Background: Postoperative adjuvant radiotherapy (RT) in the management of early stage endometrial cancer (EC) is still controversial. Here we report our institutional experience with patients who received postoperative RT for stage I-II EC over a period of 35 years and assess potential predictors of local recurrence (LR), distant metastasis (DM), and overall survival (OS). Materials and Methods: A total of 188 patients undergoing postoperative RT for stage IA-II EC between 1977 and 2012 were evaluated. Some 96 received median 46 Gy whole pelvic radiotherapy (WPRT) (range: 40-60 Gy), 37 were given WPRT with vaginal cuff therapy (VCT), and 55 received only VCT either with brachytherapy (BT) or stereotactic body radiotherapy (SBRT). Chemotherapy was given to 5 patients with uterine papillary serous carcinoma (UPSC). Logistic regression analysis was used to assess the effect of clinicopathological factors on LR, DM, and OS. Results: Median follow-up time was 11 years (range: 1-35 years). At the time of analysis, 34 patients were not alive. Of the 15 patients with LR, 7 (46.7%) recurred in the vaginal stump, 5 (33.3%) in the pelvic region, and 3 (20%) in the paraaortic nodal region, while 12 had distant metastasis. UPSC histology (p=0.027), sole VCT (p=0.041), high histologic grade (p=0.034), and age ${\geq}71$ (p=0.04) were poor prognostic factors on univariate analysis. Conclusions: In our patients receiving radiotherapy for early-stage EC, grade III disease and age ${\geq}71$ were associated with shorter OS whereas UPSC histology was an independent predictor for both LR and DM.
Purpose: To investigate the arthroscopic findings, and to evaluate the clinical outcomes of the treatment of posterior internal impingement of the shoulder in baseball players. Materials and Methods: We followed up 5 cases who were diagnosed as posterior internal impingement for the mean 15 months. All of the cases complained of the pain in the posterior shoulder at late cocking, and were positive in the relocation test added by hyper-horizontal abduction at $120^{\circ}$ abduction of the arm. We sutured posterior labral tear and SLAP lesion arthroscopically, and conducted debridement for rotator cuff. Three cases were performed of anterior capsular plication and the other two were performed of thermal capsular shrinkage. Pain, range of motion, and level of return to sports activity were assessed for the results. Results: As to the arthroscopic findings, all the five cases showed the fraying in posterosuperior labrum, and two of them was accompanied with the flap tear in posterosuperior labrum and the other one was accompanied with type 2 SLAP lesion. All the cases showed the fraying in supraspinatus, and one case showed partial tear. Meanwhile, in all the cases, the rotator cuff was impinged to the labrum at $90^{\circ}{\sim}120^{\circ}$ of abduction and external rotation. As to the postoperative results, all the cases did not complain of pain or instability while pitching, and the competition was recovered to be the mean 88%($80{\sim}100%$) of that before the injury. Conclusion: Definite diagnosis for the posterior internal impingement would be possible through arthroscopic examination. Favorable outcomes could be obtained with capsular plication or shrinkage for anterior microinstability and stretching exercise for posterior capsule tightness inducing the internal impingement.

In this paper describe about a algorithm it control and to determinate the cuff pressure, and filtering that data for measure the blood pressure. The communicating with personal computer can pressure deflation is by Solenoid valve and it uses RS-232 system in packet communication. The main using algorithm for blood pressure measurements are maximum amplitude algorithm and oscillometric algorithm. MAA(maximum amplitude algorithm) has various measured oscillation it depend on patient's age, height, weight and arm circumference size. In this paper, 1 studied the various measured oscillation apply to characteristic ratio and can get the result of systolic blood pressure, diastolic blood pressure, mean blood pressure. It was not used same ratio to measuring oscillation. In the MAA(maximum amplitude algorithm), we hope for reduce the difference with the real blood pressure and the measured blood pressure, when it applied with various specific ratio.
The purpose of this study was to evaluate the effect of multiaxial lower extremity orthosis on correction of genu valgum. 20 volunteers among people visited department of Rehabilitation Medicine, chungnam national university hospital, who had been diagnosed as genu valgum without other musculoskeletal problems were included. 10 individuals(mean age: 9.gyrs) who had been taken multiaxial lower extremity orthosis at least 12month were included in experimental group and the other 10 individuals(mean age: 11.7yrs) refused taking this orthosis in the control group. We measured the Q-angle & femorotibial angle using plain roentgenogram images at visiting day and repeat same test after 1year follow up. Multiaxial lower extremity orthosis consist of proximal horizontal bar with both thigh cuff, central vertical bar and distal horizontal bar with both shoes. we narrowed inter-shoes distance from start to 6th month and inter-thigh cuff distance together with above correction for next 6month in the frontal plane and from 10th month, dorsiflexed both shoes in sagittal plane. Also, we rotate the both shoes externally and retract the proximal vertical bar every month. This orthosis have to be taken at least 4 hours during sleep. The result were as follows 1. There were no statistical significant difference in each parameter between the right and left Q-angle before multiaxial lower extremity orthosis. 2. The left Q angle reduced $-11^{\circ}$ between 1st day and after 1year follow up showed statistical significant difference between multiaxial lower extremity orthosis taking group and non-taking(p<0.001). 3. The right Q angle reduced $-13^{\circ}$ between 1st day and after 1year follow up showed statistical significant difference between multiaxial lower extremity orthosis taking group and non-taking(p<0.001). 4. There were no statistical significant difference in each parameter between the right and left femorotibial angle before multiaxial lower extremity orthosis. 5. The left femorotibial angle reduced $-10.1^{\circ}$ between 1st day and after lyear follow up showed statistical significant difference between multiaxial lower extremity orthosis taking group and non-taking(p<0.001). 6. The right femorotibial angle reduced $-11.2^{\circ}$ between 1st day and after 1year follow up showed statistical significant difference between multiaxial lower extremity orthosis taking group and non-taking(p<0.001).

The electromyographic activity of four muscles(biceps, supraspinatus, infraspinatus and subscapularis) was measured from non-dominant shoulders of 12 volunteers by six different provocative test for the biceps pathology. The provocative tests were Speed, Yergason, Ludington, Heuter, O'Brien and the abduction-extension test. Each test was performed in a force of 30% of maximal voluntary contraction. The levels of activity of the biceps were higher than those of the other rotator cuff muscles only in Speed's test: 28% in the biceps, 26% in the infraspinatus, 25% in the supraspinatus and 21 % in the subscapularis. The levels of activity of the biceps as a percent of MMT(maximal manual test) were higher in Speed's(42%) and O'Brien's test with the arm supinatecl(42%). Speed's test can isolate the activity of biceps better than the other tests but it is a nonspecific test by which the biceps tendon was also activated within other rotator cuff muscles.
The purpose of this study was to investigate the variations in gait parameters in terms of the type of arm sling used in hemiplegic patients. Ten patients with hemiplegia and ten healthy adults participated in this study and walked at self-selected speeds on a GAITRite-instrumented carpet. The activities of the opposite shoulder girdle muscle including the latissimus dorsi, anterior deltoid, and posterior deltoid were simultaneously recorded using surface EMG during gait. They were randomly assigned a condition: without an arm sling, a single strap arm sling, a Harris hemi arm sling, a Rolyan humeral cuff arm sling, and a Bobath roll arm sling. The following gait variables were analyzed: the temporo-spatial parameters of velocity, step length, stride length, swing phase, stance phase, single support, step time and toe in/toe out. The statistical analysis was one-way ANOVA with repeated measures to compare the variation of each variable. In comparison of parameters in each trial in the hemiplegia group, the non-affected side stride length, single support, and toe in/toe out resulted in statistically significantly changes (p<.05). But without an arm sling group did not show any gait parameter differences with arm slings. This study found that several arm slings varied gait patterns in patients with hemiplegia and in healthy adults. In the EMG analysis, the Rolyan humeral cuff arm sling and the Bobath roll arm sling were higher muscle activity for the latissimus dorsi muscle than did the single strap ann sling. Further study should examine the problems that appeared in patients who worn arm slings by focusing on a larger number of subjects and by studying the variety of responses in more detail using an assessment tool that measures variation.

Blood pressure (BP) is one of the important physiological parameters for diagnosing cardiovascula diseases by means of noninvasive method. Existing noninvasive methods for measuring arterial BP have to use cuff and difficult in measuring arterial BP continuously. Systolic blood pressure (SBP) and pulse transit time (PTT) have a kind of inverse relationship. We acquired PTT data when subjects were in relaxation and also after exercise. We performed the linear regression analysis for making the regression equations for each subject and the regression equation for all subjects. We compared the estimated SBP with the measured SBP to check the accuracy of our regression equations. From the result, the regression equations for each subject was appropriate according to the American National Standards Institute of the Association of the Advancement of Medical Instrument (ANSI/AAMI) which says that BP devices should have ±5mmHg mean of error and 8mmHg standard deviation of error. However, the regression equation for all subjects was not proper to ANSI/AAMI recommendation. The result means that, without cuff, we can continuously estimate each subject's SBP through PTT and indivisual calibration.
Purpose: This study compared the SLAP lesions caused by a sports-induced injury with those caused by a non-sports-induced injury. Materials and Methods: The study was performed on 54 patients who had undergone arthroscopic surgery for a SLAP lesion. There were 21 sports-induced-injury patients (group I) and 36 non-sports-induced injury patients (group II). The mean age of the men was 36 years and that of the women was 48 years. In both groups, the frequency of a concomitant injury and the clinical outcomes at the last follow-up was evaluated using the UCLA score, Rowe score, and the ASES score. Results: According to their injury mechanism, , there were 14 cases (67%) of repeated microtrauma injury in group I and 25 cases (75%) of compression injury type in group II. As a concomitant pathology, there was 11 cases of shoulder instability and 5 cases of a rotator cuff tear in group I, and 23 cases of rotator cuff tears and 14 cases of shoulder instability in group II. At the last follow up, group I showed slightly better clinical satisfaction (P>0.05). Conclusion: The possibility of a SLAP lesion accompanying other diseases is high. Therefore, an accurate assessment of concomitant injury lesions before surgery is important for the treatment outcome.

Background: The currently available reverse shoulder arthroplasty (RSA) designs can be classified into medial glenoid/medial humerus (MGMH), lateral glenoid/medial humerus (LGMH), and medial glenoid/lateral humerus (MGLH) prosthesis designs. The purpose of this study was to radiologically analyze the effect of different RSA designs on humeral position following RSA. Methods: A total of 50 patients who underwent primary RSA were retrospectively analyzed. Among 50 patients, 33 patients (group A: MGMH) underwent RSA with Aequalis system (Wright, Inc, Bloomington, MN, USA), 6 (group B: LGMH) with Aequalis system using bony increased offset, and 11 (group C: MGLH) with Aequalis Ascend Flex system. The acromiohumeral distance, acromioepiphyseal distance (AED), lateral humeral offset (LHO), LHO from the center of rotation ($LHO^{COR}$), and deltoid length were radiologically measured to quantify the distalization and lateralization of the humerus. Results: The increment in postoperative AED was $19.92{\pm}3.93mm$ in group A, $24.52{\pm}5.25mm$ in group B, and $25.97{\pm}5.29mm$ in group C, respectively (p=0.001). The increment in postoperative LHO was $0.13{\pm}6.30mm$, $8.00{\pm}12.14mm$, and $7.42{\pm}6.88mm$, respectively (p=0.005). The increment in postoperative $LHO^{COR}$ was $20.76{\pm}6.06mm$, $22.04{\pm}5.15mm$, and $28.11{\pm}4.14mm$, respectively (p=0.002). Conclusions: The radiologic analysis of the effect of different RSA designs on humeral position following RSA showed significant differences in the increment in postoperative AED, LHO, and $LHO^{COR}$ between the 3 groups. Therefore, MGLH design seems to be more effective for humeral distalization and lateralization compared to original Grammont design.
This study was performed to determine the effect of low-intensity resistance training with blood flow restriction (BFR) on muscle volume and strength in elderly women. Sixteen elderly women (70.9±4.6 years) were divided into low (30% 1RM) and high (75% 1RM) intense resistance training groups. Tourniquet cuff (Zimmer, Germany) for BFR was applied only to the right leg during the training period. All subjects performed unilateral leg press, leg extension and leg curl (3 sets×12 repetitions) for 10 weeks (2d/wk). Blood pressure was increased from 110 to 240 mmHg during the training period at the most proximal region of exercised leg. Muscle volume and cross-sectional area (CSA) were measured by MRI and body composition was monitored by dual-energy X-ray absorptiometry (DEXA) and isokinetic muscular strength were analyzed in both legs. The quadriceps CSA (15.2%, p<.001) and muscle volume (13.8%, p<.001) were increased in high-intense trained leg with BFR and the increased rate was highest among groups. The quadriceps CSA (9.8%, p<.001) and muscle volume (6.9%, p<.001) were increased in low-intensity training group with BFR and their increased rates were higher than control groups. The strength by exercise training was significantly improved in all groups and tended to be higher in BFR groups. These results demonstrate low-intensity resistance training with blood flow restriction could be an effective way to improve muscle volume and strength in elderly women.
