개의 성대수술 후 발성음의 음성학적 분석 (Spectrographic Analysis of the Sound After Vocal Cordectomy in Dogs)

  • This study eas conducted to assess the spectrographic analysis agter vocal cordectomy in which procedures included biopsy punch per os, laryngofissure and lactic acid injection to the vocal cord. There were significant difference in dominant frequency(DF) of spectrograms between normal and all surgical procedures for vocal cordectomy at 1 day (p<0.01). Vocal cordectomy(laryngofissure) revealed significant differences in minimum frequency of call (MIFC) and DF during the period of observation after surgery (p<0.01). In lactic acid injextion group, there were also significant differences in DF and maximum frequency of call (MAFC) variables (p<0.01).

한방치료가 위 미주신경 활성 저하와 유문부 기능 장애가 병발된 기능 소화불량중 환자에 미치는 선택적 효능 (A Selective Effect of Combined Treatment of Electroacupuncture at Zusanli(ST36), Manual Acupuncture, and Pyengwi-san in Function Dyspepsia Patients with Pyloric Valve Disturbance and Hypoactivity of Gastric Vagus Nerve)

  • 목 적 : 위 미주신경 활성 저하와 유문부 기능 장애를 동시에 가지고 있는 기능성 소화불량증 환자에서 전침 및 한약의 복합처치 치료가 이 두가지 면에서 각각 어떤 영향을 미치는지를 살펴보았다. 방 법 : 장음검사상 위 미주신경 활성 저하와 유문부 기능 장애를 동시에 가지고 있는 환자 10명을 대상으로 치료 전후의 장음 지표 변화를 비교하였다. 장음지표는 % of bowel sound (%BS)와 주 주파수(dominant frequency, DF)를 사용하였으며, %BS(6인 경우를 위 미주신경 활성저하 DF ratio(식후/식 전)<1를 유문부 기능장애로 평가하였다. 환자들에게 2주간 족삼리전 침과 체침이 매일 시술되었으며 평위산 전탕액이 하루 3회 투여되었다. 결 과 : 2주간의 치료 후 DF ratio는 $0.93\pm0.06$에서 $1.06\pm0.04$으로 유의성있게 증가했으며, 10명 중 9명의 환자가 DF ratio>1의 정상치로 회복되었다. %BS 역시 $2.97\pm1.17%$에서 4.27%로 증가했지만 통계적 유의성은 없었으며, 3명의 환자에서 %BS가 6이상으로 정상회복되었다. 결 론 : 위 미주신경 활성 저하와 유문부 기능 장애를 동시에 가지고 있는 기능성 소화불량증 환자에 대한 족삼리 전침과 일반 체침, 평위산의 치료는 미주신경 활성 저하보다는 유문부 기능 장애에 더 유의한 효과를 보였다.

Incomplete Relationship between Dominant Power of Electrogastrography and Gastric Myoelectrical Activity in Patients with Functional Dyspepsia

  • Background & Aims : The aim of this study was to investigate the change of dominant power with observation of gastric myoelectrical activity and its parameter linkage in electrogastrography. Methods : Electrogastrography was performed on a total of 123 subjects (113 patients with functional dyspepsia, 10 healthy controls) for 30 min in fasting state and 50 min in postprandial state. Average myoelectrical activity per frequency and accumulated electrical activity of 5 min duration in each bradygastria, normogastria, and tachygastria were measured at the moment of frequency switchover of slow wave. Assumed parameter linkages were also investigated among dominant frequency, % of normal regularity, and dominant power (or power ratio). Results : Average myoelectrical activity per frequency was highest in bradygastria (mean 1.10-1.47 Volt/s), next highest in normogastria (mean 0.50-0.82 Volt/s), and lowest in tachygastria (mean 0.44-0.47 Volt/s). Average accumulated myoelectrical activity was highest in normogastria (mean 114.90-126.29 Volt/ss), next highest in tachygastria (mean 71.02-90.00 Volt/ss), and lowest in bradygastria (mean 12.93-51.94 Volt/ss). Significance of parameter linkages were noted in dominant frequency (p< 0.01) and in % of normal regularity (p< 0.01), but not in dominant power in case of frequency shift from bradygastria to normogastria (p=0.376). Conclusion : Dominant power is not a parameter that reflects the gastric myoelectrical activity related with only gastric contraction. Bradygastric dominant power does not follow the inter-parameter linkage of electrogastrography for gastric motility assessment.

기능성 소화불량증 환자의 위 미주신경 활성 및 유문부 기능에 대한 족삼리(足三里) 전침과 일반 체침 자극의 복합 효능 (Combination Effects of Zusanli(ST36) Electroacupuncture and Manual Acupuncture of other Acupoints on Gastric Vagal Nerve Activity and Pyloric Valve Function in Patients with Functional Dyspepsia.)

  • Objectives : The aim of this study was to investigate changes of gastric vagal nerve activity and pyloric valve function after execution of combination treatments of both electroacupuncture at Zusanli(ST36) and manual acupuncture at other acupoints in patient with functional dyspepsia. Methods : Bowel sounds of 49 patients (18 male, 31 female) were recorded and their % of bowel sound (%BS) and ratio of dominant frequency (DF) were analyzed. Postprandial %BS was used to indicate the gastric vagal activity after eating. Ratio of postprandial/fasting dominant frequency was used to present the degree of pyloric valve function. According to values of %BS and DF ratio, each patient was classified into normal or abnormal (<6 %BS, hypoactivity: <1 DF ratio, dysfunction) group. For 2 weeks, patients received a treatment consisting of both electroacupuncture stimulation at Zusanli (ST36) and acupuncture at other meridian points. Variation of parameters shifting normal to abnormal or abnormal to normal was observed, and total cure rate was calculated. Results : Total cure rate of %BS was 16%, and that of DF was 37%. Patients who improved to normal value from abnormal or aggravated to abnormal level showed both significant difference in both vagal nerve hypoactivity and pyloric valve dysfunction, respectively. Conclusions : Analysis of bowel sound might be useful to evaluate both gastric vagal nerve activity and pyloric valve function. Combination effects of Zusanli (ST36) electroacupuncture and manual acupuncture of other acupoints showed a bidirectional effect in which their activity and function were in general improved. sometimes from aggravated to abnormal level.

후두마비 개에 대한 피열연골고정술 (Arytenoidopexy with Bilateral Vocal Cord Excision (laryngofissure) for the Relief of Laryngeal Paralysis)

  • This study was conducted to assess the application of spectrograms in the diagnosis of laryngeal paralysis and the arytenoidopexy with bilateral vocal cord excision (laryngofissure) for the relief of laryngeal paralysis. Laryngeal paralysis represented two types of stridor A and B (AN, PN). There were significant differences in Dominant Frequency ( DF, p<0.01), which were $3590{\pm} 209.81 Hz, 7445\{pm}418.54 Hz$, respectively. After arytenoidopexy with bilateral vocal cord excision (laryngofissure), $PaO_{2} levels (100.57{\pm}7.59 mmHg) were more increased than that (86.5 {\pm} 3.34 mmHg)$ of pre-operative dogs.

Poodle의 발성음 (Common Calls of Poodle)

  • This study was performed to analyse the common calls of poddle spectrographically : bark, growl, howl, snore, yelp and whine. The sonograms of 6 common calls were shown their own specific features. There were significant differences among each types of common callsin the parceter of minimun frequency of call (MIFC), maximun frequency of call (MAFC), duration of call (DC), interval between call (IBC), dominant frequency (DF), F1 formant, F2 formant and F3 formant (P<0.01). It was considered that it was possible to record the main common calls dogs by sonograms and it sould be applied to objective basic data for understanding the psychological stats of dogs, the social relationship among them and the relationship sith human being.

위전도(胃電圖)에 영향을 미치는 요인(要因)에 대한 고찰(考察) (A Review on factors that influence Electrogastrography)

  • Objectives : To review to determine difference in the effect of age, gender, location of electrode, test meal and period of recording that influence Electrogastrography (EGG) parameters and to study its oriental medical application. Methods : We investigate the research results through internet search engines, Pub Med, Karger, Mary Ann Libert and Pro Quest. Results : Age and gender influence postprandial Dominant Frequency Instability Coefficient (DFIC). Body mass index (BMI) influence fasting and postprandial EGG parameters. Menstrual cycle phase influence fasting Dominant Frequency (DF). Test meal volume and composition influence postprandial EGG parameters. But reduced-calorie test meal have no effect on EGG parameters. Conclusions : Because the amplitude of surface-recorded SW(slow wave) is very weak, EGG is influenced by many factors. Age, gender, BMI, menstrual cycle phase, test meal volume and test meal composition influence EGG parameters. but reduced-calorie test meal have no effect on EGG parameters.

위장 비율동성 당뇨환자에서 족삼리(足三里)($ST_{36}$) 전침처치의 위전도 조절 영향 사례군 연구 (Assessment of Regulatory Effect of Electro-acupuncture at Jogsamni($ST_{36}$), on Electrogastrography in Diabetic Patients with Gastric Dysrhythmia;A Case Series Study)

  • Objectives : This study was to investigate the regulatory effect of electro-acupuncture on Jogsamni ($ST_{36}$) in bradygastric condition or gastroparesis which is a neuropathic complication of type 2 diabetes mellitus(T2DM), assessed using 4-channel electrogastrography (EGG). Methods : It was a case series study. Each patient was given electro-acupuncture(EA) one point, Jogsamni($ST_{36}$) or Gyeonu($LI_{15}$) bilaterally for thirty minutes with 2Hz bipolar square wave frequency and moderate intensity of stimulation. With attaching electrodes around stomach region throughout pre-acupuncture, acupuncture, and post-acupuncture session, these parameters were produced; percent rate of bradygastria power compared to sum of power in all kind of gastric rhythm(% bradygastria), dominant frequency(DF) and dominant power(DP). Parameter was analysed per each channel's variable change from pre-acupuncture session by paired t-test. Results : There was statistically significant decrease in % bradygastria parameters on one of the $ST_{36}$ treated patients from pre-acupuncture session and to post-acupuncture session(P=0.015, P=0.008 respectively). A marginal significant decrease of DP was shown in the other $ST_{36}$ treated patient from pre-acupuncture session through post-acupuncture session(P=0.049). Combined data of two $ST_{36}$ Treated patients showed that there was significant decrease of %bradygastria from pre-acupuncture session to acupuncture session(P=0.020), and was decrease of DP from pre-acupuncture session to post- acupuncture session(P=0.020). Conclusions : EA at $ST_{36}$ resulted in statistically significant decrease in %bradygastria and DP for dysrhythmic condition in T2DM gastroparesis patients. Considering this limited results, further study will be needed to elucidate the effect of multiple-acupuncture on gastric myoelectrical activities.

기능성 소화불량증 환자에서 침구 처치 후 다채널 위전도 소견의 변화 (Changes in Multi-channel Electrogastrography after Acupuncture Treatmet in Patients with Functional Dyspepsia)

  • The aim of this study was to investigate the effects of traditional Korean medical therapy in patients with functional dyspepsia evaluted using multi-channel electrogastrography. Before the treatment, the EGG of 14 outpatients (male 3, female 11) with functional dyspepsia were recorded and their dominant frequency(DF), percentage of gastric slow waves, percentage of Power ratio, and percentage of slow wave couplings were analysed. For 8 weeks, the patients received acupuncture treatment. After 8 weeks, EGG was rechecked and compared with the previous data. After treatment, normogastria was significantly increased in channel 1 and channel 3($53.70{\pm}18.94$% vs $72.64{\pm}15.49$% in channel 1, P<0.05; $79.74{\pm}18.09$ vs $90.86{\pm}8.86$ in channel 3, P<0.05). arrythmia was significantly decreased in channel 3($14.34{\pm}15.21$ vs $5.93{\pm}6.90$, P<0.05). The patients who had less severe decrease in EGG indexes showed improvement of those indexes through 8 weeks of outpatient care. EGG channel 3 was shown to be the most adequate standard of evaluation. But there was no significant improvement in slow wave coupling rate which is a new index of multichannel EGG.
