Experimental Evaluation of Proton Dose Calculations in Phantoms Simulating a Clinical Heterogeneity in Patients

  • In a treatment planning for actual patients with a complex internal structure, we often expect that proton beams, which pass through both a bolus and the heterogeneity in body, will form complex dose distributions. Therefore, the accuracy of the calculated dose distributions has to be verified for such a complex object. Then dose distributions formed by proton beams passing through both the bolus and phantoms simulating a clinical heterogeneity in patients were measured using a silicon semiconductor detector. The calculated results by the range-modulated pencil beam algorithm (RMPBA) produced large errors compared with the measured dose distributions since dose calculation using the RMPBA could not predict accurately the edge-scattering effect both in the bolus and in clinical heterogeneous phantoms. On the other hand, in spite of this troublesome heterogeneity, calculated results by the simplified Monte Carlo (SMC) method reproduced the experimental ones well. It is obvious that the dose-calculations by the SMC method will be more useful for application to the treatment planning for proton therapy.

유방촬영 시 몬테칼로 전사모사를 이용한 유방 성형 환자의 장기선량 평가 (Assessment of Organ Dose in Mammoplasty Patient by Monte Carlo Simulation during Mammography)

  • Although the screening with a mammography has been shown to be economical, simple and effective in detecting breast cancer, it is accompanied by the risk from radiation. Therefore, this study analyzed the glandular dose and organ dose according to the target-filter combination and the presence and absence of implants using Monte Carlo simulation. The results indicate that at a tube voltage of 30 kV and a tube current of 50 mAs, the dose increased in the order of Mo/Mo. Mo/Rh, Rh/Rh and W/Rh in proportion to the atomic number of the target-filter. In addition, in phantom without implant a reduction in dose was seen when compared to the phantom with implant. The organ dose was highest in the lens except for the breast on the examination side regardless of the presence or absence of the implant. These results may contribute to use basic data for the diagnostic reference level of breast plastic surgery patients.

The Effects of Nonmagnetic Bolus on Contralateral Breast Skin Dose during Tangential Breast Irradiation Therapy

  • In this study the contralateral breast skin dose was decreased. It was to apply the results to the clinical study after analysis of different radiation dose amounts to contralateral breast with nonmagnetic bolus and without nonmagnetic bolus. A Rando phantom was computed tomography (CT) simulated, five treatment plans were generated: open tangents, open field in field, wedge 15, wedge 30, and intensity-modulated radiotherapy (IMRT) plan with 50.4 Gy to cover sufficient breast tissue. Contralateral breast skin dose was measured at 8 points using a glass dosimeter. The average contralateral breast dose using nonmagnetic bolus showed better excellence in decreasing the absorbed dose in the order of $168{\pm}11.1$ cGy, $131{\pm}10.2$ cGy (29%), $112{\pm}9.7$ cGy (49%), and $102{\pm}9.5$ cGy (64%) than changing the treatment plan. This study focused on decreasing the effect of scattered dose by use of a nonmagnetic bolus on the contralateral breast during radiotherapy in breast cancer patients and an intriguingly significant decrease was observed parallel to the opposed beam.

  • In head and neck region, the critical organ and tissue doses were determined, and the risks were estimated from lateral, posteroanterial and basilar cephalometric radiography. For each cephalometric radiography, 31 TLDs were placed in selected sites(18 internal and 13 external sites) in a tissue-equivalent phantom and exposed, then read-out in the TLD reader. The results were as follows: 1. From lateral cephalometric radiography, the highest effective dose recorded was that delivered to the salivary gland(3.6pSv) and the next highest dose was that received by the bone marrow(3pSv). 2. From posteroanterial cephalometric radiography, the highest effective dose recorded was that delivered to the salivary gland(2pSv) and the next highest dose was that received by the bone marrow(1.8pSv). 3. From basilar cephalometric radiography, the highest effective dose recorded was that delivered to the thyroid gland(31A p Sv) and the next highest dose was that received by the salivary gland(13.3 p Sv). 4. The probabilities of stochastic effect from lateral, posteroanterial and basilar cephalometric radiography were $0.72{\times}10^{-6}$, $0.49{\times}10^{-6}$ and $3.51{\times}10^{-6}$, respectively

The Effectiveness of Volumetric Modulated arc Radiotherapy to Treat Patients with Metastatic Spinal Tumors

  • Among the possible stereotactic body radiation therapy (SBRT) modalities used to treat patients with metastatic spinal tumors, this study compared Cyberknife, tomotherapy, and volumetric modulated arc radiotherapy (VMAT). We established treatment plans for each of them modality and quantitatively analyzed the dose evaluation factors of the dose-volume histogram (DVH) for all spinal bones, focusing on the tumor and spinal cord, in order to examine the usefulness of VMAT. For the treatment planning dose, the mean dose ($D_{max}$) and $D_{5%}$ showed statistical differences in the target dose, but no difference was shown in the spinal cord dose. For the DVH indices, tomotherapy showed the best performance was the best in terms of uniformity index, while VMAT showed better performance was better than the other two modalities in terms of the conformity index and the dose gradient index. VMAT had a much shorter treatment time than Cyberknife and tomotherapy. These findings suggest that VMAT FFF is the most effective therapy for SBRT of patients with metastatic spinal tumors for whom a high dose of radiation is prescribed.

유방암 근접치료 시 나노입자의 밀도에 따른 흡수선량 평가 (Evaluation of Absorbed Dose According to Nanoparticle Density During the Breast Cancer Brachytherapy)

  • The purpose of this study was to evaluate the efficacy of brachytherapy of breast cancer by dose assessment which a steady increased in Korea women. The dose assessment was performed using the MCNPX program, a MonteCarlo simulation technique. The sources used for brachytherapy was 192Ir. And nanoparticle which used for dose enhancement was gold. The density of nanoparticle was 7, 18 and 30 mg. Evaluation of absorbed dose according to distance is measured at a distance of 30, 50, 100 and 200 cm from the patient. As a result, The breast absorbed dose results increased in proportion to the density of nanoparticle. And the surrounding organs were not significantly different according to the density. But, in some organs, the absorbed dose decreased as the density of nanoparticles increased. Absorbed dose according to the distance was in inverse proportion to distance.

Recent Trend of Occupational Exposure to Ionizing Radiation in Korea, 2015-2019

  • Background: Radiation exposure can occur as a result of occupational activities utilizing sources of radiation. The average level of occupational exposure is generally similar to the global average, but some workers receive more than this. In this study, the occupational exposure data for workers in Korea to check the recent trend of radiation exposure. Materials and Methods: The data collection and analysis are carried out by two separate periods based on the United Nations Scientific Committee on the Effects of Atomic Radiation (UNSCEAR) survey. One is the year 2003 to 2014 for a recent survey, and the other is 2015 to 2019. All available data were collected by annual reports from radiation dose registry organizations. Results and Discussion: The annual dose over the record level to the total workers did not change much compared with the total increasing number of workers in this period. The dose to the nuclear fuel cycle field has a tendency to decrease. It resulted from the efforts of radiation dose reduction with high technology introduced to this area. Also, it is important result that the radiation dose to the workers in radiography is remarkably reduced. Conclusion: The number of radiation workers and average doses were analyzed for occupational categories in Korea. It still needs cooperative efforts between the dose registry organizations for the efficient dose management of Korean radiation workers.

A GPU-based point kernel gamma dose rate computing code for virtual simulation in radiation-controlled area

  • Virtual reality technology has been widely used in the field of nuclear and radiation safety, dose rate computing in virtual environment is essential for optimizing radiation protection and planning the work in radioactive-controlled area. Because the CPU-based gamma dose rate computing takes up a large amount of time and computing power for voxelization of volumetric radioactive source, it is inefficient and limited in its applied scope. This study is to develop an efficient gamma dose rate computing code and apply into fast virtual simulation. To improve the computing efficiency of the point kernel algorithm in the reference (Li et al., 2020), we design a GPU-based computing framework for taking full advantage of computing power of virtual engine, propose a novel voxelization algorithm of volumetric radioactive source. According to the framework, we develop the GPPK(GPU-based point kernel gamma dose rate computing) code using GPU programming, to realize the fast dose rate computing in virtual world. The test results show that the GPPK code is play and plug for different scenarios of virtual simulation, has a better performance than CPU-based gamma dose rate computing code, especially on the voxelization of three-dimensional (3D) model. The accuracy of dose rates from the proposed method is in the acceptable range.

A Review of Organ Dose Calculation Methods and Tools for Patients Undergoing Diagnostic Nuclear Medicine Procedures

  • Exponential growth has been observed in nuclear medicine procedures worldwide in the past decades. The considerable increase is attributed to the advance of positron emission tomography and single photon emission computed tomography, as well as the introduction of new radiopharmaceuticals. Although nuclear medicine procedures provide undisputable diagnostic and therapeutic benefits to patients, the substantial increase in radiation exposure to nuclear medicine patients raises concerns about potential adverse health effects and calls for the urgent need to monitor exposure levels. In the current article, model-based internal dosimetry methods were reviewed, focusing on Medical Internal Radiation Dose (MIRD) formalism, biokinetic data, human anatomy models (stylized, voxel, and hybrid computational human phantoms), and energy spectrum data of radionuclides. Key results from many articles on nuclear medicine dosimetry and comparisons of dosimetry quantities based on different types of human anatomy models were summarized. Key characteristics of seven model-based dose calculation tools were tabulated and discussed, including dose quantities, computational human phantoms used for dose calculations, decay data for radionuclides, biokinetic data, and user interface. Lastly, future research needs in nuclear medicine dosimetry were discussed. Model-based internal dosimetry methods were reviewed focusing on MIRD formalism, biokinetic data, human anatomy models, and energy spectrum data of radionuclides. Future research should focus on updating biokinetic data, revising energy transfer quantities for alimentary and gastrointestinal tracts, accounting for body size in nuclear medicine dosimetry, and recalculating dose coefficients based on the latest biokinetic and energy transfer data.

투시 조영 검사 시 확대율에 따른 피폭선량에 관한 고찰 (Study on Radiation dose in according to Magnification's rate in fluoroscopy)

  • Purpose : The purpose of this study is the magnification rates depending on the area of patient dose (DAP) and glass dosimeter see the change of the dose according to the dose characteristics of low-magnification aims to raise standards. Materials and Method : Direct DR equipment Sonialvision DAR-8000f, Shimadzu was used, the patient entrance dose measurements to the surface of the Rando Phantom of the neck and the abdomen was placed on the Xi unfors. glass dosimeter for measuring organ doses at the same time the Rando Phantom of the major organs in place by inserting a 9 ", 12", 15 ", 17" and 30 seconds for each magnification were measured according in fluoroscopy. DAP meter area of the patient dose was measured. Result : Esophagography at 17" 143% than 9"magnification the average area dose was increased. Organ dose of Esophagography at 17" was decreased 25.32% than 9" magnification. UGI at 17" was increased 129.73% DAP than 9" magnification. Organ dose of UGI at 17" was decreased 23.32% than 9" magnification. Where the major organs of magnification at 17" were decreased(lung -25.96%, stomach -33.09%, spleen -27.81%, liver -4.92%) than 9" magnification. Conclusion : Expected to get better quality image While using the proper magnification, and have recognition that difference Organ doses and DAP meter in fluoroscopy.
