의사를 위한 호스피스 교육 프로그램의 개발 - 2004 호스피스.완화의료 시범사업을 중심으로 - (Development of Education Program for Physicians Based on the 2004 Hospice Palliative Model Project for Terminal Cancer)

  • 목적: 2004년 호스피스 완화의료 시범사업을 실시하면서 의사를 대상으로 한 사업을 위하여 호스피스 완화의료에 관한 교육 프로그램을 개발하고 이를 기초로 교육을 실시하고 성과를 분석하였다. 방법: 교육 분과위원회에서 3차례의 회의를 통해 교육 요구도(총 79문항) 설문지를 개발한 후 125개 전국 호스피스 관련 기관에 종사하는 의사를 대상으로 우편을 이용하여 설문지를 발송하였다. 교육 중요도는 호스피스 전문가 27명에게 이메일을 통하여 조사하였다. 결과: 교육 요구도 및 중요도 조사 결과, 다양한 문헌 검토 및 외국의 호스피스 완화의료 관련 교육과정을 기초로 호스피스 완화의료 교육 프로그램이 개발되었으며 교육 시간은 총 36시간(이론 21, 실습 15시간)이었다. 이를 토대로 1일 교육 코스를 전국에서 4회에 걸쳐 시행하였으며 47명의 의사가 참여하였다. 교육 이수 후 참가자들은 교육 과정이 향후 업무에 도움이 된 정도는 항목마다 $3.0{\sim}3.6$점, 교과 강사의 적절성은 $3.1{\sim}3.3$점으로 평가하였다(1: 전혀 아니다 ${\sim}4$: 매우 그렇다). 전반적인 만족도는 100점 중 80점이었다. 결론: 개발된 교육 프로그램은 전체적으로는 운영되지 않았으며 일부만 시험교육으로 운영하였다. 대상자들은 대체로 만족하였으나 강의보다는 사례, 토론 중심의 교육 및 주제별 심화교육을 원하였다. 향후 지속적인 요구도 조사와 프로그램 개발 및 시행이 필요하다.

  • Purpose : An aging phenomenon was recognized as only negative aspect, which impediment a development with the decrease of a developing population and increase of social burden. Nevertheless, the problem of aging cannot be treated as a problem of individuals or social burden. Because Every mankind sometime will be the person directly involved the aging. Methods : Korea, predict to reach the aging society with over 20 percent of elderly in the whole population in 2020, should find an active political plan for responding. Because a previous discussion about the aging phenomenon focused on social welfare, the housing plan for the elderly with a consideration of the physical environment for sustaining elderly's life with happiness will be primary task. Results : This research considers housing policy, focusing on the medical service. It is because statistics, which 85 percent of elderly people suffer from chronic diseases because of physical aging signify the importance of medical service to the elderly in every-day-life. Implications : We prescribe the elderly as a population over 65. In this duration, the elderly retirees from the workplace e and spend the majority of time in their home. Thus, for the elderly, the residential space is a field of action and passageway connecting with the surroundings For the solution of physical space preparing the aging phenomenon, the combination of residential and medical function creates a new type of a lifestyle.

  • 최근 맞벌이 가구의 수가 늘어나고 출산율도 함께 저하되면서 집에 홀로 방치되는 초등학교 학생이 전체 초등학생의 3분의 1이 넘는다고 한다. 전국 초등학교에서는 방과 후 돌봄 교실이란 정책을 시행하고 있지만 그것마저도 교실의 수를 정부에서 줄이고 있는 상황이다. 이처럼 보호자들은 아이들의 집에 혼자 두는 것을 피할 수 없고 방치 된 아이들의 하루를 궁금해 할 수밖에 없는 현실이다. 이런 보호자들과 초등학교 고학년 학생들을 위해 자기 일과를 스스로 계획하고 실천 할 수 있는 서비스를 제안하고자 한다. 이 서비스로 인해 아이는 자기조절학습능력을 키울 수 있고 보호자들은 홀로 집에 방치되어 있는 아이가 스스로 어떤 계획을 세우고 실천하고 있는지 앱을 통해 피드백을 받아 안심할 수 있다.

수술실 간호인력의 수요측정 및 간호제공량분석 - 수술대기시간과 수술시간을 중심으로 - (A Study of Nursing Manpower Requirements based on the Nursing Times spent in Operating Room of an University Hospital)

  • This Study was an attempt to estimate the optimum numbers of Operating Room Nursing Manpower by measuring the amount of service hours required by the patients in Operating Room in relation to the service amount actually provided by the nurses. The major concern of this study was placed on the measurement of Nursing Service Requirements by using the Operating Room (O. R) Patient Acuity System recently developed by M. M. Hart to classify the O. R. patients into four groups according to the degree of the complexity of operative procedure and some other elements which increase nursing activities in respect of patient care; Acuity IV group is the one requires nursing services most, on the other hand Acuity I requires least. nu sing The objectives of this study were as follows; 1. To analyze functions of the nursing personnel in O. R. by time unit and to estimate the average time a nurse can activate for productive functions. 2. To measure the actual amount of nursing times provided by nurses to the surgical patients. 3. To develop a patient classification system in order to measure the amount of Nursing services required by the patients. 4. To calculate an appropriate number of nursing manpower to meet the needs of the patients. In order to conduct the research both selected nurses and patients in 'S' University Hospital were Studied by utilizing the O. R. Patient Acuity System as well as the Classification Chart developed by Association of Operating Room Nurses (A. O. R. N) as a means of classifying functions of O. R. nurses. That is; Functions of the 10 selected O. R. nurses observed during the period of June 30 to July 4, 1986, whereas the amount of nursing services required by or provided to the 974 patients who had received surgeries during the period of June 9 to July 4, 1986. The results of this study were as follows; 1) The actual working hours per a nurse averaged 6.7 hours a day. 2) Each nurse's daily routine schedule consists of $71.4\%$ for Technical Functions, $16.1\%$ for Nonprodective Functions, $6.6\%$ for Assessment and Evaluation, $3.9\%$ for Overseeing and Supervision and the rest $2.0\%$ for Patient Preparation respectively. 3) Preoperative waiting time per a patient was 24.1 minutes on the average; for the first case was 10.7 minutes, whereas for the following cases was 32.0 minutes. 4) Total Operation time for the 974 patients during the period of observation for this study amounted to 2759.6 hours, weekly hour was equivalent to 689.9 hours, Whereas daily operation time averaged 130 hours. Meanwhile the average operation time per patient was 2.8 hours ; for the case of Acuity IV was 5.6 hours, 5. 1 hours for the case of Acuity III, 2.3 hours for Acuity II and 1.1 hours for Acuity I. 5) According to the O. R. Patient Acuity System, $64.5\%$ of the whole patients belonged to Acuity II, $23.7\%$ to Acuity III, 11. $3\%$ to Acuity IV and $0.7\%$ to Acuity I respectively. 6) Required amount of nursing times based on the preoperative waiting time and operation time was 7167.8 person hours, which showed that $5.5\%$ of them needed for preoperative nursing care, whereas the rest $94.5\%$ for intraoperative nursing care. In terms of the O. R. Patient Acuity System, $49.7\%$ of total nursing service requirements was needed for Acuity II patients, $27.4\%$ for Acuity III patients, $17.2\%$ for Acuity IV patients and $0.2\%$ for Acuity I patients. 7) The rate of the nursing services provided against the required nursing times was about $81.4\%$ on the average; some departments, like those of Plastic Surgery, Otolaryngology and Ophthalmology whose patients mostly belonged to Acuity II recorded hegher provision rate than average, whereas other departments of Thoracic Surgery. Neurosurgery and Orthopedic Surgery whose patients belonged to Acuity III and Acuity IV as well as Acuity II recorded lower provision rate than average. 8) Subsequently, required numbers of nursing manpower was 10.7 nurses additionally. Based on the above findings the following recommendations will be made; 1) this study recommends, develops. and adopts an accurate and realistic O. R. Patient Acuity System which can help measure the nursing service requirements objectively to elicit the rationales of allocation of nursing personnels. 2) this study proposes storongly place nurses who take the role of preoperative nursing care exclusively for the waiting patients in O. R. and shortening their waiting time by close communication between the designated O. R. and the ward.

시간제 간호사 및 인턴간호사의 직무만족도에 관한 비교연구 (Comparative Study on the Job Satisfaction of Part-time Nurses and Internship Nurses)

  • Recently, Korean health care industry managers are paying more attention to customer oriented service, the rationalization of business administration, and quality control of service to adjust their business to outer environment since IMF bailout program. They are focusing on the cost reduction through remodeling the size and structure of man power, viz. labor flexibility. Nurses are not free from this juncture and contingent nurses are increasing fast. This article intends to verify the actual condition of two types of contingent nurses -part-time nurses and internship nurses-and to compare their job satisfaction to provide basic resource for efficient management of nurse man power. The concrete goals of this article are; Firstly, to verify the actual condition of their employment. Secondly, to compare their job satisfaction. And Thirdly, to the relation between employment condition and job satisfaction. To accomplish these research goals, a statistical survey was executed, in which 384 questionnaires - 66 for manager nurses, 318 for contingent nurses-were given to nurses working at 66 general hospitals-which have at least 100 beds-in Seoul. Among them, 121 questionnaires-of 28 general hospitals-were returned. Then, the data coded and submitted to mean, standard deviation, T-test, variance analysis (ANOVA), correlation analysis, multiple regression analysis with SAS program. The research results of the contingent nurses are followings: 1. Two types of contingent nurses shows similar age spans: they are mostly 21 - 30 years old and unmarried. But internship nurses have high level of educational career. Part - time nurses spread at general beds, out -patient part, intensive care part, operation part, etc, but internship nurses work mostly at general beds. 2. Two groups shows difference in actual employment condition: average employment career of part -time nurses is 7.0 months but internship nurses' is 2.0 months: average duty-on days per month of part - time nurses are 23.7 days but internship nurses' are 24.8 days. But there are little difference in average working time per day: 7.7 hours for part -time nurses and 0 hours for internship nurses. 3. The average wage per month for part -time nurses is 836,026 won but for internship nurses is 557,428 won-66.7% of part-time nurses'. Both groups are enjoying little additional pay. 4. Both groups are getting job not so much through advertisement of newspaper or hospitals as through acquainted person or college. 5. Both groups show very high level of job satisfaction: 3.2195 for part -time nurses and 3.2881 for internship nurses. But they show very low satisfaction on payment level compared with other categories and two groups show meaningful difference(P<.0001). 6. The multiple regression test reveals the factors related with job satisfaction: wage level, working part(OR or ICU), age, job career, and motive of contingent job-taking('because I can take care of family duties at the same time') influence positively: motive of contingent job-taking('because I can work regularly without alternation') influences negatively.

농어촌지역 한방 외래 다빈도 상병의 의료이용 변이분석 (Variation Analysis of Medical Service Utilization in Oriental Medicine Frequent Disease of Rural Area)

  • 본 연구는 의과 일부 상병에 국한된 지역간 의료이용변이 연구의 범위를 한방으로 확대하여 다양한 실증적 연구기반을 마련하고 의과 상병의 지역간 의료이용변이가 한방에서도 존재하는지, 존재한다면 그 요인은 무엇인지를 파악하여 한방 의료서비스의 합리적 이용을 위한 정책수립에 필요한 기초 자료를 제공하는데 그 목적이 있다. 2010~2011년 건강보험 자료에서 의과, 한방외래 다빈도 10대 상병을 선정하고 86개 군 지역을 대상으로 지역 간 의료이용 변이여부를 분석하였다. 분석결과 한방에서도 지역별의료이용변이가 존재하는 것으로 나타났는데 내원일당진료비는 공급자측면 보다는 수요자특성이 주요한 요인으로 작용하였고 수진자당진료비는 공급자와 수요자특성이 동시에 주요한 요인으로 작용하였다. 따라서 한방 의료서비스의 합리적 이용을 위한 의료의 과다이용과 미 충족 방지를 위해 한방 의료서비스 표준화, 한방의료 역할강화, 올바른 의료이용과 불필요한 의료행위를 줄이기 위한 정보제공 및 정보공유 확대정책이 필요하다.

국내 장기요양보험서비스를 제공 받는 치매 환자 대상 작업 중심 회상치료 효과에 대한 체계적 고찰 (A systematic Review on the Effects of Occupational Reminiscence Therapy in Dementia Offered National Long Term Care Insurance)

  • 목적 : 본 연구는 국내 장기요양보험서비스를 제공받는 치매환자를 대상으로 다양한 분야에서 이루어진 작업 중심 회상치료 프로그램을 중재 적용한 연구 효과에 대해 검토해보고자 체계적 고찰을 시행하였다. 연구방법 : 2009년부터 2013년 까지 한국교원학술정보원을 통해서 확인되는 논문을 검색하였고 주요 검색용어로는 치매, 시설 서비스, 주간 보호 센터, 회상치료, 작업 중심 프로그램 등의 단어를 사용하였다. 연구결과 : 분석 결과 총 6편의 논문이 선정되었으며 선정된 연구에서의 회상치료는 모두 작업 중심 프로그램을 병행한 집단 회상치료이다. 세부 중재 형태는 의사소통 중심 회상치료, 작업 중심 회상치료로 크게 2가지 형태로 나눌 수 있었다. 치매 환자 대상의 회상치료의 효과는 인지기능 유지 및 회상기능 향상, 우울감소, 문제행동 감소, 의사소통 및 사회적 상호작용 향상, 삶의 질 향상 등 총 5가지 효과가 보고되었으며, 연구의 질적인 측면에서는 적은 대상군, 짧은 연구기간, 추적조사 미시행으로 인해 전반적으로 낮은 수준의 내적, 외적 타당도를 보였다. 결론 : 추후의 연구에서는 보다 많은 수의 대상군의 확보와 추적조사를 통한 지속적인 치료적 효과에 대한 연구가 이루어져야 할 것이며 치매환자 대상의 다양한 형태의 작업 중심 회상치료의 연구가 이루어져야 할 것으로 사료된다.

대학병원 외래환자의 일반적 특성과 만족도 - 서울시내 5개 대학병원을 대상으로 - (General Characteristics and Satisfaction of Out-patients of University Hospitals in Seoul, Korea)

  • Purpose : the management of University Hospital is being challenged in maintenance of reasonable level of income and high ranked reputation by domestic competition with each other and emergence of enterprised owned hospitals. It is imperative that University Hospitals have to make management for patient satisfaction. Furthermore, increased patient's requirement for qualified hospital services (quality assurance) and low-estimated service fee also repress the hospital management condition as well as medical markets open following with Urguay Connection. Due to these unforable conditions surrounding hospital management, -University Hospitals are being pressed to seek improved management strategies. To develope the strategies, we need to have basic understanding about the problems on hospital management and detail information for various patient's requirement. Methods: For this study, we have analysed out-patients from five different University Hospitals located in Seoul, Korea. To obtain the data, we have carried out personal interviews with patients who patients who visit the Out-Patient Clinics of five different University Hospitals using a previously prepared questionnaire. Result: Around 65.7 percent of the visits to University Hospitals were indwelt in the vicinity of 1 hour and motuvation of visiting University Hospital was expending high wuality medical csre in 49.3 percent. The 79.3 percent of the patients have experienced inconvenience during medical care in University Hospital. The most inconvenient condition was waiting for doctors. The 57.2 percent of total patient have experienced rudness. The most rudness condition was registration and receipt desk in 44.4 percent. Patient expect that doctors working in University Hospitals as professors have high and updated medical knowledge(50.4%) and University Hospitals have a high quality medical care system(79.4%). The patient satisfaction was relatively low in 61.1 percent of total patient and revealed high frequency of again visit University Hospital in satisfaction group. Comparison of interhospital analytical study showed quite difference on various problems. Conclusion: Almost patients who visit to Out-Patient Clinic of University Hospital havevisiting motivation to high quality medical care. University Hospitals have several different unsatisfactorial factors and revealed different degree of patient satisfaction. In a future day, University Hospitals have to make use of another University Hospital's merits for approach of Benchmarking and also should be studied decision factors of patient satisfaction and interhospital difference of them.

보육교사의 전문성 인식, 학습민첩성, 긍정심리자본이 직무만족도에 미치는 영향 (The Effects of Child Care Teachers' Professionalism, Learning Agility and Positive Psychological Capital on Their Job Satisfaction)

  • 본 연구의 목적은 보육교사의 전문성 인식, 학습민첩성, 긍정심리자본 등이 직무만족도에 미치는 영향을 확인하여 보육교사의 긍정적인 직무 수행에 필요한 정책적 시사점을 제안하는 것이다. 이를 위해 전남 지역의 보육교사 283명을 대상으로 온라인 설문조사를 실시하였고, 분석결과는 다음과 같다. 첫째, 5년 이상의 경력을 가진 보육교사들이 전문성 인식과 긍정심리자본에 있어서 높은 수준을 보여주었다. 둘째, 보육교사의 직무만족도는 전문성 인식, 긍정심리자본, 학습민첩성의 순으로 높은 상관을 보였다. 셋째, 보육교사의 전문성 인식과 긍정심리자본이 직무만족도에 긍정적인 영향을 미치는 것으로 나타났다. 본 연구결과를 통해 보육교사의 전문성 인식, 학습민첩성, 긍정심리자본은 직무만족도를 설명하는 유의미한 변인임을 확인하였다. 따라서 어린이집에서의 직무만족도를 높이기 위해서는 전문 연수과정과 컨설팅 등을 지원하여 스스로 전문직업인이라는 인식을 갖도록 하고 나아가 교사들에게 보다 많은 자율성을 부여하며 수직적인 어린이집의 운영 시스템을 개선할 필요가 있다.

Retrospective Analysis of Long-Term Survival in Very Elderly (Age ≥80) Critically Ill Patients of a Medical Intensive Care Unit at a Tertiary Care Hospital in Korea

  • Background: The purpose of this study was to evaluate the long-term survival rates of very elderly (age ≥80) critically ill patients admitted to a medical intensive care unit (MICU) at a regional tertiary-care hospital in Korea. Methods: We retrospectively analyzed data from patients who survived after discharged from the MICU of our hospital. Survival rates at 90 days, 1 year, 2 years, and 3 years were assessed between patients age ≥80 and those age <80. Survival status was evaluated using the National Health Insurance Service data. Results: A total of 468 patients were admitted, 286 (179 males, 97 females; mean age, 70.18±13.2) of whom survived and were discharged soon after their treatment. Among these patients, 69 (24.1%) were age ≥80 and 217 (75.9%) were age <80. The 90-day, 1-year, 2-year, and 3-year survival rates of patients age ≥80 were significantly lower than those in patients age <80 (50.7%, 31.9%, 15.9% and 14.5% vs. 68.3%, 54.4%, 45.6%, and 40.1%, respectively) (p<0.01). The Kaplan-Meier survival curves showed significantly lower survival rates in patients age ≥80 than in those age <80 (p=0.001). Conclusion: The poor rates of long-term survival in very elderly (age ≥80) and critically ill patients admitted to an ICU should be considered while managing and treating them.
