Effects of Obesity on Presentation of Breast Cancer, Lymph Node Metastasis and Patient Survival: A Retrospective Review

  • Background: As data on the relation between obesity and lymph node ratio are missing in the literature, we here aimed to assess the impact of obesity on this parameter and other clinicopathological features of breast cancer cases and patient survival. Materials and Methods: Medical data of 646 patients, all referred to two centers in Tehran, Iran, were reviewed. Factors that showed significant association on univariate analysis were entered in a regression model. Kaplan-Meier and Cox-regression were employed for survival analysis. Results: Obesity was correlated with the expression of estrogen and progesterone receptor (p=0.004 and p=0.039, respectively), metastasis to axillary lymph nodes (p=0.017), higher lymph node rate (p<0.001) and larger tumor size (p<0.001). The effect of obesity was stronger in premenopausal women. There was no association between obesity and expression of human epidermal growth factor receptor. Three factors showed independent association with BMI on multivariate analysis; tumor size, estrogen receptor and lymph node ratio. Obesity was predictive of shorter disease-free survival with a hazard ratio of 3.324 (95%CI: 1.225-9.017) after controlling for the above-mentioned variables. Conclusions: The findings of this study support the idea that obese women experience more advanced disease with higher axillary lymph node ratio, and therefore higher stage at the time of diagnosis. Furthermore, obesity was associated with poorer survival independent of lymph node rate.

Analysis of the Chest Expansion and Pulmonary Function in the 20s men Obesity according to Position Change (20대 남성 비만인의 자세에 따른 가슴우리 확장과 폐기능 특성분석)

  • Purpose : The purpose of this study were to determine whether changes of position might effect the chest expansion and pulmonary function of the 20s men obesity. Methods : Thirty subjects with obesity(M:30, % fat>25.0) and thirty normal subjects(M:30, % fat<24.9) were participated in experiment. Subjects were assessed according to position changes(supine position, $45^{\circ}$ lean sitting position, $90^{\circ}$ sitting position) using chest length(chest length for resting, chest expansion) and pulmonary function (Tidal volume, Inspiratory capacity, Vital capacity, Inspiratory reserve volume, Expiratory reserve volume) by the CardioTouch 3000S(BIONET, USA). Repeated measure ANOVA was used to compare each region data of chest length and pulmonary function according to changes of position with obesity and normal subjects. Results : These findings suggest that the obesity can be appear to low chest expansion and pulmonary function than normal subjects on position method. In comparison of three experimental position, supine position was more low. Conclusion : This study showed position of the obesity appear low chest expansion and function of pulmonary volume than normal subjects, and thus it indicates that the pulmonary function of the obesity will be suggest objective respiratory data through the exercise program.

Evaluation of Chronic Disease and Nutritional Intake by Obesity of Korean Elderly - Data from Korea National Health and Nutrition Examination Survey 2016~2018 - (한국 노인의 비만도에 따른 만성질환 및 영양섭취 실태조사 - 국민건강영양조사 2016~2018 자료를 이용하여 -)

  • The purpose of this study was to investigate the current status of chronic diseases according to obesity in the elderly older than age 65 using data from the 7th Korea National Health and Nutrition Examination Survey (2016~2018). The subjects of the survey were 3,245 elderly older than age 65 who participated in the health survey and nutrition survey, 45.8% of the subjects were males and 54.2% of the subjects were female. 37.5% of all the elderly were obese, and the females (42.4%) were more obese than the males (31.8%) (p<0.001). Diabetes, hypertension, and hypertriglyceridemia had a similar prevalence of 24.6%, 63.0%, and 12.7% in males and females, respectively. Hypercholesterolemia was higher in the elderly females (44.2%) than in the elderly males (24.9%) (p<0.001). Through the logistic regression analysis, it was found that the prevalence of chronic diseases was higher in obesity than normal in elderly males and females (p<0.001). The total food intake increased from 'under weight' to 'obesity' in males (p<0.01) and females (p<0.001). In the case of the elderly males, the higher the degree of obesity, the higher the energy intake (p<0.05), and 'pre-obesity' consumed the most energy in elderly females. As a result of this study, the higher the obesity rate of the elderly, the higher the prevalence of chronic diseases.

The Study on Relation of Obesity and Low Back Pain Based on Body Composition using Segmental Bioelectrical Impedance Analysis and Radiological Parameter (체성분 분석 변수와 X-선 소견을 근거로 한 비만과 요통의 관계 연구)

  • Objectives : This study was performed in order to investigate the relation of body composition analysis and radiological parameter(lumbosacral angle, lumbar lordortic angle, lumbar gravity line). Methods : This study was carried out with the data from comprehensive medical testing. 75 subject aged 20-59 performed the segmental bioelectrical impedance analysis, questionnaire. And lumbosacral angle, lumbar lordortic angle and lumbar gravity line were measured in the standing position x-ray. Then we analyzed the data. Results : Low back pain(LBP) prevalence in high obesity index(Body Mass Index(BMI), Percentage of Body Fat(PBF), Waist Hip Ratio(WHR)) group was higher than LBP prevalence in normal obesity index group(p<0.01). In LBP group, lumbosacral angle, lumbar lordortic angle were significantly lager than Non-LBP group(p<0.001). And 75% of LBP group indicated abnormal lumbar gravity line ratio(0.67 < Normal lumbar gravity line ratio <1.00). When it comes to analyze relation between obesity index and radiological parameter, no-significant change was seen. Conclusions : This study carried as following research after the study on relation of obesity, LBP and trunk muscle strength. Results from this investigation showed positive correlation between obesity and LBP prevalence. But obesity index didn't indicate significant correlation with structural changes of lumbar vertebrae. When considering prior research, trunk muscle strength changes were more related to LBP prevelence in obese people. This results are expected to explain causes of LBP in obese group.

A Retrospective Study on the Status of Obesity and Eating and Weight Control Behaviors of Elementary School Children in Inchon (인천지역 초등학생의 후향적 연구방법에 의한 비만이행추이 및 식생활습관과 체중조절행태)

  • Two hundred sixty six children were retrospectively surveyed from first to fifth grade to find out the status of obesity during four years. The obesity index was gained by height and weight data from student's health file and the data about eating behaviors and weight control behaviors, etc. were collected by questionnaires. Children assigned to one of the following groups : underweight, normal, overweight and obese. The results were as follows ; 1. Obese children were 18.6%(n=27) of boys and 10.8%(n=13) of girls at fifth grade. 2. Twenty eight children among forty obese children at fifth grade were also overweight or obese at first grade, and the other twelve was normal. 3. Breakfast skipping rate was significantly high among underweight and obese group of boys(p<0.01).The most children(87.6%) had prejudice for special foods. 4. The rate of agreement between obesity index and self recognition of body image was 60.7% in boys and 63.6% in girls. 5. Forty two point five percent of children have ever tried weight control as exercise(35.7%), reducing sugars and snacks(27%), fasting(23.8%), avoiding fatty foods(12.7%), taking diet food or drug(0.8%). In conclusion, the prevalence of children obesity is gradually increasing and children tried weight control without right guidances. In order to prevent progress into adulthood obesity, nutrition education should be followed with school foodservice including the right perception for obesity, proper weight control and eating behavior modification.

Fatty Acids Intake and Its Association with Abdominal Obesity and Hyperglycemia in Korean Adults: Korea National Health and Nutrition Survey, 1998~2007 (한국 성인의 지방산 섭취와 복부비만 및 고혈당과의 관련성 -1998~2007 국민건강영양조사 자료에 근거하여-)

  • Abdominal obesity is an important factor of metabolic syndrome and is known as an independent risk factor of cardiovascular disease. The purpose of this study was to examine the fatty acid intake pattern as well as the association between fatty acid intake and abdominal obesity and hyperglycemia in Korean adults. This study was based on data from the Korea National Health and Nutrition Examination Surveys (KNHANES) I, II, III, and IV. After excluding subjects who had missing data for dietary intakes, anthropometric measurments, and laboratory tests, a total of 12,320 subjects aged 20~64y (5,266 men and 7,054 women) were included in the study. Trends in individual consumption of dietary fatty acids by year were analyzed by One-way ANOVA test, and the association between fatty acid intake and abdominal obesity and hyperglycemia was analyzed by logistic regression analysis after adjusting for age, energy intake, marital status, job, education level, alcohol intake, and smoking status. In men, intakes of TFA, SFA, MUFA, FUFA, and n-6 PUFA significantly increased with year. On the other hand, intakes of TFA, SFA and FUFA significantly increased with year in women. Logistic regression analysis showed that the risk of abdominal obesity was significantly associated with intakes of PUFA, LNA and n-3 PUFA in women. However, there was no association with any fatty acid intakes in men. The risk of hyperglycemia was significantly and positively associated with intakes of TFA, SFA, PUFA, and LNA in men, but, there was no significant association with any fatty acid intakes in women. These results show that abdominal obesity and hyperglycemia are significantly associated with individual intake of fatty acids, suggesting that fatty acid type may be associated with risk of abdominal obesity and hyperglycemia. Futher, the macronutrients of measl must be properly balanced.

Prevalences of Hypertension and Obesity of Children in Kangnung (강릉지역 국민학생의 고혈압 및 비만의 이환율에 관한 연구)

  • Prevalences of hypertension and obesity of primary school children in Kangnung were investigated, and the correlations between blood perssure and measured anthropometry were also studied to identify the factors that influence on blood pressure. 1,164 children (602 boys and 562 girls) from five elementary schools were chosen for this study. Anthropometric measurements were taken for blood pressure, body weight, height, triceps skinfold, and circumferences of chest and arm. Data of family history of hypertension were obtained by means of prepared questionnaires. Prevalences of hypertension were 3.3% in boys and 3.1% in girls. The group with elevated blood pressure had relatively a greater frequency of familial hypertension and significantly higher mean body weight and obesity index. The prevalence of obesity was defined as percentage of children whose rates of current weight to standard weight for height and sex in Korean children were more than 120%. Prevalence of obesity were 16.3% in boys, 10.3% in girls. There findings clearly revealed the influences of above-average weight and familial hypertension on blood pressure. These results suggest that children with familial hypertension should be avoid excess adiposity and be measured blood pressure regularly.

Factors associated with Obesity among Korean Adolescents based on the Seventh Korea National Health and Nutrition Examination Survey (2016)

  • Purpose: This study was conducted to examine factors associated with obesity among Korean adolescents. Methods: The participants were 436 Korean adolescents aged 12~18 years who took part in the seventh Korea National Health and Nutrition Examination Survey (2016). The data were analyzed by descriptive statistics, the $x^2$ test, one-way analysis of variance, and multinomial logistic regression using complex sample analysis. Results: Among the participants, 9.7% were classified as being obese, and 11.9% as being overweight. Several health parameters, including systolic blood pressure, diastolic blood pressure, waist circumference, and triglyceride, fasting blood sugar, and alanine aminotransferase levels, varied according to obesity. Distorted perceptions of body shape showed a significant negative association with overweight, and carbohydrate intake was negatively associated with obesity. Conclusion: These findings indicate that obesity in Korean adolescents was associated with lifestyle and health parameters. Therefore, intervention programs for obese or overweight adolescents should focus on preventing them from developing health problems.

Current Barriers of Obesity Management of Children Using Community Child Care Centers and Potential Possibility of Utilizing Mobile Phones: A Qualitative Study for Children and Caregivers (지역아동센터 이용 어린이의 비만관리의 한계점과 모바일폰의 잠재적인 활용 가능성: 어린이와 보호자 대상의 질적 연구)

  • Objectives: This study was performed to identify the current barriers of obesity management for children using Community Child Care Centers and their caregivers (parents and teachers working in the Centers). Further, this study explored the possibility of utilizing a mobile phone application for tailored obesity prevention and management programs to overcome the current difficulties associated with children's obesity management. Methods: The qualitative data were collected through in-depth interviews with 20 obese and overweight children or children who wanted to participate in this study using Community Child Care Centers, 12 teachers working at the Centers, and a focus group interview with five parents of children using the Centers. Data were analyzed with a thematic approach categorizing themes and sub-themes based on the transcripts. Results: The current barriers of obesity management of obese and overweight children using Community Child Care Centers were lack of self-directed motivation regarding obesity management (chronic obesity-induced lifestyles and reduced self-confidence due to stigma) and lack of support from households and Community Child Care Centers (latchkey child, inconsistency in dietary guidance between the Center and household, repetitive pressure to eat, and absence of regular nutrition education). Mobile phone applications may have potential to overcome the current barriers by providing handy and interesting obesity management based on visual media (real-time tracking of lifestyles using behavior records and social support using gamification), environmental support (supplementation of parental care and network-based education between the Community Child Care Center and household), and individualized intervention (encouragement of tailored and gradual changes in eating habits and tailored goal setting). It is predicted that the real-time mobile phone program will provide information for improving nutritional knowledge and behavioral skills as well as lead to sustainable children's coping strategies regarding obesity management. In addition, it is expected that environmental factors may be improved by network-based education between the Community Child Care Centers and households using the characteristics of mobile phones, which are free from space and time constraints. Conclusions: The tailored education program for children using Community Child Care Centers based on mobile phones may prevent and reduce childhood obesity by overcoming the current barriers of obesity management for children, providing environmental and individualized support to promote healthy lifestyles and quality of life in the future.

A Study on the Relationship between School-age Obesity and Type of Feeding in Infant Period (영아시기 수유 방식과 초기 학동기 비만과의 관련성에 관한 연구)

  • Purpose : The aim of this study was to evaluate whether breast milk feeding at infancy has the effect of a programming agent preventing school-age obesity, or whether formula milk feeding is the cause of elementary school-age obesity. Methods : We randomly selected 4 elementary school in Masan and Changwon city. We calculated the BMI and obesity degree from height and weight data on the school record of 1,275 children of first and second grade. The parents of 1,275 children were asked to a questionnaire about their birth history and feeding modality during infancy. Based on these data, we categorized them into 4- different groups : breast fed for less than 2 months; breast fed for 2-6 months; breast fed more than 6 months; mixed fed. We compared the mean BMI, obesity degree and prevalence rates of obesity of each groups. Results : There was no statistical difference on the prevalence rates of overweight and obesity, which was defined by BMI, according to the duration of breast feeding. The same was true for mild obesity and moderate obesity, which was defined by obesity degree. The mean BMIs and the mean obesity degree were not different among different feeding types. Conclusion : There seems to be no protective effect of breast feeding during infancy on reducing the prevalence rates of overweight and obesity. However, even wider and larger studies considering confounding factor might be needed to reach a definite conclusion in the future.
