Ectopic eruption is defined as the eruption of the tooth in an abnormal position or orientation. In the molar region, ectopic eruption may cause distal root resorption and premature exfoliation of the neighbor teeth and uncontrolled space loss is followed. Prolonged partial impaction of the permanent molar may also cause undetected caries or abscess formation of the neighbor teeth. While 66% of ectopically erupting permanent molars are corrected spontaneously without treatment, the treatment is necessary for the irreversible ectopic eruption cases. The optimal treatment approach depends on a number of factors including the clinical eruption status of the molar, amount of enamel ledge and the mobility of the neighbor tooth, and the presence of pain or infection. This case report presents the results of treatment of the ectopically erupting maxillary first permanent molars and mandibular second molars using elastic separators or modified Halterman appliance with or without surgical approach.

Background: Fiberoptic nasotracheal intubation (FNI) is performed if it is difficult to open the mouth or if intubation using laryngoscope is expected to be difficult. However, training is necessary because intubation performed by inexperienced operators leads to complications. Methods: Every resident performed intubation in 40 patients. Success of FNI was evaluated as the time of FNI. First intubation time was restricted to 2 min 30 s. If the second attempt was unsuccessful, it was considered a failed case, and a specialist performed nasotracheal intubation. If the general method of intubation was expected to be difficult, awake intubation was performed. The degree of nasal bleeding during intubation was also evaluated. Results: The mean age of the operators (11 men, 7 women) was 27.8 years. FNI was performed in a total of 716 patients. The success rate was 88.3% for the first attempt and 94.6% for the second attempt. The failure rate of intubation in anesthetized patients was 4.9%, and 13.6% in awake patients. When intubation was performed in anesthetized patients, the failure rate from the first to fifth trial was 9.6%, which decreased to 0.7% when the number of trials increased to > 30 times. In terms of awake intubation, there was no failed attempt when the resident had performed the FNI > 30 times. The number of FNIs performed and nasal bleeding were important factors influencing the failure rate. Conclusion: The success rate of FNI increased as the number of FNI performed by residents increased despite the nasal bleeding.
Objectives: For the deviation phenomenon occurring during the treatment process in temporo-mandibular balancing medicine (TBM), hypotheses were established regarding the cause and mechanism of formation from the perspective of neuro-science and material mechanics, and a verification method was proposed. Methods: The deviation phenomenon was theoretically analyzed based on the structure theories of material mechanics of the joint and the neurological pain mechanism. Results: Deviation occurs due to temporary yield by the accumulation of heterogeneous stress in the temporo-mandibular joint and the affected joint. Because the joint structures are corresponding with material mechanics showing compressive and tensile properties. The size of the deviation is expressed in terms of strain. The occlusal surface of the teeth is level with the axial joint. Since the magnitude of the deviation has a proportional relationship with the degree of abnormality of the temporo-mandibular joint, the magnitude of the deviation calculated by the balance measurement can be replaced by the strain. The major variables involved in the occurrence of deviations are the strength of joint structures and neurological conditions. Therefore plastic deformation and adaptation occur as a long-term depression of neural circuits is strengthened in different ways at different locations each time in various clinical situations. This is the reason why the sequence of the restoration process while correcting deviations is following reverse order of the accumulation in many layers in the muscular nervous system. Conclusions: From the above results, it can be inferred that the occurrence and correction of the deviations are corresponding with the plastic deformation and neuro-plasticity.

BACKGROUND: Dobutamine and adenosine stress cardiac magnetic resonance (CMR) imaging is relatively contraindicated in patients with moderate to severe aortic valve stenosis (AS). We aimed to determine the safety of dobutamine and adenosine stress CMR in patients with moderate to severe AS. METHODS: In this retrospective study patients with AS who underwent either dobutamine or adenosine stress CMR for exclusion of obstructive coronary artery disease were enrolled. We recorded clinical data, CMR and echocardiography findings, and complications as well as minor symptoms. Patients with AS were compared to matched individuals without AS. RESULTS: A total of 187 patients with AS were identified and compared to age-, gender- and body mass index-matched 187 patients without AS. No severe complications were reported in the study nor the control group. The reported frequency of non-severe complications and minor symptoms were similar between the study and the control groups. Nineteen patients with AS experienced non-severe complications or minor symptoms during dobutamine stress CMR compared to eighteen patients without AS (p = 0.855). One patient with AS and two patients without AS undergoing adenosine stress CMR experienced minor symptoms (p = 0.562). Four examinations were aborted because of chest pain, paroxysmal atrial fibrillation and third-degree atrioventricular block. Inducible ischaemia, prior coronary artery bypass grafting, prior stroke and age were associated with a higher incidence of complications and minor symptoms. CONCLUSIONS: Moderate to severe AS was not associated with complications during CMR stress test. The incidence of non-severe complications and minor symptoms was greater with dobutamine.
This study is an attempt to analyze the physical load by the type of the nursing tasks at the neurosurgical ICU nurses through the quantitative analysis of the working postures by the type of the nursing tasks with the OWAS(Ovako Working Postures Analysis System). Data collection was conducted through the video recording of the 13 nurses working at the neurosurgical ICU. For the analysis of the work postures by the type of the nursing tasks, and were interviewed regarding the subjective degree of the difficulties with the work postures related to the tasks. Collected data was analyzed through the WinOWAS program. The results were as follows : AC3 or AC4 tasks among the 18 nursing tasks types are "occupied bed making and change of the patient gown", "back massage", "suction", "elimination management", "change of position", "adjustment of bed", "helping the patient to move","measurement of CVP"and "measurement of urine volume". It appears that these tasks are harmful to the musculoskeletal system and the improvement or change of the work is required. The results stated above indicate that improper working postures during the nursing tasks influence the musculoskeletal system. Therefore, making use of assistant devices for the improvement of the working environment at the nursing tasks, based on human technological diagnoses, is required regarding the duty types with massive work pressure known to be harmful to the musculoskeletal system among those performed by the nurses. And there is a need of the education about the employment and maintenance of the vocational back pain prevention.
The arthritis patients suffer from psychological, social and spiritual problems as well as physical problems because the arthritis is not curable and has chronic pain, joint deformity, limitation of activity and physical dysfunction for all of his life. Especially if they do not find the meaning in their lives, they will experience spiritual distress seriously. Therefore, it is important that nurses help the patients to find the meaning in their lives and to reduce spiritual distress. The purpose of this study is to provide a basis for nursing intervention strategies to minimize the arthritis patients' spiritual distress and understand the relationship between the meaning of life and the spiritual distress in arthritis patients. The samples were composed of 157 arthritis patients. Data collection was carried out from October 1, 1998 to February 28, 1999. Data were analyzed using a SAS program for descriptive statistic, Pearson correlation, t-test, ANOVA, linear regression. The results were as follow; 1. The scores on the meaning of life scale ranged from 51 to 130 with a mean of 93. 2. The scores on the spiritual distress scale ranged from 26 to 91 with a mean of 60. 3. There were significant correlations between the meaning of life and the spiritual distress(r=.53, p=.00). 4. The linear regression analysis showed that the meaning of life explained 13% of the spiritual distress. 5. In the degree of the meaning of life and the spiritual distress according to the general characteristics, the level of the meaning of life in arthritis patients was different by the duration of incidence(F=2.71, p=.03). In conclusion, the nursing intervention strategies to reduce the spiritual distress in arthritis patients must take into account the meaning of life.
Angelica polymorpha Maxim root extract (APRE) is a popular herbal medicine used for treating stomachache, abdominal pain, stomach ulcers, and rheumatism; however the effect of APRE on cancer cells has not yet been explored. Here, we examined APRE cytotoxicity seen on target neuroblastoma cells (NB) using cell viability assays, DAPI visualization of fragmented DNA, and Western blotting analysis of candidate signaling pathways involved in proliferation and apoptosis. We demonstrated that APRE reduced cell viability in NB to a greater extent than in fibroblast cells. In addition, we found that APRE could inhibit the three classes of MAPK proteins and could also down-regulate the PI3K/AKT/GSK-$3{\beta}$ activity all being relevant for proliferation and survival. APRE could also up-regulate Bax expression and down-regulate Bcl-2 and Mcl-1. With APRE treatment, depolarization of mitochondria membrane potential and activation of caspase-3 was demonstrated in the SH-SY5Y cells. We could not found increased activity of death receptor and caspase-8 as markers of the extrinsic apoptosis pathway for the APRE treated cells. In presence of a caspase-3 siRNA and a pan-caspase inhibitor, APRE could not reduce the viability of NB cells to a significant degree. So we predicted that with APRE, the intrinsic pathway was solely responsible for inducing apoptosis as we also showed that the non-caspase autophagy pathway or ER stress-ROS mediated pathways were not involved. These findings demonstrate that an intrinsic mitochondria-mediated apoptosis pathway mediates the apoptotic effects of APRE on SH-SY5Y cells, and that APRE shows promise as a novel agent for neuroblastoma therapy.

This retrospective descriptive study was conducted to survey the use of alternative therapy by chronicly ill patients. The subject of this study were 205 chronicly ill patients at Pusan National University Hospital. The data for this study were patient record. The data were gathered from August, 1 to September, 30 1998 and analyzed through SPSSWIN program for frequency, percentile and $x^2-test$. The important results of this study are as followings. Of population-sociological characteristics on studied objects, sexual distribution showed a similar percentage male 47.8% and the female 52.2%. Age distribution showed that the 60-69 year old group made up 31.5%, and was the highest. Of religion Buddism was the most frequest. The diagnosis distribution showed cancer at 28.8% and coronary artery disease at 16.1%. According to the results of the study, 51.2% of subjects had used alternative therapy. The most common type of alternative therapy was dietary and nutritional therapy, 40.2%, the place of use, home, 44.0%, duration, less than 3 months, 51.5%. The most common motive was a recommendation by friends or family. The degree of satisfaction after the use of alternative therapy was high for 21.0%, and slight for 53.3%. The reaction after the use of alternative therapy was increasing power for patients with coronary artery disease and cancer, sugar control for those with DM, and pain control for those with chronic arthrits. The study showed that for chronicly ill patients, age, religion, diagnosis name have an important effect on the use of alternative therapy. In conclusion, chronicly ill patients showed high-use of alternative therapy but they did not used alternative therapy as a healing method. Therefore we must provide an education program nurses and physicians. And we must provide more information or healing method's and support chronicly ill patients.
Objectives This study was conducted in order to analyze the distribution of Sasang constitution, herbmed, exterior-interior disease and concomitant symptoms of 36 facial nerve paralysis patients. Methods A clinical study was done on 36 patients, treated as facial nerve paralysis. Sasang constitution specialist diagnosed their constitution and gave prescription. Degree of improvement was evaluated by assessment scale or change of patient's complaint. Results 1) Distribution of sasang constituion : Taeeumin 21 patients, Soyangin 8 patients, Soeumin 7 patients. 2) Distribution of exterior-interior disease : Taeeumin and Soeumin were diagnosed as interior disease more than exterior disease, Soyangin was reverse. 3) Hyeongbangdojeok-san and Hyeongbangjihwang-tang (Soyangin), Galgeunhaegi-tang(Taeeumin) and Gwakhyangjeonggi-san(Soeumin) were used the most in each sasang constitution. 4) Seventeen patients had concomitant symptoms : digestive disorder, sleep disorder, tinnitus, hypertension, diplopia, itching, gout, neck-shoulder pain, constipation and dizziness. Conclusions Patients had different Sasang constitution and in the same Sasang constitution, they had different symptomatology.
Background : The study aimed at examining the awareness and satisfaction level of Korean medicine treatment of musculoskeletal patients and non-musculoskeletal patients. Method : The frequency and percentage were calculated to identify the overall characteristics, and to identify the characteristics of the respondents who visited the hospital to treat musculoskeletal diseases and those who visited for the treatment of non-musculoskeletal diseases, the correlation between the variables was analyzed using the chi-square analysis (χ2-test). Furthermore, analysis items were compared depending on detailed diseases within the musculoskeletal disorder (lumbar pain, sprains, arthritis, frozen shoulders, spondylitis, disc) Result : Respondents who used Korean medicine for the purpose of treating musculoskeletal diseases had answered that costs involved in Korean medicine was expensive, and answered that herbal decoction was the preferred Korean medicine treating method for expanding health insurance benefits. Regarding the safety awareness of Korean medicinal herbs, responses that said it was safe was high, and their willingness to use and recommend Korean medicine in future was also high. Respondents who used Korean medicine for the purpose of treating musculoskeletal diseases said they were overall satisfied along with the attitude of Korean medical doctors, treatment results, and costs of treatments. Conclusion : The study was aimed at securing basic data to indirectly identify the national demand for Korean medicine, through investigating the level and degree of differences that exist in the perception and satisfication level and further find a point where policy intervention is possible in future.
