The Effects of Counseling about Death and Dying on Perceptions, Preparedness, and Anxiety Regarding Death among Family Caregivers Caring for Hospice Patients: A Pilot Study

  • Purpose: This pilot study aimed to examine the influence of death counseling on perceptions, preparedness, and anxiety regarding death and dying among family caregivers of hospice patients. Methods: Death counseling developed based on the SPIKES model was provided to 37 family caregivers in a hospice and palliative care unit. Perceptions, preparedness, and anxiety regarding death were assessed with a self-administered structured questionnaire, and participants' scores before and after counseling were compared using the paired t-test. Results: Significant changes were found in perceptions, preparedness, and anxiety regarding death after counseling. Compared to before counseling, the scores for perceptions of death (t=-4.90, P<0.001) and preparedness for death and dying (t=-16.23, P<0.001) improved, while anxiety (t=3.72, P=0.001) decreased after counseling. Some changes were also found in the types of support that family caregivers needed to prepare for the death of their family members in the hospice care unit. Conclusion: The findings demonstrate that death counseling could help family caregivers prepare for the death of their loved ones. Hospice and palliative care providers should play a key role in supporting family caregivers of hospice patients by developing strategies for counseling.

  • 본 논문은 고령사회를 맞이한 우리 사회가 안고 있는 노인의 문제에 주목하여 노년의 삶을 이해하고 노후준비와 죽음준비의 필요성을 인식하도록 하기 위한 것을 목적으로 한다. 영화 '죽여주는 여자'의 주인공 소영의 서사를 중심으로 작품 속 맥락에 따라 문학 치료 관점으로 리뷰 분석한다. 영화 속 작품 서사에는 빈곤, 질병, 치매, 외로움, 죽음의 두려움 등 노인들이 겪는 고통의 문제를 잘 보여준다. 초고령 시대를 맞이하여 국가적 차원의 소외된 노인에 대한 제도적 보완장치 마련과 사회적 관심과 안전망이 확보되어야 하며, 개인적으로는 노후설계를 통한 구체적인 노후준비가 필요하다. 나아가 노후준비 및 죽음을 준비하기 위한 노후준비 교육 프로그램과 죽음 교육 프로그램의 연구와 교육을 위한 지원이 지속적으로 이루어져야 할 것이다.

  • Purpose: This study examined the current state of death preparedness and factors related to the death preparedness among community-dwelling older adults in Korea. Methods: A total of 6,879 older adults' data, selected from the Korean National Survey on the Elderly 2014 data, were analyzed. The criteria for selection were responding to the questionnaires directly and normal cognitive function. Stratified sampling logistic regression analysis was conducted on the compiled data. Results: It was seen that 37.3% of older adults were prepared for death; specifically, 28.6% had arranged for resting places, 10.7% had purchased burial clothes, 6.8% had subscribed to mutual aid societies, 0.7% participated in death education, and 0.6% had signed a will. The results of logistic regression indicated that the older adults who were females, older age, religious, highly educated, more satisfied with their economic status, living in rural areas, and participating in more social activities were more likely to prepare for their deaths compared to their counterparts. Conclusion: Education programs that prepare for death need to target marginalized groups in the community, such as male older adults and whose participation in social activities or satisfaction with their economic status are relatively low.

수술실 의료진의 뇌사자 장기기증 태도 관련 요인 (Factors Affecting Attitudes toward Brain Death Organ Donation among Nurses and Doctors in an Operating Room)

  • Purpose: This study was conducted to identify the factors affecting the attitudes toward brain death organ donation among nurses and doctors in an operating room. Methods: A descriptive research was used. The participants included 90 nurses and 30 doctors who had experience of operating organ transplantation for brain death organ donation. Data were collected from March 12 to May 23, 2020 in the one tertiary general hospital. The outcome measures were perception and attitude of death and attitude towards brain death organ donation. Results: Attitudes toward brain death organ donation was influenced by type of occupation, intention of organ donation and attitude toward death. In addition, the explanatory power of the total variance was 52.1%. Conclusions: Based on the results, it is necessary to prepare an intervention to improve awareness of the brain death and the brain death organ donation.

보건진료소에서의 말기 암 환자 간호사례 (Case Report of Terminal Cancer Patient by Community Health Practitioner)

  • Purpose: This study was done to describe a community health practitioner's nursing case management for a terminal cancer patient registered in the public health post. Methods: For this purpose, data were collected through the patient and family through home visits, health clinic offices, and phone calls. The nursing process was carried out from August to November 2019. Results: The patient suffered the most from anorexia and lack of energy. Also he expressed psychologically uncertainty about disease and death anxiety caused by long-term treatment. In order to reduce the death anxiety, Community Health Practitioner (CHP) asked him to express his life stories and listened to him. CHP provided information of appropriate medications and alternative foods for symptoms such as gastrointestinal disorders and anorexia to the patient and family. Observing the situation of the patient and family, CHP guided the patient and family to prepare for death and has confirmed to them that the process was not with the patient alone. Conclusion: CHP's this experience has shown the possibility for CHP to help the terminal cancer patient and family to prepare peaceful death in their communities.

Notification of Terminal Status and Advance Care Planning in Patients with Cancer

  • As population aging increases the burden of cancer, the quality of death of patients with cancer is emerging as an important issue alongside their quality of life. To improve the quality of death, it is necessary to prepare for death, allowing patients to die comfortably and with dignity at the end. Considering these issues, I aim to discuss the practical aspects of notifying the patient of the terminal phase of cancer and planning for end-of-life care (i.e., advance care planning). When cancer treatment that can extend the patent's lifespan becomes difficult, the patient enters a treatment transition period. Treatment is shifted from life-prolonging care to life-enhancing care, and end-of-life care must be well planned. Medical providers often worry too much about whether the patient will be disappointed or psychologically traumatized when notified of the terminal phase of their cancer, thus delaying plans for end-of-life care. In fact, patients can accept their condition and prepare for end-of-life care better than we expect. During the treatment transition period, notification of terminal status should be given, and a well-prepared advance care plan should be established early when the patient has decision-making ability. In addition to conveying information, it is always necessary to be sensitive to whether the patient and caregiver understand the information and respond to their emotions.

죽음준비교육 참여군과 비 참여군의 삶의 의미 및 죽음에 대한 태도 비교 (Comparison of Meaning in Life and Death Attitude between Participants and Non-participants in Well-dying Education)

  • Purpose: The purposes of this study was to compare the meaning in life and death attitude between the participants and nonparticipants of the well-dying education program. Methods: This study adopted the descriptive comparative design. Data were collected by interviewing 85 participants and 94 non-participants of well-dying education. The instruments used for this study were a self-report questionnaire. Results: There were significant differences in age, gender, marital status, health status, and volunteer experience. The program participants showed higher scores in the death attitude than non-participants. There were significant correlations between meaning in life and death attitude in participant group. Death attitude was significantly associated with meaning in life in participant group with 6.0% variance. Conclusion: Based on the results, well-dying education program was effective to prepare good death with more comprehensive vision. Therefore, this program should be served for patient with life-threatening illness by nurse and this is the expended role of oncology and hospice palliative nurses.

Death and the Inoperative Community in the Works of Gabriel Garcia Marquez and Merlinda Bobis

  • Gabriel García Márquez's short story "The Handsomest Drowned Man in the World" ["El ahogado más hermoso del mundo," 1968] and the novel Chronicle of a Death Foretold [Crónica de una muerte anunciada, 1981] and Merlinda Bobis's novel Fish-Hair Woman (2012) and short story "O Beautiful Co-Spirit" (2021) feature unusual scenarios of death: the arrival of a drowned man's corpse at an island; the inaction of the community to stop the foretold death of a supposedly-innocent man; a woman with long hair that can fetch dead bodies at the bottom of the village river; and a Filipino Catholic and a Malaysian Muslim working together to prepare an Italian Catholic's corpse for a funeral. These narratives demand critical attention as all deaths make the community's existence meaningful as they alter its social reality. Looking into the works of the aforementioned Colombian writer and Filipino writer and unveiling how death affects the community, this paper relies on Jean-Luc Nancy's theory on death and inoperative community.

예비노인의 사전연명의료의향서 작성 여부에 따른 죽음불안과 품위있는 죽음태도의 차이 (Death Anxiety and Dignified Death Attitudes of Pre-Elderly Who Opt for Signing Advance Directives)

  • Purpose: This study attempts to provide basic data for establishing and implementing an advanced directive (AD) system that helps identify dignified death attitudes and reduces death anxiety of the preliminary, through preparation of an AD. Methods: Data were collected from 135 preliminary elderly aged 55 to 64 years, recruited from health centers and welfare facilities. All data were analyzed using SPSS 22.0. Results: Overall scores of the participants were 2.26±0.64 for death anxiety scale, and 3.16±0.44 for dignified death attitude. Scores of individuals who prepared an AD were determined to be 2.19±0.64 for death anxiety, and 3.34±0.44 for dignity of death. The difference in scores obtained for dignified death attitude was significantly higher for the group that signed an AD, as compared to the group with no AD (F=14.81, p<0.001). Conclusions: Results of this study reveal that preliminary elderly who sign an AD have a higher dignified death attitude score as compared to subjects who do not sign an AD. Additionally, the former group of participants desire a dignified end to their life. This indicates a necessity to promote public campaigns for ADs, and to develop educational programs that assist the elderly to prepare for a dignified death and make autonomous decisions.

간호대학생의 사회심리적 특성에 따른 죽음에 대한 태도 (A Study of Nursing College Students' Attitude towards Death by Psychosocial Characteristics)

  • Purpose: This study was performed to identify nursing college students' attitudes towards death and was conducted as a pre-investigation to prepare them for coping with terminally ill patients in their clinical practice. Methods: The subjects consisted of 321 nursing students. Their attitudes towards death were measured by using the Collett-Lester Fear of Death Scale(CLFODS). The data obtained were analyzed by using the one-wayANOVA and Duncan's multiple test using the SAS 9.2 soft ware program. Results: The mean CLFODS score was 2.99. Among the 4 subscales, there were significant differences among the students with respect to grade, indirect experience of death, social isolation, daily life stress, and suicidal ideation in the past year. Conclusion: A negative attitude towards death was observed in the psychosocial characteristics, with indirect experience of death, social isolation, daily life stress, and suicidal ideation in the past year. It is necessary to educate students on how to care for dying patients before clinical practice. This education can be carried out by developing a death education program after considering the above mentioned psychosocial characteristics noted in this study.
