전문간호사 업무범위안 제정에 따른 전문의의 업무 위임 의향 (Intention to Delegate Clinical Practice of Medical Specialists in Accordance with the Enactment of the Scope of Practice for Advanced Practice Nurses)

  • Purpose: This study aimed to investigate the nationwide intention to delegate clinical practice of medical specialists in accordance with the enactment of the scope of practice for advanced practice nurses (APNs). Methods: Data were collected from October to December 2021 using Google Surveys. In total, 147 medical specialists from 12 provinces responded to the survey. The survey questionnaire was categorized into four legislative draft duties, according to the scope of practice (a total of 41 tasks): Twenty-nine tasks on treatments, injects, etc., performed under the guidance of a physician and other activities necessary for medical treatment (treatment domain); two tasks on collaboration and coordination; six tasks on education, counseling, and quality improvement; four regarding other necessary tasks. Participants were asked whether they were willing to delegate the tasks to APN. Results: The intention to delegate tasks to APN was higher for non-invasive tasks such as blood sampling (97.3%) or simple dressing (96.6%). Invasive tasks such as endotracheal tube insertion (10.2%), sampling: bone marrow biopsy & aspiration (23.8%) showed low intention to delegate in the treatment domain. Participants who were older, male, and had more work careers with APN, showed a higher intention to delegate tasks. Conclusion: To prevent confusion in the clinical setting, a clear agreement on the scope of APN practice as APN delegated by physicians should be established. Based on this study, legal practices that APN can perform legally should be established.

Carbohydrate counting 을 이용한 제2형 당뇨병 환자의 식사 관리 (The Meal Management of Korean Type 2 Diabetes Patients Using Carbohydrate Counting)

  • Carbohydrate(CHO) counting is a meal planning approach used with diabetic patients that focuses on carbohydrate as the primary nutrient affecting post-prandial glycemic response. However, it has not been used in meal management of diabetic patients in Korea. CHO counting can be used by clients with type 1 and 2 diabetes. The purpose of the study was to determine the barriers to utilize the CHO counting when three levels of CHO counting were educated to type 2 diabetic patients who started continuous subcutaneous insulin infusion (CSⅡ) therapy by nutrition lectures and counseling. And the CHO-to-insulin ratios were determined for the individual patients who followed the carbohydrate counting as a meal management, and the factors to influence the CHO-to-insulin ratios were selected through the stepwise regression analysis. Twenty- four subjects were received three lectures, and one or two nutritional counseling for a month. The average age of the subjects was 50.7 years, and the duration of diabetes was 9.4 years. Their body mass index (BMI) was 21.5 kg/$m^2$. The difficulties of using CHO counting were 1) confusing the CHO exchange system to diabetic food exchange system, 2) lack of basic nutrition and not distinguishing nutrients such as CHO, fat and calorie, and 3) lack of motivation to make effort to count and record the amount of carbohydrates eaten. Nutritional counseling replenished the nutrition education and made patients practice CHO counting. Average CHO-to-insulin ratios at breakfast, lunch and dinner were 4.1$\pm$3.3, 2.9$\pm$2.6 and 2.9$\pm$3.0units/23g of CHO, respectively. CHO-to-insulin ratios were influenced by gender, age, BMI, post-prandial blood glucose levels and post-prandial c-peptide levels. The effective education and nutritional counseling of CHO counting can make CHO counting applicable to type 2 diabetic patients as meal management for improving glycemic control with less hypoglycemic episode.

초등학생을 위한 해결중심 집단상담프로그램의 효과에 관한 메타분석 (A Meta-Analysis on the Solution Focused Group Program Effects for Elementary School Children)

  • 본 연구에서는 초등학생을 위한 해결중심 집단상담프로그램의 효과크기를 통합하여 제시하고, 보다 효과적인 해결중심 집단상담프로그램에 대한 실천적 방안을 제시하기 위하여 메타분석을 실시하였다. 자료 수집은 2014년 5월까지 국내에서 발표된 석 박사 학위논문 및 학술지에 게재된 논문 중 메타분석의 대상선정기준에 따라 논문을 선정하여 분석하였다. 메타분석을 위해서는 CMA(Comprehensive Meta-Analysis) software 2.0을 사용하였으며, 연구의 주요 결과는 다음과 같다. 첫째, 초등학생을 대상으로 국내에서 실시된 해결중심 집단상담프로그램의 평균효과크기를 살펴보면, 자아존중감의 효과크기가 1.61으로 가장 높게 나타났고, 학교적응능력이 1.35, 대인관계능력이 1.07, 자기효능감이 1.03 의 순으로 나타나서 네 가지 변수 모두 큰 효과크기를 보였다. 둘째, 해결중심 집단상담프로그램의 효과크기가 조절변수에 따라 어떻게 차이가 나는지 살펴보았다. 통계적으로 유의미한 조절변수는 자아존중감의 경우에 '총회기수'이었고 학교적응능력의 경우에는 '총회기수'와 '1회기당 시간'으로 나타났다. 본 연구결과를 바탕으로 초등학생을 위한 해결중심 집단상담프로그램의 실천적 함의와 전략을 제언하였다.

애도상담자 전문역량 향상을 위한 교육과정 탐색 (A Study on Curriculum to Improve Grief Counselor's Competency)

  • 상실은 죽음 뿐 아니라 인생 전환기의 변화까지 포함하는 개념으로 확대되었다. 이와 더불어 자연재해, 예기치 않은 사고 등으로 주변의 죽음을 직 간접적으로 경험하는 비율도 증가하였다. 따라서 애도 상담에 대한 수요 증가와 전문 애도 상담에 대한 필요성이 제기되고 있다. 이에 본 연구는 애도 상담자의 전문역량 향상을 위해 애도 상담 역량 요인을 조사하고, 무엇을 학습하고 훈련해야 하는지 문헌을 검토하고 조사하는데 초점을 두었다. 구체적으로 애도 상담 이론 및 모델, 애도의 평가, 죽음 교육, 상담자 자기-관리로 나누었고 각 교육 내용을 기존 상담교육과정에 통합시키고 경험적 실습 방법을 제시하였다. 본 연구는 애도 상담자 교육에 대한 관심을 고취시키고 애도 상담자 역량 모형 및 체계적인 교육과정 개발에 기초자료로 활용될 수 있다.

식습관 및 운동 목표 달성의 촉진요인과 방해요인: 시내버스 운전자와 상담 영양사의 포커스그룹 인터뷰 (Facilitators and barriers to achieving dietary and physical activity goals: focus group interviews with city bus drivers and counseling dietitians)

  • Objectives: Our previously published study showed that a workplace nutrition intervention program with personalized goal setting and smartphone-based nutrition counseling improved dietary habits and physical activity in city bus drivers who were overweight/obese. This study explored the facilitators and barriers that participants faced in achieving their dietary and physical activity goals six months after the intervention. Methods: The study included bus drivers and dietitians who participated in the intervention program. Three focus group interviews were conducted with 10 bus drivers (divided by two groups based on their achievement of set goals) and five dietitians who had provided nutrition counseling. Results: Willpower was the most important intrapersonal facilitator for drivers to achieve their goals. Other factors that promoted behavioral changes were nutrition counseling by dietitians, trackable physical activity using smartwatches, and setting of practical and achievable goals. Meanwhile, the most important barriers identified were occupational factors such as long driving hours, short breaks, and shift work. Other barriers were environmental factors such as availability of snackable food, accessibility to convenience stores, and cold weather. Family and colleagues were perceived as both facilitators and barriers. In addition, dietitians identified a lack of knowledge about healthy diet as one of the barriers. Conclusions: Our results suggested that the workplace environment should be improved and that nutrition intervention programs at the workplace could encourage bus drivers to practice healthy eating habits. The facilitators and barriers identified in this study should be considered when planning a nutrition intervention program for bus drivers.

보건교사의 비만관리실태 조사연구 (A study on practice of obesity control by school nurses)

  • This study was a research study to find out the real picture and practice of student obesity control by primary, middle and high school school nurses. The study sampled 196 school nurses working in the Busan City and Northern Gyungsang Province, who agreed on the purpose of the study. When it comes to the study process, at the meeting of the whole school nurses, the researcher explained the purpose of the study and asked them to complete questionnaires and return on the spot. The study is designed to provide basic data for school nurses in field to systematically and efficiently control student obesity by examining perception of school obesity control and the real picture or practice of obesity control as well as its problems. The study used SPSS 10.0 to analyze data. The results are as follows: Annual plans about obesity control appeared significant at the primary school, 89.3%, higher than 77.3% of the middle and high school(p<0.05). Annual project evaluation showed a significant difference by regions, 56.0% of metro cities vs. small cities(p<0.01). The number of measuring weight of obese students was higher at the primary school than at the middle and high school(p<0.05). Also small cities conducted the weight measurement more often than metro cities(p<0.001). Among the health education methods for obesity, counseling appeared statistically significant(p<.01). The health education showed a significant difference in more experienced school nurses. Among the educational media, environment postings were used at the primary school, 24.0%, higher than 5.3% of the middle and high school. It showed a significant difference(p<.001). OHP or epidiascopes were used at the primary school, 36.4%, significantly higher than the middle and high school(p<.001). In addition, computer was used at the primary school, 21.5%, higher than the middle and high school(p<.05). Problems of obesity education data included a lack of textbooks and programs and a short of educational media. However it didnt show a significant difference in schools or in regions. Counseling activities for obesity showed a significant difference in schools(p<.01) and in regions(p<.001). When parents with obese students were in counseling, those in their 20s used home correspondence and those in their 30s and 40s used phone call(p<.01). The rate of equipment to measure obesity was higher at the middle and high school than the primary school, showing a significant difference(p<.001). Therefore, regular health education curriculums for school nurses should be systematically created to promote student health. Furthermore, it needs to make standard textbooks for health education by grades as well as by levels and to develop various educational programs and educational media.

대구지역 초등학생의 학교급식 위생에 대한 인식, 지식, 실천 수준 및 위생교육 요구도 분석 (Analysis on perception, knowledge, and practice level for school food hygiene and need for hygiene education of elementary school students in Daegu)

  • The purpose of this research was to investigate perception, knowledge, and practice level for school food hygiene and need for hygiene education with 300 students of elementary schools in Daegu. The average of correct answer percentage on knowledge about food hygiene of the respondents was 65.3%. For practice level of personal hygiene in school food, the average score was 3.75 out of 5 points. The ratio of educational experience of hygiene and dietary behaviors at home was 73.2%. Almost half of the respondents answered that 'food ingredients' should be the most hygienic. About 90% of the respondents recognized 'hand washing' had effect on prevention of foodborne illness. In addition, the respondents wanted to get the information about food hygiene from 'cooking practice at school', 'school broadcasting', 'education program on TV', and 'school newsletter' in order. These results suggest that continuous education on food hygiene for elementary school students are required and supervision to let them have proper hygiene habit is needed.

치위생(학)과 학생들의 성격유형이 임상실습에 미치는 영향 (The Effects of Personality Types of Dental Hygienist Students on Clinical Practice)

  • 본 연구는 부산광역시, 울산광역시 및 경상남도 일부 지역 대학교 433명의 학생을 대상으로 성격유형을 파악하고 임상실습 만족도에 미치는 영향을 조사하였다. 주요 변인들과의 관련성 결과, 외향성 및 저신경증적 경향의 대상자에서 임상실습 만족의 하위요인들에서 유의미한 결과를 보였다. 외향성 요인에 영향을 미치는 요인으로는 지도만족(p<0.001), 환경만족과 실습만족(p<0.05)이 유의한 수준으로 나타났고, 신경증적 경향성에 영향을 미치는 요인으로는 내용만족(p<0.05), 지도만족(p<0.001), 대인관계만족(p<0.05) 및 실습만족(p<0.05)이 유의한 수준으로 나타났다. 따라서, 치위생(학)과 학생들의 임상실습의 만족을 높이기 위해서는 재학 기간 중 성격유형 검사가 반드시 선행되어야 하며, 검사 결과를 토대로 맞춤형 상담프로그램의 강화와 대인관계 능력을 향상시킬 수 있는 다양한 프로그램이 개발되어야 할 것이다.

제2형 당뇨병 환자의 식품섭취 및 식행동에 대한 웹기반 영양상담의 효과 (Effects of Web-Based Nutrition Counseling on Dietary Behavior and Food Intake of Type II Diabetic Patients)

  • 본 연구는 웹기반 영양상담 프로그램을 이용하여 당뇨병 환자들을 대상으로 8주 동안 6회의 영양상담을 실시한 후 웹기반 영양상담이 당뇨병 환자의 식행동 및 식품섭취양상의 변화에 미치는 영향과 영양상담 프로그램을 평가하였다 웹 기반 영양상담 프로그램은 식사관리, 영 양상담, 영양평가, 추후관리 프로그램으로 구성되었다. 추후관리 프로그램은 혈당수첩, 식습관 조사, 식품섭취빈도 조사, 온라인 상담파일로 구성되었으며 온라인 상담파일에 대화방을 두어 환자와 실시간으로 상담할 수 있도록 하였다 웹기반 영양상담후 운동습관은 상당히 개선되었으며 음주와 흡연은 상담 전 이미 그 빈도가 매우 낮음에 기인하여 큰 차이를 보이지 않았다. 식사요법에 대한 지식의 변화는 모든 항목에서 식사요법 지식이 향상되었으며 전체적으로도 비합병증군과 합병증군 두군 모두에서 유의 (p<0.001)하게 높아졌다. 식 행동 변화는 영앙상담 후 두군 모두에서 유의(p<0.001)하게 개선됨으로서, 8주 동안의 지속적인 영양상담이 환자 스스로 올바른 식행동 형성의 의지를 갖게 함으로서 당뇨병 관리의 가능성을 보여주었다. 식품섭취빈도의 변화는 두군 모두에서 어패류, 채소류, 해조류의 섭취 빈도가 영양상담후에 유의하게 높아졌으며 당류, 가공식품, 감자류, 빵류의 섭취빈도가 낮아져 환자들의 식품섭취패턴이 바람직하게 개선되었다. 웹기반 프로그램 및 영양상담에 대한 평가점수는 연령이 낮을 수록 평가점수가 높게 나타났으며, 컴류터 사용이 익숙하지 않아 보호자의 도움을 필요로 하고 있는 60대 이상에서는 상대적으로 낮은 점수를 보였다. 따라서 본 연구의 결과는 웹상에서의 영양상담을 통하여 지속적이고 반복적인 추후 관리를 하는 것이 당뇨병 환자의 바람직한 식품섭취양상 및 식행동 변화를 정착시켜 질병을 관리하는데 기여 할수 있음을 제시한다. 또한 웹을 이용한 프로그램 및 상담에 대한 평가결과도 연령에 따라 차이는 있지만 긍정적으로 나타남으로서 정보화시대에 맞는 새로운 영양상담 매체로서의 인터넷의 가능성을 제시한다.

한국 소아청소년정신과 의사의 전문적 치료 현황 (A Practice Patterns of Child and Adolescent Psychiatrists in South Korea)

  • Objectives : The aim of this study is to assess the career paths and practice patterns of child and adolescent psychiatrists in South Korea. Methods : A survey on demographic data, work environment, opinion on fellowship training program, and current practice patterns was conducted in 76/223 (34.0%) of all child and adolescent psychiatrists (N=223) were from 1980 to 2008. The survey data from the responders were analyzed. Results : More than half of those surveyed were women. The mean age of survey participants was 40.8 years; most were married, and lived in the Seoul National Capital Area. In general, the responders were very satisfied with their work. They worked in private practice, public sector, group practice, and as consultants. Up to 87% of the patients treated were children and adolescents. The most common treatment modalities were medication and parent psychoeducation. Social skills training and play therapy were applied mainly for children aged 4 to 12 years old. Cognitive behavioral therapy and counseling were preferred in adolescents. Conclusion : The current study used a database approach in order to define current practice patterns. These data may be helpful in consideration of future training programs, workforce issues in child and adolescent psychiatry in South Korea, and also in collaboration with other mental health providers.
