A Study on the Radiation Dose of $^{131}I$ in the Thyroid Gland during the Treatment of Hyperthyroidism (갑상선 기능항진증의 $^{131}I$ 치료시 갑상선 조사량에 관한 연구)

  • 53 patients with hyperthyroidism have been analyzed with special reference to therapeutic response to radioactive iodine ($^{131}I$) treatment. Mean effective half-life, 24 hour uptake rate and radiation dose of $^{131}I$ in hyperthyroid patients included in this study were respectively. 1. Mean effective half-life of $^{131}I\;was\;4.7{\pm}1.5$ days in the tracer dose and $5.0{\pm}1.5$ days in the therapeutic dose. 2. Mean 24 hour uptake rate of $^{131}I\;was\;72.7{\pm}11.1%$ in the tracer dose and $73.4{\pm}12.3%$ in the theapeutic dose. 3. Mean radiation dose of $^{131}I\;was\;5,319{\pm}2,648$ RAD as predicted and $5,692{\pm}2,843$ RAD as actual. A single dose of radioactive iodine treatment was satisfactory in 34 patients (radioiodine sensitive) and multiple doses of radioactive iodine treatments were required in 19 patients (radioiodine resistant). A radioiodine resistant group of patients with hyperthyroidism was distinctively characteristic in the following aspects. 1. Mean thyroid weight calculated in the resistant group ($63.9{\pm}14.0gm$) was significantly (p<0.01) greater than that of the sensitive group ($46.6{\pm}13.3gm$). 2. Mean 24 hour uptake rate of the tracer dose in the resistant group ($67.3{\pm}10.7%$) was significantly (p<0.01) lower than that of the sensitive group ($75.7{\pm}10.5%$). 3. Mean 24 hour uptake rate of the therapeutic dose in the resistant group ($68.5{\pm}13.7%$) was significantly (p<0.05) lower than that of the sensitive group ($76.1{\pm}10.9%$). 4. Mean predicted radiation dose, of $^{131}I$ in the resistant group ($3,684{\pm}1,745$ RAD) was significantly (p<0.01) lower than that of the sensitive group ($6,232{\pm}2,683$ RAD). 5. Mean actual radiation dose of $^{131}I$ in the resistant group ($4,100{\pm}1,691$ RAD) was significantly (p<0.01) lower than that of the sensitive group ($6,582{\pm}3,024$ RAD). 6. No significant difference was detected in terms of effective half-life of $^{131}I$ among the groups (p>0.05). 7. The average mean % difference of effective half-life, uptake rate and radiation dose measured following the tracer and therapeutic dose of $^{131}I$ were not statistically significant (p>0.05). Therefore effective half-life, uptake rate and radiation dose of the therapeutic dose of $^{131}I$ were readily predictable following the tracer dose of $^{131}I$. 8. It is concluded that the possibility of resistance to radioactive iodine treatment may be anticipated in patients with thyroid gland large in size and compromised $^{131}I$ uptake rate.

  • In EVAR procedure using long time C-arm, we studied exposure dose reduction and effeciency through measuring surgical staff's ESD by installing lead curtain operating table next to. The height 3 the spot (50cm, 100cm, 150cm) dose was measured on 2 locations for 600sec in the X-ray radiation considering the surgical staff's movement. To install the curtains, we compared before and after the dose. As a result, it can confirm that dose of the installation former of 50cm height and after is reduced about 75% and 91% in 2 locations. In 100cm height, the reduction of the dose was a bit confirmed. There as to dose value, measured on 150cm height the installation former and after was nearly no change. This research examined the exposure dose about the radiation of the surgical staff during EVAR procedure in which the operation time is the long time. It was implemented in the object that it reduces the radiation exposure. Feasibility of normal tissue dose reduction in radiotherapy using low strength magnetic field

  • Purpose: Toxicity of mucosa is one of the major concerns of radiotherapy (RT), when a target tumor is located near a mucosal lined organ. Energy of photon RT is transferred primarily by secondary electrons. If these secondary electrons could be removed in an internal cavity of mucosal lined organ, the mucosa will be spared without compromising the target tumor dose. The purpose of this study was to present a RT dose reduction in near target inner-surface (NTIS) of internal cavity, using Lorentz force of magnetic field. Materials and Methods: Tissue equivalent phantoms, composed with a cylinder shaped internal cavity, and adjacent a target tumor part, were developed. The phantoms were irradiated using 6 MV photon beam, with or without 0.3 T of perpendicular magnetic field. Two experimental models were developed: single beam model (SBM) to analyze central axis dose distributions and multiple beam model (MBM) to simulate a clinical case of prostate cancer with rectum. RT dose of NTIS of internal cavity and target tumor area (TTA) were measured. Results: With magnetic field applied, bending effect of dose distribution was visualized. The depth dose distribution of SBM showed 28.1% dose reduction of NTIS and little difference in dose of TTA with magnetic field. In MBM, cross-sectional dose of NTIS was reduced by 33.1% with magnetic field, while TTA dose were the same, irrespective of magnetic field. Conclusion: RT dose of mucosal lined organ, located near treatment target, could be modulated by perpendicular magnetic field.

A Comparison between Effects of Anorexia Induced by Consecutive Low-Dose Cisplatin and High-Dose Cisplatin on Hindlimb Muscles of Rats (시스플라틴에 의해 유발된 식욕부진이 쥐의 뒷다리근에 미치는 영향: 저용량 연속투여요법과 고용량 투여요법 간의 비교)

  • Purpose: The purpose of this study was to compare the effects of anorexia induced by consecutive low-dose and high-dose of cisplatin (CDDP) on the hindlimb muscles of rats. Methods: Male Sprague-Dawley rats were assigned to three groups: Control group (C) received a saline (the same dose and duration as the low CDDP group), the high-dose cisplatin (High CDDP) group received a single 5 mg/kg dose of cisplatin, the consecutive low-dose cisplatin (Low CDDP) group had 1 mg/kg of cisplatin administered for five consecutive days. On the 8th day the soleus and gastrocnemius muscles were dissected. Body weight, food intake, activity, muscle weight, Type I, II fiber cross-sectional area (CSA) of the dissected muscles were measured. Results: Body weight, food intake, muscle weight and Type I, II fiber CSA of the High CDDP and Low CDDP groups were significantly less than the C group. The High CDDP group showed significant decreases, compared to the Low CDDP group, in body weight, food intake, activity score, muscle weight and Type I, II fiber CSA. Conclusion: Hindlimb muscle atrophy occurs due to anorexia induced by both consecutive low-dose and high-dose cisplatin. The muscle atrophy induced by consecutive low-dose cisplatin is less apparent than high-dose cisplatin.

The Properties of Beam Intensity Scanner(BInS) in IMRT with Phantom for Three Dimensional Dose Verification

  • Objectives: Patient dose verification is clinically the most important parts in the treatment delivery of radiation therapy. The three dimensional(3D) reconstruction of dose distribution delivered to target volume helps to verify patient dose and determine the physical characteristics of beams used in intensity modulated radiation therapy(IMRT). We present Beam Intensity Scanner(BInS) system for the pre treatment dosimetric verification of two dimensional photon intensity. The BInS is a radiation detector with a custom made software for relative dose conversion of fluorescence signals from scintillator. Methods: This scintillator is fabricated by phosphor Gadolinium Oxysulphide and is used to produce fluorescence from the irradiation of 6MV photons on a Varian Clinac 21EX. The digitized fluoroscopic signals obtained by digital video camera will be processed by our custom made software to reproduce 3D relative dose distribution. For the intensity modulated beam(IMB), the BInS calculates absorbed dose in absolute beam fluence, which are used for the patient dose distribution. Results: Using BInS, we performed various measurements related to IMRT and found the followings: (1) The 3D dose profiles of the IMBs measured by the BInS demonstrate good agreement with radiographic film, pin type ionization chamber and Monte Carlo simulation. (2) The delivered beam intensity is altered by the mechanical and dosimetric properties of the collimating of dynamic and/or static MLC system. This is mostly due to leaf transmission, leaf penumbra, scattered photons from the round edges of leaves, and geometry of leaf. (3) The delivered dose depends on the operational detail of how to make multileaf opening. Conclusions: These phenomena result in a fluence distribution that can be substantially different from the initial and calculative intensity modulation and therefore, should be taken into account by the treatment planing for accurate dose calculations delivered to the target volume in IMRT.

A Study on the Performance Evaluation of Portable Radiation Shielding Apparatus (이동형 방사선 차폐장치의 성능평가에 관한 연구)

  • When using a mobile X-ray unit, primary radiation creates medical images and secondary radiation scatters in many directions, which reduces image quality and causes exposure to patients, care givers and medical personnel. The purpose of this study was to develop a radiation shielding system for effectively shielding secondary radiation and evaluate its effectiveness. Using a mobile X-ray unit, spatial dose according to presence of human equivalent phantom and spatial dose using the developed shielding device were measured, and the phantom at 80 cm equidistance from center of X-ray was compared with spatial dose according to use of a shield. Measurements were taken at intervals of 10 cm every $30^{\circ}$ from the head direction($-90^{\circ}$) to the body direction($+90^{\circ}$). In the spatial dose measurement with and without the phantom, when the human equivalent Phantom was used, the spatial dose was increased by 40% in all directions from 40 cm to 100 cm from the central X-ray, and about 88% of the space dose was reduced when using the developed shields with the phantom. The equidistance dose at 80 cm from the central X-ray was increased by 39% from $5.1{\pm}0.26{\mu}Gy$ to $7.1{\pm}0.15{\mu}Gy$ when the human equivalent phantom was used, and when phantom was used and shielding was used, the spatial dose was reduced by about 90% from $7.1{\pm}0.15{\mu}Gy$ to $0.7{\pm}0.07{\mu}Gy$. The spatial dose of natural radiation was measured to be about $0.2{\pm}0.04{\mu}Gy$ when using the developed shielding with Phantom at a distance of 1 m or more. It is expected that by using the developed shielding system, it will be possible to effectively reduce secondary radiation dose received in all directions and to ensure safe imaging.

Therapeutic Advantages of Treatment of High-Dose Curcumin in the Ovariectomized Rat

  • Objective : Although curcumin has a protective effect on bone remodeling, appropriate therapeutic concentrations of curcumin are not well known as therapeutic drugs for osteoporosis. The purpose of this study was to compare the bone sparing effect of treatment of low-dose and high-dose curcumin after ovariectomy in rats. Methods : Forty female Sprague-Dawley rats underwent either a sham operation (the sham group) or bilateral ovariectomy (OVX). The ovariectomized animals were randomly distributed among three groups; untreated OVX group, low-dose (10 mg/kg) curcumin administered group, and high-dose (50 mg/kg) curcumin group. At 4 and 8 weeks after surgery, serum biochemical markers of bone turnover were analyzed. Bone histomorphometric parameters of the 4th lumbar vertebrae were determined by micro-computed tomography (CT). In addition, mechanical strength was determined by a three-point bending test. Results : High-dose curcumin group showed significantly lower osteocalcin, alkaline phosphatase, and the telopeptide fragment of type I collagen C-terminus concentration at 4 and 8 weeks compared with the untreated OVX group as well as low-dose curcumin group. In the analyses of micro-CT scans of 4th lumbar vertebrae, the high-dose curcumin treated group showed a significant increase in bone mineral densities (p=0.028) and cortical bone mineral densities (p=0.036) compared with the low-dose curcumin treated group. Only high-dose curcumin treated group had a significant increase of mechanical strength compared with the untreated OVX group (p=0.015). Conclusion : The present study results demonstrat that a high-dose curcumin has therapeutic advantages over a low-dose curcumin of an antiresorptive effect on bone remodeling and improving bone mechanical strength.

Shielding 140 keV Gamma Ray Evaluation of Dose by Depth According to Thickness of Lead Shield (140 keV 감마선 차폐 시 납 차폐체 두께에 따른 깊이별 선량 평가)

  • The present study made a phantom for gamma ray of 140 keV radiated from $^{99m}Tc$, examined shielding effect of lead by thickness of the shielding material, and measured surface dose and depth dose by body depth. The OSL Nano Dot dosimeter was inserted at 0, 3, 15, 40, 90, and 180 mm depths of the phantom, and when there was no shield, 0.2 mm lead shield, 0.5 mm lead shield, The depth dose was measured. Experimental results show that the total cumulative dose of dosimeters with depth is highest at 366.24 uSv without shield and lowest at 94.12 uSv with 0.5 mm lead shield. The shielding effect of 0.2 mm lead shielding was about 30.18% and the shielding effect of 0.5 mm lead shielding was 74.30%, when the total sum of the accumulated doses of radiation dosimeter was 100%. The phantom depth and depth dose measurements showed the highest values at 0 mm depth for all three experiments and the dose decreases as the depth increases. This study proved that the thicker a shielding material, the highest its shielding effect is against gamma ray of 140 keV. However, it was known that shielding material can't completely shield a body from gamma ray; it reached deep part of a human body. Aside from the International Commission on Radiation Units and Measurements (ICRU) recommending depth dose by 10 mm in thickness, a plan is necessary for employees working in department of nuclear medicine where they deal with gamma ray, which is highly penetrable, to measure depth dose by body depth, which can help them manage exposed dose properly.

  • Oral health behavior such as toothbrushing one's teeth, using dentifrice and such are an important part of improving one's oral health and therefore quality of life. However, it is also necessary to research exposure to harmful chemical substances. Therefore, this study investigated the factors that affect researching fluorine exposure resulting from oral health behavior initiation so that correct oral health guidelines can be provided. As a result of applying the fluorine compound's oral exposure in the ConsExpo 5.0 model, adult males' oral external dose was at 0.000196 mg/kg, oral acute (internal) dose at 0.000196 mg/kg/day and oral chronic (internal) dose at 0.000465 mg/kg/day. In the case of females, the oral dose was at $4.1{\times}10^{-6}mg/kg$, oral acute (internal) dose at $4.1{\times}10^{-6}mg/kg$ and oral chronic (internal) dose at $9.99{\times}10^{-6}mg/kg/day$.

Transyaginal Ultrasound-guided Oocytes Collection in Superovulated Korean Native Cows (과배란처리 한우에서 초음파 유도에 의한 난자채란에 대한 연구)

  • The present study was carried out to evaluate the effect of superovulation treatments on ovarian responses, oocyte recovery rates and grades of collected oocytes using an ultrasound-guided transvaginal approach in Korean native cows. Superovulation in cows was induced with two different regimenes: 1) FSH-decreasing dose(n=8): the cows were received twice per day for three days of the total dose of 400 mg of FSH-p, 2) FSH-single dose(n=9): the cows were administrated a single dose of 400 mg of FSH-p in 25% PVP. The Observation of visible follicles and collection of oocytes were performed 12 hours following the last FSH in FSH-decreasing dose group and 48 hours after the FSH-single dose injection. All visible follicles larger than 6 mm were punctured and aspirated with a 6.5 MHz convex-array ultrasound transducer designed for intravaginal use. The mean number of visible follicles(> 6 mm) was significantly(P<0.05) higher in the FSH-decreasing dose treatment (22.811.9) and FSH-single dose treatment (20.612.0) groups than the non-treatment group(7.0$\pm$8). The mean recovery rate of oocytes was not significantly(P<0.05) different between the treatment and control groups, but the mean number of collected oocytes was significantly(P<0.05) higher in the FSH-decreasing dose treatment( 12.611.5) and FSH-single dose treatment (11.813.6) groups than the non-treatment group(3.7$\pm$0.5). In conclusion, the FSH-single dose treatment at superovulation in cows for ultrasound-guided aspiration might increase the number of aspiratable follicles and the recovery rate of follicular oocytes as the FSH-decreasing dose treatment.
