한국재래산양 머리뼈에 대한 두개계측학적 연구 (Craniometric studies on the skull of Korean native goat)

  • This study was carried out to identify the craniometric characteristics of the skull of Korean native goat. The results were as follows; The skull index, cranial index, facial index, right orbital index and left orbital index were $55.86{\pm}2.14$, $59.97{\pm}3.68$, $107.03{\pm}5.71$, $92.22{\pm}4.54$and $90.47{\pm}5.48$, respectively. The ratio of facial length to cranial length was 1:1.15 and the ratio of length of cranial base to palatal length was 1:1.35. The skull length was more correlated to the facial length than to the cranial length(p<0.01). The skull width was more correlated to the width between foramina supraorbitales than to the width between foramina infraorbitales. It also showed negative correlation to the medial width between bases of processus cornualis but positive correlation to the lateral width between bases of processus cornualis(p<0.01). The width between tips of both horns showed high positive correlation to the medial width between processus cornualis, but negative correlation to the lateral width(p<0.01).

  • This study was performed to report the effectiveness of upper cervical chuna and release of cranial base on patients with dry eye syndrome. The patients diagnosed as dry eye syndrome and treated with upper cervical chuna and release of cranial base. Outcomes were measured by ocular surface disease index(OSDI). After treatments, discomfort and ocular surface disease index decreased. These results suggest that upper cervical chuna and release of cranial base were effective on patients with dry eye syndrome.

Effectiveness of Helmet Cranial Remodeling in Older Infants with Positional Plagiocephaly

  • Background: Management of positional plagiocephaly by wearing a cranial molding helmet has become a matter of growing medical interest. Some research studies reported that starting helmet therapy early (age 5 to 6 months) is important and leads to a significantly better outcome in a shorter treatment time. The aim of the present study was to evaluate the effectiveness of cranial remodeling treatment with wearing helmet for older infants (${\geq}18$ months). Methods: We conducted a retrospective study of 27 infants with positional plagiocephaly without synostosis, who were started from 2008 to 2012. Every child underwent a computerized tomography (CT) before starting helmet therapy to exclude synostosis of the cranial sutures and had CT performed once again after satisfactory completion of therapy. Anthropometric measurements were taken on using spreading calipers in every child. The treatment effect was compared using cranial vault asymmetry (CVA) and the cranial vault asymmetry index (CVAI), which were obtained from diagonal measurements before and after therapy. Results: The discrepancy of CVA and CVAI of all the patients significantly decreased after cranial molding helmet treatment in older infants (${\geq}18$ months) 7.6 mm from 15.6 mm to 8 mm and 4.51% from 9.42% to 4.91%. Six patients had confirmed successful outcome, and all subjects were good compliance patients. The treatment lasted an average of 16.4 months, was well tolerated, and had no complication. Additionally, the rate of the successful treatment (final CVA ${\leq}5mm$) significantly decreased when the wearing time per was shorter. Conclusion: This study showed that treatment by cranial remodeling orthosis was effective if the patient could wear the helmet longer and treatment duration was somewhat longer than in younger patients, well tolerated in older infants and had no morbidity. This therapeutic option is available and indicated in these older infants before other cranial remodeling surgery.

정상견에서 전방 췌장십이장 동맥의 초음파학적 혈관 저항지수 (Ultrasonographic Resistive Index of the Cranial Pancreaticoduodenal Artery in Normal Conscious Dogs)

  • 임상적으로 건강한 비글견 17두와 췌장염으로 진단된 5두(요크셔테리어 3두, 토이푸들 2두)의 견에서, 화학보정 없이 전방 십이지장동맥(cranial pancreaticodudenal artery: cPDA) 혈관저항지수(resitive index: RI)를 측정하고 이를 비교 평가하였다. RI 측정을 위해 이중 펄스 색도플러 초음파와 11-MHz 선형 위상차 배열 탐촉자를 이용하였다. 췌장염견(0.75$\pm$0.04)견의 평균 RI는 정상견(0.63$\pm$0.04)과 비교하여 유의성(t = 5.79, p = 0.001)있게 증가된 소견을 보였다. 췌장염에 이환 또는 의증 환축에서는 cPDA의 RI 평가 또한 보조적 진단 가치가 있음을 확인하였다.

The Jugular Foramen Schwannomas : Review of the Large Surgical Series

  • Objective: Jugular foramen schwannomas are uncommon pathological conditions. This article is constituted for screening these tumors in a wide perspective. Materials: One-hundred-and-ninty-nine patients published in 19 articles between 1984 to 2007 years was collected from Medline/Index Medicus. Results: The series consist of 83 male and 98 female. The mean age of 199 operated patients was 40.4 years. The lesion located on the right side in 32 patients and on the left side in 60 patients. The most common presenting clinical symptoms were hearing loss, tinnitus, disphagia, ataxia, and hoarseness. Complete tumor removal was achieved in 159 patients. In fourteen patients tumor reappeared unexpectedly. The tumor was thought to originate from the glossopharyngeal nerve in forty seven cases; vagal nerve in twenty six cases; and cranial accessory nerve in eleven cases. The most common postoperative complications were lower cranial nerve palsy and facial nerve palsy. Cerebrospinal fluid leakage, meningitis, aspiration pneumonia and mastoiditis were seen as other complications. Conclusion: This review shows that jugular foramen schwannomas still have prominently high morbidity and those complications caused by postoperative lower cranial nerve injury are life threat.

Efficacy of Cranial Orthosis for Plagiocephaly Based on 2D and 3D Evaluation

  • Background With the advent of cranial orthoses as therapeutic medical devices for the treatment of severe positional head deformities in Japan, an increasing number of patients are being treated with them. However, assessing the effectiveness of a treatment is often difficult due to the use of different metrics. This study aimed to evaluate the effectiveness of cranial orthoses for deformational plagiocephaly using two- (2D) and three-dimensional (3D) evaluation metrics. Methods We conducted a retrospective study of infant patients with deformational plagiocephaly who underwent cranial orthosis treatment. We evaluated the severity of deformational plagiocephaly using cranial asymmetry (CA) and the cranial vault asymmetry index (CVAI) as 2D metrics, and anterior and posterior symmetry ratios as 3D metrics. The patients were divided into 24 subgroups based on the initial severity of each outcome and their age at the start of treatment. We analyzed the changes in outcomes and correlations within improvements across the age and severity categories. Results Overall, 1,038 infants were included in this study. The mean CA, CVAI, and anterior and posterior symmetry ratios improved significantly after cranial orthosis treatment. The improvement in each score was greater in patients with more severe initial deformities and in those who underwent treatment at a younger age. Conclusion Cranial orthosis treatment was effective in correcting deformational plagiocephaly in infants, as demonstrated by improvements in both 2D and 3D metrics. Patients with more severe initial deformities and those who underwent treatment at a younger age showed greater improvement.

머리전방자세에 따른 상체의 생체역학적 상관분석 (The Biomechanical Correlation Analysis of Upper Body according to Forward Head Posture)

  • Background: The purpose of this study is to analysis of correlation upper body according to forward head posture. Methods: The subjects of this study were 40 female university students who were equally and randomly allocated to a forward head posture group, normal group. Using general posture system, electromyograph, visual analogue scale, tape measurement, neck disability index were evaluated. Results: There was positive correlation between posture analysis and Sternocleidomastoid, neck flexion (p<.05). There was positive correlation between Craniovertebral angle (CVA) and trapezius upper, VAS (p<.05). There was negative correlation between posture analysis and CVA (p<.05). There was negative correlation between Cranial rotation angle and CVA (p<.05). Conclusion: Increased forward head posture lead to increase of pain, muscles activity, so it suggests to be necessary on the prevention of dysfunction and limited activities daily living.

사두증 환자를 위한 맞춤형 헬멧 몰드 디자인 시스템 개발 (Development of a Customized Helmet Design System for Patients with Plagiocephaly)

  • This study developed a three-dimensional helmet mold design software that can design helmets for treating the infant plagiocephaly (flat head syndrome) using three-dimensional head scan data. For this, the three-dimensional head data of sixth SizeKorea body measurement project as well as the data produced by a head modeling software were used to prepare the head shape data of plagiocephaly patients. A total of 14 landmarks and 10 dimensions of heads required for helmet mold shape design and plagiocephaly diagnosis were automatically measured using an anthropometric analysis software. Using the software developed in this study, plagiocephaly can be diagnosed not only visually by three-dimensional head data visualization but also quantitatively by calculating the medically defined indices such as cranial index, which measures the proportions of the head, and the cranial vault asymmetry index, which measures the asymmetry of the head. The basic shape of the helmet mold was automatically generated based on the head scan data. Additionally, it is possible to fine tune the shape of the mold to reflect individual characteristics by using a free form deformation technique. Finally, the designed helmet mold was converted into the data that can be printed on a three-dimensional printer for generating the actual prototype.

한국인 성인 안면두개골의 형태적 분류와 특징두부방사선 계측학적 연구 (CRANIOFACIAL MORPHOLOGIC PATTERNS RELAYED TO DIFFERENT FACIAL TYPES IN KOREAN ADULTS : A CEPHALOMETRIC ANALYSIS)

  • The purpose of the present study was to investigate craniofacial patterns in Korean male and female adults, and to compare morphologic differences between different facial types. In order to get configurational groupings, standardized lateral and P-A cephalograms of 174 Korean adults were measured and analyzed with seven angular, nineteen linear, four ratio, and two index measurements. Ward's minimum variance cluster analysis was employed to divide the sample into groups having similar craniofacial morphology, and thereafter, inductive statistics(t-test) was used to characterize morphologic differences of the divided groups. And also, Pearson correlation coefficients were calculated to assess correlation between the cranial dimensions and upper/lower anterior facial height(UAFH, LAFH) in each facial types for both male and female The results were as follows; 1. There existed sexual dimorphism in most of variables except SN-FH angle, palatal plane angle, facial axis angle, facial index, lower anterior facial height ratio and upper facial height ratio. 2. A brachycephalic headform ($0.84{\pm}0.05$) with leptoprosopic facial form ($0.92{\pm}0.05$) was appeared in male, and a mesocephalic headform ($0.86{\pm}0.06$) with leptoprosopic facial form ($0.92{\pm}0.04$) was exhibited in female. 3. Facial types in both male and female subjects were divided into two groups, Group A (mesoprosopic facial type) and Group B (leptoprosopic facial type). The morphologic differences between Group A and Group B were as follows: a. Cranial measurements including index, angular and linear measurements were not found to be different between Group A and Group B in both sexes. b. Increased UAFH, LAFH, and upper/lower anterior dental heights were exhibited in Group Bs of both male and female. c. In both male and female, ramus height and mandible length showed no difference between Group A and Group B. However, genial angle was found to be larger in Group B than Group A. Therefore, the morphologic differences between two groups in male and female were closely related to less favorable anatomic morphology of the mandible. 4. LAFH and UAFH showed no relationships with cranial dimensions in male and female.

Stereological and Morphometric Analysis of MRI Chiari Malformation Type-1

  • Objective : In this study, we aimed to investigate the underlying ethiological factors in chiari malformation (CM) type-I (CMI) via performing volumetric and morphometric length-angle measurements. Methods : A total of 66 individuals [33 patients (20-65 years) with CMI and 33 control subjects] were included in this study. In sagittal MR images, tonsillar herniation length and concurrent anomalies were evaluated. Supratentorial, infratentorial, and total intracranial volumes were measured using Cavalieri method. Various cranial distances and angles were used to evaluate the platybasia and posterior cranial fossa (PCF) development. Results : Tonsillar herniation length was measured $9.09{\pm}3.39mm$ below foramen magnum in CM group. Tonsillar herniation/concurrent syringomyelia, concavity/defect of clivus, herniation of bulbus and fourth ventricle, basilar invagination and craniovertebral junction abnormality rates were 30.3, 27, 18, 2, 3, and 3 percent, respectively. Absence of cisterna magna was encountered in 87.9% of the patients. Total, IT and ST volumes and distance between Chamberlain line and tip of dens axis, Klaus index, clivus length, distance between internal occipital protuberance and opisthion were significantly decreased in patient group. Also in patient group, it was found that Welcher basal angle/Boogard angle increased and tentorial slope angle decreased. Conclusion : Mean cranial volume and length-angle measurement values significantly decreased and there was a congenital abnormality association in nearly 81.5 percent of the CM cases. As a result, it was concluded that CM ethiology can be attributed to multifactorial causes. Moreover, congenital defects can also give rise to this condition.
