전자궁적출술후 시행한 정맥내 통증자가조절법의 임상 경험 (Clinical Experience on Intravenous Patient Controlled Analgesia after Total Abdominal Hysterectomy)

  • Background: We studied 150 patients who received intravenous patient controlled analgesia (PCA) after total abdominal hysterectomy to evaluate pain relief, analgesic consumption, patient's satisfaction and side effects. Methods: We made total 40 ml of analgesic mixture with morphine 40 mg, ketorolac 120 mg, droperidol 3 mg and normal saline. Loading/bolus/basal infusion dose and lockout interval was 2 ml, 1.5 ml, 0.5 ml/hr and 10 min, respectively. Numerical rating scale (NRS) pain score, cumulative analgesic consumption, degree of satisfaction, and incidence of side effects were evaluated. Also, correlation of age and edu ion with analgesic consumption were evaluated. Results: The average pain scores using NRS were $3.1{\pm}1.7$ (6 h), $2.1{\pm}1.5$ (24 h), $1.7{\pm}1.5$ (48 h). The average cumulative analgesic consumption were $11.7{\pm}5.0$ ml (6 h), $23.0{\pm}6.7$ ml (24 h), $32.1{\pm}3.7$ ml (48 h). The degree of satisfaction in postoperative pain control was good in 94% of patients. There was no correlation between degree of satisfaction and analgesic consumption. Also age and level of edu ion did not correlated with analgesic consumption. Conclusions: Intravenous PCA with morphine, ketorolac, and droperidol is an effective method of postoperative pain control because it provides adequate pain relief and a few side effects with high patient's satisfaction. However, age and level of education did not correlated with analgesic consumption.

밸런스테이핑이 퇴행성관절염 환자의 다리통증과 일상 활동장애에 미치는 효과 (A Study on Effects of Balance Taping Therapy applied on the patients with Arthritis caused upon the Pain in Legs and Obstacle in Daily Activity)

  • Purpose: This study is to examine the effect of balance taping therapy applied on the old people who suffer from arthritis upon the pain in legs and obstacle in daily activity. Methods: The research period was from Dec. 2003 to Feb. 2004, and among the old female people who visited one of the welfare halls in Seoul downtown and 4 assembly hall for old people who aged over 60 years. 30 of the experimental group and 33 of control group were optionally collected. Results: The pain degree in legs and the uneasiness degree of the obstacle in the daily life of the experimental group who were treated by the balance taping was decreased than those of the control group(p=0.015, p=0.000). Conclusions: Based upon the above result, it was confirmed that the balance taping therapy has influence in decreasing the degree of the uneasiness in the leg pain and daily activity obstacle of the old people who suffer from the arthritis, and therefore it is thought that this therapy can be usefully applied for one of the nursing methods for the old patients, and besides the repeated future researches are needed.

동통 평가도구 개발을 위한 연구 -한국 통증 어휘별 강도 순위의 유의도 및 신뢰도 검사- (A Study for Development of a Korean Pain Measurement Tool(II). A Study for Testing Ranks of Words in each Subclass of a Korean Pain Measurement Tool)

  • The main purpose of this study is to systematically classify words indicating pain in terms of their ranks in each subclass. This study is a part of developing a Korean Pain Measurement Tool. This study didnot include exploration of each word's dimension such as sensory or affective. Eighty three Korean words tentatively classified in 19 subclasses in previous study were used for this study. At least three to six words were included in each subclass and the words were randomly placed in which each subject indicates their rank of pain degree. One hundred and fifty nursing students and one hundred clinical nurses were requested to indicate the rank of each word. One hundred and sixteen students and eighty three nurses completed the ratings for analysis. The data were collected from June 1983 to July 1983. The data using ordinal scale were analyzed by Friedman ANOVA to test significant difference between rank means. All of pain words indicated significant rank mean difference in all of 19 subclasses. Some of the words were either cancelled or replaced by other words, or rearranged for their ranks. Subclasses of which words were cancelled were 1) Simple stimulating pain, 2) Punctuate pressure, 3) peripheral nerve pain, 4) radiation pain, 5) punishment-related pain, and 6) suffering-related pain. Subclasses of which words were replaced or rearranged were 1) incisive pressure, 2) constrictive pressure, 3) dull pain, 4) tract pain, 5) digestion-related pain and 6) fear-related pain. Four subclasses such as traction pressure, thermal, cavity pressure, and fatigue- elated pain indicated significant differences among rank means in each subclasses and showed no visible overlaps of the ranks among means. Further research is needed using high level measurement of pain degree of each word and more sophisticated analysis of the pain degrees. Three pain words which would be related to chemical stimulation were newly explored and included as a new subclass. Through this study, the total number of subclasses increases from 19 to 20 and the total number of Korean words in the scale decreases from 83 to 80.

요가운동이 만성요통 완화에 미치는 영향 (The Effect of Yoga Exercise on the Relieve of Chronic Low Back Pain)

  • The purpose of this study was to test whether Yoga exercise, one of muscle relaxation, helps to relieve chronic low back paln. Sample were selected from nurses who were worked at K medical center in the period from March 18 to April 23, 1996. The sample size was fifteen. The research design was one-group pretest-posttest design. To test the major research question, this study had the following procedure. The pretest included measuring individuals' degree of pain and discomfort depending on the scope and types of an range of motion, and their pain by their activity of daily living(ADL). The treatment was conducted twice every other week. The reason why this study had twice observations was to control history effect and maturation which treat internal validity in the research design of this study. In this research design, a treatment was to expose Yoga exercise to samples. The exercise was taken in 30 minutes per day for four days in a week (Two consecutive days and twice every other day). The posttest included re-measuring the individuals' the degree of pain and discomfort, and their pain by their ADL. Several hypotheses concerning effect of Yoga exercise was analyzed by the paired t-test, comparing the difference scores between pre and post tests. The results of this study was as follows. The first hypothesis that the post-treatment group taking the Yoga exercise had the pain score lower than the pre-treatment group was supported(t=3.31, p=.005). The second hypothesis that the discomfort score of the post-treatment group had lower than does that of the pre-treatment group was supported(t=2.75, p=.016). The third hypothesis that the post-treatment group had the pain score by ADL lower than does the pre-treatment group was supported(t=5.52, p=.000). In summary, this study examined the effect of a yoga exercise, one of muscle relaxation, on those who were suffered from chronic low back pain. The effect measured by the degree of pain and discomfort with a visual analog scale was statistically significant. The degree of pain according to postures in ADL also showed statistical significance. These findings showed that a yoga exercise was effective to alleviation of chronic low back pain. A pretest-posttest control group design, however, needs to get more accurate results since the design satisfies Internal validity. Consequently, yoga as an exercise cure improves an interaction between muscles and articulations, and performance of ADL for those suffered from chronic low back pain. Furthermore, this positive impact may be an effective method as nursing intervention for their physical, mental, emotional, social, and psychological recovery.

화상치료에 있어서 동종유래표피세포의 유용성 (The Usefulnesssof Cultured Allogenic Keratinocyte for Burn Treatment)

  • Purpose: When choosing dressing method to treat skin defect by second degree or higher burn, we have to consider method of rapid epithelization and minimization of pain during the treatment. In this study, we used biologic dressing with cultured allogenic keratinocytes for skin defect due to burn. We followed up the degree of epithelization, the degree of pain, and patient satisfaction. Methods: From June 2003 to June 2006, among the patients with skin defect due to burn, 31 cases with second degree burn(moderate to severe) were selected and biological dressing with cultured allogenic keratinocytes were done. 21 cases did not use cultured allogenic keratinocytes. Most of the patients had second degree burn. We applied cultured allogenic keratinocyte by Kaloderm. For wounds that were not deep enough to effect the dermis, escharectomy was done before applying Kaloderm. After the operation, moist wound site was maintained by dressing with saline gauze for 5 - 7 days. We compared the condition of the wound site before and after applying Keloderm by grading epithelization by standardized percentage scoring scale(1 - 5), and degree of pain and patient satisfaction by visual analogue scale(0 - 10). Results: When cultured allogenic keratinocytes were applied for the same period of time, the mean score of epithelization were $3.29{\pm}0.529$(mean ${\pm}$ S.D.). Without the application, the mean score of epithelization were $2.86{\pm}0.655$(mean ${\pm}$ S.D.). The degree of pain was $7.71{\pm}1.419$(mean ${\pm}$ S.D.) and $2.35{\pm}0.950$(mean ${\pm}$ S.D.) before and after the application, respectively. The patients' satisfaction score was $6.45{\pm}0.850$(mean ${\pm}$ S.D.) and $8.45{\pm}0.961$(mean ${\pm}$ S.D.) before and after the application, respectively. Conclusion: Applying biological dressing with cultured allogenic keratinocyte to skin defect due to second degree burn showed satisfactory results in the degree of the epithelization, degree of pain and patients' satisfaction.

  • Prognosis in oriental medicine gathers information by four examination methods. It provides important information to understand the degree of deficiency - excessiveness of a patient to treat properly. To generalize the degree of deficiency - excessiveness can be found by seeing the patient's muscle response and pain perception to the palpitations.The theoretical basis to generalize deficiency - excessiveness, oriental and western medical understanding of pain perception and the elasticity of muscle were discussed.The usual symptoms for the excessiveness could include Pain (dislikeness to the palpitation), Stiffness of nape and limbs, Contracture of the limbs, Clonic convulsion and Fast pain. The symptoms for the deficiency could include Pain (likeness to the palpitation, Gastrocnemius muscle spasm, Flaccid paralysis of limbs and Slow pain. More theoretical bases for generalization of deficiency - excessiveness are needed along with the simplifying the complex clinical symptoms. In this way, we can discuss about deficiency - excessiveness with the regard to western medicine to help its generalization.

전기치료와 키네시오 테이핑 요법이 테니스 엘보 환자의 통증과 악력에 미치는 영향 (The Effects of Electrical Therapy and Kinesio Taping on the Grip Strength and Pain in Tennis Elbow Patients)

  • Background : The purpose of this study is to identify the level of positive influence on the pain and the gripping power in patients with tennis elbow by comparing the TENS treatment method and the kinesio taping therapy with good accessibility among the kinesio taping therapy and electrical therapy. Methods : Targeting the patients diagnosed with tennis elbow, the subjects were categorized in groups of four, each of electrical therapy group and kinesio tape therapy group where the degree of pain reduction through VAS before and after the treatment of each group and the measurement of the degree of gripping power increase using the gripping power measurement tool as well as the pain reduction changes and gripping power increase before and after the treatment between the two groups were compared. Result : As a result, the electrical therapy group and the kinesio tape group were able to obtain the results of pain reduction and gripping power increase through VAS before and after the treatment, but for all pain reduction and gripping power increase, it had no statistically significant changes(p>.05). In addition, in the changes of pain reduction and changes of gripping power increase before and after the treatment, there were no statistically significant differences between the electrical therapy group and the kinesio tape group(p>.05). Conclusion : Based on the results so far, it is thought that the kinesio taping therapy can positively influence the pain reduction of patients with tennis elbow and increase of gripping power.

종합병원에 근무하는 요통 간호사의 간호업무활동, 신체역학원리 및 직무 스트레스와의 관련성 (Relationships among Nursing Activities, the Use of Body Mechanics, and Job Stress in Nurses with Low Back Pain)

  • Purpose: This study was conducted to identify relationships among nursing activities, the use of body mechanics, and job stress in nurses with low back pain. Methods: Participants were 225 nurses with low back pain working at an university hospital. The data were collected with self-reported questionnaires at March, 2012 and analyzed by descriptive statistics, t test, ANOVA, Scheff$\acute{e}$e test and multiple regression analyses. Results: There was significantly a higher degree of pain in subjects who working at special units including intensive care units and emergency room than at general wards. Low back pain was negatively correlated with the use of body mechanical principles while it was positively associated with the degree of job stress. According to results of multiple regression analyses, low back pain was significantly associated with the use of body mechanics and job stress in nurses working at general ward and special units including intensive care units and emergency room. Conclusion: The use of the principles of body mechanics and reducing job stress are important to prevent low back pain in nurses. It is necessary to develop and apply stress management and education program about the use of the principles of body mechanics.

Effects of Music Therapy on Pain, Discomfort, and Depression for Patients with Leg Fractures

  • Purpose. To determine the effects of music therapy on pain, discomfort, and depression for patients with leg fractures. Methods. Data were collected from 40 patients admitted in an orthopedic surgery care unit. The subjects included 20 intervention group members and 20 control group members. Music therapy was offered to intervention group members once a day for 3 days for 30-60 minutes per day. Pain was measured with a numeric rating scale and by measuring vital signs. Discomfort and depression were measured with self-administered questionnaires. Results. Patients who received music therapy had a lower degree of pain than patients who did not receive music therapy as measured by the numeric pain score (p < 0.001), systolic blood pressure (p < 0.01), diastolic blood pressure (p < 0.001), pulse rate (p < 0.001) and respiration (p < 0.001). Patients who were provided with music therapy also had a lower degree of discomfort than patients who were not provided with this therapy (p < 0.01). Effects of Music Therapy on Pain, Discomfort, and Depression for Patients with Leg Fractures Conclusions. These results demonstrate that music therapy is an effective method for decreasing pain and discomfort for patients with leg fractures.

자궁적출술을 경험한 여성의 통증과 냉증완화를 위한 쑥뜸요법 효과의 예비연구 (The Effects of Hand Moxibustion Therapy on Decreasing Pain and Relieving Coldness of Women with a Hysterectomy)

  • The purpose of this pilot study was to identify the effects of hand moxibustion therapy to decrease pain and relieve coldness of the body in women who had a hysterectomy. The conceptual framework of this study was derived from Ying-Yang, Khi and other corresponding theories. The data were collected from February to May 1997. A Graphic Rating scale was used to measure the degree of pain and Digital Infrared Thermographic Imaging(D.I.T.I) was used to examine the degree of coldness. The subjects were women who resided in Seoul and had a hysterectomy within five years. An experimental group was composed of five women who received moxibustion and a control group was composed of five who did not. The research procedure began with having both the experimental group and control group describe their general characteristics and the degree of pain they perceived. Then, Digital Infrared Thermographic Imaging(D.I.T.I) was conducted. Moxibustion was only given to the experimental group on both hands twice every day for a total of five weeks. Their perception of pain and D.I.T.I were examined weekly. The control group received no therapy. After finishing therapy, the perception of pain and D.I.T.I for both groups were also measured. According to this study, moxibustion therapy resulted in a change of body temperature on the right shoulder joint(p=0.00074), abdomen(p= 0.0047), waist(p=0.0068) and hands(p=0.0317) respectively. Also, the study results showed significant decrease (p=0.0001) in pain over time and significant improvement over body coldness.
