in the pelvic angle was significantly greater in the WAM group than in the normal group (p<0.05). The GM onset time was found to be not significant in all comparisons (p>0.05). Conclusion : Therefore, it can be concluded that after performing the ADI exercise, the pelvic anterior tilt during PHE is decreased in normal women and those with WAM, especially in the WAM group, suggesting that the ADI exercise can reduce the compensatory pelvic anterior tilt more effectively by delaying the ES onset times.

  • A numerical study on oscillatory thermocapillary flow in half-zone has performed to understand the effect of axial rotation. 2d unsteady code is developed to observe the onset of oscillation. 2cs Silicone oil with Prandtl number of 26.5 is used as a working fluid. The critical temperature difference at onset of oscillation is investigated under the different aspect ratios and rotation modes. It is shown that the onset of oscillation is delayed when aspect ratio reduces and rotating speed increases. The oscillatory flow is strongly reduced under top rotation and co-rotation modes, while it is augmented under bottom rotation and counter-rotation modes. It is thought that interaction between return flow and bottom wall is important to explain the oscillatory flow.

고유수용성신경근촉진법의 스케이터와 스프린터 통합패턴이 몸통근육의 근활성도와 근수축 개시에 미치는 영향 (Effects of Skater and Sprinter Combined Pattern in PNF on Muscle Contraction Onset Time and Muscle Activation of Trunk Muscle)

  • Purpose : The purpose of this study was to investigate the effect of the sprinter and skater combined patterns on muscle contraction onset time and muscle activation of body stabilizing muscles. Method : Our study included young and healthy men in their 20s. The participants used the sprinter and skater combined patterns of the proprioceptive neuromuscular facilitation (PNF) methods to measure muscle activation and muscle contraction onset time of the trunk muscles. To measure muscle contraction onset time and muscle activation, electrodes were attached to the rectus abdominis (RA), external oblique (EO), internal oblique (IO), and erector spinalis (ES) regions. Significant differences were identified using a paired t-test and a one-way analysis of variance (ANOVA) analysis. Result : In the sprinter combined pattern, the muscle with the fastest onset time of contraction was the RES, and that with the slowest was the RRA (p<.001). In the skater combined pattern, the muscle with the fastest onset time of contraction was the LES, and that with the slowest was the LRA (p<.001). In the sprinter combined pattern, the REO and LIO presented medium muscle contraction onset times (p<.001). In the skater combined pattern, the LEO and RIO presented medium muscle contraction onset times (p<.001). Conclusions: Based on these results, these patterns could be used as exercise methods for the elderly with delayed proactive response speeds of the body stabilization methods due to imbalances in body stabilizing muscles or limbs movement.

  • Opioid-induced rigidity is a potentially life-threatening complication that can occur after treatment with large doses of opioids, but with early recognition it can be treated effectively with naloxone or with muscle relaxants. Regarding its onset time, there have been few case reports that have described delayed manifestations of opioid-induced rigidity. The mechanism of this complication is not well understood. In this report we describe a case of incidental overdose injection of sufentanil and subsequently review the confusing clinical features that require immediate diffenrentiation and the possible mechanim of this complication.

  • Blunt carotid artery injury is uncommon, yet not rare. However, it is often underdiagnosed because of inconsistent early symptoms or masking by the presence of coexisting brain and spinal injuries. The delay between the accident and the onset of cerebral ischemic symptoms is variable and has been reported to range from minutes to ten years. However, to our knowledge, there has been no report on a case presented with delayed intracerebral hemorrhage 25months after blunt carotid artery injury. We report on a case with discussion of supporting evidence and possible mechanisms.

지연성 근육통에 대한 Triamcinolone Acetonide의 이온도입 경피투과 (Iontiophoretic Transdermal Delivery of Triamcinolone Acetonide on Delayed Onset Muscle Soreness)

  • This study investigated the effects of triamcinolone acetonide by iontophoretic transdermal drug delivery on anti-inflammatory action into the human which had excentric exercise-induced delayed onset muscle soreness in the non-dominant arm. The degree of anti-inflammation was evaluated creatine posphokinase(CPK) by serum enzyme activity and subjective pain threshold by soreness muscle scale in clinical study. The results Were as follows; 1. In a subjective pain scale, all groups showed non-significant difference but, showed a tendency to decrease numerical value in human. 2. In the serum CPK level, iontophoresis group showed more significant reduction than other groups at 24, 48 and 72 hours. From the results, the iontophoresis with triamcinolone acetonide is more effective than using each groups. The continuous study is needed for many interesting issues of iontophoretic transdermal drug delivery in new future.
