Traumatic Aneurysm of Descending Thoracic Aorta -A Case Report- (외상성 하행 흉부 대동맥류 -치험 1 례 보고-)

  • We experienced a case of traumatic aneurysm of descending thoracic aorta by an automobile accident. The patient was 23-year-old-male with a traumatic aortic aneurysm [6x12cm] on the descending thoracic aorta just distal to the origin of the left subclavian artery. Exposure was obtained through a left posterolateral thoracotomy incision in the fourth intercostal space and then partial femoro-femoral cardiopulmonary bypass was established.After aortic cross- clamping, the aneurysmal sac was opened and repaired with interposition of Dacron vascular graft and aortic cross-clamping period lasted for 100 minutes. Postoperative bleeding and vocal cord paralysis were complicated, but bleeding was controlled by reoperation and vocal cord paralysis was improved. Follow up was continued for 14months and postoperative course was uneventful.

  • We experienced one case of surgical treatment of abdominal aortic aneurysm using retroperitoneal approach. The transperitoneal route has been the standard surgical approach for elective and emergency treatment of aneurysmal or occlusive disease. Among its cited advantages for aortic reconstruction are familarity with the exposure, easy access to the infrarenal aorta and iliac vessels, possibility of simultaneous inspection of the intra-abdominal viscera, and speed of opening and closure.Despite the proved versatility of the transperitoneal approach, it is commonly associated with prolonged ileus, increased third space fluid loss, and significant pulmonary complications. The retroperitoneal approach, on the other hand, has many advantages; excellent exposure for the repair of juxtarenal or suprarenal abdominal aortic aneurysms and visceral vessel occlusive disease, fewer postoperative complications, decreased postoperative third-space fluid losses from intraoperative evaporation and ileus, and improved postoperative pulmonary function. Atherosclerosis was most common cause of abdominal aortic aneurysm. Dacron graft[18mm] was replaced successfully.Postoperative course was uneventful and he was discharged in good condition.

Surgical Treatment of Atrioventricular Septal Defect (방실중격결손증의 외과적 치료)

  • Twelve patients had undergone repair of atrioventricular septal defects. Age at operation ranged from 2.4 years to 17 years[mean, 8.25 years]. Five patients were male and seven were female. Three patients had complete atrioventricular septal defect[Rastelli type A] associated with Down`s syndrome. One of the three patient with complete atrioventricular septal defect had tetralogy of Fallot. Three patients had the intermediate form and seven patients had the partial form. The primum atrial septal defect was closed with pericardial patch. The atrioventricular valve septal commissure[mitral cleft] was closed with pledgeted sutures. Three complete atrioventricular septal defect were undergone by two-patch technique. A crescent-shaped Dacron patch was used to occlude the ventricular septal defect. One patient of partial form was sudden death 5 days postoperatively. There were no another complications after surgery. One patient underwent reoperation for opened mitral cleft 2.5 years postoperatively. New York Heart Association functional class of patients was improved postoperatively.

  • Pulmonary artery aneurysm usually carries an ominous prognosis due to the associated pulmonary hypertension. In July 1981, a patient with a huge aneurysm of main pulmonary artery secondary to pulmonary hypertension and bacterial endocarditis due to a patent ductus arteriosus was treated by resection of the aneurysm and Dacron patch graft replacement and closure of the patent ductus arteriosus. The immediate postoperative result was excellent. We now report the surgical treatment, clinical course, and one and half years follow up of the patient.

  • Since the first report by Freeman of renal artery repair for renovascular hypertension in 1954, there has been a gradual improvement in the results of renovascular reconstruction. This is case report of renovascular hypertension due to diffuse primary arteritis, which was performed aorto-bllateral renal bypass graft with using of Y -woven dacron graft. This 33 y-o male has complained intermittent headache, facial edema & malignent hypertension symptoms for 1.5 years. He had the history of Rt. B-K amputation due to unhealed wound after trauma of the Rt. great toe about 7 years ago. The abdominal aortography revealed nearly not visualized the Rt. renal artery & severe narrowing of the Lt. renal arterly. During postop. course, Blood pressure was well controlled. At postop. 3rd week, systolic pressure was down to 130-140mmHg But, diastolic pressure was remained to 100-110mmHg. At postop. 30th day, exploration was done due to intestinal obstruction signs. But severe ischemic enteritis was occured due to fibrotic obstruction of the superior mesenteric artery. The next day, he was dead. in spite of Rt. common iliac artery-sup. mesenteric artery bypass graft.

One case report of syphilitic aortic aneurysm (매독성 대동맥루: 1 수술 치험예)

  • The incidence of syphilitic aortic aneurysm was decreased now a day. The predilection site of cardiovascular syphilis is the thoracic aorta, especially ascending portion. The form of syphilitic aneurysm is characterized by saccular or fusiform. We have experienced 58 year old female complained of intermittent left chest pain for these 2 years. Saccular aneurysm of 7 cm in diameter at the descending thoracic aorta just distal to the left subclavian artery was confirmed with aortogram, and etiologic lesion was suspected as syphilis by her strong positive finding of serum VDRL test. Excision of the aneurysm was done under temporary bypass with heparinized 10mm silicone bypass cannula, and 23 mm Dacron aortic prosthetic graft in 10 cm segment was replaced, and excised aortic segment was confirmed as syphilitic aneurysm on light microscopic examination. Postoperative hospital course was uneventful, and discharged 3 weeks after operation in good normotensive condition. The patient died of CVA 3 months after discharge at home.

Clinical evaluation of atrial septal defect (심방중격결손증의 외과적 고찰)

  • For the purpose of clinical analysis of secundum atrial septal defect, the patients with atrial septal defect operated on during the period from Aug. 1959 to Aug. 1983 were analysed. The age of the patients was ranged from 3 months to 60 years and there were 132 males [45.3%] patients and 145 female patients [54.6%] in this series. The most frequent type was oval type defect in 219 [86.9%]. The PAPVC were present in 7 patients of high defect group. Clinical analysis revealed 53 patients [19.9%] were asymptomatic. The QP/QS above than 3.61 were 45 patients and less than 1.5 were 24 patients. The pulmonary artery pressure was 10 to 100 mmHg. The majority of the defects were closed in primary suture and closed with a Dacron patch in 19 patients. The postoperative complications occurred in 35 patients [13.1%] and the most frequent complication was wound problem which was present in 10 patients. The hospital mortality rate was 1.6%.

  • The patient in whom corrected transposition of the great vessels is an isolated congenital anomaly has a physiologically normal circulation. A 19 year old male was perforemd with surgical correction for congenitally corrected transposition of great arteries (SLL) associated with ventricular septal defect (type I) and severe pulmonary stenosis. After right sided ventriculotomy, ventricular septal defect was repaired with Dacron patch and to provide adequate relief of severe valvar stenosis of pulmonary artery, valvulotomy was done. On immediate postoperative period, complete heart block and lower cardiac output state were ocurred, but adequate blood pressure could be maintained under using of inotropic agent and temporary pace-maker.

Dissecting Aneurysm of Aorta: report of a case (해리성 대동맥류 1례 보고)

  • Dissecting aortic aneurysm is a disease which is characterized by hemorrhagic intramural seperation of aortic wall and extension for varlng distances proximally, distally, or both from the site of the intimal tear. Most aortas show some type of medial degeneration most commonly described as cystic medial necrosis. DeBackey classified this disease according to involved aorta and site of intimal tear to 3 basic types, such as type I, II and III. Type III is defined that dissecting process arrises in the descending thoracic aorta just distal to origin of the left subclavian artery and extends distally for a varing distance. We expirienced a case of dissecting aneurysm, type III of DeBackey's classification which dissecting process is limited to the descending thoracic aorta in the Department of Thoracic and Cardiovascular Surgery, Kyungpook National University Hospital. This patient was 40 year old woman and she had suffered from intermittent sharp back pain for 3 years .before admission. Excision of the aneurysm and Dacron graft were placed successfully under the left atrio-femoral bypass with artificial pump. The hospital course was uneventful.

흉부외상에 의한 하행흉부대동맥파열 (수술 치험 1예 보고)

  • A successful repair of transected descending thoracic aorta was performed in a 44-year-old man. The patient had once been hospitalized in a local clinic for 7 days after a steering wheel injury. Dealing with right Colle`s fracture, he was transferred to this hospital to rule out aortic injury. On admission, a chest PA film and concomitant aortogram revealed an aneurysm of the descending thoracic aorta just distal to the origin of the left subclavian artery measuring 6 cm in diameter and 8 cm in length. He underwent urgent thoracotomy and the injured part of the aorta was replaced with a woven Dacron graft utilizing a Gott`s heparinized aortic shunt. The postoperative course was very smooth except hoarseness and left phrenic nerve palsy due to a blind clamping of the proximal aorta during the operation.
