EXPERIMENTAL STUDY ON THE ANTIBIOTIC SUSCEPTIBILITY TEST AGAINST MICROORGANISMS ISOLATED FROM INFECTED ROOT CANALS (감염근관내(感染根管內) 수종균(數種菌)에 대(對)한 항생제(抗生劑)의 감수성검사(感受性檢査)에 관(關)한 실험적(實驗的) 연구(硏究))

  • The rational approach to antimicrobial therapy of infected root canals is based on accurate identification of the infecting organism and on the organism's susceptibility to antimicrobial agents as measured by standardized techniques. In establishing criteria for the selection of antibiotics, a susceptibility test should be performed. The purpose of this study was to investigate the susceptibility of 224 aerobic and anaerobic microbial strains isolated from infected root canals to various antibiotics. This was performed by using 7 antibiotic sensi-disc: Penicillin (10 units), Ampicillin (10 mcg), Tetracycline (30 mcg), Streptomycin (10 mcg), Kanamycin (30 mcg), Lincomycin (2 mcg), and Clindamycin (2 mcg). The results were as follows; 1. Strains isolated from infected root canals was shown to be most susceptible to Clindamycin, while Streptomycin exhibited least antibacterial properties. 2. Anaerobes were found to be susceptible to Penicillin, Ampicillin, and Clindamycin. 3. Streptomycin and Kanamycin were shown to be effective against Staphylococcus aureus and Staphylococcus epidermis, however, unidentified G(+) cocci organisms were found to be resistant to these agents. 4. Bifidobacterium sp. was susceptible to Lincomycin while G(+) rods were resistant to it. 5. Staphylococcus aureus, Micrococcus sp., and anaerobes were highly susceptible to Clindamycin. 6. All of the antibiotics tested were shown to be very effective against Eubacterium sp.

Radiologic Analysis of Tuberculous Spondylitis (결핵성 척추염의 방사선학적 분석)

  • Among the skeletal tuberculosis, tuberculous spondylitis is high incidence and curable disease, if early diagnosis and treatment are possible. We reviewed clinical manifestations and radiologic analysis of 30 cases tuberculous spondylitis from May 1983 to Sept. 1986, at Yeungnam medical center, Yeungnam University. The results were follows : 1. The frequent involve sites were thoracolumbar vertebra. 2. The continuous lesion is 86.7% of the all cases. 3. The most common type was intervertebral type, and lytic and sclerotic lesion were same incidence. 4. Paravertebral abscess, kyphosis and disc space narrowing were demonstrated more than 80.0% of the cases. 5. Computed tomography was more accurate diagnostic method rather than conventional plain study to evaluation of extent of lesion, involvement of spinal canal and cord, and size and location of paravertebral abscess. And CT guided abscess drainage procedure was helpful to diagnosis and treatment. 6. Ultrasonography was helpful to differential diagnosis between paravertebral abscess and other solid mass, and useful to follow up study of paravertebral abscess after treatment.

Clinical Analysis of Chemonucleolysis for Treatment of Lumbar Disc Herniation (요추간판 탈출증치료에서 화학적 수핵용해술의 임상적 고찰)

  • During last 48 months, in our department 51 patients with single or multiple herniated lumbar discs have received chemonucleolysis. Forty-one of 51 cases have single level herniation and remaining cases have multiple level. Age distribution were from 19 to 59 years old. Over 4 months(longest case: 18 months) follow up revealed as follows: 40 cases(78%) revealed excellent results: 4 cases(7.8%) received discectomy 3-6 months after chemonucleolysis because of recurred symtoms : no anaphylaxis and no major complications were noted.

  • Mouth opening limitation is generally caused by masticatory muscle or temporomandibular joint pain, disc dislocation without reduction, adhesion or ankylosis of the temporomandibular joint, and muscle contracture. But otorhinolaryngologic disease, neurologic and vascular disease, tumor, inflammation and infection may cause pain and mouth opening limitation which mimics temporomandibular disorders. Re-evaluation for possibilities of inflammation, infection and tumor should be in cases that do not show symptom improvement or appear with continuous aggravation despite of proper treatment. In this case, we report of medial pterygoid muscle pain and mouth opening limitation caused by periapical abscess of third molar spread to the pterygomandibular space.

Correlation Analysis Between Radiological Result and Radiating Pain in Lumbosacral Pain (요통환자에 있어서 방사통과 영상의학검사 소견상의 연관성 분석)

  • Objectives : We studied in order to compare the differences between the symptoms of patients and findings of MRI and X-Ray of patients with herniated disc which has been diagnosed often recently. Methods : We randomly selected among the 301 patients with X-Ray and L-spine MRI films who have visited Jaseng Hospital with low-back pain and lumbar and low extremity pain from Jan.1st of 2009 to Jan.28. We used SPSS 13.0 for Windows I in analyzing statistical data of study results and the level of significance were below 0.05. Results and Conclusions : 1. If there were radiating pain, based on MRI findings, the amount of herniation was more severe(P>0.05). 2. If the finding of a X-ray showed narrowing, based on MRI findings, the amount of herniation was more severe(P>0.05). 3. There were no significant differences between the presence of radiating pain and the findings of X-Ray(P>0.05). 4. Among the 301 cases, cases which showed findings beside HIVD were 79. 7 cases of hemanggioma(2.3%), 24 cases of spinal neoplasm(cord cyst, tumor etc)(8%), 7 cases of spondylitis(2.3%), 16 cases of spinal canal stenosis(5,3%) 9 cases of ligamentum flavum thickening(3%) and 16 cases of facet syndrome(5.3%).

  • Objective : The purpose of this study is to offer a new approach to diagnostic X-ray in perspective of Chuna manual medicine for clinical application. Methods : Characteristics of each malposition in X-ray were analyzed comprehensively, based on the listing system. By verifying these results, find out the methods of X-ray diagnosis according to the each malposition. Results : 1. Vertebral malposition can be explained by alignment and relative position of vertebral body in the X-ray. To obtain more accurate estimation of subluxation, features of other structures should be considered, such as spinous process, intervertebral foramen and disc space. 2. Pelvic malposition can be determined by relative location of anterior superior iliac spine (ASIS) and posterior superior iliac spine (PSIS) in the X-ray. Also other pelvic parameters should be utilized to make a diagnosis of sacral malposition. Conclusions : Diagnostic X-ray should be applied to many clinicians for reasonable Chuna manual medicine application. And further studies are needed to use the diagnostic X-ray in the perspective of Chuna manual medicine.

The effect of Modified Sacroiliac Joint Taping on Back pain_A case report (변형된 엉치엉덩관절 테이핑의 요통 효과 사례보고)

  • This is a case study that sought to consider whether taping, which focuses on instability of the sacroiliac joint, is a potential intervention method that may be helpful for low back pain. In the case of a 58-year-old participant, we summarized the notable results from a taping training session that a man with a history of back surgery due to disc herniation and stenosis participated in to reduce ongoing pain. As an intervention method, tape was applied between the 2nd and 4th sacrum on both sides from the spinous tuberosity. It was stretched to a tension of 80% and attached past the sacroiliac joint, and then the ends were raised at about 45° on both sides and attached toward the gluteus medius muscle. Then, along the erector spinae muscle from the iliac crest. Bilateral taping up to the level of the 10th rib was applied. Through this intervention, positive case results were observed from both VAS and ODI test tools, with VAS recorded as 5 to 0 and disability index recorded as 13 to 0, respectively.

Psychological Characteristics of Living Liver Transplantation Donors using MMPI-2 Profiles (MMPI-2를 이용한 생체 간 공여자들의 심리적 특성에 대한 연구)

  • Objectives : Living donor liver transplantation (LDLT) is a life-saving therapy for patients with terminal liver disease. Many studies have focused on recipients rather than donors. The aim of this study was to assess the emotional status and personality characteristics of LDLT donors. Methods : We evaluated 218 subjects (126 male, 92 female) who visited Daegu Catholic University Medical Center from August 2012 to July 2018. A retrospective review of their preoperative psychological evaluation was done. We investigated epidemiological data and the Minnesota Multiphasic Personality Inventory-2 questionnaire. Subanalysis was done depending on whether subjects actually underwent surgery, relationship with the recipient, and their gender. Results : Mean age of subjects was $32.19{\pm}10.91years$. 187 subjects received LDLT surgery (actual donors) while 31 subjects didn't (potential donors). Donor-recipient relationship included husband-wife, parent-children, brother-sister etc. Subjects had statistical significance on validity scale L, F, K and all clinical scales compared to the control group. Potential donors had significant difference in F(b), F(p), K, S, Pa, AGGR, PSYC, DISC and NEGE scales compared to actual donors. F, D and NEGE scales were found to be predictive for actual donation. Subanalysis on donor-recipient relationship and gender also showed significant difference in certain scales. Conclusions : Under-reporting of psychological problems should be considered when evaluating living-liver donors. Information about the donor's overall psychosocial background, mental status and donation process should also be acquired.

The Case Study on Thoraco-lumbar junction and Lumbo-sacral junction Transitional Zone Syndrome. (복합적으로 병발한 등-허리, 허리-엉치 이행부위 증후군에 대한 증례를 통한 고찰)

  • Objectives: We have evaluated the effects of conservative treatment on one patient who was diagnosed to thoraco-lumbar junction and lumbo-sacral junction transitional zone syndrome. so, we investigate thoraco-lumbar junction and Jumbo-sacral junction tranzitional zone syndrome that was suggested by Maigne R. Methods : One patient was diagnosed as T12-L1 herinated intervetebral disc through MRI. We diagnosed the patient to transitional zone syndrome by symptoms and physical examination and used Acupuncture therapy, Cox technique to the patient. We measured Visual Analog Scale(VAS) before and after treatment. Results: After treating Acupuncture therapy, Cox technique in the case, We find out that the patient was improved. VAS score changed from 7 to 2 and physical examinations are improved. Conclusions: We improved the patient who had thoraco-lumbar junction and lumbo-sacral junction transitional zone syndrome by Cox technique and Acupuncture therapy, and we found that thoraco-lumbar junction related lumbo-sacral junction.

Toxic Optic Neuropathy Caused by Chlorfenapyr Poisoning (클로르페나피르 음독 후 발생한 독성 시신경병증 1예)

  • Purpose: To report a case of toxic optic neuropathy caused by chlorfenapyr ingestion accompanied by central nervous system involvement. Case summary: A 44-year-old female visited our clinic complaining of reduced visual acuity in both eyes for 7 days. She had ingested a mouthful of chlorfenapyr for a suicide attempt 2 weeks prior to the visit. Gastric lavage was performed immediately after ingestion at the other hospital. Her best-corrected visual acuity was finger count 30 cm in the right eye and hand motion in the left eye. Both pupils were dilated by 5.0 mm and the response to light was sluggish in both eyes. A relative afferent pupillary defect was detected in her left eye. Funduscopy revealed optic disc swelling in both eyes. Magnetic resonance imaging of the brain showed a symmetric hyper-intense signal in the white matter tract including the internal capsule, corpus callosum, middle cerebellar peduncle, and brainstem. The patient was diagnosed with toxic optic neuropathy induced by chlorfenapyr ingestion, and underwent high-dose intravenous corticosteroid pulse therapy. Three days later, the best-corrected visual acuity was no light perception in both eyes. Three months later, optic atrophy was observed in both eyes. Optical coherence tomography revealed a reduction in the thicknesses of the retinal nerve fiber layer and ganglion cell and inner plexiform layer in the macular area. Conclusions: Ingestion of even a small amount of chlorfenapyr can cause severe optic nerve damage through the latent period, despite prompt lavage and high-dose steroid treatment.
