  • The purpose of this study is to determine the effectiveness of routine meatal care on the reduction of catheter-associated urinary tract infection (UTI). The study was carried out on 30 patients with foley catheters in an intensive care unit of a general hospital from September 30 to April 1, 1998. Participants were both male and female. Data were collected from each patients by urinary specimen obtained with aseptic collection technique at the 3rd and the 7th day of the experiment after giving daily mental care with 10% betadine for periods ranging from 1st to 7th day to the experimental group but not to the control group. The results are as follows : 1. The rate of urinary tract infection within the experimental group was 0.0%, at the 3rd day of the experiment and 20.0% at the 7th day, but that of the control group was 20.0% at the 3rd day and 7% at the 7th day. There was a significant difference in urinary tract infection rate between the two groups at the 3rd day but no significant difference at the 7th day. 2. In the control group, the rate of UTI was 0.7% for male and 13.3% for female at the 3rd day, and 6.7% for male and 40% for female at the 7th day. In the experimental group, the rate of UTI was 6.7% for male and 0.0% for female at the 3rd day and 13.3% for male and 20% for female at the 7th day of experiment. There was a no significant difference between male and female. 3. By comparing the rate of UTI to the length of time the urinary catheter was in place, the longer the catheter was in place the more significant was the rate of UTI. 4. Microorganisms isolated in the control group were bacteria for 7 cases and fungus for 3 cases but in the experimental group, only 2 cases of bacteria were isolated.

  • Purpose: The purpose of this study was to suggest new direction for domiciliary care for elders provided by public institutions in rural areas. Method: The participants in the study were elders using one of 11 public health care institutions, of which 8 operated day care services exclusively, and 3 operated both day care and short-term respite care services. A survey was conducted using a structured questionnaire that included items on general characteristics of the service users, conditions of the services, personnel, financial status, facilities, and perception of the tasks of the staff. Result: The service content of the day care centers included Western and Chinese medical service, physical services, activities of ADL, nursing care services, meal services and transportation services. Domiciliary care centers provided a wide variety of health and social welfare service for elders. Personnel consisted of 3 to 8 staff for day care centers and 7 to 10 for domiciliary care centers. Both types of centers rely on financial support from local government for operation. The perception of the staff was the need for operation of these centers by public health facilities such as public health centers and sub-centers. Conclusion: The result suggest a need to activate the function of public institutions to provide domiciliary care for elders. For this new change, the role as a social support system must be developed.

  • The home environments, daily activities, caregiving behaviors of mothers and teachers, infant's verbal behaviors, attachment to mothers and teachers of day care infants were compared to those of home-reared infants. 30 two-year-old home-reared infants and their mothers were observed at home by the time sampling method and 31 two-year-old day care infants and their mothers and teachers were observed at home or at the center. The HOME scale, the behavioral checking lists for caregiving behaviors and for the infants' verbal behaviors, and the Attachment Q-set were used for the home and center observations. It was found that the characteristics of early environments of day care infants were different from those of home-reared infants, but that infants' attachment to the mothers were not different. Data were analyzed with descriptive analysis, Pearson's correlation analysis, and Stepwise multiple linear regression analysis using SPSS/WIN 10.0 program. The results obtained are as follows; 1. The mean score of the general need of rehabilitation day care program of stroke survivor's family was 3.10(range 1-4). The highest need among the service categories of the rehabilitation day card program was self-care and restorative activities category(3.30), and health services referral category, recreation category, psychosocial activities category in order. The needs of each category are as follows. The stepwise multiple linear regression analysis revealed following results. For the need for rehabilitation day care program service, 22.6% of the variance was initially explained by level of family's knowledge about caring method for stroke survivors, 8.8% was the level of subjective burden and 5.4% was relationship with stroke survivors. In conclusion, above characteristics should be considered to develop stroke survivors' rehabilitation day care program.

  • As the institutions of the Day Care services in Korea have merely focused on developing the issues for caring and education of children, however, other principles in the Day Care services have not been properly delivered. So, it is necessary for the institutions delivering Day Care services to approach inclusively above four perspectives to the broad sense of the child welfare because the institutions for Day care services are dealing with the groups of the parents, who are in difficulty to take care of their children, and the needs of the parents in terms of caring for their children. The study focuses to research the groups of children, which have various and mixed needs and the problems. On the other hand, the socio-economic attributes, the needs and the problems of the groups of the parents are looked for in the research. As the result of the research, the needs for Day Care services are differentiated from the different socio-economic backgrounds of the parents group. The data indicates that the low class families demands more needs than those of the middle class in the research of the categories. Analyzing from the result, the categories of caring, health care, nutritional supplements, extracurricular activities, emotional supports, providing information and referrals and community social services are hard to be defined as the necessary services based on the ages of the children. It is better to be defined as the necessary services demanding from the social needs of the parent group. Thus, the target groups such as the working couple and the low income families should be primarily considered as the beneficiary groups receiving the comprehensive day care services.

  • Background: Socioeconomic status (SES) is closely associated with health outcomes, including mortality in critically ill patients admitted to intensive care unit (ICU). However, research regarding this issue is lacking, especially in countries where the National Health Insurance System is mainly responsible for health care. This study aimed to investigate how the SES of ICU patients in South Korea is associated with mortality. Methods: This was a retrospective observational study of adult patients aged ${\geq}20$ years admitted to ICU. Associations between SES-related factors recorded at the time of ICU admission and 30-day and 1-year mortalities were analyzed using univariable and multivariable Cox regression analyses. Results: A total of 6,008 patients were included. Of these, 394 (6.6%) died within 30 days of ICU admission, and 1,125 (18.7%) died within 1 year. Multivariable Cox regression analysis found no significant associations between 30-day mortality after ICU admission and SES factors (P>0.05). However, occupation was significantly associated with 1-year mortality after ICU admission. Conclusions: Our study shows that 30-day mortality after ICU admission is not associated with SES in the National Health Insurance coverage setting. However, occupation was associated with 1-year mortality after ICU admission.
