  • In this paper, We designed the ECG-NIBP-$SpO_2$ patient monitor. This production can measure Electrocardiograph, Heart Rate, Noninvasive Blood Pressure, and Oxygen Saturation for Noninvasive Mehod and can display each information. These informations were implemented by the electrodes of ECG part, the cuff of NIBP module and the finger probe with light sensor of $SpO_2$ without injection of needle or catheter. In addition, We developed a new analysis algorithm and measurement technique for NIBP and $SpO_2$ to observe patient's conditions correctly.

Changes of blood pressure and Hematological changes induced by exposure of low- and high-level lead in the rats

  • The physiological responses caused by exposure of high- and low-level lead exhibit different phase. Low-level lead continuous hypertension, but high-level lead can in the development of hypertension. In this study it was tested which difference can be caused as lead levels and, if it can be caused, whether hematological changes are related with the hypertensive effects induced by different levels of lead exposure was tested. Lead intoxication in male SD rats was induced by exposure through drinking water containing 50, 200 and 1000 ppm lead (as lead acetate). The animals of control group was supplied drinking water containing sodium acetate ad libitum. The number of each animal group was 10. Systolic blood pressures were measured in the unanesthetized state by the tail-cuff technique at 0, 3, 7 and 16 weeks. RBC, WBC, MCV, hemoglobin, hematocrit and whole blood viscosity levels were examined.

  • 최근 교통사고의 증가, 심폐술의 보편화 및 보조호흡을 필요로 하는 환자의 증가에 따라 경구적 혹은 기관절개술을 통해 기관내 삽관을 장기간 유치하는 경우가 많아져서 이에 따른 합병증으로 기관협착증의 빈도가 높아지게 되었다. 기관내 삽관후 기관협착증을 초래하는데에는 관의 외경, 삽관시 외상, 삽관유치기간, 기계적 보조호흡시 관의 이동, cuff 의 압력 등의 요인이 작용한다. 이러한 요인에 의해 기관내 삽관을 장기간 유치시 기관점막에 대한 지속적인 압박으로 점막하부의 압박괴사 및 혈관의 폐쇄로 인해 기관연골의 무혈성 괴사를 초래하여 환상의 기관협착이 병발하게 된다. 기관협착증에 대한 치료방법으로서는 여러 가지가 있으나 환상의 기관협착증인 경우는 협착부위의 절제 및 단단문합술을 시행하고 절제부위가 광범위한 경우는 release technique 을 이용하여 문합부위의 장력을 줄여주는 것이 효과적인 치료방법으로 알려져 있다. 저자들은 최근 기관내 삽관후에 병발한 환상기관협착증 3례(다발성 골절 1례, 기관지 천식 1례, 약물중독 1례)에 대해 기관절제 및 단단문합술을 경험하였기에 보고하고자 한다.

Anchor Hole Augmentation with Bone Cement in Arthroscopic Rotator Cuff Repair (관절경적 회전근 개 봉합술에서 골 시멘트를 이용한 봉합 나사 구멍 보강술)

  • Purpose: In arthroscopic rotator cuff repair, the crucial step is secure fixation of Anchor to bone. However, osteoporosis of the tuberosity is frequently encountered in old patients, and can cause insecure fixation of anchors. The Aim of our study was to introduce a technique for anchor hole augmentation with bone cement when fixation failure of an anchor occurs, and to investigate the outcome. Materials and methods: Among 223 rotator cuff repairs performed between 2005 and 2009, anchor hole augmentation with polymethylmethacrylate was performed in 15 cases (all females; mean age of 65 years: range 49~77). Bone cement was injected into the anchor hole in a thick fluid state and the procedure was repeated to make a pot-like cement mantle. The anchor was inserted into the cement mantle while the cement hardened. The outcome was investigated, on average, at 16 months (6~32). Results: Radiographs showed cystic changes of the tuberosity. On follow-up radiographs and MRI, a change in the cement mantle was not noted. The final average UCLA score was 31 (28~35); 6 had excellent, 8 good and 1 fair results (p=0.008). Age-sex matched Constants score was 90 (74~98) (p=0.008). Conclusion: Anchor hole augmentation with bone cement is useful when fixation failure of an anchor is encountered due to bone atrophy. Anchor hole augmentation with bone cement does not negatively influence the outcome.

Reverse Total Shoulder Arthroplasty in Patients with Severe Rotator Cuff-Deficient Shoulder - A Minimum Three-Year Follow-up Study - (심한 회전근 개 부전을 동반한 환자에 대한 역형 견관절 전치환술 - 최소 3년 추시 결과 -)

  • Purpose: The goal of this study is to evaluate the minimum three-year follow-up results of reverse total shoulder arthroplasty (RTSA) for glenohumeral (GH) arthritic and pseudoparalytic patients with severe rotator cuff deficiency. Materials and Methods: We evaluated 13 patients (three males and 10 females) who underwent RTSA from July 2007 to July 2010. The average follow-up duration was 54.2 months (range, 37~74 months). Clinical results were evaluated using the Visual Analog Scale (VAS) for pain, active Range of Motion (ROM): active forward flexion (aFF); active external rotation at the side (aERs); active internal rotation to the back (aIRb), American Shoulder and Elbow Surgeons (ASES) score, Korean Shoulder Score (KSS), and intraoperative and postoperative complications. Results: VAS score improved from 7.5(6~10) points to 1.5(0~4), and ROM of active forward elevation improved from $42.7(10{\sim}100)^{\circ}$ to $129.1(110{\sim}180)^{\circ}$. In addition, ASES score improved from preoperative 32.9 (11.7~46.7) points to 80.2(58.3~95.0) postoperatively, and KSS score improved from 36.8(24~47) points to 78.4 (61~92). Twelve out of 13 cases showed various degrees of scapular notching at the last follow-up. There was one case of intraoperative anterior glenoid fracture and two cases of temporary nerve injury. Revision surgery was performed in two cases for treatment of delayed postoperative deep infection. Conclusion: According to minimum three-year follow-up results, RTSA may be an effective treatment option for glenohumeral (GH) arthritic and pseudoparalytic patients. However, considering the possibility of complications related to delayed deep infection or surgeon's technique, RTSA should be judiciously and carefully indicated by expert surgeons.

Relationship between the Changes of Catecholamines and Blood Pressure Induced by Exposure to Low- and High-levels of Lead in Rats

  • In this study, it was tested whether the changes of catecholamines and its metabolites are related with the changes of blood pressure(BP) induced by different levels of lead exposure. Adult male SD rats were exposed to lead by giving drinking water containing 50(low doses), 200 and 1,000 ppm(high doses) of lead(as lead acetate) or sodium acetate(for control groups, supplying an identical amount of acetate) for 7 or 16 weeks. The systolic BP was measured in the unanesthetized state by the tail-cuff technique. Levels of catecholamines and its metabolites in urine were measured by HPLC-ECD. Rats receiving 200 and 1,000 ppm developed an elevation of systolic BP at 3 and 7 weeks compared with week 0, but blood pressure levels at 16 weeks returned to normal. For the 50 ppm lead treated group, systolic BP increased significantly at 7 weeks and 16 weeks. The concentrations of norepinephrine and VMA in the urine of lead exposed rats changed similarly to the changes of blood pressure, but blood viscosity levels in all lead treated rats increased continuously during all lead treatment periods. This result suggests that the changes of catecholamines and its metabolites in urine by lead intoxication may influence the changes of blood pressure.

  • Purpose: Weight lifting is a good training to control body weight, to correct body shape and to relieve stress. How-ever if the training is continued by inadequate training method and technique, the risks of the shoulder injuries are relatively high. Main Subject: The rotator cuff injury is the most common disorder to wright lifters and often results from the train- ing program of upright row, military press and pectoral deck. The chances of subacromial impingement in these postures are high because the shoulder rotates under the acromion at 90 abduction state. Shoulder instability in weight lifters can develop due to various causes. aepeated microtrauma and excessive abduction and external rotation may result in laxity of the anterior capsular structure, ligament and muscles. Behind the neck and bench press are high risk training postures. Other than those injuries, idiopathic osteolysis of distal clavicle, acromioclavicular separation, pectoralis major muscle rupture, and triceps muscle rupture nay develop. Conclusion: The best treatment option of the shoulder injury to weight lifters is to eliminate the possible risk elements for the weight lifters in training program and to provide proper and prompt treatment as soon as possible.

Electrohydraulic Pump-Driven Closed-Loop Blood Pressure Regulatory System

  • An electrohydraulic (EH) pump-driven closed-loop blood pressure regulatory system was developed based on flow-mediated vascular occlusion using the vascular occlusive cuff technique. It is very useful for investigating blood pressure-dependant physiological variability, in particular, that could identify the principal mediators of renal autoregulation, such as tubuloglomerular feedback (TGF) and myogenic (MYO), during blood pressure regulation. To address this issue, renal perfusion pressure (RPP) should be well regulated under various experimental conditions. In this paper, we designed a new EH pump-driven RPP regulatory system capable of implementing precise and rapid RPP regulation. A closed-loop servo-controlwas developed with an optimal proportional plus integral (PI) compensation using the dynamic feedback RPP signal from animals. An in vivo performance was evaluated in terms of flow-mediated RPP occlusion, maintenance, and release responses. Step change to 80 mmHg reference from normal RPP revealed steady state error of ${\pm}3%$ during the RPP regulatory period after PI action. We obtained rapid RPP release time of approximately 300 ms. It is concluded that the proposed EH RPP regulatory system could be utilized in in vivo performance to study various pressure-flow relationships in diverse fields of physiology, and in particular, in renal autoregulation mechanisms.

Proposal of East-West Integrative Medicine Manual for Rehabilitation after Shoulder Surgery (어깨 수술 후 재활 치료에서의 한양방협진 매뉴얼 제안)

  • Objectives This is one of the manuals of East-west integrative medicine which was created by the Committee on integrative medicine of Chung-Yeon Korean Medicine Hospital. The purpose of this manual is to support clinical decision making and communication during the rehabilitation of patients after shoulder surgery. Methods The drafting was done by two rehabilitation specialists in Korean medicine. After a rehabilitation specialist in western medicine reviewed the draft and exchanged their ideas, a revised version that reflects the goal of consultation was made. Then the Committee agreed to adopt the manuals through the process of review and feedback in addition to face-to-face discussions. Results This manual describes clinical decision making for rehabilitation after arthroscopic rotator cuff repair, SLAP repair, and arthrolysis. Therefore it contains the schedule of rehabilitation treatment through the surgical technique, general goal of the rehabilitation by phase, guide for patients and postoperative infection management. Conclusions The proposal of this manual has a significance for it provides information about decision making process and contents of treatment in one medical institution for East-west integrative rehabilitation treatment after shoulder surgery.

A Suture Bridge Transosseous-Equivalent Technique for Bankart Lesions with Deficient Bony Stability - Technical Note - (골안정성 결손을 가진 Bankart 병변에 대한 경 골-유사 교량형 봉합술식 - 술기 보고 -)

  • Purpose: Point fixation at the margin of the glenoid is a limitation of conventional arthroscopic stabilization using suture anchors, and does not afford sufficient footprint healing, especially in glenoid bone deficiency. So, we introduce an arthroscopic suture bridge transosseous-equivalent technique for bony Bankart lesions to avoid the technical disadvantage of point contact with anchor fixation and to improve mechanical stability through cross compression of the labrum. Surgical approach: The technique was adapted from the transosseous-equivalent rotator cuff repair technique using suture bridges, which improved the pressurized contact area and mean pressure between the tendon and footprint. After preparation of the glenoid bed by removal, reshaping, or mobilization of the bony lesion, two anchors (3.0 mm Biofastak, $Arthrex^{(R)}$, Naples, FL) were inserted into the superior and inferior portion of the bony Bankart lesion. Using a suture hook, medial mattress sutures were applied around the capsulolabral portion of the IGHL complex to obtain sufficient depth of glenoid coverage. A 3.5 mm pushloc anchor ($Arthrex^{(R)}$, Naples, FL) hole was made in the articular edge of the anterior glenoid rim. distal, suture bridge was applied, and proximal was inserted to mobilize the labrum in the proximal direction. This avoided the technical disadvantage of point contact with anchor fixation and decreased the level of gap formation through cross-compression of the labrum.
