  • Because of the rapid increase of the number of elderly people, there are many problem in modern society. Especially the support of elderly people is becoming the hot issue, which results from a nuclear family and social activity of woman. The Policy of community care of the elderly people in western is caused by a heavy cost and isolation from the society of a facility care policy. Since 1980, Concept of 'home care for elderly people' has been appeared in korea. The Facility causing by these movement is 'Multipurpose senior center'. The purpose of this study is to present the alternative data to improve the quality of medical and rehabilitation part in multipurpose senior center. Data collecting skills such as drawing documents analysis, interviews, observation were used to examine the condition of medical and rehabilitation. And, case study method was used to generate the alternative medel which can be used for the spatial composition and the area calculation.

A Study on The Quality of Family Child Care (가정보육시설의 질적 특성에 관한 연구)

  • The purpose of this study was to investigate the current status of family child care. The status of family child care was explored in terms of general characteristics, program quality, and child behavior. The subjects were 17 family child care and their 102 children and 60 parents. Harms & Clifford's (1989) FDCRS(Family Day Care Rating Scale) was used to measure program quality. Child behavior was recorded by time sampling in two 30 min observations during the free play session in each facility. A questionnaire for caregiver and parents was used to investigate general characteristics. The data were analysed by frequency, percentage, and Pearson's r. Results showed that (1) a wide range of variable accounted for the general characteristics of family child care, (2) the level of program quality as rated by the FDCRS was below mid-point, (3) children engaged in solitary behavior much more than in peer interactions or caregiver-child interactions.

Development of Dementia Care Model in a Community (지역사회 치매관리 모형 개발 : 광명시의 경우)

  • There has been a dramatic increase in public awareness regarding dementia during recent years. However, dementia remains a family affair and patients do not receive adequate care in Korea. This study aims to assist patients and their caregivers by establishing Home and Community based Long-Term Care in a city. The data collected for analysis include five main categories: dementia prevalence, limitations of daily activities of patients, burden of caregivers, the services that patient's family want to utilize, the resources that handle dementia in the community. Major findings can be summarized as follows: 1)The prevalence rate of dementia for elderly people is 13.1 per 100 persons. Alzheimer's disease amount to 38.9% of dementia patients and vascular dementia account for 36.7% of them 2)Eight out of ten patients have mild dementia. Almost all patients have normal ADL. IADL, however, shows different picture. In every items of IADL, about 60% of patients reveals some limitations. 3)The proportion of patients who had medical diagnosis is as low as 20%. Families of patients think dementia as normal aging process and medical doctors in the community do not give special concern to dementia patients. 4)Caregivers does not have proper social support. They suffer from long care time, experience large obstacles in respect of health, daily living, and social activity. 5)Health center and Community welfare center have launched some programs-consultation, home-visiting nursing, day care center, voluntary force mobilization and so on-for dementia patients. But they do not perform expected roles and functions because of lack of skilled personnels and inadequate coordination of relevant organizations for dementia care. 6)Families of dementia patients prefer home helper and home-visiting nurse to hospitalization. For the future, however, demand for institution-based long-term services will increase. We develope community dementia care model based on above findings as follows: 1)Health center execute community cardiovascular control program for the prevention of vascular dementia. 2)Refer to epidemiologic characteristics of patients and preference of family, the most urgent task for dementia care in this city is to expand and organize Home and Community based Long-Term Care. 3)For the continuous and comprehensive care, care plan for a patient must be prepared. Case management team should be builded to prepare this plan and coordinate relevant resources. 4)Special long-term care unit for dementia will be needed in a near future. This unit should have multiple functions, such as day-care center, short stay facility, training center for relevant personnels, besides long-term nursing home considering effective care of dementia and efficient operation of the facility. 5)Voluntary workers deserve their due efforts. Incentive mechanisms must be developed to activate voluntary activities.

Difference of Caring Needs According to Caring Experience of the Elderly with Dementia and Caring Situation (치매노인 부양경험 유무에 따른 부양요구의 차이와 치매노인 부앵실태)

  • The purpose of this study were to find out the difference of needs of caring between care-givers and non care-givers, and to suggest the way of lessening vigorous task of care-givers for the elderly with dementia. Data were collected from 130 nationwide respondents intentionally divided into two groups; care-givers and non care-givers in the middle aged with middle and upper income. Collected data were analyzed by frequency, percentage, t-test using SPSS package. Since the result of survey, unexpectedly, showed no difference between two groups, it could be explained as that these two groups commonly had same needs of caring for the elderly with dementia. Major findings were as follow; 1) Most Koreans stiff thought family should be the main care-giver for the elderly with dementia prior to nation or society. 2) Responsibility of caring for the elderly with dementia would be better to be shared with children instead of focusing to a child. 3) They thought ideal residential facilities for the elderly with dementia were small-scale professional dementia facility(group home) rather than home or general elderly housing. 4) Professional dementia care hospital was one of the most needed facilities for the elderly with dementia, followed by short-stay and dar-care center. 5) It was revealed care-giving task was vigorous showing that most care-givers spent 1-5 hours a day for caring, while 13% of respondents spent 11-24 hours a duty. 6) 90% of care-givers took the responsibility of main care-giver because of duty of offsprings or spouses, and wanted to be free from their current circumstances. From the result of this survey researchers would like to suggest the establishment of diverse facilities for professional dementia care to lessen the caring burden for the elderly with dementia: group home, chronic hospital, short-stay, day-care center. Financial support from the government for the housing renovation of the caring families should be considered seriously afterward. It is needed to give the opportunity to select proper paid dementia care facilities according to their income and situation of household.

A Study on the User Needs for Renovation of Race Track Type of Nursing Units in K University Hospital (K대학병원 병동부 이중복도형의 개선을 위한 사용자 요구 조사연구)

  • Purpose: General hospital in korea is getting large-scaled, specialized and upgraded. So, nursing unit of race track type has been advanced along with a spatial organization, building equipment and environmental approach. This study is to search the guidelines for renovation of nursing unit with race track type in the case of K university hospital in Daejeon. Methods: 9 cases of recently opened general hospital has been analyzed for spatial and environmental design solution. Questionnaires and interviews about user needs of nursing staff, patient and visitors, have been conducted for a nursing care system and facility of nursing units in K university hospital. Results: The user needs are represented as followings. Center core public zone in each floor plan and center core nursing station and supporting areas in nursing unit are good for an adjacency and separation of spatial organization. Toilet of group patient room is necessary to equip for easy, safe and infective aspects, and so toilet install will decrease the patient number and increase the nursing care service. Hall type of station front is good for visibility and observation. It is appropriate to renovate into south facing group patient room for privacy, enough space for nursing care facility and supporting area, enough width of door to move portable medical equipment, room for medical doctor and practical student, noise absorbing of day room. Implications: Upgrading the nursing care service and facility equipment is necessary to reflect the user needs and cooperate with hospital management.

Service Quality Measurement by Perception of A Day Care Service Users: Based on SERVQUAL Model (주·야간보호서비스 이용자 인지에 의한 서비스품질 측정: SERVQUAL 모형을 중심으로)

  • The purpose of this study is to measure the utilization of a day care service and service quality by SERVQUAL model, also to discuss policy implications based on this. The data analysis is conducted with 208 users of a day care service in Deajeon metropolitan city, Chungnam and Chungbuk area. The analysis result is as follows. First, it shows positive definiteness on tangibles, reliability, responsiveness, assurance, empathy in all categories because perceived level is higher than expectations. Perceived level after using the service is more highly rated than expectations before using it by most of seniors who use the service. Second, service quality is examined in accordance with demographic characteristics, but the difference is not statistically significant. Third, examining service quality in terms of characteristics of centers, it shows statistically significant difference in perceived level depending on management type of location, a number of provided programs per week, and the size of a facility and in expectations depending on established institution. Forth, as a result of examining service quality in accordance with characteristics of service users, there is no meaningful difference statistically.

Study on actual condition of oral hygiene among admitted aged person at the some of Gyeongnam welfare facilities (경남 일부지역 노인복지시설 입소자의 구강건강 실태조사)

  • In Korea, respect for the aged and filial devotion is treated as basic ethics for human life, and family takes care of the aged person mainly. Nowadays, family support on the aged person is prioritized than the others. However, number of aged person is growing, and family, which is used to protect the aged, becomes nuclear through industrialization. In addition, social advancement of female induces weakened supporting function of family, and all these issues generate the problem of protection for the aged as significant social problems. Author conducted oral inspection and questionnaire for the aged in some welfare facilities in Gyeongnam location from December 2005 to February 2006 to improve quality of life and oral hygiene of the aged. Through gathered data, the actual condition of oral hygiene management on the aged person in welfare facility was evaluated as fundamental data for project development on oral hygiene of aged person. Through the analysis of inspected data, the following conclusions are derived. 1. Management status of oral hygiene is mostly not good, and toothbrushing per day is 'one time' for the most cases. 2. In free-of-charge facility, monthly allowance is 'under 50 thousand won' for the most cases. 3. Time for visiting oral treatment facility is on 'when toothache is occurred' for the most cases. For treatment content, 'prosthetic dentistry' takes 36.8% in charged facility, and 'tooth extraction' is 27.0% for free-of-charge facility. 4. Average DMFT index of the aged in charged facility is 16.81, and free-of-charge facility is 21.71. 5. Average number of functional teeth in charged facility is 15.22, and free-of-charge facility shows 7.29. 6. Average number of remained teeth in charged facility is 15.71, and the umbe in free-of-charge facility is 9.04. 7. Average number of extractable teeth in charged facility is 0.48, and for free-of-charge facility, the number goes up to 1.70.

Field Study of Characteristics of Airborne Bacteria Distributed in the Regulated Public Facilities (규제대상 다중이용시설내 부유세균의 분포 특성에 관한 현장 조사)

  • Concentration and identification of airborne bacteria in the regulated public facilities were examined with the six-stage cascade impactor. Geometric mean total and respirable concentrations of airborne bacteria were $404cfu/m^3$ and $194cfu/m^3$ in hospital, $931cfu/m^3$ and $358cfu/m^3$ in kindergarten, $294cfu/m^3$ and $134cfu/m^3$ in day-care center, and $586cfu/m^3$ and $254cfu/m^3$ in postpartum nurse center, respectively. As a result, culturable total and respirable concentrations of airborne bacteria were significantly highest in kindergarten and lowest in day-care center (p<0.05). The ratio of respirable to total concentration of airborne bacteria in the investigated public facilities was ranged from 30% to 40% but there was no significant difference among them (p>0.05). The mean I/O ratio of culturable total and respirable concentrations were 0.58 and 0.66 in hospital, 0.71 and 0.83 in kindergarten, 0.28 and 0.41 in day-care center, and 0.63 and 0.78 in postpartum nurse center, respectively. Day-care center showed the lowest I/O ratio of culturable total and respirable concentration of airborne bacteria (p<0.05) but a significant difference was not found among other facilities. Indoor concentration of airborne bacteria did not correlated significantly with indoor temperature and relative humidity (p>0.05) but had a significant positive correlation with $CO_2$ and surrounding condition (p<0.05). Staphylococcus spp., Micrococcus spp., Corynebacterium spp., and Bacillus spp. were dominant genera and amounted to over 95% of total airborne bacteria identified in the investigated public facilities. Size distributions of four dominant genera did not observed inconsistently regardless of type of public facility.

The Effectiveness of Nutrition Education Provided by Dietitians in Child Care Centers (보육시설의 영양사 배치를 통한 영양교육 및 효과 평가)

  • The purpose of this study was to evaluate the effectiveness of nutrition education with nutrition services provided by dietitians who were placed in child care facilities from the Korean Dietetic Association. For this, we investigated the levels of nutrition knowledge and dietary intakes of children who attended child-care centers as well as dietary practices of children assessed by their parents. The treatment for children consisted of nutrition education and food service activities that are provided by the dietitian who have 3 to 5 years experience. Nutrition education was implemented during 10 weeks, 20 times, and a total 400 min and it's effectiveness was evaluated by questionnaire. Data were obtained for 123 children aged 4 to 5 years old who attended four child-care centers, one for a control group and 3 for intervention groups. Dietary intakes were investigated by measuring one-serving size and plate waste a of child for one-day dietary records before and after nutrition education. The levels of nutrition knowledge of children improved showing 70.80 points before to 83.45 points after nutrition education (p<0.001). Dietary intakes of the children after nutrition service increased significantly on cooked rice (133.66 g), Kimchi (19.41 g), side dish of meat/fish (48.40 g), and side dish of vegetables (24.88 g). Dietary practices of children after treatment also improved especially 'eat diverse meat, fish, egg, and bean' and 'never leave plate waste'. To summarize, this study pointed out that nutrition service and nutrition education provided by dietitians had influences on increases of the nutrition knowledge, dietary intake, and dietary practices. Therefore, placement of dietitians needs to extend to child care facilities from the 100 persons-over-capacity facility to the 50 persons-over-capacity facility, for providing professional service such as nutrition education and nutrition counseling.

Measuring Accessibility of Day Care Centers for the Elderly in Seoul Using GIS Spatial Analysis Techniques (GIS 공간분석기법을 이용한 서울시 노인주간보호시설의 접근성 연구)

  • The purpose of this study is to derive policy implications in allocating day care centers for the elderly with more efficiency and/or equity by calculating and analysing the accessibility scores of individual dongs to day care centers for the elderly in Seoul using GIS spatial analysis techniques. The study finds that the spatial distribution of the centers does not respond to the distribution of the potential users very well and that dongs in Gangseo-gu and Songpa-gu at the outskirt of Seoul has the lowest level of accessibility. The findings of the study has policy implications in the management of day care centers for the elderly. First, in order to improve the accessibility of the elderly to the centers, an increase in the number and the capacity of the centers needs to be made especially in areas with lower level of services provided. Second, if policy decision is made in the way to increase the capacity of the existing centers rather than to increase the number of centers due to, for example, the budget limit, capacity expansion needs to be made in the centers with higher proximity in order for more elderly people to use the centers more frequently with easy access. Finally, this type of accessibility analysis techniques needs to be used to allocate, expand, and evaluate other types of care facilities for the elderly and social welfare facilities in order to preserve the welfare right of the users of the facility who usually have a lower mobility and to assure the necessity of the resource investment.
