골내 침범을 동반한 견관절 석회화 건염 - 증례 보고 - (Calcific Tendinitis of Shoulder Associated with Intraosseous Involvement -A Case Report-)

  • 목적: 골내 침범이 동반된 비전형적인 견관절 석회화 건염 1예를 보고하고자 한다. 대상 및 방법: 만성적인 좌측 견관절 동통의 급성 악화와 운동 제한을 주소로 내원한 59세 여자 환자로 단순 방사선 상 견봉하 석회 침착과 자기 공명 영상 상 감염이나 종양과 유사하게 골 피질을 침범하고 석회 물질이 골 내부로 연장되어 있었다. 관절경하 석회 제거술과 골내 석회의 소파술, 건 결손부에 봉합 나사못을 이용한 회전근 개 봉합술을 시행하였다. 결과: 환자의 급성 견관절 통증은 술 후 즉각적인 호전을 보였고 순조로운 재활 경과와 관절 운동범위의 회복을 보였다. 술 후 5개월에 촬영한 단순 방사선 상 석회 침착의 재발은 관찰되지 않았다. 결론: 견관절 석회화 건염은 주위 골조직으로 연장된 다양한 영상 소견을 나타 낼 수 있으며, 질환을 정확히 판별함으로써 불필요한 검사나 치료를 피할 수 있을 것으로 생각된다.

기동부자유(起動不自由) 아동(兒童)을 위한 환자복(患者服) 디자인 (Study on the Design of the Patient's Clothes for a disabled Child)

  • The purpose of this study is to propose the design of the patient's clothes for a disabled child between 4 and 6. For this, in this study, I would present the following design plan which includes 3 upper garments, 2 trousers and a one-piece dress. In case of the damage on head and neck, a neck line should be deep and wide thereby providing ease to the clothes. To facilitate injection and dressing/undressing, there should be parting from neck to cuff which can be fixed by snap or button. By rolling up sleeves with strings inside the cuffs and fixating plaster cast with snap button outside the cuffs. In case of body cast with plaster cast around body part, front adjusting part should be wider and the width should be adjusted with strings. In case of hip spica cast which covers waist, I separated front part and rear part, fixated them with snap buttons, gave more width to front adjusting part with strings to adjust width, which constitute one-piece dress. In case of shoulder spica cast, the other shoulder which is not covered with plaster cast should be exposed, and to prevent clothes' coming down, strings with snap button or velcro should be attached over a shoulder so that the length of the strings can be adjusted. In case of applying plaster cast or aid to the whole part of a leg, one part of trousers should be shorter so as to expose the injured part and there should be a parting with strings or velcros on the side for easy dressing/undressing. When plaster cast or splint is short, The strings are meant to adjust length of trousers. The partings are located 2 cm from side lines toward the center.

Vascular endothelial dysfunction after anthracyclines treatment in children with acute lymphoblastic leukemia

  • Purpose: Anthracyclines have been utilized in the treatment of children with acute lymphoblastic leukemia (ALL). Recent studies have shown that anthracyclines may induce toxicity in the vascular endothelium. This study was performed using brachial artery reactivity (BAR) to evaluate vascular endothelial function in ALL patients who were treated with anthracycline chemotherapy. Methods: We included 21 children with ALL who received anthracycline chemotherapy and 20 healthy children. The cumulative dose of anthracyclines in the ALL patients was $142.5{\pm}18.2/m^2$. The last anthracycline dose was administered to the patients 2 to 85 months prior to their examination using BAR. The diameter of the brachial artery was measured in both groups using echocardiography, and BAR was calculated as the percentage change in the arterial diameter after release of the cuff relative to the baseline vessel diameter. Results: In the anthracycline-treated group, BAR was observed to be $3.4%{\pm}3.9%$, which was significantly lower than that observed in the control group ($12.1%{\pm}8.0%$, P<0.05). The time elapsed after the last anthracycline treatment and the age at the time of treatment did not affect the change in BAR (P =0.06 and P =0.13, respectively). Conclusion: These results provided evidence that treatment of ALL patients with anthracycline results in endothelial dysfunction. A larger cohort study and a longer follow-up period will be required to clarify the relationship between endothelial dysfunction resulting from anthracycline treatment for childhood ALL and occurrence of cardiovascular diseases later in life.

자동혈압계 성능평가를 위한 인체혈압 시뮬레이터 개발 (A Simulator for the Validation of Non-invasive Blood Pressure (NIBP) Monitoring Devices)

  • Blood pressure is one of the important vital signs for monitoring the medical condition of a patient. Automated NIBP(non-invasive blood pressure) monitoring devices calculate systolic and diastolic blood pressures from the oscillation in cuff pressure caused by a pulsation of an artery. To validate the NIBP devices, we developed a simulator to supply the oscillometric waveforms obtained from human subjects. The simulator provided pressure pulses to device-under-test and device readings were compared to the auscultatory references. Fully automated simulation system including OCR(optical character recognition) were developed and used for NIBP monitoring devices. The validation results using the simulator agreed well with previous clinical validation. More validation studies using the standardized oscillometric waveforms would be required for the replacement of clinical trials to validate a new automated NIBP monitoring device.

초음파로 조기 진단한 비전위 견봉 골절 (Non-displaced Acromial Fracture diagnosed early by the Ultrasonography)

  • 대부분의 견봉 골절은 비전위 혹은 최소 전위된 형태이며, 그 빈도가 낮아서 진단이 늦어지는 경우가 많다. 떨어진 파이프에 맞은 뒤, 74세 남자에게 발생한 견봉 골절을 초음파로 조기 진단할 수 있었던 증례를 보고한다. 본 증례의 경우 좌견관절의 직접 손상 후 내원 시 좌측 견관절의 동통 및 부종, 능동 거상의 제한으로 회전근 개 및 이두건 장두의 손상에 대한 감별진단이 필요한 상태였으나 추가적인 검사 이전에 간편한 초음파 검사로 비전위 견봉 골절을 조기에 진단할 수 있었다. 초음파 검사는 견관절부의 건 손상을 동시에 확인할 수 있다는 면에서 컴퓨터 단층촬영보다 장점이 있다고 생각된다.

선천성 고혈압 렛드에서 혈압 및 내피의 기능장해에 대한 protopanaxatriol계 배당체의 효과 (Effect of Protopanaxatriol Ginsenosides on the Blood Pressure and Endothelial Dysfunction In the Aorta of Spontaneously Hypertensive Rats)

  • Chronic hypertension is associated with impaired endothelial function such as reduced synthesis/release of endothelium-derived relaxing factor(EDRF, nitric oxide) and increased synthesis/release of endothelium-derived contracting factor(EDCF) including prostaglandin endoperoxide($PGH_2$) , superoxide anion both in animals and in humans. We have previously shown that ginsenosides lower the blood pressure and enhance the release of nitric oxide(NO) from endothelial cells in the rat aorta of the normotensive rats. The aim of the present study is to examine whether in vivo treatment of spontaneously hypertensive rats(SHRs) with protopanaxatriol ginsenosides(PPT) reduces the blood pressure and improves endothelial function in the isolated thoracic aorta of SHR. In addition, the contractile response to $PGH_2$ and superoxide anion in the aorta treated with PPT was assessed. SHRs at the age of 16 weeks were savaged with PPT(30 mg/kg/ day) for 2 weeks and systolic blood pressure was measured by the tail-cuff method. Whereas blood pressure was significantly increased in SHRs by 5.4 mmHg during this period of treatment, treatment of SHRs with PPT blocked the elevation of blood pressure. Endothelium-dependent relaxation to acetylcholine was significantly increased in the PPT-treated animals. $PGH_2$- and oxygen-derived free radical-induced contractions were significantly suppressed in aortic rings without endothelium from PPT-treated SHR. These findings indicate that PPT reduces the blood pressure of SHR, which may be associated with either increase of NO release or by antagonizing superoxide anion and PGH2 in the aortic smooth muscle.

회전근개 질환에 사용되는 근위혈들의 자침 심도에 대한 연구 (The Study of Depth of Local Acupuncture Points for Rotator Cuff Disorders)

  • 목적 : 견관절 질환의 대부분의 원인을 차지하는 극상근건과 견봉하 점액낭 질환 침치료에 있어서 근위취혈의 관점으로 접근했을 때 손상된 조직인 극상근건과 견봉하 점액낭에 자입하는 것이 중요하다. 어떻게 이 조직에 접근할 것인가에 대해 X선 촬영을 통해 확인해 보고자 한다. 방법 : 4명의 지원자를 받아서 견우혈(LI 15), 거골혈(巨骨穴)(LI 16), 노상혈(UE 57)의 혈자리에 투시촬영을 이용해 자침을 하였다. 투시촬영으로 원하는 조직에 침이 자입되었음을 확인한 후 X선 촬영을 하여 확인하는 방법으로 진행하였다. 결과 : 견우혈(LI 15) 자침시(刺針時) 5cm의 깊이로 직자(直刺)할 때 원하는 조직인 근상근건과 견봉하 점액낭에 도달하는 것을 확인할 수 있었다. 거골혈(巨骨穴)(LI 16) 자침시(刺針時) 5cm의 깊이로 노상혈 부위로 하사자(下斜刺) 했을 때, 노상혈(UE 57) 자침시(刺針時) 5cm의 깊이로 거골혈(巨骨穴) 부위로 상사자(上斜刺) 했을 때 극상근건과 견봉하 점액낭에 도달함을 확인할 수 있었다. 결론 : 극상근건과 견봉하 점액낭 질환을 근위취혈의 관점으로 치료하고자 할 때 견우혈, 거골혈(巨骨穴), 노상혈에 정확한 깊이와 방향으로 자입을 해야 원하는 조직인 극상근건과 견봉하 점액낭에 자입할 수 있음을 확인할 수 있었다.

Effect of Horizontal Adduction Force on Infraspinatus and Deltoid Activities During the Side-Lying Wiper Exercise Using Pressure Biofeedback

  • Background: Shoulder external rotation exercises are commonly used to improve the stabilizing ability of the infraspinatus. Although the side-lying wiper exercise (SWE) is the most effective shoulder external rotation exercise to maximize infraspinatus activity, the effect of adduction force on the infraspinatus and posterior deltoid has not been demonstrated. Objects: This study was conducted to investigate whether horizontal adduction force increases infraspinatus activity and decreases posterior deltoid activity. Methods: Twenty-eight healthy subjects (male: 21, female: 7; $age=23.5{\pm}1.8years$; $height=170.1{\pm}7.4cm$; $weight=69.4{\pm}9.6kg$) were recruited. Subjects were asked to perform the SWE under two conditions: (1) general SWE and (2) SWE with adduction force using pressure biofeedback. Surface electromyography (EMG) signals of the infraspinatus and posterior deltoid were recorded during SWE. Paired t-tests were used to compare the EMG activity of the infraspinatus and posterior deltoid between the two conditions. Results: Posterior deltoid muscle activity was significantly decreased following SWE with adduction force ($7.53{\pm}4.52%$) relative to general SWE ($11.68{\pm}8.42%$) (p<.05). However, there was no significant difference in the infraspinatus muscle activity between the SWE with adduction force ($28.33{\pm}12.16%$) and the general SWE ($26.54{\pm}13.69%$) (p>.05). Conclusion: Horizontal adduction force while performing SWE is effective at decreasing posterior deltoid activity.

이매방 살풀이춤[중요무형문화재 제 97호] 복식 연구 (A Study on the Yi Mae-Bang's Salpurichum Costume)

  • It goes without saying that Yi Mae-Bang, as the sole owner of two of the intangible cultural assets including No. 27 Seungmu in 1987 and No. 97 Salpuri Chum, is a top-notch Korean traditional dancer. Moreover, in regards to traditional clothing, he is also known as the only traditional dancer who directly performs on stage as well as analyzes traditional clothing in order to apply them into his own dance performances by designing and producing them to meet his artistic spirit. In order to examine how Yi Mae-Bang's unique and creative stage clothing was developed, and what kind of process it went through to possess its unique style, which draws the attentions of people with its harmony of beauty and dance, this research first examined the change of the composition through the yearly picture data. Before 1984, the composition of the Salpuri Chum clothes changed by putting on 'Jeogori' on the 'Mudong-Bok' without the 'Kweia' (sleeveless), and in 1999 'Mudong-Bok' became widened with the decoration of embroidered hem making it more attractively colorful. after 2000 it became more splendidly adorned by embroidering hem on widened Mudong-Bok and tucking 'Kweja' in the layers which created silhouette like a dress that is seemingly wider and luxuriant than a skirt. One of notable features of Yi Mae-Bang's Salpuri is that its cloth and cuff ribbons are lightly colored in different colors enclosing the tip part. As for the ribbons, it was also changed as time went by from direct-cutting of the cloth just like the A-line of Mudong-Bok to drawing a diagonal line less than 5cm above the ribbon and 8cm below the ribbon so that it takes the form of getting widened as it gets to the lower part, and its length was also elongated in proportion to that of the bottom of Mudong-Bok.

국내 아웃도어 브랜드 등산복 재킷의 기능별 디자인 특성 (The Characteristic of a Functional Design for a Commercial Outdoor-Wear Jacket)

  • The objective of this study is to analyze the characteristic of a functional design for a commercial outdoor brand jacket based on 3 types of functions : windproof, waterproof, and cold proof. To analyze the design and fabric characteristics of outdoor jackets by function, 903 men's and women's outdoor jacket images were selected from the catalogues during S/S 2009 - F/W 2011 of the 6 brands: The North Face, Columbia Sports Wear, Kolon Sports, K2, Arcteryx and Mont-Bell. These brands were selected based on their high market share and brand awareness of the outdoor brands in Korea. In this study, design elements analysis was conducted as following 8 elements: hood/collar height/central front flap/waistline/pocket location/ventilation for the body part/sleeve patterns/cuff for the sleeve part. Fabrics were analyzed by the frequency of fabric types used in the 3 jacket types. According to the result derived from the analysis of the six brands of outdoor wear jacket design trends by year, detachable hood and a co-use of velcro and elastic bands have been mainly designed for all of windproof, waterproof, and cold proof jacket to all in order to respond quickly to changes in the external environment. For reinforced storage function on trekking, the number of pockets has been gradually growing, and a function was being developed to connect an electronic device to a pocket. The method, result, and collateral observations relevant to each of these phases are described. Outdoor jacket design characteristics were investigated for outdoor brands, which can provide data to suggest more accurate functional design, and satisfy functional design needs. In this regard, this research was significant in the sense that it suggested preliminary data to reconsider competitiveness of the brand products.
