  • Background : Interstitial cystitis is a disorder of the bladder characterized by urgency and frequency of urination, and pelvic pain. The cause of interstitial cystitis is not clear. Modern medical treatments include behavioral therapy, medication, intravesical infusion therapy, and surgical treatment, but the goal of treatment is based on symptom relief rather than a cure. According to a recent report, in an animal model, oriental medicine treatment, Yukmijihwang-tang improved the symptoms of interstitial cystitis. However, clinical studies and experimental research are lacking. Objectives : In this study, We investigated Chuknyojetong-tang, acupuncture, moxibustion and bee venom effect on interstitial cystitis. Methods : We targeted 25 patients diagnosed with interstitial cystitis at the university hospital and treated them for more than one month. We used acupuncture, Chuknyojetong-tang, and bee venom. The effects were evaluated using ICSI and ICPI at the begin of treatment and at the end of treatment. Results : All of the 25 patients were female, average age was $53.3{\pm}13.5$ years, and the average month of disease-duration was $69.0{\pm}59.8$. The duration of treatment was $8.0{\pm}5.4$ months, and 60% of patients were improved. After treatment, we used correlation coefficient method, multiple regression analysis. ICSI difference increased as treatment period increased. ICPI difference increased as age decreased. Total difference also increased as treatment period increased. These results imply that treatment effect was improved. Conclusions : These findings suggest that oriental medicine therapy using Chuknyojetong-tang, acupuncture, moxibustion, and bee venom would be very effective on interstitial cystitis.

Methods: Three women who had different age 27 years old, 28 years old and 51 years old women, who diagnosed chronic relapsing cystitis. were enrolled in this study. They received oriental treatments such as herbal medicine, acupuncture for several months. The 27 years old women we gave her Takchabojoongikkitang-gamibang 3 times for 3 months. The 28 years old women we gave her Takchabojoongikkitang-gamibang 2 times for 2 months. The 51 years old women we gave her Takchabojoongikkitang-gamibang 5 times for 3 months. During the treatments, we requested them to visit hospital every two weeks. If they could not come to hospital, we called them to take follow-up. Results: After the treatments, symptoms of chronic relapsing cystitis were decreased or disappeared. Conclusion: This clinical cases indicate that Takchabojoongikkitang-gamibang is effective in treatment of the chronic relapsing cystitis and can prevent chronic progress and relapse of symptoms. More clinical data and studies are requested for the treatment of chronic relapsing cystitis.

  • Background: Gastrointestinal (GI) drugs are often co-prescribed with other medications to prevent GI complications. This study aimed to evaluate the prescribing pattern of potentially unnecessary GI drugs in patients with acute cystitis who were prescribed oral antibiotics and investigate the influencing factors affecting this. Methods: We identified female patients ≥20 years with acute cystitis who visited the outpatient clinic and were prescribed oral antibiotics between July and December by analyzing Health Insurance Review and Assessment Service (HIRA)-National Patients Sample (NPS)-2019 data. Patients with no prior history of GI disorders within 180 days prior to acute cystitis, excluding or including the date of diagnosis of acute cystitis, were selected (Group A and B). Multiple logistic regression analysis was performed to estimate the factors affecting the prescription of potentially unnecessary GI drugs. Results: A total of 1,544 in Group A and 552 patients in Group B were included for the final analysis. Potentially unnecessary GI drugs were prescribed in 1,176 patients in Group A (76.2%) and 231 patients in Group B (41.8%). Third generation cephalosporines and sulfonamides showed the lower odds ratio for prescribing GI drugs than penicillins. Prescribers from Urology clinics showed more than twice odds ratio for the prescription of GI drugs compared to prescribers from internal medicine clinics. Conclusion: The results of this study showed that potentially unnecessary GI drug prescriptions for patients with acute cystitis were high in South Korea. The positive risk factors affecting the prescription of unnecessary GIs were not patient-related factor but healthcare facility and prescriber-related factors.

  • Hemorrhagic cystitis refers to massive inflammation and the diffuse vesical bleeding of the bladder. In patients with hematuria complaining of dysuria and pain, it is necessary to differentiate various causes, including cystitis, nephritis, and prostatitis. After the diagnosis of hemorrhagic cystitis, antibiotics usually treat and prevent further urinary tract infections. In the present case, a 78-year-old female patient with hemorrhagic cystitis presenting with hematuria underwent Korean medical treatment with Jeoryeong-tang-hap-Samul-tang for 29 days. The effect of the treatment was assessed with the hematuria grading scale (HGS) per week, urinalysis per two weeks, and the NRS (numeric rating scale) of dysuria per day. After treatment, both HGS and NRS scores decreased, and protein, blood, and red blood cells (RBCs) in urinalysis improved. This case report suggests that Jeoryeong-tang-hap-Samul-tang might be an effective option for hemorrhagic cystitis patients who continuously take antiplatelets.

It is liable to be chronicity and difficult to give medical treatment. Especially, Chronic Relapsing Cystitis(CRC) is the continuing and repeating cystitis without abnormality of urinalysis. This study was peformed on a 46-year-old female patient with CRC who was diagnosed as Norim(勞淋) due to Bisinyangheo(脾腎陽虛) and Hansan(寒疝) due to Hanchekanmaek(寒滯肝脈). The patient was treated with mainly Cheongsanboki-tang(淸疝補氣湯). Clinical symptoms and general conditions are improved after treatment and more extensive researches are needed.

  • Objectives: The purpose of this study is to report the clinical effectiveness of Korean medicine treatment on interstitial cystitis. Methods: The patient diagnosed with Interstitial cystitis/Bladder pain syndrome (IC/BPS) was treated with Korean medicine treatment during 22 days of hospitalization. The patient received Korean medicine treatment including herbal medicine, acupuncture, pharmacopuncture, moxibustion, cupping and extracorporeal magnetic stimulation therapy. The effect of treatment on IC/BPS was evaluated through the Numeric Rating Scale (NRS), voiding diary, and Interstitial Cystitis Symptom Index/Problem Index (ICSI/ICPI). Results: After the treatment, the clinical symptoms of IC/BPS such as pelvic pain, pelvic pressure, frequent urination and urinary urgency were improved. Also, the patient had better scores in ICSI/ICPI. Conclusions: This case report proved that the Korean medicine treatment may be effective for improving clinical symptoms of IC/BPS. However, a large-scale clinical study is needed in the future to prove the effectiveness of Korean medicine treatment for IC/BPS.

  • Interstitial cystitis is an extremely painful and distressing condition, characterized by severe suprapubic pain, which increases with bladder filling and is relieved by voiding. The daily frequency of micturition may approach 100 times, but no incontinence is observed. The symptoms persist throughout the night, which consequently affects sleep. The etiology of this condition is still unknown, but includes infection, autoimmune response, allergic reaction, neurogenic inflammation, epithelial dysfunction and inherited susceptibility. Herein, a case of interstitial cystitis, with severe symptoms, which was successfully treated with lumbar sympathetic block, is reported.
