The Smoking Habits among the OPD Patients and The Success Rates of the Physician's Cessation Order (내과 외래환자에 있어서 흡연 양상과 의사의 금연권고의 성공률)

  • Background: To evaluate the effect of doctor's cessation order Methods: From January 1989 to December 1990, Total 1981 patients (male 922, female 1059), who visited OPD of Daedong hospital were selected to investigate the cigarette smoking habits and the success rates of the physician's cessation orders. Results: 1) Among male patients, 64.43% and 27.00% revealed as smoke and non-smoker, respectively and and 8.57% as ex-smoker. In the 20 years old or less age group smoker were 36.11%. 2) Among female patients, the rates of smoker were only 2.83%, ex-smoker were 0.38%. No female patient smoked under 20 years old. 3) As compared with the number of daily smoked cigarettes among the male patients, patient who smoked less than 10 cigarettes were in 2.69%, 11-20 cigarettes in 39.23%, 21-40 cigarettes in 46.30%, more than 40 cigarettes in 11.78%, under 20 years old age gorup the number of smoked cigarettes were 11.54%, 61.54%, 23.08%, 3.84% respectively. Among female patients, smoker were only 37 patients and the number of daily used cigarette were 2.7%, 67.57%, 24.32%, 5.41% respectively. 4) As compared with systemic disease and the smoking habits, female excluded from statistics because of too small number of smoker. Among male patients ex-smoker associated with respiratory disease were 15.21% which was much higher than other disease group (4.35%-8.11%), and among cardiovascular diseae patients, smoker were 81.08% & among cardiovascular disease patients, smoker were 81.08% & among gastrointestinal disease patient 68.93% and among respiratory disease patient 60.84%. In respiratory disease patients group 16.25% smoked more than 40 cigarettes dialy and 13.01% in gastrointestinal disease patients group. 5) Among patients who treated more than 3 months via OPD the success rates of the physcian's cessation order were 62.03% of male patients (we excluded female patients) and there is no gross difference in each age group, but it was highest as 75% in 41-60 years old age group. As compared with difference of systeic disease, the success rate were highest in respiratory disease patients as 78.13% and lowest in gastrointestinal disease patients as 49.94% Conclusion: The smoking rates among the out patients including male and female of internal medical department of General Hospital were same as the general population. Although the sample size was small, on account of the success rates of physician's cessation orders were more than half, we think the cessation recommendation by physician's order is very effective. And we think the cessation recommendation are more effective. And we think the cessation recommendation are more effective than the ex-smoking education in the excluded patients due to fail to follow up more than 3 months.

A Comparison between Minimally Invasive Percutaneous Plate Osteosynthesis and Plate Fixation in the Treatment of Clavicle Midshaft Fracture (쇄골 간부 골절의 최소 침습적 경피적 금속판 고정술과 관혈적 수술 비교)

  • Purpose: To retrospectively compare minimally invasive percutaneous plate osteosynthesis (MIPPO) with open plate fixation for the treatment of clavicle midshaft fracture. Materials and Methods: Between November 2011 to May 2014, 40 cases that were followed for more than 1 year-among all cases of MIPPO and open plate fixation-were analyzed. The study population was divided into two groups: group A included 20 cases of MIPPO and group B included 20 open plate fixation cases. The comparative analysis between the two groups was based on the operative time, bone union, functional evaluation (American Shoulder and Elbow Society score), incision length, pain relief (visual analogue scale, VAS), and complication. Results: The bone union was successful for all the cases, and the functional evaluation scores of the shoulder joint were satisfactory for both groups (p>0.05). The operative time was 47.5 minutes and 58.7 minutes for group A and B, respectively (p<0.05). The incision length for group A was 6.2 cm and that for group B was 10.7 cm with statistical significance (p<0.05). Pain relief (VAS) after surgery for group A showed a quick recuperation in the early stages. For complications, there were 2 cases of dysesthesia and 1 case of malunion due to metal fixation failure in group A. There were 6 cases of dysesthesia and 2 cases of cosmetic problem due to hypertrophic scar in group B. Conclusion: Surgical results of clavicle shaft fracture were satisfactory for both groups, but group A had advantages of shorter operative time, minimal incision length, and better pain relief in the early stages compared with group B. However, we need further evaluations with long-term follow-up results and complications, such as malunion and exposure on radiation.

Clinical Nurse's Organizational Commitment and Influencing Factors (부산시내 일개 대학병원 간호사의 조직몰입에 영향을 미치는 요인)

  • This study was designed to identify organizational commitment and influencing factors in clinical nurses. A cross-sectional design was conducted with a sample of 322 clinical nurses randomly selected from university hospital in Korea. The tools used for this study were scales measuring organizational commitment(15 items), job satisfaction(41 items). The data were analyzed using SPSS 12.0 employing pearson correlation coefficients and multiple regression analysis. The mean score for organizational commitment in clinical nurses was 3.77 points. Factors influencing organizational commitment in clinical nurses were identified as job satisfaction(${\beta}$=.388), belief presence level(${\beta}$=.206). These factors explained 13.4% of organizational commitment reported by clinical nurses. The results indicate which factors are major factors influencing organizational commitment in clinical nurses. Therefore, these factors may serve as predictors of organizational commitment in clinical nurses.

  • This study presents the categorical structure for ther epresentation of a 3D human body position system in the WD stage after NP approval by the International Organization for Standardization (ISO), analyzes the needs of electronic medical record users and establishes future implementation plans for expanding its use in Korea. Research was conducted on the needs of doctors, nurses, health and medical information managers, and radiology departments, which are the main stakeholders of electronic medical records. The overall requirements for electronic medical records were derived from the results, and the requirements for each stage of use of electronic medical records were analyzed. Based on the results of the study, the study proposes plans to expand the use of the categorical structure for the representation of the 3D human body position system, and also aims to establish a standard system for health and medical terminology in Korea and contribute to the development of health and medical information standards through international standardization.

Treatment of Giant Cell Tumor of Distal Radius with Wide Resection and Proximal Fibular Graft (광범위한 절제술 후 근위 비골 이식을 이용한 원위 요골 거대세포종의 치료)

  • Purpose : Treatment of giant cell tumor of distal radius can be treated in several ways according to the agressiveness of the tumor. We treated 3 cases of widely involved giant cell tumor of distal radius with wide resection and proximal fibular graft and report the results with review of literatures. Material and Method : We have treated 3 cases of giant cell tumor of the distal radius since last 1990. Among 3 cases, two cases were grade III radiologically and treated by wide resection of distal radius and vascularized proximal fibular graft, and one case, grade II radiologically, treated by distal radial resection and non-vascularized proximal fibular graft. We followed up clinical results of above three cases 9 years, 12 years and 2 years. Result : In all three cases, tranplanted fibula graft showed solid union but grade III tumors recurred at 4 year and 6 year postoperatively. One of the case which recurred 4 year later was treated with secondary wide resection and wrist fusion with autogenous iliac bone graft, and didn't show any recurrent finding for these 5 years after re-operation. And another grade III, which recurred at 6th post-operative year, is under follow-up for 6 years after recur without 2nd operation. Grade II case didn't show any recurrent findings on 2 year follow-up. Conclusion : Grade III cases recurred at 4 year and 6 year follow-up. The cause of recurrence was thought to be invasion of remaining tumor cell in the soft tissue. To prevent recurrence, complete resection of primary tumor was necessary.

The Effects of Convergence Simulation Education Applying Problem Solving Process, Communication and Learning Satisfaction of Nursing (융합시뮬레이션교육을 적용한 간호학생의 문제해결과정, 의사소통, 학습만족도에 미치는 효과)

  • The purpose of this study was to develop and apply a to identify the effects of problem solving process and communication for nursing students. The subjects consisted of one group of 71 fourth year nursing students of D Colleage To test the application effect, a one group pre-post test design was applied. This study showed significantly higher problem solving process(t=-12.6, p<.001), and communication(t=-9.91, p<.001). Learning satisfaction was positively affected. Simulation program is one of the effective teaching and learning methods. It is recommended to develpo a scenario for simulation education applied various cases which are undergoing clinical testing, so as to apply it to the nursing training education.

  • The catch canal for a natural drainage, the excavation of tunnel for a stable irrigation, the artifical channelization of yazoo stream, many kinds of the pumping and drainage systems are found around Daedong-myeon, Gimhae-si. It could be confirmed that the deveolpment, maintenance and management of those facilities in the floodplain are mainly controlled by the geomorphological conditions. In case of Unha-chon, especially, the water can always be supplied only by openning the gate of irrigation tunnel and the Woldang pumping station as the largest pumping station in this area can supply water to Unha-chon area directly from the main river, Nakdong-gang. Because the Gamnae-chon which was an upper reach of the Unha-chon and is now connected to the Nakdong-gang through the Deoksan catch canal, the damage of flood and the burden of drainage are mitigated at the lower area of the Unha-chon.

Efficacy of Unilateral Biportal Endoscopic Decompression in Lumbar Foraminal Stenosis (요추 추간공 협착증에서 일측성 양방향 내시경적 측부 추간공 감압술의 효과)

  • Purpose: This paper reports the short-term clinical and radiological results of unilateral biportal endoscopic decompression (UBE) to prove its efficacy. Materials and Methods: Twenty patients who received unilateral biportal endoscopic far-lateral decompression (UBEFLD) were analyzed statistically using the visual analogue scale (VAS), modified Macnab criteria and Oswestry Disability Index (ODI) clinically. Radiologically, their intervertebral angle (IVA), percentage slip, disc height index (DHI) and foraminal height index (FHI) were analyzed pre- and postoperatively. Results: The VAS scores were 6.20 preoperatively, which improved to 2.05, 1.75 and 1.45 at postoperative one month, three months and one year, respectively (p<0.001). The modified macnab criteria in both the good or excellent category was 70.0%, 80.0% and 85.0% at postoperative one month, three months and one year, respectively (p=0.034). The ODI improved from 59.8% preoperatively to 35.8%, 33.2%, and 17.1% at postoperative one month, three months, and one year, respectively (p<0.001). The IVA was increased 0.40±0.88 after a surgery (p=0.057). Percentage slip was increased 0.19% after surgery (p=0.134). The DHI changed from 0.49 preoperatively to 0.62 postoperatively (p=0.359), and the FHI changed from 0.71 preoperatively to 0.79 postoperatively (p<0.001). Conclusion: UBEFLD displayed satisfactory results. Such a result highlights the potential of UBEFLD as an excellent alternative to spinal fusion or microscopic surgery.

A Study on the Evaluation of Potential Hydro-electric Power in North Korea (북한의 수력발전가능량 산정 및 평가에 대한 연구)

  • This study is to analyze and evaluate water resource development potential in North Korea. The study was conducted to analyze selected potential hydropower as an indicator to evaluate water resource development potential. Potential hydropower means theoretical value about the potential capacity of river. It is used to evaluate the amount of development through the hydropower generation. For calculating potential hydropower, monthly average and annual average of rainfall for each river basin were calculated by using the data of 27 rainfall stations in North Korea. As a result of the calculation of theoretical potential hydropower by rainfall in the seven major basins in North Korea, the Aprok River basin was analyzed to be the largest with $7,562.2{\times}10^3kW$. The efficiency and utilization rate of theoretical potential hydraulic power in South Korea and North Korea was 42.3% and 36.2%, respectively. The Daedong River basin's potential hydropower utilization rate is 12.3%, which is the lowest in North Korea. In the case of Daedong River basin, more than 40% of the total population is inhabited, so demand for water and electricity is expected to be the largest. Therefore, the Daedong River basin is considered as a priority area for water resource development. The results of this study are expected to be used as basic data for future water resource development projects and research activities in North Korea.
