안장(Sella turcica)의 형태와 지연된 치령의 연관성 평가 (Assessment of the Relationship between Sella Turcica Morphology and Delayed Dental Age)

  • 이 연구는 안장의 형태와 지연된 치령의 연관성을 알아보고자 수행되었다. 16세 미만 389명의 치령을 Demirjian 방법으로 평가하였고, 치령에서 연대 연령을 뺀 Age discrepancy (AD)를 계산하였다. 측방두부계측 방사선 사진 상 안장 형태에 따라 8가지로 분류하고 Sella turcica bridging (STB) 정도를 수치화하기 위해 Interclinoid distance를 안장의 길이로 나눈 Bridging ratio를 기준으로 4가지로 분류하였다. Kruskal Wallis 검정과 Mann Whitney 검정을 이용하여 8군과 4군에서 각각 AD의 유의성을 확인하였다. 일부 안장 형태 이상 군에서 정상보다 낮은 AD를 보였고 정상 안장 형태군과의 AD의 차이는 4 - 5개월에서 1년 3개월로 다양했다. STB 정도가 심해질수록, AD는 감소하였다. 정상 STB군과 경미한 STB군의 AD 차이는 7개월이고, 중등도 이상의 STB군에서는 1세 이상 치령이 지연되었다. 성별에 따른 안장 형태와 STB 정도는 유의한 차이가 없었다. 이 연구를 통해 일부 안장 형태 이상과 STB가 지연된 치령과 연관성이 있음을 확인하였다. 안장의 형태는 지연된 치령을 예측하는 보조적 수단으로 사용될 수 있다.

  • Osseointegration involves anchoring dental implants to stable bone rather than to soft-tissue. Clinical osseointegration is currently defined as the process whereby alloplastic material is asymptomatically and rigidly fixed and maintained in bone during functional loading. Full osseointegration is necessary for the success of long-term dental implants. Recent developments in computer assisted measurement of bone formation have improved maxillofacial examination and osseointegration. Computer assisted examination has also proved effective in dental implantology. This investigation was aimed to determine osseointegration in immediate and delayed implantation in the dog mandible using dental computed tomography (CT) and bone scintigraphy. Five adult (mean age of 2 years) mongrel dogs with a mean weight of 9.1 kg were used in this investigation. Titanium alloy implant systems with 4 mm in diameter and 10 mm in length were chosen for insertion. The second and third left mandibular premolars in each dog were extracted for the delayed implant insertion. Twelve weeks later, the second and third right mandibular premolars were extracted for the immediate implant insertion. Before the delayed and immediate implantation procedures and 0, 4, 8, and 12 weeks after the insertions, dental CT and scintigraphy were conducted. The CT and scintigraphic images indicate that reconstruction of bone surrounding of the immediate implant can be as successful as reconstruction of bone surrounding of the delayed implant.

Frequency of Delayed Diagnosis of Oral Squamous Cell Carcinoma in Pakistan

  • Objectives: To determine the frequency of delayed diagnosis of oral squamous cell carcinoma in our setup; highlighting factors responsible for any delay and their possible relevance to demographic and diagnostic features. Methods: This cross sectional study of six months duration was conducted in the Oral and Maxillofacial Surgery Department of the Armed Forces Institute of Dentistry, Rawalpindi, Pakistan. A total of 246 patients, both male and female, having a biopsy proven definitive diagnosis of OSCC were included using a consecutive sampling technique. Delay in diagnosis was assessed from the stated period of time from when the patient first noticed symptoms of disease until a definitive diagnosis was made. We concluded delayed diagnosis if this was more than 40 days. Results: The ages of patients ranged from 27 to 60 years with a mean of $46.7{\pm}10.2$ years and a marked male predominance (3.7:1). Delayed diagnosis was observed in 91.5% of cases. However, statistically no significant differences were found with age, gender, marital, education status, household income and time of biopsy. Conclusion: Our primary finding of delayed diagnosis with no prior contact with any health care professional clearly reflects a need of taking urgent measures to avoid serious impacts on morbidity and mortality associated with OSCC.

악안면 형태에 대한 상악 제1대구치 발육장애의 영향 (Effect of Developmental Disorders of Maxillary First Molars on Orofacial Morphology)

  • 이 연구의 목적은 상악 제1대구치의 결손 또는 발육 지연을 보이는 증례에서 나타나는 악안면 형태를 평가하는 것이다. 2006년부터 2017년 8월까지 부산대학교치과병원 소아치과에 내원한 어린이 2983명의 교정 진단자료를 후향적으로 평가하여 상악 제1대구치가 편측 또는 양측으로 결손되거나 발육지연을 보이는 34명을 선정하였다. Demirjian 법을 응용하여 상하악 치령을 추정하고 역령과 비교하였다. 측방두부규격방사선사진 계측을 통해 골격 및 치아치조적 부조화를 반영하는 지표들을 평가하였고, 전치부 교합관계를 확인하였다. 상악 치령은 역령과 비교할 때 유의하게 낮았다. 골격성 개방교합 경향 및 상악 열성장형 골격성 III급 부정교합을 나타내는 지표가 통계적으로 유의하였다. 전치부 피개는 반대교합과 절단교합이 주로 나타났지만, 보상에 의해 정상 피개를 보이는 증례도 있었다. 상악 제1대구치의 선천결손 및 발육지연은 상악골의 성장에 영향을 줄 가능성이 있다고 판단되었다. 따라서 제1대구치의 발육장애가 확인될 경우 사춘기 동안의 악안면 성장을 주의 깊게 관찰해야 하며 적절한 시기에 교정치료를 고려하는 것이 필요하다고 사료되었다.

성인의 구강의료접근성의 관련요인 - 구강질환의 의사진단 및 처치율을 중심으로- (Factors associated with access to dental care among Korean adults)

  • Objectives: The aim of this analysis was to investigate factors associated with dental visits in terms of getting diagnosis and receiving care for diagnosed dental diseases among Korean adults. Methods: Data used in this analysis were from the 2005 Korean National Health and Nutrition Examination Survey which involved a representative sample of 25,487 adults nineteen years of age and older who resided in Korea. This analysis used data of adults who had completed health interview survey (n = 25,215). Uni- and bi-variate analysis, Chi-square test, and logistic regression analyses. were conducted using SAS. Results: The percentages of people diagnosed (treated) dental canes, periodontal disease. and temporomandibular joint disorder were 70.4(77.1), 16.1 (55.7), and 0.6(37.4), respectively. The reception of treatment for diagnosed dental caries and periodontal disease was significantly associated with older age, high monthly household income, and high education attainment. Logistic regression model indicated that age, monthly household income, education attainment, type of health insurance, and chewing problem were significantly associated with getting diagnosis and treatments of dental diseases. Most frequently answered reason for non - or delayed treatments of diagnosed dental diseases during the last one year was cost of treatments. Conclusions: The findings indicated that socio-economic-status still significantly affected reception of needed dental treatments among Korean adults despite the national health insurance system. Oral health policy and programs should be augmented to provide further support to adults of low socio-economic-status who are more prone to dental diseases yet lack resources for needed dental treatments.

  • Eruption of the teeth can be disturbed by crowding, ectopic eruption path, retention for pathologic condition of follicle and periodontal ligament, local disturbances in the innervation, and delayed eruption for overlying obstacles in the eruption path. Eruption disturbance of permanent posterior teeth is taken with diverse therapeutic approaches according to the patient age, cooperation of patient, tooth position and maturity, degree of impaction, clinical features, and repercussion on the neighboring teeth. However, delayed treatment usually results in less favorable outcomes. Therefore, In order to prevent this situation, periodically radiographic examinations during the early mixed dentition period and early diagnosis of eruption disturbances of permanent posterior teeth are recommended.

Management of Ameloblastic Fibro-odontoma Associated with Impacted Tooth in the Posterior Mandible: Case Reports

  • Ameloblastic fibro-odontoma (AFO) is a rare odontogenic tumor, which occurs in young children before the age of 20 years. Radiologically, it is a well-defined radiolucent lesion containing radiopaque masses. Histopathologically, AFO is composed of odontogenic epithelium in a primitive-appearing connective tissue and hard tissue consisting of enamel and dentin. It is commonly found in the mandibular posterior region. AFO may be asymptomatic and is often associated with delayed tooth eruption. As it shows similar characteristics clinically and radiologically to odontoma, differential diagnosis through histopathological examination is important. Treatment of AFO is conservative enucleation, and teeth enclosed or associated with the lesion may require extraction. In this report, 2 young patients who visited our clinic with a chief complaint of delayed eruption were diagnosed as AFO with radiological and histopathological examination. After the surgery, the healing status was found to be favorable, and no evidence of recurrence was observed.

Cystic lesion between a deciduous tooth and the succeeding permanent tooth: a retrospective analysis of 87 cases

  • Objectives: The purpose of this study is to investigate the characteristics of dentigerous and radicular cysts that occur between deciduous and succeeding permanent teeth and to propose considerations for differential diagnosis of cysts at the treatment planning stage in the outpatient clinic. Materials and Methods: A total of 87 patients with a cystic lesion located between a deciduous tooth and the succeeding permanent tooth participated in the study. Twelve variables were analyzed to diagnose such a cyst. For data analysis, Fisher's exact test was used to determine the statistical significance of the variables. Results: Of the total 87 patients who participated in this study, 69 were diagnosed with dentigerous cysts and 18 were diagnosed with radicular cysts. Seven of the 12 differential factors analyzed in this study were statistically significant: age, location, symptoms, dental caries, endodontic treatment, delayed eruption, and size. Conclusion: Several criteria can be considered for diagnosis of dentigerous cysts or radicular cysts. Age, location, presence of symptoms and dental caries, previous endodontic treatment, cystic size, and delayed eruption of impacted permanent teeth are reliable factors that should be considered when diagnosing dentigerous and radicular cysts.

Clinical and Radiological Characteristics in Patients with Postoperative Maxillary Cyst: A Retrospective Study

  • Purpose: To evaluate the clinical and radiologic findings of the postoperative maxillary cysts (POMCs) and investigate the relationship between lesion size and clinical symptoms depending on the time elapsed after radical maxillary sinus surgery. Methods: A total of 29 patients who were diagnosed with POMCs at Chonnam National University Dental Hospital were selected. Clinical and radiologic findings were investigated. POMC cases were divided into two groups: those with <24 years between maxillary sinus surgery and POMC diagnosis and those with >24 years. The chi-square test was used to compare the differences between the two groups. Results: The average period from surgery to POMC detection was 24.32 years; however, the period could not be confirmed in four patients. The average patient age was 52.75 years, and 12 (41.3%) patients were in their 50s. POMC-related clinical symptoms were as follows: buccal pain and swelling, dull pain, toothache, abscess, sensory abnormality, and asymptomatic. Twenty (69.0%) cases showed unilocular radiolucency and 9 (31.0%) revealed multilocular radiolucency. Seven cases (35.0%) were misdiagnosed as odontogenic lesions, resulting in the delayed treatment of POMCs. No statistical significance was found between the two groups with respect to symptoms, expansion to the surrounding area, presence of secondary cysts, and mesiodistal length of cyst on cone-beam computed tomography (CBCT) images. However, the buccopalatal length of the cyst on CBCT images was significantly different between the two groups. Conclusions: The buccopalatal length of POMCs observed on CBCT images was related to the time elapsed since surgery. The lack of awareness of POMCs may lead to misdiagnosis as an odontogenic infection and delayed treatment. Therefore, dentists must recognize the clinical and radiologic features of POMCs to differentiate it from dental infections.

제1대구치의 맹출지연에 관한 임상적 고찰 (A CLINICAL REVIEW ON THE DELAYED ERUPTION OF 1ST MOLARS)

  • Among the permanent teeth. the first permanent molars play the greatest role in occlusion and function. So, the congenital missing, abnormal reuption or abnormal formation of the first permanent molars in the course of arch development would inflict normal development of dental arches. Therefore, early detection of abnormal cases related to first permanent molars and understanding of current and predictable clinical problems are essential for proper occlusal guidance in children. With the aim of investigating the clinical patterns of delayed eruption of first permanent molars in children, panoramic tomograms of the childern in mixed and early permanent dentition were observed and analyzed. The results were as follows: 1. Among the delayed eruption of first permanent molars, on tooth or bilateral teeths were affected most frequently. Delayed eruption was more prevalent in maxilla than in mandible. 2. The formation of tardily erupted teeth were also delayed. 3. Delayed eruption was generally limited in first molars or molar segments. 4. Delayed eruption of first permanent molars is accompanied by abnormal position of tooth germs, for example, ectopic eruption, delayed dental age, delayed localized tooth formation and generalized congenital missing. 5. There was a tendency of delayed formation or congenital missing of second molars distal to tardily erupted 1st molars. And that was more marked in maxilla than in mandible. 6. There was reported that affected 1st molars show various size and shapes. Maxillary 1st molars showing delayed eruption showed a tendency of having 3 cusps. But, tardily erupted mandibular 1st molars showed no significant reduction in mesiodistal dimension, as reported. 7. In some cases, the delayed eruption of 1st permanent molars was associated with ectopic eruption, but their formation was not usually retarded. 8. In skeletal class III cases, there showed a tendency of mandibular 1st molars to erupt earlier than maxillary 1st molars with greater interval than in normal occlusion.
