Treatment of Syringomyelia due to Chiari Type I Malformation with Syringo-Subarachnoid-Peritoneal Shunt

  • Chiari type I malformation is a tonsillar herniation more than 3 mm from the level of foramen magnum, with or without concurrent syringomyelia. Different surgical treatments have been developed for syringomyelia secondary to Chiari's malformations: craniovertebral decompression with or without plugging of the obex, syringo-subarachnoid, syringo-peritoneal, and theco-peritoneal shunt placement. Shunt placement procedures are useful for neurologically symptomatic large-sized syrinx. In this paper, authors define the first successful treatment of a patient with syringomyelia due to Chiari type I malformation using a pre-defined new technique of syringo-subarachnoid-peritoneal shunt with T-tube system.

두부전방전위자세에 자세교정법과 호흡보조근운동의 적용 시 CVA와 호흡의 변화 (Change of Craniovertebral Angle(CVA) and Respiration on Application Correction Method of Posture and Breathing Accessory Muscle Exercise in Forward Head Posture(FHP))

  • Purpose : This study was analyzed to researched the improvement of the posture and breathing ability on correction method of posture and breathing exercise in Forward Head Posture(FHP) Method : Eighten forward head posture subjects participated in this study. The control group applied to correction method of posture and the experiment group applied to correction method of posture with breathing exercises. Results : The results showed significant improvement in Craniovertebra-Angle on each two group(P<0.05). All the other result showed non-significant in respiratory(Tidal volume, Expiratory residual volume, Inspiratory residual volume) But value of result is slightly improved in after exercise. Conclusion : CVA angle is increased in each group but not increased between experiment group and control group. Total Volume(TV), Inspiratory Reserve Volume(IRV) and Expiratory Reserve Volume(ERV) are a little increased.

경부근육에 있어 두부전방자세와 압력 통증 역치와의 관계에 대한 연구 (The measurement of forward head posture and pressure pain threshold in neck muscle)

  • Poor posture of the neck and head long been recognized as a factor contributing to the onset and perpetuation of pain in the head and neck region. The purposes of the study were to evaluate the changes in forward head posture and pressure pain threshold in tension type headache group and control group. Forward head posture were measured to craniovertebral angle and cranial rotation angle. Craniovertebral angle was smaller in tension type headache group(p<0.05), and cranial rotation angle was larger in tension type headache group(p<0.05). Pressure pain threshold was similar between tension type headache group and control group with the exception of right suboccipitalis and left temporalis(p<0.05).

슈로스 운동이 시각장애학생의 시상면 자세 정렬에 미치는 효과 (The Effects of Schroth Exercise on Postural Alignment of Sagittal Plane in the Visually Impaired Students)

  • Background: This study was conducted to prove the effects of the Schroth exercise on sagittal postural alignment to the visually impaired students. Methods: The subjects were 8 students who attend the special school in Daegu and performed the Schroth exercise 3 times per week for 3 weeks. Craniovertebral angle and pelvic alignment were measured to compare the effects of the exercise. Results: Craniovertebral angle and pelvic alignment on sagittal plane showed significant differences (p<.05). Conclusions: Schroth exercise was effective to improve postural alignment of sagittal plane to the visually impaired students and these results will have a positive impact on future study of improvement of postural alignment on sagittal plane to the visually impaired students.

두개척추접합부 결핵 : 이비인후과적 관점에서의 증례 보고 (Craniovertebral Junction Tuberculosis : A Case Report in Otolaryngologic Aspect)

  • Craniovertebral junction (CVJ) tuberculosis is a rare disease, and potentially fatal due to the risk of atlantoaxial dislocation. The disease usually accompanies a retropharyngeal cold abscess, which can cause subsequential otolaryngologic symptoms such asdysphagia, odynophagia, or airway obstruction. Such a patient must be handled with great care to avoid a disaster. The disease is diagnosed with microbiological or other laboratory tests on the pus collected through puncture and aspiration, which will need otolaryngologic skills. For treatment, otolaryngologists play an important role by doing incision and drainage of the retropharyngeal abscess, or by attending the transoral vertebra surgery as partners with spine surgeons who will manage the CVJ lesions and ensure the craniocervical stability.

Effect of Joint Movement on CVA in Forward Head Posture

  • This study was conducted to examine the effect of joint mobilization on forward head posture and to propose an effective method to improve correct body posture and balance. A total of eight patients from a Maitland-applied group (n=8) received mobilization therapy to increase the mobility of the cervical joint, with Maitland's movement grade III intensity for 30 seconds of treatment and 30 seconds of rest, for ten intervals, three times a week for four weeks. The craniovertebral angle (CVA) changes before and after the intervention with the Maitland technique were measured as $56.85{\pm}2.31^{\circ}$ before, $63.23{\pm}2.23^{\circ}$ two weeks after, and $64.98{\pm}1.27^{\circ}$ four weeks after joint intervention. There were significant CVA changes before and after the Maitland technique (P <.05). The results of this study suggest that the Maitland technique is useful for improving the head vertebral angle in patients with forward head posture.

Tailored Surgical Approaches for Benign Craniovertebral Junction Tumors

  • Objective : We report our surgical experience in the treatment of 16 consecutive patients with benign craniovertebral junction (CVJ) tumor, observed from 2003 to 2008 at our department. Methods : We had treated 6 foramen magnum meningiomas, 6 cervicomedullary hemangioblastomas, 1 accessory nerve schwannoma, 1 hypoglossal nerve schwannoma, 1 C2 root schwannoma, and 1 cavernous hemangioma. Clinical results were evaluated by Karnofsky Performance Scale (KPS) and all patients underwent preoperative neuroradiological evaluation with computed tomography (CT) and magnetic resonance image (MRI). Angiography was performed in 15 patients and preoperative embolization was done in 2 patients. Results : Five far-lateral, 1 supracondylar and 10 midline suboccipital approaches were performed. Gross total removal was achieved in 15 cases (94%) and subtotal removal in 1 patient (6%). None of the patients required occipitocervical fusion. Radiological follow-up showed no recurrence in cases totally removed. Postoperative decrease of KPS scores was recorded in only 1 patient. The treatment of cervicomedullary solid hemangioblastoma presented particular issues : by preoperative embolization, we removed tumor totally without an excessive bleeding or brainstem injury. In one of foramen magnum meningioma, we carried out subtotal removal due to hard tumor consistency and encasement of neurovascular structures. Conclusion : The choice of surgical approaches and the extent of bone resection should be defined according to the location and size of individual tumors. Moreover, we emphasize that preoperative neuroradiological evaluations on presumptive tumor type could be helpful to the surgeon in tailoring the technique and providing the required exposure for different lesions, without unnecessary surgical steps.

동작관찰훈련을 통한 자세교육이 머리전방자세의 머리척추각과 머리회전각에 미치는 영향 (The Effect of Postural Training through Action Observation on Craniovertebral Angle and Cranial Rotation Angle of Forward Head Posture)

  • Purpose : The purpose of this study was to show the effect of postural training through action observation (AO) on craniovertebral angle (CVA) and cranial rotation angle (CRA) of forward head posture (FHP). Methods : From 16 subjects of having the FHP who consist of postural training through the AO (n = 8) and control group (n = 8) were training to three times per one week during three weeks. FHP measurements for pre and post the intervention use to Wiz-pacs(Wiz-Picture Achiving Communication System) from X-ray change of CVA and CRA. Results : The study for the change within the group pre and post the intervention, CVA and CRA were found the significant differences only in the postural training group through the AO (p<.05). In the comparison of the rates of change between the groups pre and post the intervention CVA and CRA all showed the significant differences (p<.05), and in the comparison of the rates of average change of individual variables in each groups, the postural training group through the AO showed more change rates. Conclusion : The study suggests that when people with FHP received the postural training, the postural training through the AO resulted in more change into the correct postures.

Surgical Treatment of Craniovertebral Junction Instability : Clinical Outcomes and Effectiveness in Personal Experience

  • Objective : Craniovertebral junction (CVJ) consists of the occipital bone that surrounds the foramen magnum, the atlas and the axis vertebrae. The mortality and morbidity is high for irreducible CVJ lesion with cervico-medullary compression. In a clinical retrospective study, the authors reviewed clinical and radiographic results of occipitocervical fusion using a various methods in 32 patients with CVJ instability. Methods : Thirty-two CVJ lesions (18 male and 14 female) were treated in our department for 12 years. Instability resulted from trauma (14 cases), rheumatoid arthritis (8 cases), assimilation of atlas (4 cases), tumor (2 cases), basilar invagination (2 cases) and miscellaneous (2 cases). Thirty-two patients were internally fixed with 7 anterior and posterior decompression with occipitocervical fusion, 15 posterior decompression and occipitocervical fusion with wire-rod, 5 C1-2 transarticular screw fixation, and 5 C1 lateral mass-C2 transpedicular screw. Outcome (mean follow-up period, 38 months) was based on clinical and radiographic review. The clinical outcome was assessed by Japanese Orthopedic Association (JOA) score. Results : Nine neurologically intact patients remained same after surgery. Among 23 patients with cervical myelopathy, clinical improvement was noted in 18 cases (78.3%). One patient died 2 months after the surgery because of pneumonia and sepsis. Fusion was achieved in 27 patients (93%) at last follow-up. No patient developed evidence of new, recurrent, or progressive instability. Conclusion : The authors conclude that early occipitocervical fusion to be recommended in case of reducible CVJ lesion and the appropriate decompression and occipitocervical fusion are recommended in case of irreducible craniovertebral junction lesion.

  • 본 연구는 경추 및 흉추부 스트레칭과 근력강화 운동프로그램이 머리전방자세에 미치는 효과를 알아보고자 연구하였다. 머리전방자세를 갖고 있는 일반인 32명을 대상자로 자세교육과 운동프로그램을 적용한 실험군 16명과 자세교육만을 받은 대조군 16명을 대상으로 시행하였다. 두 개척추각(CVA:Craniovertebral Angle)과 두개회전각(CRA:Cranial Rotation Angle)을 X-ray로 검사하고, PACS(Picture Archiving and Communications System) ${\pi}view^{TM}$ 각도를 측정하였다. 연구결과 실험군은 두개척추각과 두개회전각이 통계적으로 유의한 차이가 있었다. 그러나 대조군은 두개회전각에서만 유의한 차이가 있었다. 또한, 실험 전 그룹 간 차이는 없었으나 실험 후 그룹 간 비교에서는 통계적으로 유의한 차이가 있었다. 이 연구에서는 경, 흉추부 스트레칭 및 근력강화 운동프로그램이 연구대상자들의 머리전방자세를 감소시키는 척추자세정렬을 향상시킬 수 있었다. 이 연구 결과들은 자세증진 치료들에 있어서 향후 발전되는 운동프로 그램을 위해 기초자료가 될 수 있다.
